    Local status and power in area-based health improvement partnerships

The definitive version is available at http://hea.sagepub.comArea-based initiatives (ABIs) have formed an important part of public policy towards more socio-economically deprived areas in many countries. Co-ordinating service provision within and across sectors has been a common feature of these initiatives. Despite sustained policy interest in ABIs, little empirical work has explored relations between ABI providers and partnership development within this context remains under-theorised. This paper addresses both of these gaps by exploring partnerships as a social and developmental process, drawing on concepts from figurational sociology to explain how provider relations develop within an ABI. Qualitative methods were used to explore, prospectively, the development of an ABI targeted at a town in the north west of England. A central finding was that, although effective delivery of ABIs is premised on a high level of coordination between service providers, the pattern of interdependencies between providers limits the frequency and effectiveness of cooperation. In particular, the interdependency of ABI providers with others in their organisation (what is termed here ‘organisational pull’) constrained the ways in which they worked with providers outside of their own organisations. ‘Local’ status, which could be earned over time, enabled some providers to exert greater control over the way in which provider relations developed during the course of the initiative. These findings demonstrate how historically constituted social networks, within which all providers are embedded, shape partnership development. The theoretical insight developed here suggests a need for more realistic expectations among policy makers about how and to what extent provider partnerships can be managed.     Basic Erosive Wear Examination (BEWE): a new scoring system for scientific and clinical needs

    A new scoring system, the Basic Erosive Wear Examination (BEWE), has been designed to provide a simple tool for use in general practice and to allow comparison to other more discriminative indices. The most severely affected surface in each sextant is recorded with a four level score and the cumulative score classified and matched to risk levels which guide the management of the condition. The BEWE allows re-analysis and integration of results from existing studies and, in time, should initiate a consensus within the scientific community and so avoid continued proliferation of indices. Finally, this process should lead to the development of an internationally accepted, standardised and validated index. The BEWE further aims to increase the awareness of tooth erosion amongst clinicians and general dental practitioners and to provide a guide as to its management

    Modernisation and governance in UK national governing bodies of sport: how modernisation influences the way board members perceive and enact their roles

    Modernisation has been a key objective of many national governments for at least the last two decades. A significant element of the modernisation agenda has been the focus on improving the governance of public sector and, more recently, voluntary sector organisations. In the UK voluntary sport sector, this has involved policy statements, governance monitoring systems linked to public funding and a number of ‘good governance’ guides, aimed primarily at the boards of national governing bodies of sport (NGBs). Previous research has critically analysed modernisation and explored its effects, most often at a macro level. Very little research, to date, however, has looked at the influence of modernisation on the boards of NGBs. This article seeks to do just that, drawing on the first national survey of board-level governance in the UK and an in-depth, longitudinal case study of one UK-based NGB. It empirically examines which board roles NGBs consider most important and statistically compares large and small NGBs. It then draws on direct observation of board and committee meetings, in-depth interviews and analysis of key organisational documents to examine how modernisation influences the way board members perceive and enact their roles. In so doing, this article draws together the political science research on modernisation and the sport governance research on board roles and seeks to promote closer integration between these complementary streams of research

    Knowledge-Based Synthesis of Distributed Systems Using Event Structures

    To produce a program guaranteed to satisfy a given specification one can synthesize it from a formal constructive proof that a computation satisfying that specification exists. This process is particularly effective if the specifications are written in a high-level language that makes it easy for designers to specify their goals. We consider a high-level specification language that results from adding knowledge to a fragment of Nuprl specifically tailored for specifying distributed protocols, called event theory. We then show how high-level knowledge-based programs can be synthesized from the knowledge-based specifications using a proof development system such as Nuprl. Methods of Halpern and Zuck then apply to convert these knowledge-based protocols to ordinary protocols.     Describing and analysing primary health care system support for chronic illness care in Indigenous communities in Australia's Northern Territory – use of the Chronic Care Model

    Indigenous Australians experience disproportionately high prevalence of, and morbidity and mortality from chronic illness such as diabetes, renal disease and cardiovascular disease. Improving the understanding of how Indigenous primary care systems are organised to deliver chronic illness care will inform efforts to improve the quality of care for Indigenous people. This cross-sectional study was conducted in 12 Indigenous communities in Australia's Northern Territory. Using the Chronic Care Model as a framework, we carried out a mail-out survey to collect information on material, financial and human resources relating to chronic illness care in participating health centres. Follow up face-to-face interviews with health centre staff were conducted to identify successes and difficulties in the systems in relation to providing chronic illness care to community members. Using concrete examples, this study translates the concept of the Chronic Care Model (and associated systems view) into practical application in Australian Indigenous primary care settings. This approach proved to be useful in understanding the quality of primary care systems for prevention and management of chronic illness. Further refinement of the systems should focus on both increasing human and financial resources and improving management practice

    How Nigeria’s 2015 presidential election outcome was forecasted with geodemographics and public sentiment analytics

    In 2015, Nigeria held one of the most fiercely contested presidential elections in the nation’s recent democratic history. The outcome of the election was expected to exert significant influence on democratic practices on the African continent. The stiffness of the contest also meant that it was difficult to predict the likely winner of the election. This paper summarizes how an empirical approach was used to forecast the outcome of the election by modeling public sentiment data-set using a geodemographic framework. Results indicate that the main electorates that determined the outcome of the election were situated in thirteen battleground states. Additionally, results showed that two years before the presidential election, Goodluck Jonathan’s public approval ratings on corruption, insecurity, and the economy (the main drivers of the 2015 election) had nose-dived across many of the battleground states.
