Structural Basis for the Function of Complement Component C4 within the Classical and Lectin Pathways of Complement
Complement component C4 is a central protein in the classical and lectin pathways within the complement system. During activation of complement, its major fragment C4b becomes covalently attached to the surface of pathogens and altered self-tissue, where it acts as an opsonin marking the surface for removal. Moreover, C4b provides a platform for assembly of the proteolytically active convertases that mediate downstream complement activation by cleavage of C3 and C5. In this article, we present the crystal and solution structures of the 195-kDa C4b. Our results provide the molecular details of the rearrangement accompanying C4 cleavage and suggest intramolecular flexibility of C4b. The conformations of C4b and its paralogue C3b are shown to be remarkably conserved, suggesting that the convertases from the classical and alternative pathways are likely to share their overall architecture and mode of substrate recognition. We propose an overall molecular model for the classical pathway C5 convertase in complex with C5, suggesting that C3b increases the affinity for the substrate by inducing conformational changes in C4b rather than a direct interaction with C5. C4b-specific features revealed by our structural studies are probably involved in the assembly of the classical pathway C3/C5 convertases and C4b binding to regulators
The Indian cobra reference genome and transcriptome enables comprehensive identification of venom toxins
Snakebite envenoming is a serious and neglected tropical disease that kills ~100,000 people annually. High-quality, genome-enabled comprehensive characterization of toxin genes will facilitate development of effective humanized recombinant antivenom. We report a de novo near-chromosomal genome assembly of Naja naja, the Indian cobra, a highly venomous, medically important snake. Our assembly has a scaffold N50 of 223.35 Mb, with 19 scaffolds containing 95% of the genome. Of the 23,248 predicted protein-coding genes, 12,346 venom-gland-expressed genes constitute the \u27venom-ome\u27 and this included 139 genes from 33 toxin families. Among the 139 toxin genes were 19 \u27venom-ome-specific toxins\u27 (VSTs) that showed venom-gland-specific expression, and these probably encode the minimal core venom effector proteins. Synthetic venom reconstituted through recombinant VST expression will aid in the rapid development of safe and effective synthetic antivenom. Additionally, our genome could serve as a reference for snake genomes, support evolutionary studies and enable venom-driven drug discovery
The Pulse-Respiration Quotient: A Powerful but Untapped Parameter for Modern Studies About Human Physiology and Pathophysiology
A specific and unique aspect of cardiorespiratory activity can be captured by dividing the heart rate (HR) by the respiration rate (RR), giving the pulse-respiration quotient (PRQ = HR/RR). In this review article, we summarize the main findings of studies using and investigating the PRQ. We describe why the PRQ is a powerful parameter that captures complex regulatory states of the cardiorespiratory system, and we highlight the need to re-introduce the use of this parameter into modern studies about human physiology and pathophysiology. In particular, we show that the PRQ (i) changes during human development, (ii) is time-dependent (ultradian, circadian, and infradian rhythms), (iii) shows specific patterns during sleep, (iv) changes with physical activity and body posture, (v) is linked with psychophysical and cognitive activity, (vi) is sex-dependent, and (vii) is determined by the individual physiological constitution. Furthermore, we discuss the medical aspects of the PRQ in terms of applications for disease classification and monitoring. Finally, we explain why there should be a revival in the use of the PRQ for basic research about human physiology and for applications in medicine, and we give recommendations for the use of the PRQ in studies and medical applications
L'ablation des extrasystoles ventriculaires : expérience d'un centre tertiaire et cas particulier des foyers du sommet du ventricule gauche
Introduction : les extrasystoles ventriculaires (ESV) sont une cause frĂ©quente de dysfonction systolique ventriculaire gauche (VG). Leur ablation permet dâamĂ©liorer la fraction dâĂ©jection du ventricule gauche (FEVG) et les symptĂŽmes. Les ESV du sommet VG constituent une localisation frĂ©quente dâESV idiopathiques, mais leur ablation est complexe. Lâutilisation dâune approche anatomique pourrait permettre dâaugmenter le taux de succĂšs de ces ablations. Objectif : Lâobjectif de notre Ă©tude Ă©tait dâĂ©tudier les caractĂ©ristiques des ablations dâESV rĂ©alisĂ©es au sein du CHU de la Timone, et dâĂ©valuer lâefficacitĂ© dâune approche anatomique dans lâablation des ESV du sommet VG.MatĂ©riel et mĂ©thode : tous les patients ayant bĂ©nĂ©ficiĂ© dâune ablation dâESV entre le 1er Janvier 2015 et le 31 DĂ©cembre 2018 dans le service de Rythmologie du CHU de la Timone (APHM, Marseille, France) ont Ă©tĂ© inclus de maniĂšre rĂ©trospective. Un Holter ECG 24h Ă©tait rĂ©alisĂ© avant lâinclusion et Ă 3mois post procĂ©dure, ainsi quâune Ă©chographie transthoracique (ETT) avec mesure de la FEVG, et une Ă©valuation clinique. Le succĂšs de la procĂ©dure Ă©tait dĂ©fini par une rĂ©duction de plus 80% de la charge en ESV au 3Ăšme mois ou par une rĂ©duction de plus de 50% avec une charge rĂ©siduelle <10%. Les patients avec ESV du sommet VG pris en charge aprĂšs Juin 2016 ont bĂ©nĂ©ficiĂ© dâune ablation avec approche anatomique. RĂ©sultats : 113 procĂ©dures ont Ă©tĂ© rĂ©alisĂ©es, avec une majoritĂ© de procĂ©dures dâablation dans le VG (n= 66 (58.4%) VG seul et n=8 (7.1%) VG+VD) et chez des patients porteurs dâune cardiopathie structurelle. Le taux de succĂšs Ă 3mois Ă©tait de 76.1% toutes procĂ©dures confondues, avec un taux de succĂšs plus important pour les procĂ©dures VG (79.7%) et en cas dâESV monomorphes (76.9%). On notait une rĂ©duction significative de la charge en ESV Ă 3mois (4.2 ± 6.2% contre 19.6 ± 11.9%, p<0.001) associĂ©e Ă une amĂ©lioration significative de la dyspnĂ©e (NYHA 1.5 ± 0.8 contre 1.9 ± 0.7, p=0.002), et une tendance non significative Ă lâamĂ©lioration de la FEVG tous patients confondus. Chez les patients prĂ©sentant une FEVG <50% Ă lâadmission, on retrouvait une amĂ©lioration significative de la FEVG de +5 [0.6 ; 9.4] points lors du suivi. Le taux de complication post procĂ©dure Ă©tait de 5.3%, dont aucune complication majeure. Les ablations dâESV du sommet VG (n=22) reprĂ©sentaient 19.5% de lâensemble des procĂ©dures rĂ©alisĂ©es, et le site le plus frĂ©quent dâablation au sein du VG. Lâutilisation dâune approche anatomique aprĂšs Juin 2016 conduisait Ă un succĂšs Ă 3 mois chez 12 patients sur les 15 procĂ©dures rĂ©alisĂ©es, soit un taux de succĂšs de 80%, sans aucune complication. Conclusion : le taux de succĂšs Ă 3mois de lâensemble des procĂ©dures Ă©tait de 76.1%, avec une amĂ©lioration significative de la charge en ESV et de la dyspnĂ©e. La majoritĂ© des procĂ©dures Ă©taient pratiquĂ©es au sein du VG chez des patients porteurs dâune cardiopathie structurelle. Lâutilisation dâune approche anatomique pour les ablations dâESV du sommet VG aboutissait Ă un taux de succĂšs de 80%. Dâautres Ă©tudes sont nĂ©cessaires pour confirmer lâamĂ©lioration fonctionnelle et de la FEVG observĂ©es en post ablation chez ces patients

Assisted resiliency in child protection : development of the program for educators
Pour contrer la vision patho-centrĂ©e qui domine en protection de lâenfance en France, nous avons construit un programme de rĂ©silience assistĂ©e Ă destination des Ă©ducateurs. Nous nous sommes appuyĂ©s sur la littĂ©rature relative Ă la rĂ©silience assistĂ©e ; sur la mĂ©thodologie de la recherche-action et sur les principaux facteurs de risque et de protection que lâon retrouve chez les professionnels, chez les enfants pris en charge et leurs familles et dans le contexte de placement. Ce programme, dĂ©ployĂ© sur deux ans, a Ă©tĂ© mis en place auprĂšs dâun groupe dâĂ©ducateurs travaillant avec des enfants placĂ©s ĂągĂ©s de 6 Ă 13 ans. Nous avons Ă©valuĂ© les effets produits sur les participants et sur les enfants pris en charge par le biais dâune mĂ©thodologie mixte. Les rĂ©sultats sont encourageants et soulignent que des Ă©volutions importantes ont eu lieu au fil de cette recherche. Concernant les Ă©ducateurs, nous retrouvons notamment : une Ă©volution du regard portĂ© sur les enfants et les familles ; des changements dans les mĂ©thodes de travail ; la crĂ©ation d'une culture de travail commune autour du paradigme de la rĂ©silience assistĂ©e ; le dĂ©veloppement de la cohĂ©sion d'Ă©quipe ; lâĂ©mergence dâune crĂ©ativitĂ© nouvelle et lâaccroissement de la richesse du vocabulaire relatif aux Ă©motions. Concernant les enfants, nous constatons une meilleure conscience de leurs Ă©motions ; une diminution significative de lâanxiĂ©tĂ© et des troubles du comportement et une plus grande sĂ©curitĂ© interne. Il sâagit du premier programme de rĂ©silience assistĂ©e Ă destination dâĂ©ducateurs. La prĂ©sente Ă©tude reste exploratoire mais fournit une base de rĂ©fĂ©rence solide pour Ă©tendre cette approche en protection de lâenfance.Research context: To counter the pathological-focused functioning of the child-protection system in France, we developed an assisted-resilience program for the educators. This program is based on the literature about assisted resilience, on the action research methodology and on the main risk and protective factors related to these professionals, to the children, to their families and to their care setting. Method : This program, developed over the past two years, was implemented on a group of educators working with foster children aged between 6 and 13 years old. We assessed the effects produced on the participants and on the children using a mixed methodology, combining qualitative and quantitative data..Results: Results are encouraging as they underline that important evolutions occurred over the course of the research.With regard to the educators, we can observe an evolution of the childrenâs and familiesâ perception; change in the working methods; the creation of a common working culture around the assisted resilience paradigm; the development of a team cohesion; the emergence of a new creativity and the increase of the vocabulary related to emotions.When analyzing the results of the program on children, we can see a better conscience of their emotions; a significant decrease of anxiety and behavioral disorders and better internal security.Conclusion: This is the first program of assisted resilience developed for educators. This research is exploratory but provides a solid baseline to extend this approach in child protection in order to stop the growth of this current pathological-focused vision
