Lessons Learned from Testing the Quality Cost Model of Advanced Practice Nursing (APN) Transitional Care
Purpose To describe the development, testing, modification, and results of the Quality Cost Model of Advanced Practice Nurses (APNs) Transitional Care on patient outcomes and health care costs in the United States over 22 years, and to delineate what has been learned for nursing education, practice, and further research. Organizing Construct The Quality Cost Model of APN Transitional Care. Methods Review of published results of seven randomized clinical trials with very low birth-weight (VLBW) infants; women with unplanned cesarean births, high risk pregnancies, and hysterectomy surgery; elders with cardiac medical and surgical diagnoses and common diagnostic related groups (DRGs); and women with high risk pregnancies in which half of physician prenatal care was substituted with APN care. Ongoing work with the model is linking the process of APN care with the outcomes and costs of care. Findings APN intervention has consistently resulted in improved patient outcomes and reduced health care costs across groups. Groups with APN providers were rehospitalized for less time at less cost, reflecting early detection and intervention. Optimal number and timing of postdischarge home visits and telephone contacts by the APNs and patterns of rehospitalizations and acute care visits varied by group. Conclusions To keep people well over time, APNs must have depth of knowledge and excellent clinical and interpersonal skills that are the hallmark of specialist practice, an in-depth understanding of systems and how to work within them, and sufficient patient contact to effect positive outcomes at low cost
Measuring large-scale structure with quasars in narrow-band filter surveys
We show that a large-area imaging survey using narrow-band filters could
detect quasars in sufficiently high number densities, and with more than
sufficient accuracy in their photometric redshifts, to turn them into suitable
tracers of large-scale structure. If a narrow-band optical survey can detect
objects as faint as i=23, it could reach volumetric number densities as high as
10^{-4} h^3 Mpc^{-3} (comoving) at z~1.5 . Such a catalog would lead to
precision measurements of the power spectrum up to z~3-4. We also show that it
is possible to employ quasars to measure baryon acoustic oscillations at high
redshifts, where the uncertainties from redshift distortions and nonlinearities
are much smaller than at z<1. As a concrete example we study the future impact
of J-PAS, which is a narrow-band imaging survey in the optical over 1/5 of the
unobscured sky with 42 filters of ~100 A full-width at half-maximum. We show
that J-PAS will be able to take advantage of the broad emission lines of
quasars to deliver excellent photometric redshifts, \sigma_{z}~0.002(1+z), for

Tree planting in organic soils does not result in net carbon sequestration on decadal timescales
Tree planting is increasingly being proposed as a strategy to combat climate change through carbon (C) sequestration in tree biomass. However, total ecosystem C storage that includes soil organic C (SOC) must be considered to determine whether planting trees for climate change mitigation results in increased C storage. We show that planting two native tree species (Betula pubescens and Pinus sylvestris ), of widespread Eurasian distribution, onto heather (Calluna vulgaris ) moorland with podzolic and peaty podzolic soils in Scotland, did not lead to an increase in net ecosystem C stock 12 or 39 years after planting. Plots with trees had greater soil respiration and lower SOC in organic soil horizons than heather control plots. The decline in SOC cancelled out the increment in C stocks in tree biomass on decadal timescales. At all four experimental sites sampled, there was no net gain in ecosystem C stocks 12â39 years after afforestationâindeed we found a net ecosystem C loss in one of four sites with deciduous B. pubescens stands; no net gain in ecosystem C at three sites planted with B. pubescens ; and no net gain at additional stands of P. sylvestris . We hypothesize that altered mycorrhizal communities and autotrophic C inputs have led to positive âprimingâ of soil organic matter, resulting in SOC loss, constraining the benefits of tree planting for ecosystem C sequestration. The results are of direct relevance to current policies, which promote tree planting on the assumption that this will increase net ecosystem C storage and contribute to climate change mitigation. Ecosystemâlevel biogeochemistry and C fluxes must be better quantified and understood before we can be assured that largeâscale tree planting in regions with considerable preâexisting SOC stocks will have the intended policy and climate change mitigation outcomes

Abstract: IL-6 excess is central to the pathogenesis of multiple inflammatory conditions and this is targeted in clinical practice by immunotherapy that blocks the IL-6 receptor encoded by IL6R. We describe two patients with homozygous mutations in IL6R who presented with recurrent infections, abnormal acute phase responses, elevated IgE, eczema, and eosinophilia. This study identifies a novel primary immunodeficiency, clarifying the contribution of IL-6 to the phenotype of patients with mutations in IL6ST, STAT3 and ZNF341, genes encoding different components of the IL-6 signalling pathway, and alerts us to the potential toxicity of drugs targeting the IL-6R.J.E.D.T. is supported by the MRC (RG95376 and MR/L006197/1). Tension between SN and BAO: current status and future forecasts
Using real and synthetic Type Ia SNe (SNeIa) and baryon acoustic oscillations
(BAO) data representing current observations forecasts, this paper investigates
the tension between those probes in the dark energy equation of state (EoS)
reconstruction considering the well known CPL model and Wang's low correlation
reformulation. In particular, here we present simulations of BAO data from both
the the radial and transverse directions. We also explore the influence of
priors on Omega_m and Omega_b on the tension issue, by considering 1-sigma
deviations in either one or both of them. Our results indicate that for some
priors there is no tension between a single dataset (either SNeIa or BAO) and
their combination (SNeIa+BAO). Our criterion to discern the existence of
tension (sigma-distance) is also useful to establish which is the dataset with
most constraining power; in this respect SNeIa and BAO data switch roles when
current and future data are considered, as forecasts predict and spectacular
quality improvement on BAO data. We also find that the results on the tension
are blind to the way the CPL model is addressed: there is a perfect match
between the original formulation and that by the low correlation optimized, but
the errors on the parameters are much narrower in all cases of our exhaustive
exploration, thus serving the purpose of stressing the convenience of this

Measures of Galaxy Environment - I. What is "Environment"?
The influence of a galaxy's environment on its evolution has been studied and
compared extensively in the literature, although differing techniques are often
used to define environment. Most methods fall into two broad groups: those that
use nearest neighbours to probe the underlying density field and those that use
fixed apertures. The differences between the two inhibit a clean comparison
between analyses and leave open the possibility that, even with the same data,
different properties are actually being measured. In this work we apply twenty
published environment definitions to a common mock galaxy catalogue constrained
to look like the local Universe. We find that nearest neighbour-based measures
best probe the internal densities of high-mass haloes, while at low masses the
inter-halo separation dominates and acts to smooth out local density
variations. The resulting correlation also shows that nearest neighbour galaxy
environment is largely independent of dark matter halo mass. Conversely,
aperture-based methods that probe super-halo scales accurately identify
high-density regions corresponding to high mass haloes. Both methods show how
galaxies in dense environments tend to be redder, with the exception of the
largest apertures, but these are the strongest at recovering the background
dark matter environment. We also warn against using photometric redshifts to
define environment in all but the densest regions. When considering environment
there are two regimes: the 'local environment' internal to a halo best measured
with nearest neighbour and 'large-scale environment' external to a halo best
measured with apertures. This leads to the conclusion that there is no
universal environment measure and the most suitable method depends on the scale

Systematically reviewing and synthesizing evidence from conversation analytic and related discursive research to inform healthcare communication practice and policy: an illustrated guide
Background
Healthcare delivery is largely accomplished in and through conversations between people, and healthcare quality and effectiveness depend enormously upon the communication practices employed within these conversations. An important body of evidence about these practices has been generated by conversation analysis and related discourse analytic approaches, but there has been very little systematic reviewing of this evidence.
Methods
We developed an approach to reviewing evidence from conversation analytic and related discursive research through the following procedures:
⢠reviewing existing systematic review methods and our own prior experience of applying these
⢠clarifying distinctive features of conversation analytic and related discursive work which must be taken into account when reviewing
⢠holding discussions within a review advisory team that included members with expertise in healthcare research, conversation analytic research, and systematic reviewing
⢠attempting and then refining procedures through conducting an actual review which examined evidence about how people talk about difficult future issues including illness progression and dying
Results
We produced a step-by-step guide which we describe here in terms of eight stages, and which we illustrate from our âReview of Future Talkâ. The guide incorporates both established procedures for systematic reviewing, and new techniques designed for working with conversation analytic evidence.
Conclusions
The guide is designed to inform systematic reviews of conversation analytic and related discursive evidence on specific domains and topics. Whilst we designed it for reviews that aim at informing healthcare practice and policy, it is flexible and could be used for reviews with other aims, for instance those aiming to underpin research programmes and projects. We advocate systematically reviewing conversation analytic and related discursive findings using this approach in order to translate them into a form that is credible and useful to healthcare practitioners, educators and policy-makers
Cross-cutting principles for planetary health education
Since the 2015 launch of the Rockefeller Foundation Lancet Commission on planetary health,1 an enormous groundswell of interest in planetary health education has emerged across many disciplines, institutions, and geographical regions. Advancing these global efforts in planetary health education will equip the next generation of scholars to address crucial questions in this emerging field and support the development of a community of practice. To provide a foundation for the growing interest and efforts in this field, the Planetary Health Alliance has facilitated the first attempt to create a set of principles for planetary health education that intersect education at all levels, across all scales, and in all regions of the worldâie, a set of cross-cutting principles
The Sloan Digital Sky Survey: Technical Summary
The Sloan Digital Sky Survey (SDSS) will provide the data to support detailed
investigations of the distribution of luminous and non- luminous matter in the
Universe: a photometrically and astrometrically calibrated digital imaging
survey of pi steradians above about Galactic latitude 30 degrees in five broad
optical bands to a depth of g' about 23 magnitudes, and a spectroscopic survey
of the approximately one million brightest galaxies and 10^5 brightest quasars
found in the photometric object catalog produced by the imaging survey. This
paper summarizes the observational parameters and data products of the SDSS,
