A new path motion to aid in the tracing of P and the reflection generated SV wave paths in the continuum plate is developed. A theoretical output voltage from the receiving transducer is calculated for a tone burst. The study enhances the quantitative and qualitative understanding of the nondestructive evaluation of fiber composites which can be modeled as transversely isotropic media

    Input-output characterization of fiber composites by SH waves

    Input-output characterization of fiber composites is studied theoretically by tracing SH waves in the media. A fiberglass epoxy composite is modeled as a homogeneous transversely isotropic continuum plate. The reflection of an SH wave at a stress-free plane boundary in a semi-infinite transversely isotropic medium is considered first. It is found that an incident SH wave reflects only a similar SH wave back into the medium. It is also established that the angle of reflection of the reflected wave is equal to the angle of incidence of the incident wave. The phase velocity of the SH waves and the delay time of the SH waves in reaching the receiving transducer are computed as functions of a reflection index, defined as the number of reflections of the SH waves from the bottom face of the continuum plate. The directivity function corresponding to the shear stress associated with the SH waves in the continuum plate is also derived as a function of the reflection index. A theoretical output voltage from the receiving transducer is calculated for a tone burst (a periodic input voltage of finite duration). The output voltage is shown for tone bursts of duration 60 microseconds and center frequencies of 0.75, 1.00, and 1.25 MHz.     Effective responder communication improves efficiency and psychological outcomes in a mass decontamination field experiment: implications for public behaviour in the event of a chemical incident

    The risk of incidents involving mass decontamination in response to a chemical, biological, radiological, or nuclear release has increased in recent years, due to technological advances, and the willingness of terrorists to use unconventional weapons. Planning for such incidents has focused on the technical issues involved, rather than on psychosocial concerns. This paper presents a novel experimental study, examining the effect of three different responder communication strategies on public experiences and behaviour during a mass decontamination field experiment. Specifically, the research examined the impact of social identity processes on the relationship between effective responder communication, and relevant outcome variables (e.g. public compliance, public anxiety, and co-operative public behaviour). All participants (N = 111) were asked to visualise that they had been involved in an incident involving mass decontamination, before undergoing the decontamination process, and receiving one of three different communication strategies: 1) Health-focused explanations about decontamination, and sufficient practical information; 2) No health-focused explanations about decontamination, sufficient practical information; 3) No health-focused explanations about decontamination, insufficient practical information. Four types of data were collected: timings of the decontamination process; observational data; and quantitative and qualitative self-report data. The communication strategy which resulted in the most efficient progression of participants through the decontamination process, as well as the fewest observations of non-compliance and confusion, was that which included both health-focused explanations about decontamination and sufficient practical information. Further, this strategy resulted in increased perceptions of responder legitimacy and increased identification with responders, which in turn resulted in higher levels of expected compliance during a real incident, and increased willingness to help other members of the public. This study shows that an understanding of the social identity approach facilitates the development of effective responder communication strategies for incidents involving mass decontamination

    A simple tool for comparing benefits and “costs” of COVID-19 lockdown exit strategies

    As several countries begin to exit various forms of COVID-19 “lockdown,” a simple epidemiological tool – Population Attributable Risk – can help guide decisions about specific exit policy options, by quantifying the proportion of “serious” (hospitalised) COVID-19 cases likely attributable to various combinations of individual risk factors at the population level, such as age alone, versus age combined with the presence of any chronic disease/risk factor. Using recent COVID-19 hospitalisation data from a large hospital network, and current Scottish population age structure and risk-factor prevalences, we show that the likely impact on adult hospitalisations would be very similar (an approximate halving, compared to full lockdown exit for all adults) for the most “moderate” and yet reasonably effective options for continued lockdown: continuing to restrict the social contacts of all persons over 65, compared to all persons over 50 with any chronic disease/risk factor. Other considerations are therefore of critical importance to this decision, such as the equity and acceptability of these two policy options, as well their likely economic impacts.PostprintPeer reviewe

    Does the operations of the National Health Insurance Scheme (NHIS) in Ghana align with the goals of primary health care? Perspectives of key stakeholders in northern Ghana

    In 2005, the World Health Assembly (WHA) of the World Health Organization (WHO) urged member states to aim at achieving affordable universal coverage and access to key promotive, preventive, curative, rehabilitative and palliative health interventions for all their citizens on the basis of equity and solidarity. Since then, some African countries, including Ghana, have taken steps to introduce national health insurance reforms as one of the key strategies towards achieving universal health coverage (UHC). The aim of this study was to get a better understanding of how Ghana's health insurance institutions interact with stakeholders and other health sector programmes in promoting primary health care (PHC). Specifically, the study identified the key areas of misalignment between the operations of the NHIS and that of PHC.; Using qualitative and survey methods, this study involved interviews with various stakeholders in six selected districts in the Upper East region of Ghana. The key stakeholders included the National Health Insurance Authority (NHIA), district coordinators of the National Health Insurance Schemes (NHIS), the Ghana Health Service (GHS) and District Health Management Teams (DHMTs) who supervise the district hospitals, health centers/clinics and the Community-based Health and Planning Services (CHPS) compounds as well as other public and private PHC providers. A stakeholders' workshop was organized to validate the preliminary results which provided a platform for stakeholders to deliberate on the key areas of misalignment especially, and to elicit additional information, ideas and responses, comments and recommendations from respondents for the achievement of the goals of UHC and PHC.; The key areas of misalignments identified during this pilot study included: delays in reimbursements of claims for services provided by health care providers, which serves as a disincentive for service providers to support the NHIS; inadequate coordination among stakeholders in PHC delivery; and inadequate funding for PHC, particularly on preventive and promotive services. Other areas are: the bypassing of PHC facilities due to lack of basic services at the PHC level such as laboratory services, as well as proximity to the district hospitals; and finally the lack of clear understanding of the national policy on PHC.; This study suggests that despite the progress that has been made since the establishment of the NHIS in Ghana, there are still huge gaps that need urgent attention to ensure that the goals of UHC and PHC are met. The key areas of misalignment identified in this study, particularly on the delays in reimbursements need to be taken seriously. It is also important for more dialogue between the NHIA and service providers to address key concerns in the implementation of the NHIS which is key to achieving UHC
