    Sleep patterns in patients treated for non-secreting intra- and parasellar tumors

    PURPOSE: In this study we evaluate sleep patterns of patients treated for non-secreting intra- and parasellar tumors and age- and sex-matched healthy controls. METHODS: We conducted a self-report cross-sectional case-control study with 104 patients treated for non-secreting intra- and parasellar tumors and 1800 healthy controls in an 1:8 matching. All subjects answered the Munich ChronoType Questionnaire, whereas patients were provided the Pittsburgh Sleep Quality Index, the Epworth Sleepiness Scale, the Short-Form 36 Health survey, the Beck Depression Inventory and the State-Trait Anxiety Inventory additionally. RESULTS: Patients treated for non-secreting intra- and parasellar tumors go to bed earlier, fall asleep earlier, need less time to prepare to sleep but also to get up. Additionally, they lie and sleep longer. The subgroup analysis showed that patients with secondary adrenal insufficiency compared to controls experienced shorter daily light exposure and longer sleep latency. Higher hydrocortisone dose (>20mg) was associated with worse score in global, physical and mental health, shorter time to prepare to sleep, earlier sleep onset and longer sleep duration. CONCLUSION: Our study shows that patients treated for non-secreting intra- and parasellar tumors, even if successfully treated, experience altered sleep patterns compared to controls. We suggest that managing clinicians should enlighten these possible sleep alterations to their patients and use specific questionnaires to document sleep disturbances. Additionally, when treating patients surgically, especially by transcranial approach, damaging the suprachiasmatic nucleus should be avoided. Furthermore, circadian hydrocortisone replacement therapy ideally with dual-release hydrocortisone - if possible, in a dose not more than 20mg daily - that resembles physiological cortisol levels more closely may be beneficial and could improve sleep patterns and sleep-related quality of life

    Exploring the dust content of SDSS DR7 damped Lyman alpha systems at 2.15zab<\le z_{ab}< 5.2

    We have studied a sample of 1084 intervening absorption systems with 2.15zab\le z_{ab}\le5.2, having log(NHI_{\rm HI}) >> 20.0 in the spectra of QSOs in Sloan Digital Sky Survey (SDSS) data release 7 (DR7), with the aim of understanding the nature and abundance of the dust and the chemical abundances in the DLA absorbers. Composite spectra were constructed for the full sample and several subsamples, chosen on the basis of absorber and QSO properties. Average extinction curves were obtained for the samples by comparing their geometric mean composite spectra with those of two samples of QSOs, matching in zem_{em} and i magnitude with the DLA sample, one sample without any absorbers along their lines of sight and the other without any DLAs along their lines of sight irrespective of the presence of other absorption systems. While the average reddening in the DLA sample is small, we find definite evidence for the presence of dust in subsamples based on absorber properties, in particular the strength of metal absorption lines. DLAs along lines of sight to QSOs which are not colour selected are found to be more dusty compared to those along the lines of sight to the more numerous colour selected QSOs. From these studies and from the strengths of absorption lines in the composite spectra, we conclude that \le 10% of the DLAs in SDSS DR7 cause significant reddening, have stronger absorption lines and have higher abundances as compared to the rest of the sample. The rest of the sample shows little reddening. Due to the dominant color selection method used to target QSOs in the SDSS DR7, this fraction of 10% likely represents a lower limit for the global fraction of dusty DLAs at high-z.Comment: 12 pages, 9 figures. Therefore, a clear understanding of chemokines and immune checkpoint modulators is essential for the treatment of this disease. This review discusses the origins and categories of chemokines and the mechanisms that are responsible for activating immune checkpoints in cervical dysplasia and cancer, chemokines as biomarkers, and therapy development that targets immune checkpoints in cervical cancer research

    Comparison of 13CO Line and Far-Infrared Continuum Emission as a Diagnostic of Dust and Molecular Gas Physical Conditions: III. Systematic Effects and Scientific Implications

    Far-infrared continuum data from the {\it COBE}/{\it DIRBE} instrument were combined with Nagoya 4-m \cOone spectral line data to infer the multiparsec-scale physical conditions in the Orion A and B molecular clouds, using 140\um/240\um dust color temperatures and the 240\um/\cOone intensity ratios. In theory, the ratio of far-IR, submillimeter, or millimeter continuum to that of a \cO (or \Co) rotational line can place reliable upper limits on the temperature of the dust and molecular gas on multi-parsec scales; on such scales, both the line and continuum emission are optically thin, resulting in a continuum-to-line ratio that suffers no loss of temperature sensitivity in the high-temperature limit as occurs for ratios of CO rotational lines or ratios of continuum emission in different wavelength bands. Two-component models fit the Orion data best, where one has a fixed-temperature and the other has a spatially varying temperature. The former represents gas and dust towards the surface of the clouds that are heated primarily by a very large-scale (i.e. 1\sim 1 kpc) interstellar radiation field. The latter represents gas and dust at greater depths into the clouds and are shielded from this interstellar radiation field and heated by local stars. The inferred physical conditions are consistent with those determined from previously observed maps of \COone and \Jtwo that cover the entire Orion A and B molecular clouds. The models require that the dust-gas temperature difference is 0±2\pm 2 K. If this surprising result applies to much of the Galactic ISM, except in unusual regions such as the Galactic Center, then there are a number implications.Comment: The work of Schnee et al. 2006 is relevant here.     Elderly and patients with large breast volume have an increased risk of seroma formation after mastectomy — results of the SerMa pilot study

    The collective of the SerMa pilot study included 100 cases of primary breast cancer or Carcinoma in situ who had undergone a mastectomy procedure with or without reconstruction of the breast using an implant or expander at Augsburg University Hospital between 12/2019 and 12/2022. The study aimed to investigate possible causes of seroma formation; reported here are the clinicopathological correlations between seroma formation and tumor biology and surgical procedures. Seroma occurred significantly more often in patients with older age (median patient age in cases with seroma was 73 years vs. 52 years without seroma; p < 0.001). In addition, patients with larger mastectomy specimen were significantly more likely to develop seroma (median ablation weight in cases with seroma 580 g vs. 330 g without seroma; p < 0.001). Other significant parameters for seroma formation were BMI (p = 0.005), grading (p = 0.015) and tumor size (p = 0.036). In addition, with insertion of implant or expander, a seroma occurred significantly less frequently (p < 0.001). In a binary logistic regression, age in particular was confirmed as a significant risk factor. In contrast, tumor biological characteristics, number of lymph nodes removed or affected showed no significant effect on seroma formation. The present study shows the need for patient education about the development of seroma in particular in older patients and patients with large breast volumes within the preoperative surgical clarification. These clinicopathological data support the previously published results hypothesizing that seroma formation is related to autoimmune/inflammatory processes and will be tested on a larger collective in the planned international multicenter SerMa study

    The direct oxygen abundances of metal-rich galaxies derived from electron temperature

    We aim to derive the electron temperature Te in the gas of metal-rich star-forming galaxies, which can be obtained from their ratios of auroral lines [O II]7320,7330 to nebular lines [O II]3727, in order to establish a more robust mass-metallicity relationship, and compare the Te-based (O/H) abundances with those from empirical strong-line calibrations, such as R23. We obtained 27 spectra by stacking the spectra of several hundred (even several thousand) star-forming galaxies selected from the SDSS-DR4 in each of the 27 stellar mass bins from log(M*) ~8.0 to 10.6 (logMsun). This "stack" method sufficiently improves the signal-to-noise ratio of the auroral lines [O II]7320,7330, which allow us to reliably obtain the electron temperature t2 in the low ionization region from the ratio of [O II]7320,7330 to [O II]3727, then t3 in the high ionization region from t2 by using a relation, and then the direct (O/H) abundances from Te. The results show that the empirical R23 method will overestimate the log(O/H) by 0.2 to 0.6 dex for these moderate metal-rich galaxies. The new metal-mass relationship of the galaxies with moderate metallicities is fitted by a linear fit (12+log(O/H) =6.223+0.231*logM*) confirming that empirical methods significantly overestimate (O/H). We also derived their (N/O) abundance ratios on the basis of the Te method, which are consistent with the combination of the primary and secondary components of nitrogen. For actual use, we re-derive the relations of 12+log(O/H)(Bay) vs. logM* and 12+log(O/H)(Bay) vs. logR23 from the SDSS-DR4 data, which are a bit different from those derived from DR2.Comment: 13 pages, 12 figures, 2 tables, accepted by A&

    Th2/Th17 cell associated cytokines found in seroma fluids after breast cancer surgery

    Purpose The development of a seroma after breast cancer surgery is a common postoperative complication seen after simple mastectomy and axillary surgery. We could recently demonstrate that breast cancer patients undergoing a simple mastectomy with subsequent seroma formation developed a T-helper cell increase within the aspirated fluid measured by flow cytometry. The same study revealed a Th2 and/or a Th17 immune response in peripheral blood and seroma fluid of the same patient. Based on these results and within the same study population, we now analyzed the Th2/Th17 cell associated cytokine content as well as the best known clinical important cytokine IL-6. Methods Multiplex cytokine measurements (IL-4, IL-5, IL-13, IL-10, IL-17, and IL-22) were done on 34 seroma fluids (Sf) after fine needle aspiration of patients who developed a seroma after a simple mastectomy. Serum of the same patient (Sp) and that of healthy volunteers (Sc) were used as controls. Results We found the Sf to be highly cytokine rich. Almost all analyzed cytokines were significantly higher in abundance in the Sf compared to Sp and Sc, especially IL-6, which promotes Th17 differentiation as well as suppresses Th1 differentiation in favor of Th2 development. Conclusion Our Sf cytokine measurements reflect a local immune event. In contrast, former study results on T-helper cell populations in both Sf and Sp tend to demonstrate a systemic immune process
