Impact of introducing an electronic physiological surveillance system on hospital mortality
YesBackground Avoidable hospital mortality is often attributable to inadequate patient vital signs monitoring, and failure to recognise or respond to clinical deterioration. The processes involved with vital sign collection and charting; their integration, interpretation and analysis; and the delivery of decision support regarding subsequent clinical care are subject to potential error and/or failure.
Objective To determine whether introducing an electronic physiological surveillance system (EPSS), specifically designed to improve the collection and clinical use of vital signs data, reduced hospital mortality.
Methods A pragmatic, retrospective, observational study of seasonally adjusted in-hospital mortality rates in three main hospital specialties was undertaken before, during and after the sequential deployment and ongoing use of a hospital-wide EPSS in two large unconnected acute general hospitals in England. The EPSS, which uses wireless handheld computing devices, replaced a paper-based vital sign charting and clinical escalation system.
Results During EPSS implementation, crude mortality fell from a baseline of 7.75% (2168/27â
959) to 6.42% (1904/29â
676) in one hospital (estimated 397 fewer deaths), and from 7.57% (1648/21â
771) to 6.15% (1614/26â
241) at the second (estimated 372 fewer deaths). At both hospitals, multiyear statistical process control analyses revealed abrupt and sustained mortality reductions, coincident with the deployment and increasing use of the system. The cumulative total of excess deaths reduced in all specialties with increasing use of the system across the hospital.
Conclusions The use of technology specifically designed to improve the accuracy, reliability and availability of patientsâ vital signs and early warning scores, and thereby the recognition of and response to patient deterioration, is associated with reduced mortality in this study
The Importance of Time Congruity in the Organisation.
In 1991 Kaufman, Lane, and Lindquist proposed that time congruity in terms of an individual's time preferences and the time use methods of an organisation would lead to satisfactory performance and enhancement of quality of work and general life. The research reported here presents a study which uses commensurate person and job measures of time personality in an organisational setting to assess the effects of time congruity on one aspect of work life, job-related affective well-being. Results show that time personality and time congruity were found to have direct effects on well-being and the influence of time congruity was found to be mediated through time personality, thus contributing to the personâjob (PâJ) fit literature which suggests that direct effects are often more important than indirect effects. The study also provides some practical examples of ways to address some of the previously cited methodological issues in PâJ fit research
Onset of magnetism in B2 transition metals aluminides
Ab initio calculation results for the electronic structure of disordered bcc
Fe(x)Al(1-x) (0.4<x<0.75), Co(x)Al(1-x) and Ni(x)Al(1-x) (x=0.4; 0.5; 0.6)
alloys near the 1:1 stoichiometry, as well as of the ordered B2 (FeAl, CoAl,
NiAl) phases with point defects are presented. The calculations were performed
using the coherent potential approximation within the Korringa-Kohn-Rostoker
method (KKR-CPA) for the disordered case and the tight-binding linear
muffin-tin orbital (TB-LMTO) method for the intermetallic compounds. We studied
in particular the onset of magnetism in Fe-Al and Co-Al systems as a function
of the defect structure. We found the appearance of large local magnetic
moments associated with the transition metal (TM) antisite defect in FeAl and
CoAl compounds, in agreement with the experimental findings. Moreover, we found
that any vacancies on both sublattices enhance the magnetic moments via
reducing the charge transfer to a TM atom. Disordered Fe-Al alloys are
ferromagnetically ordered for the whole range of composition studied, whereas
Co-Al becomes magnetic only for Co concentration >0.5.Comment: 11 pages with 9 embedded postscript figures, to be published in

Dynamical properties of liquid Al near melting. An orbital-free molecular dynamics study
The static and dynamic structure of liquid Al is studied using the orbital
free ab-initio molecular dynamics method. Two thermodynamic states along the
coexistence line are considered, namely T = 943 K and 1323 K for which X-ray
and neutron scattering data are available. A new kinetic energy functional,
which fulfills a number of physically relevant conditions is employed, along
with a local first principles pseudopotential. In addition to a comparison with
experiment, we also compare our ab-initio results with those obtained from
conventional molecular dynamics simulations using effective interionic pair
potentials derived from second order pseudopotential perturbation theory.Comment: 15 pages, 12 figures, 2 tables, submitted to PR
A weakly stable algorithm for general Toeplitz systems
We show that a fast algorithm for the QR factorization of a Toeplitz or
Hankel matrix A is weakly stable in the sense that R^T.R is close to A^T.A.
Thus, when the algorithm is used to solve the semi-normal equations R^T.Rx =
A^Tb, we obtain a weakly stable method for the solution of a nonsingular
Toeplitz or Hankel linear system Ax = b. The algorithm also applies to the

Electroexcitation of the at low momentum transfer
We report on new p measurements at the
resonance at the low momentum transfer region. The mesonic
cloud dynamics is predicted to be dominant and rapidly changing in this
kinematic region offering a test bed for chiral effective field theory
calculations. The new data explore the low dependence of the resonant
quadrupole amplitudes while extending the measurements of the Coulomb
quadrupole amplitude to the lowest momentum transfer ever reached. The results
disagree with predictions of constituent quark models and are in reasonable
agreement with dynamical calculations that include pion cloud effects, chiral
effective field theory and lattice calculations. The reported measurements
suggest that improvement is required to the theoretical calculations and
provide valuable input that will allow their refinements
'Education, education, education' : legal, moral and clinical
This article brings together Professor Donald Nicolson's intellectual interest in professional legal ethics and his long-standing involvement with law clinics both as an advisor at the University of Cape Town and Director of the University of Bristol Law Clinic and the University of Strathclyde Law Clinic. In this article he looks at how legal education may help start this process of character development, arguing that the best means is through student involvement in voluntary law clinics. And here he builds upon his recent article which argues for voluntary, community service oriented law clinics over those which emphasise the education of students
Virtual Compton Scattering and Neutral Pion Electroproduction in the Resonance Region up to the Deep Inelastic Region at Backward Angles
We have made the first measurements of the virtual Compton scattering (VCS)
process via the H exclusive reaction in the nucleon resonance
region, at backward angles. Results are presented for the -dependence at
fixed GeV, and for the -dependence at fixed near 1.5 GeV.
The VCS data show resonant structures in the first and second resonance
regions. The observed -dependence is smooth. The measured ratio of
H to H cross sections emphasizes the different
sensitivity of these two reactions to the various nucleon resonances. Finally,
when compared to Real Compton Scattering (RCS) at high energy and large angles,
our VCS data at the highest (1.8-1.9 GeV) show a striking -
independence, which may suggest a transition to a perturbative scattering

Complex relationships among personality traits, job characteristics, and work behaviors
The aim of the study was to investigate the additive, mediating, and moderating effects of personality traits and job characteristics on work behaviors. Job applicants (N = 161) completed personality questionnaires measuring extraversion, neuroticism, achievement motivation, and experience seeking. One and a half years later, supervisors rated the applicants' job performance, and the job incumbents completed questionnaires about skill variety, autonomy, and feedback, work stress, job satisfaction, work self-efficacy, and propensity to leave. LISREL was used to test 15 hypotheses. Perceived feedback mediated the relationship between achievement motivation and job performance. Extraversion predicted work self-efficacy and job satisfaction. Work stress mediated the relationship between neuroticism and job satisfaction. Job satisfaction and experience seeking were related to propensity to leave. Autonomy, skill variety, and feedback were related to job satisfaction
