    A revised edition of the readiness to change questionnaire (treatment version)

    The UK Alcohol Treatment Trial provided an opportunity to examine the factor structure of the Readiness to Change Questionnaire-Treatment Version (RCQ[TV]) in a large sample (N = 742) of individuals in treatment for alcohol problems who were given the RCQ[TV] at baseline, 3-months and 12-months follow-up. Confirmatory factor analysis of the previously reported factor structure (5 items for each of Precontemplation, Contemplation and Action scales) resulted in a relatively poor fit to the data. Removal of one item from each of the scales resulted in a 12-item instrument for which goodness-of-fit indices were improved, without loss of internal consistency of the three scales, on all three measurement occasions. Inspection of relationships between stage allocation by the new instrument and negative alcohol outcome expectancies provided evidence of improved construct validity for the revised edition of the RCQ[TV]. There was also a strong relationship between stage allocation at 3-months follow-up and outcome of treatment at 12 months. The revised edition of the RCQ[TV] offers researchers and clinicians a shorter and improved measurement of stage of change in the alcohol treatment population

    Does cardiac rehabilitation meet minimum standards: an observational study using UK national audit?

    Objective: To assess the extent by which programmes meet national minimum standards for the delivery of cardiac rehabilitation (CR) as part of the National Certification Programme for Cardiovascular Rehabilitation (NCP_CR). Methods: The analysis used UK National Audit of Cardiac Rehabilitation (NACR) data extracted and validated for the period 2013–2014 set against six NCP_CR measures deemed as important for the delivery of high-quality CR programmes. Each programme that achieved a single minimum standard was given a score of 1. The range of the scoring for meeting the minimum standards is between 1 and 6. The performance of CR programmes was categorised into three groups: high (score of 5–6), middle (scores of 3–4) and low (scores of 1–2). If a programme did not meet any of the six criteria, they were considered to have failed. Results: Data from 170 CR programmes revealed statistically significant differences among UK CR programmes. The principal findings were that, based on NCP_CR criteria, 30.6% were assessed as high performance with 45.9% as midlevel performance programmes, 18.2% were in the lower-level and 5.3% failed to meet any of the minimum criteria. Conclusions: This study shows that high levels of performance is achievable in the era of modern cardiology and that many CR programmes are close to meeting high performance standards. However, substantial variation, below the recommended minimum standards, exists throughout the UK. National certification should be seen as a positive step to ensure that patients, irrespective of where they live, are accessing quality services

    Progressive stage transition does mean getting better: a further test of the Transtheoretical Model in recovery from alcohol problems

    Aims To test two central assumptions of the Transtheoretical Model (TTM) regarding recovery from alcohol problems: (i) individuals making a forward transition from pre-action to action stages will show greater drinking improvements than those remaining in pre-action stages; and (ii) individuals remaining in pre-action stages will not demonstrate improvements in drinking outcomes. Design and setting Large, multi-centre, randomized controlled trial of treatment for alcohol problems [United Kingdom Alcohol Treatment Trial (UKATT)]. Measurements Stage of change, drinks per drinking day and percentage days abstinent at baseline, 3- and 12-month follow-ups. Findings In support of TTM assumption 1, improvements in drinking outcomes were consistently greater among clients who showed a forward stage transition (Cohen's d = 0.68) than among those who did not (d = 0.10). Two tests of assumption 2 showed a significant improvement in drinking outcomes in non-transition groups, inconsistent with the TTM; one test showed a significant deterioration and the other showed equivalent drinking outcomes across time. An explanation is offered as to why, under the relevant assumption of the TTM, clients in non-transition groups showed small changes in drinking outcomes. Conclusions In contrast to a previous study by Callaghan and colleagues, our findings largely support the TTM account of recovery from alcohol problems in treatment. The discrepancy can be explained by the use in our study of a more reliable and valid method for assigning stage of change

    Influence of management practice on the microbiota of a critically endangered species: a longitudinal study of kākāpō chick faeces and associated nest litter

    BACKGROUND: The critically endangered kākāpō is a flightless, nocturnal parrot endemic to Aotearoa New Zealand. Recent efforts to describe the gastrointestinal microbial community of this threatened herbivore revealed a low-diversity microbiota that is often dominated by Escherichia-Shigella bacteria. Given the importance of associated microbial communities to animal health, and increasing appreciation of their potential relevance to threatened species conservation, we sought to better understand the development of this unusual gut microbiota profile. To this end, we conducted a longitudinal analysis of faecal material collected from kākāpō chicks during the 2019 breeding season, in addition to associated nest litter material. RESULTS: Using an experimental approach rarely seen in studies of threatened species microbiota, we evaluated the impact of a regular conservation practice on the developing kākāpō microbiota, namely the removal of faecal material from nests. Artificially removing chick faeces from nests had negligible impact on bacterial community diversity for either chicks or nests (p > 0.05). However, the gut microbiota did change significantly over time as chick age increased (p < 0.01), with an increasing relative abundance of Escherichia-Shigella coli over the study period and similar observations for the associated nest litter microbiota (p < 0.01). Supplementary feeding substantially altered gut bacterial diversity of kākāpō chicks (p < 0.01), characterised by a significant increase in Lactobacillus bacteria. CONCLUSIONS: Overall, chick age and hand rearing conditions had the most marked impact on faecal bacterial communities. Similarly, the surrounding nest litter microbiota changed significantly over time since a kākāpō chick was first placed in the nest, though we found no evidence that removal of faecal material influenced the bacterial communities of either litter or faecal samples. Taken together, these observations will inform ongoing conservation and management of this most enigmatic of bird species

    Spatial models of carbon, nitrogen, and sulfur stable isotope distributions (isoscapes) across a shelf sea: an INLA approach

    Spatial models of variation in the isotopic composition of structural nutrients across habitats (isoscapes) offer information on physical, biogeochemical and anthropogenic processes occurring across space, and provide a tool for retrospective assignment of animals or animal products to their foraging area or geographic origin. The isotopic differences among reference samples used to construct isoscapes may vary spatially and according to non‐spatial terms (e.g. sampling date, or among individual or species effects). Partitioning variance between spatially dependent and spatially independent terms is a critical but overlooked aspect of isoscape creation with important consequences for the design of studies collecting reference data for isoscape creation and the accuracy and precision of isoscape models. We introduce the use of integrated nested Laplace approximation (INLA) to construct isoscape models. Integrated nested Laplace approximation provides a computationally efficient framework to construct spatial models of isotopic variability explicitly addressing additional variation introduced by including multiple reference species (or other recognized sources of variance). We present carbon, nitrogen and sulphur isoscape models extending over c. 1 million km2 of the UK shelf seas. Models were built using seven different species of jellyfish as spatial reference data and a suite of environmental correlates. Compared to alternative isoscape prediction methods, INLA‐spatial isotope models show high spatial precision and reduced variance. We briefly discuss the likely biogeochemical explanations for the observed spatial isotope distributions. We show for the first time that sulphur isotopes display systematic spatial variation across open marine shelf seas and may therefore be a useful additional tool for marine spatial ecology. The INLA technique provides a promising tool for generating isoscape models and associated uncertainty surfaces where reference data are accompanied by multiple, quantifiable sources of uncertainty

    Tungsten divertor sources in WEST related to impurity inventory and local plasma conditions

    A dedicated series of L-mode deuterium discharges were performed in WEST in which a broad parameter space in terms of divertor plasma temperature (inner divertor at &lt;10 eV, outer divertor up to 50 eV) and impurity flux density was achieved by variation of X point elevation, upstream plasma density and fuelling position and rate. Density steps provided stable reference plasma conditions whilst density scans up to the point of detachment revealed the sputtering threshold behaviour of the dominant sputtering species. Tungsten gross erosion was quantified via the common WI 400.9 nm line and correlated with the residual gas content.</p
