The βδ fusion (hybrid) chain is synthesized inefficiently and normal β and δ chain production is abolished. The homozygotes show thalassaemia intermediate and heterozygotes show thalassaemia trait. In Hb Lepore, transcription is from the δ globin promoter so the hybrid gene is expressed at the same low level as δ (~2% of the β). Hb Lepore is a βthalassaemia allele, anti-Lepore is essentially normal. Purpose: Presentation of the first case of Lepore hemoglobinopathy from the activity of Hematology and General Pediatrics Department, Institute of Mother and the Child. Material and methods: Paediatric patient, girl, age 11 years presents moderate anemia, pronounced pallor of skin, mucous and refracterity to the previous antianemic therapy. On the basis of the complete blood count, the clinical findings, and the electrophoretic studies it was possible to establish the diagnosis of Lepore hemoglobinopathy. Results: Clinical and paraclinical tests established moderate anemia, hepatomegaly and a minor hemolytic syndrome. Only the specific test - hemoglobin electrophoresis, highlighted the abnormal fraction - Lepore hemoglobin - 10.1%. Conclusions: Anemic syndrome is common in children, but usually it is a real challenge when it comes to establish a certain diagnosis. In the Republic of Moldova, this case is unique in that it creates difficulties confirming the diagnosis

    Molecular basis and therapeutic targets in prostate cancer: A comprehensive review

    Prostate cancer is one of the most significant causes of morbidity and mortality in male patients. The incidence increases with age, and it is higher among African Americans. The occurrence of prostate cancer is associated with many risk factors, including genetic and hereditary predisposition. The most common genetic syndromes associated with prostate cancer risk are BRCA-associated hereditary breast and ovarian cancer (HBOC) and Lynch syndrome. Local-regional therapy, i.e., surgery is beneficial in early-stage prostate cancer management. Advanced and metastatic prostate cancers require systemic therapies, including hormonal inhibition, chemotherapy, and targeted agents. Most prostate cancers can be treated by targeting the androgen-receptor pathway and decreasing androgen production or binding to androgen receptors (AR). Castration-resistant prostate cancer (CRPC) usually involves the PI3K/AKT/mTOR pathway and requires targeted therapy. Specific molecular therapy can target mutated cell lines in which DNA defect repair is altered, caused by mutations of BRCA2, partner and localizer of BRCA2 (PALB2), and phosphatase and tensin homolog (PTEN) or the transmembrane protease serine 2-ERG (TMPRSS2-ERG) fusion. Most benefits were demonstrated in cyclin dependent-kinase 12 (CDK12) mutated cell lines when treated with anti-programmed cell death protein 1 (PD1) therapy. Therapies targeting p53 and AKT are the subject of ongoing clinical trials. Many genetic defects are listed as diagnostic, prognostic, and clinically actionable markers in prostate cancer. Androgen receptor splice variant 7 (AR-V7) is an important oncogenic driver and an early diagnostic and prognostic marker, as well as a therapeutic target in hormone-resistant CRPC.     Extracorporeal Membrane Oxygenation for Severe Acute Respiratory Distress Syndrome associated with COVID-19: An Emulated Target Trial Analysis.

    RATIONALE: Whether COVID patients may benefit from extracorporeal membrane oxygenation (ECMO) compared with conventional invasive mechanical ventilation (IMV) remains unknown. OBJECTIVES: To estimate the effect of ECMO on 90-Day mortality vs IMV only Methods: Among 4,244 critically ill adult patients with COVID-19 included in a multicenter cohort study, we emulated a target trial comparing the treatment strategies of initiating ECMO vs. no ECMO within 7 days of IMV in patients with severe acute respiratory distress syndrome (PaO2/FiO2 <80 or PaCO2 ≥60 mmHg). We controlled for confounding using a multivariable Cox model based on predefined variables. MAIN RESULTS: 1,235 patients met the full eligibility criteria for the emulated trial, among whom 164 patients initiated ECMO. The ECMO strategy had a higher survival probability at Day-7 from the onset of eligibility criteria (87% vs 83%, risk difference: 4%, 95% CI 0;9%) which decreased during follow-up (survival at Day-90: 63% vs 65%, risk difference: -2%, 95% CI -10;5%). However, ECMO was associated with higher survival when performed in high-volume ECMO centers or in regions where a specific ECMO network organization was set up to handle high demand, and when initiated within the first 4 days of MV and in profoundly hypoxemic patients. CONCLUSIONS: In an emulated trial based on a nationwide COVID-19 cohort, we found differential survival over time of an ECMO compared with a no-ECMO strategy. However, ECMO was consistently associated with better outcomes when performed in high-volume centers and in regions with ECMO capacities specifically organized to handle high demand.     Discutindo a educação ambiental no cotidiano escolar: desenvolvimento de projetos na escola formação inicial e continuada de professores

    A presente pesquisa buscou discutir como a Educação Ambiental (EA) vem sendo trabalhada, no Ensino Fundamental e como os docentes desta escola compreendem e vem inserindo a EA no cotidiano escolar., em uma escola estadual do município de Tangará da Serra/MT, Brasil. Para tanto, realizou-se entrevistas com os professores que fazem parte de um projeto interdisciplinar de EA na escola pesquisada. Verificou-se que o projeto da escola não vem conseguindo alcançar os objetivos propostos por: desconhecimento do mesmo, pelos professores; formação deficiente dos professores, não entendimento da EA como processo de ensino-aprendizagem, falta de recursos didáticos, planejamento inadequado das atividades. A partir dessa constatação, procurou-se debater a impossibilidade de tratar do tema fora do trabalho interdisciplinar, bem como, e principalmente, a importância de um estudo mais aprofundado de EA, vinculando teoria e prática, tanto na formação docente, como em projetos escolares, a fim de fugir do tradicional vínculo “EA e ecologia, lixo e horta”.Facultad de Humanidades y Ciencias de la Educació

    AgriMicro&mdash;A Microservices-Based Platform for Optimization of Farm Decisions

    The paper presents AgriMicro, a modern Farm Management Information System (FMIS) designed to help farmers monitor and optimize corn crops from sowing to harvest, by leveraging cloud technologies and machine learning algorithms. The platform is built on a modular architecture composed of multiple components implemented through microservices such as the weather and soil service, recommendation and alert engine, field service, and crop service&mdash;which continuously communicate to centralize field data and provide real-time insights. Through the ongoing exchange of data between these services, different information pieces about soil conditions, crop health, and agricultural operations are processed and analyzed, resulting in predictions of crop evolution and practical recommendations for future interventions (e.g., fertilization or irrigation). This integrated FMIS transforms collected data into concrete actions, supporting farmers and agricultural consultants in making informed decisions, improving field productivity, and ensuring more efficient resource use. Its microservice-based architecture provides scalability, modularity, and straightforward integration with other information systems. The objectives of this study are threefold. First, to specify and design a modular FMIS architecture based on microservices and cloud computing, ensuring scalability, interoperability and adaptability to different farm contexts. Second, to prototype and integrate initial components and Internet of Things (IoT)-based data collection with machine learning models, specifically Random Forest and XGBoost, to provide maize yield forecasting as a proof of concept. Model performance was evaluated using standard predictive accuracy metrics, including the coefficient of determination (R2) and the root mean square error (RMSE), confirming the reliability of the forecasting pipeline and validated against official harvest data (average maize yield) from the Romanian National Institute of Statistics (INS) for 2024. These results confirm the reliability of the forecasting pipeline under controlled conditions; however, in real-world practice, broader regional and inter-annual variability typically results in considerably higher errors, often on the order of 10&ndash;20%. Third, to present a Romania based case study which illustrates the end-to-end workflow and outlines an implementation roadmap toward full deployment. As this is a design-oriented study currently under development, several services remain at the planning or early prototyping stage, and comprehensive system level benchmarks are deferred to future work
