    Comparing treatment of proximal phalangeal fractures with intramedullary screws versus plating.

    PURPOSE Phalangeal fractures are the most common injuries in humans and account for approximately 10% of all fractures. With plate fixation, anatomic reduction is achievable in most cases, but extension lag is seen in up to 67%. Intramedullary headless screw offers treatment of unstable proximal phalangeal fractures using a minimally invasive procedure with very few complications. One of the major disadvantages of this technique is the transarticular screw position, damaging the articular surface and thus preventing very proximal fractures from being treated with a distally inserted screw. In this study, we present a modified approach to the fixation of the proximal phalangeal fractures and compare outcomes with plate osteosynthesis. MATERIALS AND METHODS Twenty-nine patients with 31 comparable fractures of the proximal phalanx were treated either with a plate (14) or with minimal invasive cannulated compression screw (17). Pain, strength, range of motion (ROM), work disability and QuickDASH score were assessed. RESULTS TAM was significantly better in the screw group. The extension lag was worse in the plate group. Plate removal had to be performed in 13 of 14 the cases, while the screw had to be removed in only 3 cases. The average duration of work disability was 9.9 weeks in the plate group, compared to 5.6 weeks in the screw group. CONCLUSION Minimally invasive screw osteosynthesis not only has the advantage of significantly shorter work disabilities, but also shows remarkably improved postoperative range of motion. In contrast to plate osteosynthesis, removal of the screw is only necessary in exceptional cases. With the antegrade screws position, even difficult fractures close to the base can be treated without destroying any articular surface. In proximal phalanx fractures with both options of plate or single-screw osteosynthesis, we recommend minimal invasive cannulated screw osteosynthesis

    Base Station Antenna with Enhanced Cross Polarization Discrimination Performance by Using Horizontal Meandered Dipole and Vertical Parasitic Elements

    This study is related with the design of a ± 45° dual polarized base station antenna with improved cross-polarization discrimination (XPD) values. Parasitic elements are added to antenna design formed by orthogonal two compact meandered dipole above ground plane. The antenna designed with CST Microwave Studio program has VSWR ≀ 2 within 1.71-2.69 GHz frequency band, which covers GSM 1800/3G/LTE bands. The antenna has minimum of 0 dBi gain in the beamwidth of 120° ( 60°) at azimuth plane (ϕ = 0°) along the band, and XPD values being minimum of 2 dB at 1.71-2.4 GHz for  60° without parasitic elements are improved to 10 dB with parasitic elements. This design initially had two horizontal straight monopoles on the ground plane perpendicular to each other. Afterwards, antenna with microstrip balun feed applied but the XPD values were not appropriate to expected results. Because of that, by using image theory, vertical parasitic elements were added to get appropriate XPD values. Later, meandered structure used to make antenna smaller. Finally, according to base station applications, antenna frequencies optimized to 1.71 GHz and 2.69 GHz. The designed and optimized antenna produced and measured in laboratory environment. Return losses for port 1 and port 2 are measured above the 10 dB and isolation between the port 1 and port 2 are measured above the 20 dB. In addition, the maximum gain values are measured between 3 dB and 7 dB in 1.71 GHz and 2.69 GHz frequency band.  Finally, XPD values are measured more than 10 dB in bandwidth

    Morphometric features of the thyroid gland: a cadaveric study of Turkish people

    Background: Although racial and ethnic variations in the morphology of anatomical structures are defined well, the size, shape, and weight of the thyroid gland have not previously been reported in Turkish people. This study provides data about the morphometric features of the thyroid gland, thyroid lobes, and pyramidal lobe, and highlights some anatomical variations in people from the Marmara region in Turkey. Material and methods: The material for the present study consisted of thyroid glands obtained from 75 male and 15 female adult cadavers aged between 18 and 80 years. A dissection was carried out and the thyroid glands were exposed. The glands were weighed and measured according to the various age groups of the patients. Results: A pyramidal lobe was found to be present in 57.8% of the cadavers (52/90). During midline dissection of the neck 2 males out of 90 cadavers, giving an incidence of 2.22%, did not show an isthmus. The mean thyroid weight was 26.11 ± 8.14 g. In males it was 26.93 ± 7.96 g while in females it was 21.93 ± 7.98 g. Conclusions: This is the first reported morphometric study on cadaveric thyroid glands from Turkey and it highlights individual and ethnic/racial variations. In order to perform safe and effective surgery and for the accurate diagnosis of thyroid disorders, knowledge of normal anatomy and the variations of the thyroid gland are essential.     Optimization of conventional and ultrasound assisted extraction of flavonoids from grapefruit (Citrus paradisi L.) solid wastes

    Flavonoid compounds from grapefruit wastes were obtained by conventional solid liquid extraction (CE) and ultrasound assisted extraction (USE). Naringin was by far the most abundant flavonoid in the extracts ranging from 18 to 28 mg/g dw for CE and 24-36 mg/g dw for USE. Response surface methodology allowed obtaining predictive models for total phenolic content (TPC) and total antioxidant activity (TAA) as a function of the process variables ethanol concentration (Etc) (defined as weight of ethanol/weight of solution), temperature (T) and time (t) with reasonable success (CE-TPC, R-2 = 0.86, CE-TAA, R-2 = 0.85; USE-TPC, R-2 = 0.82; USE, TAA, R-2 = 0.86). USE was very effective when compared with conventional solvent extraction, allowing higher extraction yields (on average TPC 50% and TAA 66% higher) with lower temperatures and extraction times. Although the optimum process conditions indicate the use of a low ethanol concentration and ultrasounds (T = 25 degrees C, EtC = 0.4 (g/g) (40 g/100 g) and t = 55 min leading to TPC = 80.0 mg GAE/g dw and TAA = 38.3 mmol trolox/g dw), it has been proved that an USE treatment free of organic solvent (EtC = 0 g/g), at moderate temperature (25 degrees C) and short time (t = 3 min) leads to similar results (TPC = 753 mg GAE/g dw and TAA = 31.9 mmol trolox/g dw), suggesting its use for economic and environmental purposes.The authors acknowledge the Universitat Politecnica de Valencia (Spain) for its financial support through the project 1965 (PAID05-11).Garcia-Castello, EM.; RodrĂ­guez LĂłpez, AD.; Mayor LĂłpez, L.; Ballesteros, R.; Conidi, C.; Cassano, A.     A novel procedure to measure the antioxidant capacity of Yerba maté extracts

    Yerba maté extracts have in vitro antioxidant capacity attributed to the presence of polyphenolic compounds, mainly chlorogenic acids and dicaffeoylquinic acid derivatives. DPPH is one of the most used assays to measure the antioxidant capacity of pure compounds and plant extracts. It is difficult to compare the results between studies because this assay is applied in too many different conditions by the different research groups. Thus, in order to assess the antioxidant capacity of yerba maté extracts, the following procedure is proposed: 100 ”L of an aqueous dilution of the extracts is mixed in duplicate with 3.0 mL of a DPPH 'work solution in absolute methanol (100 ”M.L-1), with an incubation time of 120 minutes in darkness at 37 ± 1 °C, and then absorbance is read at 517 nm against absolute methanol. The results should be expressed as ascorbic acid equivalents or Trolox equivalents in mass percentage (g% dm, dry matter) in order to facilitate comparisons. The AOC of the ethanolic extracts ranged between 12.8 and 23.1 g TE % dm and from 9.1 to 16.4 g AAE % dm. The AOC determined by the DPPH assay proposed in the present study can be related to the total polyphenolic content determined by the Folin-Ciocalteu assay

    Seroprevalence and risk factors for toxoplasma infection among pregnant women in Aydin province, Turkey

    BACKGROUND: The aims of the present study were to determine the prevalence of toxoplasmosis in pregnant women at first trimester of their pregnancy and to follow up the seroconversion for next two trimesters, and to identify the risk factors and possible contamination routes in Aydin province, Turkey. METHOD: The sample size was calculated as 423 on a prevalence of 50%, d=0.05 at a confidence level of 95% with 10% addition. It was a cross-sectional study with multistage sampling. After a questionnaire applied to the pregnant women, anti-Toxoplasma IgG antibodies were studied with ELISA and IFA, values in conflict with DA test, where IgM antibodies were studied with ELISA and for borderline or positive values of IgM avidity test was used. RESULTS: The mean age of 389 (92.9%) of pregnant women in the study was 24.28+/-4.56 years, the seroprevalence of anti-Toxoplasma IgG antibodies for toxoplasmosis was 30.1%. Seroprevalence was increased with age (p=0.001) and with drinking water consumption other than bottled water (p=0.042). No significant relations were observed between anti-Toxoplasma IgG antibodies and education level, being native or migrant, abortion history, consumption of meat, vegetable and milk/milk products, personal or kitchen hygiene habits, cat owning at home of the pregnant women. No IgM antibody was detected. CONCLUSION: One of every three pregnant women in Aydin was at risk of toxoplasmosis at the first trimester of their pregnancy. Increased seroprevalance with age was a predictable result because of increasing time of exposure. Increased seroprevalence with consumption of municipal and uncontrolled water (well/spring water) supplies was similar with latest epidemiological findings

    Genome sequencing in families with congenital limb malformations

    The extensive clinical and genetic heterogeneity of congenital limb malformation calls for comprehensive genome-wide analysis of genetic variation. Genome sequencing (GS) has the potential to identify all genetic variants. Here we aim to determine the diagnostic potential of GS as a comprehensive one-test-for-all strategy in a cohort of undiagnosed patients with congenital limb malformations. We collected 69 cases (64 trios, 1 duo, 5 singletons) with congenital limb malformations with no molecular diagnosis after standard clinical genetic testing and performed genome sequencing. We also developed a framework to identify potential noncoding pathogenic variants. We identified likely pathogenic/disease-associated variants in 12 cases (17.4%) including four in known disease genes, and one repeat expansion in HOXD13. In three unrelated cases with ectrodactyly, we identified likely pathogenic variants in UBA2, establishing it as a novel disease gene. In addition, we found two complex structural variants (3%). We also identified likely causative variants in three novel high confidence candidate genes. We were not able to identify any noncoding variants. GS is a powerful strategy to identify all types of genomic variants associated with congenital limb malformation, including repeat expansions and complex structural variants missed by standard diagnostic approaches.     Neutrophil to Lymphocyte Ratio and Outcomes in Patients with New-Onset or Worsening Heart Failure with Reduced and Preserved Ejection Fraction

    Inflammation is thought to play a role in heart failure (HF) pathophysiology. Neutrophil-to-lymphocyte ratio (NLR) is a simple, routinely available measure of inflammation. Its relationship with other inflammatory biomarkers and its association with clinical outcomes in addition to other risk markers have not been comprehensively evaluated in HF patients. Methods We evaluated patients with worsening or new-onset HF from the BIOlogy Study to Tailored Treatment in Chronic Heart Failure (BIOSTAT-CHF) study who had available NLR at baseline. The primary outcome was time to all-cause mortality or HF hospitalization. Outcomes were validated in a separate HF population. Results 1622 patients were evaluated (including 523 ventricular ejection fraction [LVEF] < 40% and 662 LVEF ≄ 40%). NLR was significantly correlated with biomarkers related to inflammation as well as NT-proBNP. NLR was significantly associated with the primary outcome in patients irrespective of LVEF (hazard ratio [HR] 1.18 per standard deviation increase; 95% confidence interval [CI] 1.11–1.26, P < 0.001). Patients with NLR in the highest tertile had significantly worse outcome than those in the lowest independent of LVEF (<40%: HR 2.75; 95% CI 1.84–4.09, P < 0.001; LVEF ≄ 40%: HR 1.51; 95% CI 1.05–2.16, P = 0.026). When NLR was added to the BIOSTAT-CHF risk score, there were improvements in integrated discrimination index (IDI) and net reclassification index (NRI) for occurrence of the primary outcome (IDI + 0.009; 95% CI 0.00–0.019, P = 0.030; continuous NRI + 0.112, 95% CI 0.012–0.176, P = 0.040). Elevated NLR was similarly associated with adverse outcome in the validation cohort. Decrease in NLR at 6 months was associated with reduced incidence of the primary outcome (HR 0.75; 95% CI 0.57–0.98, P = 0.036). Conclusions Elevated NLR is significantly associated with elevated markers of inflammation in HF patients and is associated with worse outcome. Elevated NLR might potentially be useful in identifying high-risk HF patients and may represent a treatment target
