    An isolated proximal tibiofibular joint dislocation in a young male playing soccer: a case report

    Isolated dislocation of the proximal tibiofibular joint is a rare injury. We present a 23-year-old caucasian man who sustained a traumatic anterolateral dislocation of the proximal tibiofibular joint. There is no consenus on definitive management, and we review the different published treatment and rehabilitation regimens for this injury. Our patient was successfully treated by open reduction and temporary Kirschner-wire fixation. The authors recommend their structured rehabilitation process involved using cast brace immobilization as allows for excellent soft tissue healing

    Modelling Automation–Human Driver Handovers Using Operator Event Sequence Diagrams

    This research aims to show the effectiveness of Operator Event Sequence Diagrams (OESDs) in the normative modelling of vehicle automation to human drivers’ handovers and validate the models with observations from a study in a driving simulator. The handover of control from automation to human operators has proved problematic, and in the most extreme circumstances catastrophic. This is currently a topic of much concern in the design of automated vehicles. OESDs were used to inform the design of the interaction, which was then tested in a driving simulator. This test provided, for the first time, the opportunity to validate OESDs with data gathered from videoing the handover processes. The findings show that the normative predictions of driver activity determined during the handover from vehicle automation in a driving simulator performed well, and similar to other Human Factors methods. It is concluded that OESDs provided a useful method for the human-centred automation design and, as the predictive validity shows, can continue to be used with some confidence. The research in this paper has shown that OESDs can be used to anticipate normative behaviour of drivers engaged in handover activities with vehicle automation in a driving simulator. Therefore, OESDs offer a useful modelling tool for the Human Factors profession and could be applied to a wide range of applications and domains.</jats:p

    Effect of sulfur content in wet or dry distillers grains fed at several inclusions on cattle growth performance, ruminal parameters, and hydrogen sulfide

    Effects of S from wet or dry distillers grains with solubles (DGS) containing 0.82 or 1.16% S on animal growth performance, carcass characteristics, apparent total tract digestibility, and ruminal parameters were evaluated. In Exp. 1, crossbred beef steers (n = 120; 345 ± 34 kg BW) were individually fed ad libitum using Calan gates. Treatments were applied as a 2 × 2 × 3 + 1 factorial treatment arrangement with factors of DGS type (wet or dry), S content in DGS (0.82 or 1.16% DM basis), and DGS inclusion (20, 30, and 40%, DM basis), as well as a corn control diet (no DGS). In Exp. 2, ruminally cannulated crossbred beef steers (n = 6; 381 ± 31 kg BW) were assigned to 1 of 5 diets in a 5 × 6 unbalanced Latin Square design and fed ad libitum through five 14-d periods. A 2 × 2 + 1 factorial treatment arrangement was used with the factors of DGS type and S content in DGS (similar to Exp. 1). Inclusion of DGS was 40%, except for a MATCH diet containing wet 1.16% S DGS included at 31.4% (DM basis). Intake of DM decreased linearly (P \u3c 0.01) and quadratically (P \u3c 0.01) for steers fed wet and dry DGS that was 1.16% S, respectively. In addition, steers fed dry DGS consumed 9% more DM (P \u3c 0.01) than those fed wet. Gain decreased linearly (P = 0.02) when wet 1.16% S DGS increased in the diet, representing a 12% drop in ADG between the Control and 40% DGS inclusion. A quadratic (P = 0.02) improvement in G:F was observed for steers fed wet DGS compared with dry, regardless of S content (P = 0.52). Feeding diets with wet 1.16% S DGS linearly decreased (P = 0.03) HCW. In Exp. 2, molar proportion of propionate declined (P = 0.01) 9% and A:P ratio tended (P = 0.13) to be greater when 1.16 compared with 0.82% S DGS was fed. Apparent total tract DMD was not affected (P \u3e 0.16) and only subtle changes (P \u3c 0.01) in ruminal pH parameters were observed. Greater (P = 0.02) ruminal H2S concentration for steers fed wet compared with dry DGS was observed, while 1.16% S DGS tended (P = 0.12) to produce greater ruminal H2S than 0.82% S. Sulfur in wet DGS appears to be more prone to be converted to ruminal H2S, because feeding 1.16% S as wet DGS had a greater impact on ADG, DMI, and ruminal H2S compared with dry DGS

    Clear cell chondrosarcoma of the head and neck

    Clear cell chondrosarcoma is a rare variant of chondrosarcoma that mostly involves the end of long bones. However, nine cases have been reported in the head and neck: four in larynx, two in nasal septum, two in maxilla and one in the skull. These cases form the basis of this review. Head and neck cases accounts for less than 5% of Clear cell chondrosarcomas in the whole body and the larynx is the most common place. The histological findings of head and neck cases are consistent with general features of this entity in the whole body and nearly all tumors in this case series had a component of conventional chondrosarcoma. Clear cell chondrosarcoma is an intracompartmental tumor and retains "Grenz zone" just beneath the epithelium. Therefore, the overlying mucosa remained intact in all laryngeal cases. Nasal tumor caused ballooning of the septum and the maxillary lesion did not involve the oral mucosa. This tumor presents various radiographic features in the head and neck area. Chondroblastoma, chondroma, osteoblastoma, osteosarcoma and metastatic renal cell carcinoma are included in the histologic differential diagnoses. Differentiation from chondroblastic osteosarcoma is important in the maxilla. A wide resection is adequate in most cases. However, some laryngeal cases show tendency to recur. Clear cell chondrosarcoma is a slow growing tumor and this necessitates a long time follow-up of patients. Due to the extreme rarity in the head and neck, diagnosis of Clear cell chondrosarcoma in this area, must be confirmed by histochemical and immunohistochemical studies

    Safety and Efficacy of the Addition of Lapatinib to Perioperative Chemotherapy for Resectable HER2-Positive Gastroesophageal Adenocarcinoma: A Randomized Phase 2 Clinical Trial

    Importance: Perioperative chemotherapy and surgery are a standard of care for operable gastroesophageal adenocarcinoma. Anti-HER2 therapy improves survival in patients with advanced HER2-positive disease. The safety and feasibility of adding lapatinib to perioperative chemotherapy should be assessed. Objectives: To assess the safety of adding lapatinib to epirubicin, cisplatin, and capecitabine (ECX) chemotherapy and to establish a recommended dose regimen for a phase 3 trial. Design, Setting, and Participants: Phase 2 randomized, open-label trial comparing standard ECX (sECX: 3 preoperative and 3 postoperative cycles of ECX with modified ECX plus lapatinib (mECX+L). This multicenter national trial was conducted in 29 centers in the United Kingdom in patients with histologically proven, HER2-positive, operable gastroesophageal adenocarcinoma. Registration for ERBB/HER2 testing took place from February 25, 2013, to April 19, 2016, and randomization took place between May 24, 2013, and April 21, 2016. Data were analyzed May 10, 2017, to May 25, 2017. Interventions: Patients were randomized 1:1 open-label to sECX (3 preoperative and 3 postoperative cycles of 50 mg/m2 of intravenous epirubicin on day 1, 60 mg/m2 intravenous cisplatin on day 1, 1250 mg/m2 of oral capecitabine on days 1 through 21) or mECX+L (ECX plus lapatinib days 1 through 21 in each cycle and as 6 maintenance doses). The first 10 patients in the mECX+L arm were treated with 1000 mg/m2 of capecitabine and 1250 mg of lapatinib per day, after which preoperative toxic effects were reviewed according to predefined criteria to determine doses for subsequent patients. Main Outcomes and Measures: Proportion of patients experiencing grade 3 or 4 diarrhea with mECX+L. A rate of 20% or less was considered acceptable. No formal comparison between arms was planned. Results: Between February 2013, and April 2016, 441 patients underwent central HER2 testing and 63 (14%) were classified as HER2 positive. Forty-six patients were randomized; 44 (24 sECX, 20 mECX+L) are included in this analysis. Two of the first 10 patients in the mECX+L arm reported preoperative grade 3 diarrhea; thus, no dose increase was made. The primary endpoint of preoperative grade 3 or 4 diarrhea rates were 0 of 24 in the sECX arm (0%) and 4 of 20 in the mECX+L arm (21%). One of 24 in the sECX arm and 3 of 20 in the mECX+L arm stopped preoperative treatment early, and for 4 of 19 in the mECX+L arm, lapatinib dose was reduced. Postoperative complication rates were similar in each arm. Conclusions and Relevance: Administration of 1250 mg of lapatinib per day in combination with ECX chemotherapy was feasible with some increase in toxic effects, which did not compromise operative management.     Outcome related to impact on daily living: preliminary validation of the ORIDL instrument

    <p>Abstract</p> <p>Background</p> <p>The challenge of finding practical, patient-rated outcome measures is a key issue in the evaluation of health care systems and interventions. The ORIDL (Outcome in Relation to Impact on Daily Living) instrument (formerly referred to as the Glasgow Homoeopathic Hospital Outcomes Scale or GHHOS) has been developed to measure patient's views of the outcome of their care by asking about change, and relating this to impact on daily life. The aim of the present paper is to describe the background and potential uses of the ORIDL, and to report on its preliminary validation in a series of three studies in secondary and primary care.</p> <p>Methods</p> <p>In the first study, 105 patients attending the Glasgow Homoeopathic Hospital (GHH) were followed-up at 12 months and changes in health status were measured by the EuroQol (EQOL) and the ORIDL. In the second study, 187 new patients at the GHH were followed-up at 3, 12, and 33 months, using the ORIDL, the Short Form 12 (SF-12), and the Measure Yourself Medical Outcome Profile (MYMOP). In study three, 323 patients in primary care were followed for 1 month post-consultation using the ORIDL and MYMOP. In all 3 studies the Patient Enablement Instrument (PEI) was also used as an outcome measure.</p> <p>Results</p> <p>Study 1 showed substantial improvements in main complaint and well-being over 12 months using the ORIDL, with two-thirds of patients reporting improvements in daily living. These improvements were not significantly correlated with changes in serial measures of the EQOL between baseline and 12 months, but were correlated with the EQOL transitions measure. Study 2 showed step-wise improvements in ORIDL scores between 3 and 33 months, which were only weakly associated with similar changes in SF-12 scores. However, MYMOP change scores correlated well with ORIDL scores at all time points. Study 3 showed similar high correlations between ORIDL scores and MYMOP scores. In all 3 studies, ORIDL scores were also significantly correlated with PEI-outcome scores.</p> <p>Conclusion</p> <p>There is significant agreement between patient outcomes assessed by the ORIDL and the EQOL transition scale, the MYMOP, and the PEI-outcome instrument, suggesting that the ORIDL may be a valid and sensitive tool for measuring change in relation to impact on life.</p

    The study protocol of a cluster-randomised controlled trial of family-mediated personalised activities for nursing home residents with dementia

    <p>Abstract</p> <p>Background</p> <p>Following admission to a nursing home, the feelings of depression and burden that family carers may experience do not necessarily diminish. Additionally, they may experience feelings of guilt and grief for the loss of a previously close relationship. At the same time, individuals with dementia may develop symptoms of depression and agitation (BPSD) that may be related to changes in family relationships, social interaction and stimulation. Until now, interventions to alleviate carer stress and BPSD have treated carers and relatives separately rather than focusing on maintaining or enhancing their relationships. One-to-one structured activities have been shown to reduce BPSD and also improve the caring experience, but barriers such as a lack of resources impede the implementation of activities in aged care facilities. The current study will investigate the effect of individualised activities based on the Montessori methodology administered by family carers in residential care.</p> <p>Methods/Design</p> <p>We will conduct a cluster-randomised trial to train family carers in conducting personalised one-to-one activities based on the Montessori methodology with their relatives. Montessori activities derive from the principles espoused by Maria Montessori and subsequent educational theorists to promote engagement in learning, namely task breakdown, guided repetition, progression in difficulty from simple to complex, and the careful matching of demands to levels of competence. Persons with dementia living in aged care facilities and frequently visiting family carers will be included in the study. Consented, willing participants will be randomly assigned by facility to a treatment condition using the Montessori approach or a control waiting list condition. We hypothesise that family carers conducting Montessori-based activities will experience improvements in quality of visits and overall relationship with the resident as well as higher self-rated mastery, fewer depressive symptoms, and a better quality of life than carers in the waiting list condition.</p> <p>Discussion</p> <p>We hypothesise that training family carers to deliver personalised activities to their relatives in a residential setting will make visits more satisfying and may consequently improve the quality of life for carers and their relatives.     The Apache Point Observatory Galactic Evolution Experiment (APOGEE)

    The Apache Point Observatory Galactic Evolution Experiment (APOGEE), one of the programs in the Sloan Digital Sky Survey III (SDSS-III), has now completed its systematic, homogeneous spectroscopic survey sampling all major populations of the Milky Way. After a three-year observing campaign on the Sloan 2.5 m Telescope, APOGEE has collected a half million high-resolution (R ~ 22,500), high signal-to-noise ratio (>100), infrared (1.51–1.70 ÎŒm) spectra for 146,000 stars, with time series information via repeat visits to most of these stars. This paper describes the motivations for the survey and its overall design—hardware, field placement, target selection, operations—and gives an overview of these aspects as well as the data reduction, analysis, and products. An index is also given to the complement of technical papers that describe various critical survey components in detail. Finally, we discuss the achieved survey performance and illustrate the variety of potential uses of the data products by way of a number of science demonstrations, which span from time series analysis of stellar spectral variations and radial velocity variations from stellar companions, to spatial maps of kinematics, metallicity, and abundance patterns across the Galaxy and as a function of age, to new views of the interstellar medium, the chemistry of star clusters, and the discovery of rare stellar species.
