    Electrochromic Windows: Return on Investment Analysis

    In recent years, sustainability in construction has become a topic of undeniable interest. With sustainability certifications becoming more prominent and desirable, members of the construction industry have begun to seek alternatives for standard systems to keep up with market trends. One of the main issues that the construction industry faces is accessibility of technology. While there are new products and technologies being released frequently, members of the construction industry lack knowledge on the specific details and implications of working with newer systems. One piece of technology that was recently introduced is electrochromic glass, or smart glass. Electrochromic windows analyze sunlight and heat exposure in their surrounding environment and change their opacity accordingly to better utilize the natural environment to increase indoor environmental quality. This report will explore the advantages, accessibility, and feasibility of utilizing electrochromic windows, ultimately concluding in a Return on Investment (ROI) analysis. There currently is limited data on the usage and/or effectiveness of this technology as it is relatively new, having only appeared in the industry 12 years ago. Through material data analysis and conducting an interview with a smart glass supplier, the current effectiveness and implications of electrochromic technology have been analyzed in this report

    Sertraline and Phenytoin Drug Interaction in a Geriatric Patient

    This report presents the case of a 78-year-old man residing in a nursing home who presented with a 2-month history of increasing lethargy and confusion. These symptoms coincided with the initiation of sertraline in the patient. Among other medications, he was also taking phenytoin. The medical team concluded that the cause of the patient’s lethargy and confusion was a drug interaction between sertraline and phenytoin. Phenytoin was held, while the sertraline was slowly tapered to discontinuation. The patient’s symptoms resolved soon thereafter. Future research is needed to better guide clinicians in appropriate selection, dosing, and monitoring of selective serotonin reuptake inhibitors with concomitant phenytoin use.Key words: phenytoin, sertraline, SSRIs, drug interactio

This new heterogeneous catalyst has been shown to be effective in the asymmetric epoxidation of olefins by oxone.     Medical Staff Organization in Nursing Homes: Scale Development and Validation

    PURPOSE: To construct a multidimensional self-report scale to measure nursing home (NH) medical staff organization (NHMSO) dimensions and then pilot the scale using a national survey of medical directors to provide data on its psychometric properties. DESIGN AND METHODS: Instrument development process consisting of the proceedings from the Nursing Home Physician Workforce Conference and focus groups followed by cognitive interviews, which culminated in a survey of a random sample of American Medical Directors Association (AMDA) affiliated medical directors. Analyses were conducted on surveys matched to Online Survey Certification and Reporting (OSCAR) data from freestanding nonpediatric nursing homes. A total of 202 surveys were available for analysis and comprised the final sample. RESULTS: Dimensions were identified that measured the extent of medical staff organization in nursing homes and included staff composition, appointment process, commitment (physiciancohesion; leadership turnover/capability), departmentalization (physician supervision, autonomy and interdisciplinary involvement), documentation, and informal dynamics. The items developed to measure each dimension were reliable (Cronbach's alpha ranged from 0.81 to 0.65).Intercorrelations among the scale dimensions provided preliminary evidence of the construct validity of the scale. IMPLICATIONS: This report, for the first time ever, defines and validates NH medical staff organization dimensions, a critical first step in determining the relationship between physician practice and the quality of care delivered in the NH

    Feasibility study on earthquake early warning and operational earthquake forecasting for risk mitigation at nuclear power plants

    International audienceWithin the framework of the EC-funded project REAKT (Strategies and Tools for Real Time Earthquake Risk Reduction, FP7, contract no. 282862, 2011-2014, www.reaktproject.eu), a task concerns feasibility study and initial implementation of Earthquake Early Warning (EEW) and timedependent seismic hazard analyses aimed at mitigating seismic risk at nuclear power plants (NPPs) in Switzerland. This study is jointly carried out by academic institutions (the Swiss Seismological Service at ETHZ and BRGM) and in cooperation with swissnuclear, the nuclear energy section of swisselectric, an umbrella organisation for the nuclear power plants in Switzerland, which provide about 40% of the electricity needs of the country. Briefly presented in this contribution are the main investigations carried out and results obtained throughout the development of this task, with special focus on: a) evaluating the performances of the selected EEW algorithm (the Virtual Seismologist, VS) in Switzerland and California, in terms of correct detections, false alerts, and missed events; b) embedding the VS algorithm into the earthquake monitoring software SeisComP3 (www.seiscomp3.org) routinely used by the Swiss Seismological Service for earthquake detections and locations; c) customising the User Display (a graphical interface originally developed at the California Institute of Technology (Caltech) during Phase II of the ShakeAlert project in California) for optimised use at Swiss NPPs; d) presenting synthetic time-dependent hazard scenarios for Switzerland and e) attempting to associate the above input data with potential mitigation actions and related cost and benefits for NPPs in Switzerland

    Review of methods for measuring β-cell function: Design considerations from the Restoring Insulin Secretion (RISE) Consortium

    The Restoring Insulin Secretion (RISE) study was initiated to evaluate interventions to slow or reverse the progression of β-cell failure in type 2 diabetes (T2D). To design the RISE study, we undertook an evaluation of methods for measurement of β-cell function and changes in β-cell function in response to interventions. In the present paper, we review approaches for measurement of β-cell function, focusing on methodologic and feasibility considerations. Methodologic considerations included: (1) the utility of each technique for evaluating key aspects of β-cell function (first- and second-phase insulin secretion, maximum insulin secretion, glucose sensitivity, incretin effects) and (2) tactics for incorporating a measurement of insulin sensitivity in order to adjust insulin secretion measures for insulin sensitivity appropriately. Of particular concern were the capacity to measure β-cell function accurately in those with poor function, as is seen in established T2D, and the capacity of each method for demonstrating treatment-induced changes in β-cell function. Feasibility considerations included: staff burden, including time and required methodological expertise; participant burden, including time and number of study visits; and ease of standardizing methods across a multicentre consortium.     Corporate governance compliance and disclosure in the banking sector: using data from Japan

    Using regression model this study investigates which characteristics of a bank is associated with the extent of corporate governance disclosure in Japan. The findings suggest that on average 8 banks out of a sample of 46 disclose optimal corporate governance information. The regression model results reveal in general that non-executive directors, cross-ownership, capital adequacy ratio and type of auditors are associated with the extent of corporate governance disclosure. Of these four variables, non-executive directors have a more significant impact on the extent of disclosure contrary to total assets and audit firms of banks in the context of Japan. The findings of this paper are relevant for corporate regulators, professional associations and developers of corporate governance code when designing or updating corporate governance code

    The Threat of Capital Drain: A Rationale for Public Banks?

    This paper yields a rationale for why subsidized public banks may be desirable from a regional perspective in a financially integrated economy. We present a model with credit rationing and heterogeneous regions in which public banks prevent a capital drain from poorer to richer regions by subsidizing local depositors, for example, through a public guarantee. Under some conditions, cooperative banks can perform the same function without any subsidization; however, they may be crowded out by public banks. We also discuss the impact of the political structure on the emergence of public banks in a political-economy setting and the role of interregional mobility

    Adiposity and hepatic lipid in healthy full-term, breastfed, and formula-fed human infants: a prospective short-term longitudinal cohort study

    Background: The effect of mode of infant feeding on adiposity deposition is not fully understood. Objective: The objective was to test the hypothesis that differences in total and regional adipose tissue content and intrahepatocellular lipid (IHCL) arise in early infancy between breast- and formula-fed infants and to describe longitudinal changes. Design: This prospective longitudinal cohort study was performed in 2 hospitals in the United Kingdom. Healthy, full-term, appropriate weight-for-gestational age infants were recruited; adipose tissue volume and distribution were directly quantified by using whole-body magnetic resonance imaging; IHCL was assessed by in vivo proton magnetic resonance spectroscopy. Measurements were performed after birth (median age: 13 d) and at 6–12 wk of age. Method of infant feeding was recorded prospectively by using maternally completed feeding diaries. Breastfed was defined as >80% of feeds consisting of breast milk at both points; formula-fed was defined as >80% of feeds consisting of formula milk at both points. Results: Longitudinal results were obtained from 70 infants (36 breastfed, 9 mixed-fed, and 25 formula-fed). No differences were found in total or regional adipose tissue or IHCL between breastfed and formula-fed infants. In pooled analyses including all feeding groups, IHCL and total adipose tissue approximately doubled between birth and 6–12 wk: IHCL after birth (median: 0.949; IQR: 0.521–1.711) and at 6–12 wk (1.828; 1.376–2.697; P < 0.001) and total adipose tissue after birth (0.749 L; 0.620–0.928 L) and at 6–12 wk (1.547 L; 1.332–1.790 L; P < 0.001). Increasing adiposity was characterized by greater relative increases in subcutaneous than in internal adipose tissue depots. Conclusions: No differences were detectable in adipose tissue or IHCL accretion between breastfed and formula-fed infants up to 2 mo. The substantial increase in IHCL seen over this period in both breastfed and formula-fed infants is a novel observation, which suggests that hepatic storage of lipids may be physiologic up to 2 mo.     Cardiometabolic risk factor clustering in patients with deficient branched‐chain amino acid catabolism: A case‐control study

    AbstractClassical organic acidemias (OAs) result from defective mitochondrial catabolism of branched‐chain amino acids (BCAAs). Abnormal mitochondrial function relates to oxidative stress, ectopic lipids and insulin resistance (IR). We investigated whether genetically impaired function of mitochondrial BCAA catabolism associates with cardiometabolic risk factors, altered liver and muscle energy metabolism, and IR. In this case‐control study, 31 children and young adults with propionic acidemia (PA), methylmalonic acidemia (MMA) or isovaleric acidemia (IVA) were compared with 30 healthy young humans using comprehensive metabolic phenotyping including in vivo 31P/1H magnetic resonance spectroscopy of liver and skeletal muscle. Among all OAs, patients with PA exhibited abdominal adiposity, IR, fasting hyperglycaemia and hypertriglyceridemia as well as increased liver fat accumulation, despite dietary energy intake within recommendations for age and sex. In contrast, patients with MMA more frequently featured higher energy intake than recommended and had a different phenotype including hepatomegaly and mildly lower skeletal muscle ATP content. In skeletal muscle of patients with PA, slightly lower inorganic phosphate levels were found. However, hepatic ATP and inorganic phosphate concentrations were not different between all OA patients and controls. In patients with IVA, no abnormalities were detected. Impaired BCAA catabolism in PA, but not in MMA or IVA, was associated with a previously unrecognised, metabolic syndrome‐like phenotype with abdominal adiposity potentially resulting from ectopic lipid storage. These findings suggest the need for early cardiometabolic risk factor screening in PA
