    Race and Inflammatory Bowel Disease in an Urban Healthcare System

    Inflammatory bowel disease (IBD) is increasingly common among non-Caucasian populations, but interracial differences in disease characteristics and management are not well-characterized. We tested the hypothesis that disease characteristics and management vary by race among IBD patients in an ethnically diverse healthcare system. A retrospective study of the safety net healthcare system of San Francisco, CA, from 1996 to 2009 was undertaken. Patient records with International Classification of Diseases, 9th Revision (ICD9) codes 555.xx, 556.xx, and 558.xx were reviewed. Adult patients with confirmed IBD diagnoses were included. Interracial variations in disease characteristics and management were assessed broadly; focused between-race comparisons identified specific differences. The 228 subjects included 77 (33.4%) with Crohn’s disease (CD), 150 (65.8%) with ulcerative colitis, and 1 (0.4%) with IBD, type unclassified. The race distribution included 105 (46.1%) white, 34 (14.9%) black, 35 (15.4%) Hispanic, and 51 (22.4%) Asian subjects. Asians and Hispanics were diagnosed at older ages (41.0 and 37.1 years, respectively) and had shorter disease durations (5.4 and 5.2 years, respectively) than whites (30.5 years at diagnosis and 8.6 years duration, P < 0.05) and blacks (31.7 years at diagnosis and 12.1 years duration, P < 0.05). CD was more common among blacks (50% of subjects) than Asians (25.5% of subjects, P = 0.015). The Montreal classification of IBD was similar among races. Hispanics were less likely than others to be treated with 5-aminosalicylates (5-ASA), immunomodulators, and steroids. Medical and surgical management was otherwise similar among races. Modest race-based differences in IBD characteristics exist in this racially diverse healthcare system, but the management of IBD is similar among race groups

    A large genome-wide association study of age-related macular degeneration highlights contributions of rare and common variants.

The final version is available from Nature Publishing Group via http://dx.doi.org/10.1038/ng.3448Advanced age-related macular degeneration (AMD) is the leading cause of blindness in the elderly, with limited therapeutic options. Here we report on a study of >12 million variants, including 163,714 directly genotyped, mostly rare, protein-altering variants. Analyzing 16,144 patients and 17,832 controls, we identify 52 independently associated common and rare variants (P < 5 × 10(-8)) distributed across 34 loci. Although wet and dry AMD subtypes exhibit predominantly shared genetics, we identify the first genetic association signal specific to wet AMD, near MMP9 (difference P value = 4.1 × 10(-10)). Very rare coding variants (frequency <0.1%) in CFH, CFI and TIMP3 suggest causal roles for these genes, as does a splice variant in SLC16A8. Our results support the hypothesis that rare coding variants can pinpoint causal genes within known genetic loci and illustrate that applying the approach systematically to detect new loci requires extremely large sample sizes.We thank all participants of all the studies included for enabling this research by their participation in these studies. The incidence of CDI has also increased in surgical patients. Optimization of management of C difficile, has therefore become increasingly urgent.     Modelling the effect of temperature and CO 2 on microbial spoilage of chilled gilthead seabream fillets

    The effect of temperature (0-15°C) and CO 2 (0-80%) on the growth of spoilage bacteria (total viable count, Pseudomonas sp. and lactic acid bacteria) in marine cultured gilthead seabream fillets was studied and kinetically modelled using an Arrhenius type equation. Pseudomonas sp. dominated the spoilage microflora of aerobically packed fillets whereas LAB defined spoilage at the MAP conditions. Modified atmosphere packaging led to a significant shelf-life extension of fillets, in terms of microbial growth and organoleptic deterioration. The preservative effect of CO 2 was greater as the CO 2 concentration in the atmosphere increased. Under this context, MAP can practically extend the shelf life of chilled gilthead seabream fillets. The growth of LAB was modelled as a function of temperature and CO 2 concentration in modified atmosphere packaging. The applicability of the models was validated at fluctuating temperature conditions. The nominal maximum CO 2 concentration (CO 2,max) for growth and the specific growth rate (k ref) at T ref (4°C) in the absence of carbon dioxide were determined to be 98.3% and 0.015 h -1, respectively. The activation energy parameter of the model, which indicates the temperature dependence of the growth rates, was 101.0 kJ/mol showing a strong dependence of LAB growth on storage temperature. Based on organoleptic acceptability the end of shelf life coincided with 10 7/g Pseudomonas sp. and 10 6/g lactobacilli for aerobically and MAP stored samples respectively. Based on the developed models this translates to 7, 8, 12 and 32 days of shelf life at 5°C and 0, 20, 50 and 80% CO 2 respectively

    Application and validation of the TTI based chill chain management system SMAS (Safety Monitoring and Assurance System) on shelf life optimization of vacuum packed chilled tuna

    The objective of the study was to establish a validated kinetic model for growth of spoilage bacteria on vacuum packed tuna slices in the temperature range of 0 to 15 degrees C and to evaluate the applicability of the TTI (Time Temperature Integrators) based SMAS (Safety Monitoring and Assurance System) system to improve tuna product quality at the time of consumption in comparison to the conventional First In First Out (FIFO) approach. The overall measurements of total flora and lactic acid bacteria (LAB) on the tuna samples used in a laboratory simulated field test were in close agreement with the predictions of the developed kinetic model. The spoilage profile of the TTI bearing products, handled with SMAS, was improved. Three out of the thirty products that were handled randomly, according to the FIFO approach, were already spoiled at the time of consumption (logN(LAB)>6.5) compared to no spoiled products when handled with the SMAS approach
