Discourse: Assessment and Therapy
Discourse is essential for interaction and for the expression of ideas, feelings and opinions. Telling personal stories, such as talking about your day or recounting what happened in the playground, is essential for communication and establishing relationships. However, due to their language impairments, people with aphasia (PWA) and children with developmental language disorder (DLD) often have problems with everyday discourse which impact on their lives more widely. While improvement in language skills is supported by speech-language pathology (therapy), it tends to focus on smaller linguistic components, such as single words and sentences. This chapter outlines how speakers construct discourse in everyday situations and focuses on the meanings that people use discourse to convey, as well as the lexical and grammatical resources they use to convey these meanings. Current methods for discourse analysis will be outlined and key developments in narrative discourse production therapy will be reviewed

âTOTâ phenomena: Gesture production in younger and older adults
This study explored age-related changes in gesture in order to better understand the relationship between gesture and word retrieval from memory. The frequency of gestures during âTip-of-the-Tongueâ (TOT) states highlights this relationship. There is a lack of evidence describing the form and content of iconic gestures arising spontaneously in such TOT states, and a parallel gap addressing age-related variations. In this study, TOT states were induced in 45 participants from two age groups (older and younger adults) using a pseudoword paradigm. The type and frequency of gestures produced was recorded during two experimental conditions (single-word retrieval/ narrative task). We found that both groups experienced a high number of TOT states, during which they gestured. Iconic co-TOT gestures were more common than non-iconic gestures. Whilst there was no age-effect on the type of gestures produced, there was a significant, task-specific, age difference in the amount of gesturing. That is, younger adults gestured more in the narrative task, whereas older adults generated more gestures on the single-word retrieval task. Task-specific age differences suggest that there are age-related differences in terms of the cognitive operations involved in TOT gesture production
Pulmonary tuberculosis in a South African regional emergency centre: Can infection control be improved to lower the risk of nosocomial transmission?
Background. George Regional Hospital (GRH) is a 272-bed regional referral hospital for the Eden and Central Karoo districts, Western Cape Province, South Africa. The perception among emergency centre (EC) staff is that a high burden of tuberculosis (TB) is being diagnosed and that infection control procedures are currently lacking, leading to a high risk of nosocomial transmission.Objectives. To establish the burden of pulmonary TB (PTB) presenting to GRH via the EC and audit current infection prevention and control practices, to quantify the risk of transmission of PTB in the EC and to establish whether infection control measures are inadequate.Methods. An audit of infection control based on the Centers for Disease Control audit tool for TB, analysis of results, and implementation of new infection control measures including a new standard operating procedure based on a set of triage criteria.Results. Implementation of new triage criteria and a standard operating procedure led to the longest length of stay of a patient with suspected TB in the EC being reduced by 40% (from 142 hours to 84 hours). The average time between seeing a doctor and leaving the EC for patients with suspected TB was reduced by 20% (from 20 hours 40 minutes to 16 hours 45 minutes).Conclusion. Simple measures implemented in the EC led to a significant reduction in the time patients with suspected or confirmed TB spent in the EC. This should lead to a reduced risk of nosocomial transmission of TB to both staff and patients
The 4U 0115+63: Another energetic gamma ray binary pulsar
Following the discovery of Her X-1 as a source of pulsed 1000 Gev X-rays, a search for emission from an X-ray binary containing a pulsar with similar values of period, period derivative and luminosity was successful. The sporadic X-ray binary 4U 0115-63 has been observed, with probability 2.5 x 10 to the minus 6 power ergs/s to emit 1000 GeV gamma-rays with a time averaged energy flux of 6 to 10 to the 35th power

EAGLE multi-object AO concept study for the E-ELT
EAGLE is the multi-object, spatially-resolved, near-IR spectrograph
instrument concept for the E-ELT, relying on a distributed Adaptive Optics,
so-called Multi Object Adaptive Optics. This paper presents the results of a
phase A study. Using 84x84 actuator deformable mirrors, the performed analysis
demonstrates that 6 laser guide stars and up to 5 natural guide stars of
magnitude R<17, picked-up in a 7.3' diameter patrol field of view, allow us to
obtain an overall performance in terms of Ensquared Energy of 35% in a 75x75
mas^2 spaxel at H band, whatever the target direction in the centred 5' science
field for median seeing conditions. The computed sky coverage at galactic

Representation-theoretic derivation of the Temperley-Lieb-Martin algebras
Explicit expressions for the Temperley-Lieb-Martin algebras, i.e., the
quotients of the Hecke algebra that admit only representations corresponding to
Young diagrams with a given maximum number of columns (or rows), are obtained,
making explicit use of the Hecke algebra representation theory. Similar
techniques are used to construct the algebras whose representations do not

On-sky tests of the CuReD and HWR fast wavefront reconstruction algorithms with CANARY
CuReD (Cumulative Reconstructor with domain Decomposition) and HWR (Hierarchical Wavefront Reconstructor) are novel wavefront reconstruction algorithms for the ShackâHartmann wavefront sensor, used in the single-conjugate adaptive optics. For a high-order system they are much faster than the traditional matrixâvector-multiplication method. We have developed three methods for mapping the reconstructed phase into the deformable mirror actuator commands and have tested both reconstructors with the CANARY instrument. We find out that the CuReD reconstructor runs stably only if the feedback loop is operated as a leaky integrator, whereas HWR runs stably with the conventional integrator control. Using the CANARY telescope simulator we find that the Strehl ratio (SR) obtained with CuReD is slightly higher than that of the traditional least-squares estimator (LSE). We demonstrate that this is because the CuReD algorithm has a smoothing effect on the output wavefront. The SR of HWR is slightly lower than that of LSE. We have tested both reconstructors extensively on-sky. They perform well and CuReD achieves a similar SR as LSE. We compare the CANARY results with those from a computer simulation and find good agreement between the two
Krebs von den Lungen 6 (KL-6) as a marker for disease severity and persistent radiological abnormalities following COVID-19 infection at 12 weeks
IntroductionAcute presentations of COVID-19 infection vary, ranging from asymptomatic carriage through to severe clinical manifestations including acute respiratory distress syndrome (ARDS). Longer term sequelae of COVID-19 infection includes lung fibrosis in a proportion of patients. Krebs von den Lungen 6 (KL-6) is a mucin like glycoprotein that has been proposed as a marker of pulmonary epithelial cell injury. We sought to determine whether KL-6 was a marker of 1) the severity of acute COVID-19 infection, or 2) the persistence of symptoms/radiological abnormalities at medium term follow up.MethodsProspective single centre observational study.ResultsConvalescent KL-6 levels were available for 93 patients (male 63%, mean age 55.8 years) who attended an 12-week follow up appointment after being admitted to hospital with COVID-19. For 67 patients a baseline KL-6 result was available for comparison. There was no significant correlations between baseline KL-6 and the admission CXR severity score or clinical severity NEWS score. Furthermore, there was no significant difference in the baseline KL-6 level and an initial requirement for oxygen on admission or the severity of acute infection as measured at 28 days. There was no significant difference in the 12-week KL-6 level and the presence or absence of subjective breathlessness but patients with abnormal CT scans at 12 weeks had significantly higher convalescent KL-6 levels compared to the remainder of the cohort (median 1101 IU/ml vs 409 IU/ml).ConclusionsThe association between high KL-6 levels at 12 weeks and persisting CT abnormalities (GGO/fibrosis), is a finding that requires further exploration. Whether KL-6 may help differentiate those patients with persisting dyspnoea due to complications rather than deconditioning or dysfunctional breathing alone, is an important future research question
