Divergent drivers of the microbial methane sink in temperate forest and grassland soils
Aerated topsoils are important sinks for atmospheric methane (CH4) via oxidation by CH4âoxidizing bacteria (MOB). However, intensified management of grasslands and forests may reduce the CH4 sink capacity of soils. We investigated the influence of grassland landâuse intensity (150 sites) and forest management type (149 sites) on potential atmospheric CH4 oxidation rates (PMORs) and the abundance and diversity of MOB (with qPCR) in topsoils of three temperate regions in Germany. PMORs measurements in microcosms under defined conditions yielded approximately twice as much CH4 oxidation in forest than in grassland soils. High landâuse intensity of grasslands had a negative effect on PMORs (â40%) in almost all regions and fertilization was the predominant factor of grassland landâuse intensity leading to PMOR reduction by 20%. In contrast, forest management did not affect PMORs in forest soils. Upland soil cluster (USC)âα was the dominant group of MOBs in the forests. In contrast, USCâÎł was absent in more than half of the forest soils but present in almost all grassland soils. USCâα abundance had a direct positive effect on PMOR in forest, while in grasslands USCâα and USCâÎł abundance affected PMOR positively with a more pronounced contribution of USCâÎł than USCâα. Soil bulk density negatively influenced PMOR in both forests and grasslands. We further found that the response of the PMORs to pH, soil texture, soil water holding capacity and organic carbon and nitrogen content differ between temperate forest and grassland soils. pH had no direct effects on PMOR, but indirect ones via the MOB abundances, showing a negative effect on USCâα, and a positive on USCâÎł abundance. We conclude that reduction in grassland landâuse intensity and afforestation has the potential to increase the CH4 sink function of soils and that different parameters determine the microbial methane sink in forest and grassland soils.Deutsche Forschungsgemeinschaft

What do healthcare workers need? A qualitative study on support strategies to protect mental health of healthcare workers during the SARS-CoV-2 pandemic
The role of migration in mental healthcare: treatment satisfaction and utilization
Migration rates increase globally and require an adaption of national mental health services to the needs of persons with migration background. Therefore, we aimed to identify differences between persons with and without migratory background regarding (1) treatment satisfaction, (2) needed and received mental healthcare and (3) utilization of mental healthcare. In the context of a cross-sectional multicenter study, inpatients and day hospital patients of psychiatric settings in Southern Germany with severe affective and non-affective psychoses were included. Patientsâ satisfaction with and their use of mental healthcare services were assessed by VSSS-54 and CSSRI-EU; patientsâ needs were measured via CAN-EU. In total, 387 participants (migratory background: nâ=â72; 19%) provided sufficient responses for analyses. Migrant patients were more satisfied with the overall treatment in the past year compared to non-migrant patients. No differences between both groups were identified in met and unmet treatment needs and use of supply services (psychiatric, psychotherapeutic, and psychosocial treatment). Despite a comparable degree of met and unmet treatment needs and mental health service use among migrants and non-migrants, patients with migration background showed higher overall treatment satisfaction compared to non-migrants. The soil microbiome as a driver of methane fluxes in temperate grassland and forest soils
Methane (CH4) is a potent greenhouse gas with rising atmospheric concentrations.
Microorganisms are essential players in the global methane cycle. In fact, the largest part of methane emissions derives from microbial production by methanogenic Archaea (methanogens). Microorganisms do not only produce methane: methanotrophs can also oxidize the methane produced by methanogens. In addition, soil methanotrophs are the only biological methane sink, oxidizing up to 30-40 Tg of this potent greenhouse gas per year worldwide.
However, intensified management of grasslands and forests may reduce the methane sink capacity of soils.
In general, the interaction of methanogens and methanotrophs determines whether a soil is a source or a sink for methane. It is, therefore, crucial to understand the microbial part of the methane cycle and which factors influence the abundance and activity of methane-cycling microbes. However, capturing the soil microbiome's abundances, activity, and identity is
challenging. There are numerous target molecules and myriad methods, each with certain
limitations. Linking microbial markers to methane fluxes is therefore challenging. This thesis aimed to understand how methane-cycling microbes in the soil are related to soil methane fluxes and how soil characteristics and human activity influence them.
The first publication investigated the biotic and abiotic drivers of the atmospheric methane sink of soils. It assessed the influence of grassland land-use intensity (150 sites) and forest management type (149 sites) on potential atmospheric methane oxidation rates (PMORs) and the abundance and diversity of CH4-oxidizing bacteria (MOB) with qPCR in topsoils of three temperate regions in Germany. In the second publication, we used quantitative metatranscriptomics to link methane-cycling microbiomes to net surface methane fluxes throughout a year in two grassland soils. Methane fluxes were highly dynamic: both soils were net methane sources in autumn and winter and net methane sinks in spring and summer. Correspondingly, methanogen mRNA abundances per
gram soil correlated well with methane fluxes. Methanotroph to methanogen mRNA ratios were higher in spring and summer when the soils acted as net methane sinks. Furthermore, methane uptake was associated with an increased proportion of USCα and γ pmoA and pmoA2 transcripts. High methanotroph to methanogen ratios would indicate methane sink properties.
Our study links the seasonal transcriptional dynamics of methane-cycling soil microbiomes for the first time to gas fluxes in situ. It suggests mRNA transcript abundances as promising indicators of dynamic ecosystem-level processes.
We conclude that reduction in grassland land-use intensity and afforestation can potentially increase the methane sink function of soils and that different parameters determine the microbial methane sink in forest and grassland soils. Furthermore, this thesis suggests mRNA transcript abundances as promising indicators of dynamic ecosystem-level processes. Mikroorganismen sind wesentliche Akteure im globalen Methankreislauf. TatsÀchlich stammt
der gröĂte Teil der Methanemissionen aus der mikrobiellen Produktion durch methanogene
Archaeen (Methanogene). Mikroorganismen produzieren nicht nur Methan: Methanotrophe
Organismen können das von Methanogenen produzierte Methan auch oxidieren. DarĂŒber
hinaus sind Methanotrophe im Boden die einzige biologische Methansenke, die bis zu 30-40
Tg dieses starken Treibhausgases pro Jahr oxidiert. Eine intensivere Bewirtschaftung von
GrĂŒnland und WĂ€ldern kann jedoch die KapazitĂ€t der Böden als Methansenke verringern.
Im Allgemeinen bestimmt das Zusammenspiel von Methanogenen und Methanotrophen, ob ein
Boden eine Quelle oder eine Senke fĂŒr Methan ist. Daher ist es wichtig den mikrobiellen Teil
des Methankreislaufs zu kennen um zu verstehen, welche Faktoren die HÀufigkeit und AktivitÀt
der methanbildenden Mikroben beeinflussen. Die Abundanz, AktivitÀt und IdentitÀt des
Bodenmikrobioms zu bestimmen ist jedoch sehr anspruchsvoll. Es gibt zahlreiche ZielmolekĂŒle
und eine Vielzahl von Methoden, die jeweils bestimmte EinschrÀnkungen aufweisen. Die
VerknĂŒpfung von mikrobiellen Markern mit MethanflĂŒssen ist daher eine Herausforderung.
Ziel dieser Arbeit war es, zu verstehen, wie Bodenmikroben mit MethanflĂŒsse von Böden
zusammenhÀngen und wie sich Bodeneigenschaften und menschliche AktivitÀten auf die
MethanflĂŒsse von Böden auswirken.
In der ersten Veröffentlichung wurden die biotischen und abiotischen Faktoren untersucht, die
die atmosphÀrische Methansenke der Böden beeinflussen. Dabei wurden die Auswirkungen der
IntensitĂ€t der GrĂŒnlandnutzung (150 Standorte) und der Art der Waldbewirtschaftung (149
Standorte) auf die potenziellen atmosphÀrischen Methanoxidationsraten (PMORs) sowie die
HĂ€ufigkeit und Vielfalt von methanoxidierenden Bakterien (MOB) mit qPCR in Proben von
Oberböden aus drei gemĂ€Ăigten Regionen in Deutschland untersucht. Die in Mikrokosmen
unter definierten Bedingungen gemessenen PMORs waren in Waldböden etwa doppelt so hoch
wie in GrĂŒnlandböden. Die hohe LandnutzungsintensitĂ€t von GrĂŒnland wirkte sich in fast allen
Regionen negativ auf die PMOR-Werte aus (-40 %). Von den verschiedenen Aspekten der
LandnutzungsintensitĂ€t wirkte sich die DĂŒngung am stĂ€rksten negativ aus und verringerte die
PMORs um 20 %. Im Gegensatz dazu hatte die Waldbewirtschaftung keinen Einfluss auf die
PMORs in Waldböden. Das Upland Soil Cluster (USC)α war die dominierende Gruppe von
MOBs in den WÀldern. Im Gegensatz dazu fehlte USCγ in mehr als der HÀlfte der Waldböden,
war aber in fast allen GrĂŒnlandböden vorhanden. Die Abundanz von USCα wirkte sich in
WÀldern direkt positiv auf PMOR aus, wÀhrend sich im Grasland die Abundanz von USCα und
USCγ positiv auf PMOR auswirkte, wobei der Beitrag von USCγ stÀrker war als der von USCα.
In der zweiten Veröffentlichung setzten wir quantitative Metatranskriptomik ein, um die
Methan-zyklierenden Mikrobiome mit den NettomethanflĂŒssen zweier Graslandböden ĂŒber ein
Jahr hinweg zu verbinden. Die MethanflĂŒsse waren sehr dynamisch: Beide Böden waren im
Herbst und Winter Nettomethanquellen und im FrĂŒhjahr und Sommer Nettomethansenken.
Dementsprechend korrelierten die mRNA-HĂ€ufigkeiten der Methanogene pro Gramm Boden
gut mit den MethanflĂŒssen. Das VerhĂ€ltnis von Methanotrophen zu Methanogenen mRNA war
im FrĂŒhjahr und Sommer höher, wenn die Böden eine Nettomethansenken waren. AuĂerdem
war die Methanaufnahme mit einem erhöhten Anteil an USCα- und γ pmoA- und pmoA2-
Transkripten verbunden. Die Methanaufnahme der Böden war von einem hohen VerhÀltnis von
Methanotrophen zu Methanogenen begleitet. Diese Thesis stellt erstmals einen Zusammenhang
zwischen der saisonalen Transkriptionsdynamik von Methan-zyklierenden Bodenmikrobiomen
und den GasflĂŒssen in situ her.
Wir kommen zu dem Schluss, dass eine Verringerung der IntensitĂ€t der GrĂŒnlandnutzung und
Aufforstung die Methansenkenfunktion von Böden potenziell erhöhen kann und dass
unterschiedliche Parameter die mikrobielle Methansenke in Wald- und GrĂŒnlandböden
bestimmen. DarĂŒber hinaus zeigt diese Arbeit, dass die Abundanz von mRNA Transkripten ein
vielversprechender Indikator fĂŒr dynamische Prozesse auf Ăkosystemebene sein können. Die
Abundanz von Methanogen-Transkripten kann als Indikator fĂŒr VerĂ€nderungen der Netto-
OberflĂ€chen-Methanemissionen von GrĂŒnlandböden verwendet werden
Untersuchung der Benetzungshemmung im Boden
Benetzungsgehemmte Böden können weltweit gefunden werden. Benetzungshemmung in Böden verursacht eine rĂ€umliche VariabilitĂ€t der Bodenfeuchte und einen höheren OberflĂ€chenabfluss. Sie hat einen Einfluss auf die Formierung von prĂ€ferenziellen FlieĂwegen und damit auf den Stofftransport und die Grundwasserneubildung aber auch auf das Pflanzenwachstum. Das Ziel der Arbeit war es, das Auftreten und die Auswirkungen der Benetzungshemmung in Böden besser verstehen zu können. Der Einfluss der aktuellen Bodenfeuchte auf den Grad der Benetzungshemmung wurde an feldfrischen Proben untersucht. Aber die vorherrschende Bodenfeuchte und somit die Benetzungshemmung verĂ€ndern sich mit der Zeit. DafĂŒr ist es notwendig diesen dynamischen Prozess der Bildung und des Verschwindens der Benetzungshemmung in sandigen Böden zu beschreiben und den verĂ€nderlichen Anteil von benetzungsgehemmten und benetzbaren Bereichen im Boden zu quantifizieren. Zwei benetzungsgehemmte, sandige Standorte wurden ĂŒber drei Jahre untersucht: eine Wiese im Tiergarten und eine GrĂŒnlandflĂ€che auf den ehemaligen Rieselfeldern Berlin-Buch. Es wurden Bodenproben mit hoher rĂ€umlicher Auflösung in regelmĂ€Ăigen AbstĂ€nden genommen und auf ihren Wassergehalt, den Gehalt an organischer Substanz und auf ihre aktuelle und potenzielle Benetzungshemmung (mit dem WDPT Test) untersucht. An beiden Standorten wurden mit automatischen Messstellen Wassergehalte gemessen und klimatische Faktoren wie Lufttemperatur, relative Luftfeuchte und Niederschlag aufgenommen. Benetzungsgehemmte Proben mit hohen Benetzungszeiten treten bei niedrigen Wassergehalten auf. Bei hohen Wassergehalten waren die Proben benetzbar (Benetzungszeit <5s). Dazwischen befindet sich ein Ăbergangsbereich, die transition zone, in dem fĂŒr einen Wassergehalt sowohl benetzbare als auch benetzungsgehemmte Proben auftreten. Die Benetzbarkeit der Proben kann hier nicht aus dem Wassergehalt gefolgert werden. Diese unscharfe Trennung kann nur schwer in numerische Modelle eingearbeitet werden. Es konnte gezeigt werden, dass die Unterscheidung von benetzbaren und benetzungsgehemmten Proben anhand des Wassergehaltes deutlich verbessert werden kann, wenn neben dem Wassergehalt auch der Gehalt an organischer Substanz der Proben berĂŒcksichtigt wird. Mit einer linearen Funktion war es möglich, einen kritischen Wassergehalt crit in AbhĂ€ngigkeit des Gehaltes an organischer Substanz zu ermitteln und somit klarere Aussagen ĂŒber die Benetzbarkeit der Proben zu treffen. Die Ergebnisse von 32 Beprobungen ĂŒber 3 Jahre sowie die stĂŒndlichen Wassergehaltsmessungen einer automatischen TDR-Anlage mit 63 Sonden weisen auf eine jahreszeitliche Ănderung des Anteils der benetzungsgehemmten Bereiche hin. Um die HeterogenitĂ€t und somit den Grad des prĂ€ferenziellen Flusses zu quantifizieren, wurde ein Parameter eingefĂŒhrt: die effective cross section (ECS). Der Parameter wurde durch Anpassung der Beta-funktion an die kummulativen WassergehaltsĂ€nderungen in einer Tiefe von 25cm bei Niederschlagsereignissen gewonnen. Sowohl die ermittelten Werte fĂŒr die ECS als auch die Ergebnisse der Beprobungen zeigen den gleichen jahreszeitlichen Trend. Im Sommer ist die Neigung zum prĂ€ferenziellen Fluss mit ECS-Werten um 0,4 besonders ausgeprĂ€gt, wĂ€hrend im FrĂŒhjahr mit ECS-Werten um 0,9 fast die gesamte Bodenmatrix am FlieĂgeschehen teilnimmt. Der Vergleich der ermittelten ECS-Werte mit Klimadaten zeigt eine AbhĂ€ngigkeit der ECS von der Ausgangsfeuchte im Boden (in 10 cm Tiefe), der Niederschlagsdauer und âintensitĂ€t und der potentiellen Evapotranspirationsrate. Die TDR-Messungen zeigen auch eine Verschiebung der FlieĂwege bzw. der benetzungsgehemmten Bereiche im Boden. Der Einfluss der jahreszeitlichen Ănderung der Benetzungshemmung auf den Wasser- und Stofftransport wurde in einem Doppeltracerexperiment untersucht, bei dem zwei konservative Tracer zu unterschiedlichen Zeitpunkten auf einem Transekt ausgebracht wurden. Ein Br-Tracer wurde im April 2002 und ein Cl-Tracer im Oktober 2002 ausgebracht und die Verteilung der Konzentrationen im Profil bei der Beprobung im Januar 2003 untersucht. Es zeigte sich, dass wĂ€hrend der Versuchsdauer unterschiedliche FlieĂregime bestimmend waren. Es konnte eine Einteilung in ganzjĂ€hrig benetzbare HauptflieĂwege, in zeitweise genutzte NebenflieĂwege und ganzjĂ€hrig benetzungsgehemmte Bereiche ohne Wasserfluss vorgenommen werden. Die Ergebnisse dieses Experiments untermauern das Konzept der Effective Cross Section.Water repellent soils were reported all over the world. Water repellency in soils creates a spatial variability in soil moisture and a higher surface runoff. It has an impact on the formation of preferential flow paths, influences the solute transport and ground water recharge and is effecting the plant growth. The aim of this study was to get a better understanding of occurrence and consequences of water repellency. The influence of actual soil moisture on the degree of water repellency was observed for field-moist samples. But the ambient moisture and thus the wettablility in soils is changing by time. Therefore, it was necessary to describe the dynamics of forming and vanishing of water repellent spots in sandy soils and to quantify the varying area share of water repellent and wettable soil regions. Two sandy sites â a meadow in the Tiergarten park and a plot of grassland on the former sewage fields Berlin Buch - were observed over three years. Soil samples were taken with high spatial and temporal resolution in regular intervals and were analyzed for water content, organic matter content and the degree of actual and potential water repellency (using the Water-Drop-Penetration-Time test. On both sites automated measurement arrays recorded water content and the climatic conditions air temperature relative humidity and rainfall. Water repellent samples can be found at lower water contents with high penetration times and the wettable samples at higher water contents and penetration times < 5 s. The transition zone is the water content range where some samples repel while other samples are wettable â the wettability behaviour of samples in the transition zone is not clear. This fuzzy behaviour is hardly applicable to numerical models. It was shown that the distinction between wettable and repellent samples can be improved by taking the soil organic matter content, SOM, into account for the calculation of a critical water content. A linear function made it possible to distinguish between wettable or water repellent behaviour by using an easily measurable parameter the water content. The results of the 32 sampling campaigns at different times over a 3-year period and the hourly water content measurements on a TDR-array with 63 probes indicate a seasonal variation in the area share of water repellent soil regions. To quantify the heterogeneity (i.e. the degree of preferential flow) we introduced a parameter - the effective cross section, ECS, for water flow. This parameter was calculated by fitting the beta function to the cumulative values of the water content change over a horizontal cross section at a depth of 25 cm. Sampling and TDR measurements showed similar seasonal dynamics of preferential flow, with the highest occurrence in summer and early autumn with a ECS of 0.4 and a maximum accessible soil volume in the spring with an ECS of 0.9. Comparing the ECS-values with climatic data it was possible to calculate the effective cross section using a linear relationship with the initial soil moisture at 10 cm depth, the duration and amount of precipitation, and the potential evapotranspiration rate. The TDR-measurements also show that preferential flow paths do not remain at their positions. A shifting of flow channels e.g. of dry and water repellent spots was observed. The influence of the seasonal dynamics of water repellency on the solute transport was observed in a time delayed tracer experiment. Two conservative tracers Br- and Cl- were applied on one plot at different times, one in spring and one in autumn 2002. The varying distribution of the bromide and chloride tracer in the soil profile at the sampling in January 2003 makes it clear that different flow regimes predominated at the time of the tracer transport. The soil profile ranges were segmented in always wettable main flow paths, in sparse and moderate flow paths which become temporarily water repellent and in no flux regions which were water repellent over the whole period. Organic farming development in China and Japan
Diese Arbeit befasst sich mit unterstĂŒtzenden politischen, wirtschaftlichen und sozialen Strukturen fĂŒr die ökologische Landwirtschaft in China und Japan. Dabei wird untersucht, ob diese in beiden LĂ€ndern existieren, und in welchem MaĂ dies der Fall ist. Das Ziel ist, die-se unterstĂŒtzenden Faktoren beider LĂ€nder zu vergleichen.
DafĂŒr werden Gegebenheiten identifiziert, die nach Meinung internationaler Wissenschaftler einen positiven oder negativen Einfluss auf die Entwicklung ökologischer Landwirt-schaft im Allgemeinen haben. Die einzelnen Aspekte werden anschlieĂend im Bezug auf China und Japan untersucht. Strukturen, welche sich als beeinflussend auf die Entwicklung des Bio-Landbaus erweisen, werden im politischen, wirtschaftlichen und gesellschaftlichen Umfeld verortet. Im Hinblick auf die beiden asiatischen LĂ€nder wird demnach einer Analyse folgender Faktoren nachgegangen: Staatliche Richtlinien, finanzielle UnterstĂŒtzung, Bereitstellung von Beratungs- und Informationsdiensten durch den Staat, Zertifizierung, Kennzeichnung von Bio-Produkten, Regelungen und Gesetze, Finanzierung und Förderung von Forschung und Entwicklung, Erziehung und InformationsĂŒbermittlung, Markt fĂŒr Bio-Produkte und ökologische Futtermittel, VertriebskanĂ€le, Bio-AufschlĂ€ge, nichtstaatliche Akteure, Motivation von Ăko-Landwirten durch Preisauszeichnungen, Konsumenten von Bio-Produkten, Werbung fĂŒr Bio-Produkte sowie existierende Hindernisse fĂŒr ökologische Land-wirtschaft. Die Analyse dieser Faktoren beruht auf den entsprechenden chinesischen und ja-panischen rechtlichen Bestimmungen sowie Quellen in der SekundĂ€rliteratur.
Die Ergebnisse dieser Arbeit zeigen, dass China und Japan grundlegende Elemente zur UnterstĂŒtzung ihres ökologischen Agrarsektors eingefĂŒhrt haben, was auf ein groĂes Potential zur weiteren Entwicklung dieses Bereichs hinweist. Ăhnlichkeiten bezĂŒglich der UnterstĂŒtzungselemente in China und Japan bestehen vor allem im politischen Kontext, wohingegen Strukturen in den Gesellschaften beider LĂ€nder die gröĂten Unterschiede aufweisen. Hinzu kommt die Erkenntnis, dass es aufgrund bestehender Barrieren und Probleme im chinesischen und japanischen Bio-Landbau groĂer BemĂŒhungen beider LĂ€nder bedarf, um ein umfassendes System zu schaffen, welches die weitere Entwicklung dieses Sektors in beiden LĂ€ndern vorantreibt.This thesis deals with supportive governmental, economic, and social structures in the Chinese and Japanese organic farming sector. It thereby examines whether such structures exist in each country, and if so, to what extent they have evolved. The aim is to allow a com-parison of the supportive structures for the Japanese and the Chinese organic farming sector.
Therefore factors regarded by international scientists as having a positive or negative influence on the development of organic farming in general are identified. The individual aspects are subsequently researched for China and for Japan. Factors that influence organic farming development are found in the political, economic, and social sphere of a country. Hence, the following points are examined for the two countries: National guidelines, financial support, official authorities that provide assistance and information, certification, labeling, laws and standards, funding and promotion of research and development, education and information, the market for organic products and organic feed, sales channels, price premiums, non-governmental actors, motivation of organic farmers through rewards and prizes, consumers of organics, the promotion of organic products, and existing barriers. The analysis of the factors is based on Chinese and Japanese laws and secondary sources.
The results of this thesis reveal that China and Japan have succeeded in establishing fundamental supportive elements for their organic sectors, and thus possess great potential for further development. Similarities primarily exist among supportive factors in the political sphere, whereas the greatest differences have been identified among the social sphere. Additionally, the fact that major insufficiencies within the two organic agriculture sectors have been revealed shows that extensive efforts are needed in order to ensure a supportive framework that will facilitate organic farming development in both countries in the future
Revision of 'covered sclerotomy' in glaucoma patients
Hintergrund: Ein erhöhter intraokularer Druck (IOD) wird als einer der
bedeutendsten okulĂ€ren Risikofaktoren fĂŒr einen Glaukomschaden betrachtet. Zur
Drucksenkung kann eine fistulierende Glaukomoperationen durchgefĂŒhrt werden.
Die dadurch gebildeten Sickerkissen vernarben jedoch oftmals und eine
Revisionsoperation muss folgen. Die Revision der gedeckten Sklerotomie stellt
einen minimalinvasiven, gut durchzufĂŒhrenden Eingriff dar. Um der erhöhten
Vernarbungstendenz entgegenzuwirken, wird bei der Revision der Antimetabolit
Mitomycin C intraoperativ verwendet. Methoden: In einer retrospektiven
klinischen Untersuchung wurden 40 Augen (37 Patienten), die alle eine Revision
nach einer gedeckten Sklerotomie erhielten, untersucht. Alle
Revisionsoperationen erfolgten durch Reaktivierung der Fistulation und unter
intraoperativer Mitomycin-Applikation (0,02 % fĂŒr 3 min). ĂberprĂŒft wurden im
postoperativen Verlauf der IOD, die Vernarbungsrate und mögliche
Komplikationen fĂŒr einen Beobachtungszeitraum von einem Jahr. Ergebnisse: Ein
Revisionseingriff war im Durchschnitt nach 14,7 ± 14 Monate nach der primÀren
gedeckten Sklerotomie erforderlich. Der IOD lag vor der Revision im
Durchschnitt bei 27,0 ± 5,4 mmHg. 4 Tage postoperativ im Durchschnitt 9,8 ±
5,8 mmHg ohne zusÀtzliche Glaukommedikamente. Im weiteren Verlauf betrug der
IOD 18,4 ± 7,5 mmHg nach 4 Wochen, 18,0 ± 4,4 mmHg nach 3 Monaten, 16,6 ± 4,8
mmHg nach 6 Monaten und 17,5 ± 4,4 mmHg nach einem Jahr. 13 Patienten (32,5 %)
mussten im Durchschnitt 4,2 ± 2,9 Monate nach der Revisionsoperation
wiederholt operiert werden (CPC, Needling, erneute Revision, gedeckte
Sklerotomie an neuer Stelle). Als Komplikationen traten ein kurzzeitiges
positives SeidelphÀnomen (32,5%), eine Hypotonie (IOD †5 mmHg) in 30% und
eine Aderhautamotio in 15% der FĂ€lle auf. Die Komplikationen bildeten sich
alle im Laufe von höchstens 4 Wochen wieder zurĂŒck. Schlussfolgerung: Bei 17%
der Patienten konnte ein absoluter Erfolg (IOD †21 mmHg ohne antiglaukomatöse
Medikamente) und bei 56% ein relativer Erfolg (IOD †21 mmHg mit
antiglaukomatösen Medikamente) durch die Revisionsoperation erzielt werden bei
einer geringen Komplikationsrate.Backround: Scarring of the filtering bleb is one of the most significant
postoperative complications after glaucoma filtration surgery. Subsequent
elevation of the intraocular pressure [IOP] requires a revision of the fistula
in most of the cases. Revision of the scarred filtering bleb after covered
sclerotomy is a minimal invasive procedure. To prevent subsequent scarring
Mitomycin C is used intraoperatively. Methods: In a retrospective clinical
investigation 40 eyes (37 patients) were examined. In each case revision of
covered sclerotomy combined with intraoperative Mitomycin C application (0,02%
3 minutes) was performed by removing cicatricial tissue and thereby
reactivating the fistulization. Postoperatively IOP, scarring and
complications were observed for a period of one year . Results: Revision of
the covered sclerotomy was carried out 14,7 ± 14 months on average after the
covered sclerotomy had been undertaken. Intraocular pressure before
reoperation was 27,0 ± 5,4 mmHg on average. IOP was reduced postoperatively to
9,8 ± 5,8 mmHg on average 4 days after surgery, 18,4 ± 7,5 mmHg on average
after 4 weeks, 18,0 ± 4,4 mmHg on average after 3 months, 16,6 ± 4,8 mmHg on
average after 6 months and 17,5 ± 4,4 mmHg on average after one year. In 13
cases (32,5 %) further antiglaucomatous surgery (cyclophotocoagulation,
needling, repeated revision, covered sclerotomy at a new place) was required
4,2 ± 2,9 months on average after the first revision. As short-term
complications bleb-fistulization (positive Seidel phenomenon) was observed in
32,5% of the examined cases, hypotony (IOP †5 mmHg) in 30% and amotio
choroideae in 15%. All these complications, however, regressed within 4 weeks,
at the latest. Conclusion: After the revision of covered sclerotomy 17% of the
cases showed absolute success (IOP †21 mmHg without antiglaucomatous
medicine) and 56% of the cases relative success (IOP †21 mmHg with
antiglaucomatous medicine). The intra- and postoperative complication rate was
low
