The Effect of Coaching on Physical Activity and Quality of Life in Children and Adolescents with Cystic Fibrosis: A Quality Improvement Pilot Study
Purpose: Physical activity is well known to improve or maintain exercise capacity and overall health in patients with cystic fibrosis (CF). However, many patients do not meet recommended guidelines for physical activity. The purpose of this study was to determine if individualized, targeted coaching could improve the levels of physical activity and quality of life in children and adolescents with CF. Subjects: Twelve children with CF, ages 7 to 17, participated in this study.Method: Each participant completed a physical activity questionnaire (PAQ) and a disease-specific quality of life questionnaire (CFQ) at baseline and three months later. The participants were given pedometers and a 10,000 step per day target. Participants also participated in discussions regarding ways to increase physical activity appropriate for their interest, abilities and age. Weekly follow-up phone calls were made to obtain pedometer counts and discuss physical activities performed. Results: Physical activity scores (PAQ) improved in 6 of 12 participants. Quality of life improved in many dimensions of the CFQ, particularly in vitality (3 of 5 improved); emotional (8 of 12 improved) and respiratory (7 of 12 improved). Conclusions: The results of this small pilot study suggest that an individualized coaching approach to physical activity in children and adolescents with CF may improve physical activity levels and improve important aspects of quality of life in some children with CF
Numinosity and Adaptation to Stress: The Role of Religiosity, Spirituality and Religious Coping in Adaptation to Significant Life Events
The objectives of this study were to examine what role the numinous traits of spirituality, religiosity and religious coping play in one’s adaptation to stress associated with divorce, above and beyond the predictive role of the robust construct called personality. Using scores from instruments measuring the above referenced domains from participants who had recently experienced a separation or divorce (N = 509), this study employed correlational statistics and multiple regression analysis to assess the unique contributions which each domain made to the variance in adaptation to stress. This study elucidates the contributions made by numinous constructs (above that of personality) to one’s adaptation to stress. Results indicated that spirituality, religiosity, and religious coping produced significant contributions to the variance in adaptation to stress over personality. This suggests that these constructs are important considerations for counselors and their clients who are struggling with stressful events like marital separation and divorce
The growth and erosion of cinder cones in Guatemala and El Salvador: Models and statistics
Morphologic data for 147 cinder cones in southern Guatemala andwestern El Salvador are comparedwith data from
the San Francisco volcanic field, Arizona (USA), Cima volcanic field, California (USA), Michoácan–Guanajuato
volcanic field, Mexico, and the Lamongan volcanic field, East Java. The Guatemala cones have an average height of
110+/-50 m, an average basal diameter of 660+/-230 m and an average top diameter of 180+/-150 m. The
generalmorphology of these cones can be described by their average cone angle of slope (24+/-7), average heightto-
radius ratio (0.33+/-0.09) and their flatness (0.24+/-0.18). Although the mean values for the Guatemalan
cones are similar to those for other volcanic fields (e.g., San Francisco volcanic field, Arizona; Cima volcanic field,
California; Michoácan–Guanajuato volcanic field, Mexico; and Lamongan volcanic field, East Java), the range of
morphologies encompasses almost all of those observed worldwide for cinder cones.
Three new 40Ar/39Ar age dates are combined with 19 previously published dates for cones in Guatemala and El
Salvador. There is no indication that the morphologies of these cones have changed over the last 500–1000 ka.
Furthermore, a re-analysis of published data for other volcanic fields suggests that only in the Cima volcanic field (of
those studied) is there clear evidence of degradation with age.
Preliminary results of a numerical model of cinder cone growth are used to show that the range of morphologies
observed in the Guatemalan cinder cones could all be primary, that is, due to processes occurring at the time of
eruption

Feasibility of Using Low-Cost Motion Capture for Automated Screening of Shoulder Motion Limitation after Breast Cancer Surgery
Objective
To determine if a low-cost, automated motion analysis system using Microsoft Kinect could accurately measure shoulder motion and detect motion impairments in women following breast cancer surgery. Design
Descriptive study of motion measured via 2 methods. Setting
Academic cancer center oncology clinic. Participants
20 women (mean age = 60 yrs) were assessed for active and passive shoulder motions during a routine post-operative clinic visit (mean = 18 days after surgery) following mastectomy (n = 4) or lumpectomy (n = 16) for breast cancer. Interventions
Participants performed 3 repetitions of active and passive shoulder motions on the side of the breast surgery. Arm motion was recorded using motion capture by Kinect for Windows sensor and on video. Goniometric values were determined from video recordings, while motion capture data were transformed to joint angles using 2 methods (body angle and projection angle). Main Outcome Measure
Correlation of motion capture with goniometry and detection of motion limitation. Results
Active shoulder motion measured with low-cost motion capture agreed well with goniometry (r = 0.70–0.80), while passive shoulder motion measurements did not correlate well. Using motion capture, it was possible to reliably identify participants whose range of shoulder motion was reduced by 40% or more. Conclusions
Low-cost, automated motion analysis may be acceptable to screen for moderate to severe motion impairments in active shoulder motion. Automatic detection of motion limitation may allow quick screening to be performed in an oncologist\u27s office and trigger timely referrals for rehabilitation

Suture-method versus Through-the-needle Catheters for Continuous Popliteal-sciatic Nerve Blocks: A Randomized Clinical Trial.
BACKGROUND:The basic perineural catheter design has changed minimally since inception, with the catheter introduced through or over a straight needle. The U.S. Food and Drug Administration recently cleared a novel perineural catheter design comprising a catheter attached to the back of a suture-shaped needle that is inserted, advanced along the arc of its curvature pulling the catheter past the target nerve, and then exited through the skin in a second location. The authors hypothesized that analgesia would be noninferior using the new versus traditional catheter design in the first two days after painful foot/ankle surgery with a primary outcome of average pain measured with the Numeric Rating Scale. METHODS:Subjects undergoing painful foot or ankle surgery with a continuous supraparaneural popliteal-sciatic nerve block 5 cm proximal to the bifurcation were randomized to either a suture-type or through-the-needle catheter and subsequent 3-day 0.2% ropivacaine infusion (basal 6 ml/h, bolus 4 ml, lockout 30 min). Subjects received daily follow-up for the first four days after surgery, including assessment for evidence of malfunction or dislodgement of the catheters. RESULTS:During the first two postoperative days the mean ± SD average pain scores were lower in subjects with the suture-catheter (n = 35) compared with the through-the-needle (n = 35) group (2.7 ± 2.4 vs. 3.4 ± 2.4) and found to be statistically noninferior (95% CI, -1.9 to 0.6; P < 0.001). No suture-style catheter was completely dislodged (0%), whereas the tips of three (9%) traditional catheters were found outside of the skin before purposeful removal on postoperative day 3 (P = 0.239). CONCLUSIONS:Suture-type perineural catheters provided noninferior analgesia compared with traditional catheters for continuous popliteal-sciatic blocks after painful foot and ankle surgery. The new catheter design appears to be a viable alternative to traditional designs used for the past seven decades
Ar-40/Ar-39 Ages for Maskelynites and K-Rich Melt from Olivine-Rich Lithology in (Kanagawa) Zagami
We report Ar/Ar release patterns for small maskelynite grains and samples of a K-rich phase separated from the basaltic shergottite Zagami. The purpose of the work is to investigate the well-known discrepancy between published Ar/Ar ages of Zagami, >200 Ma, and its age of approx. 170 Ma as determined by other methods [1-6]. Niihara et al. [7] divide less abundant darker material present in Zagami into an olivine-rich lithology (ORL), from which most of our samples came, and a pyroxene-rich one (Dark Mottled-Lithology: DML) [8, 9]. ORL consists of vermicular fayalitic olivine, coarse-grained pyroxene, maskelynite, and a glassy phase exceptionally rich in K (up to 8.5 wt%), Al, and Si, but poor in Fe and Mg. The elemental composition suggests a late-stage melt, i.e., residual material that solidified late in a fractional crystallization sequence. Below we refer to it as "K-rich melt." The K-rich melt contains laths of captured olivine, Ca-rich pyroxene, plagioclase, and opaques. It seemed to offer an especially promising target for Ar-40/Ar-39 dating
Ar-40/Ar-39 Studies of Martian Meteorite RBT 04262 and Terrestrial Standards
Park et al. recently presented an Ar-40/Ar-39 dating study of maskelynite separated from the Martian meteorite RBT 04262. Here we report an additional study of Ar-40/Ar-39 patterns for smaller samples, each consisting of only a few maskelynite grains. Considered as a material for Ar-40/Ar-39 dating, the shock-produced glass maskelynite has both an important strength (relatively high K concentration compared to other mineral phases) and some potentially problematic weaknesses. At Rutgers, we have been analyzing small grains consisting of a single phase to explore local effects that might be averaged and remain hidden in larger samples. Thus, to assess the homogeneity of the RBT maskelynite and for comparison with the results of, we analyzed six approx. 30 microgram samples of the same maskelynite separate they studied. Furthermore, because most Ar-40/Ar-39 are calculated relative to the age of a standard, we present new Ar-40/Ar-39 age data for six standards. Among the most widely used standards are sanidine from Fish Canyon (FCs) and various hornblendes (hb3gr, MMhb-1, NL- 25), which are taken as primary standards because their ages have been determined by independent, direct measurements of K and A-40
Antitumor activity and safety of the PARP inhibitor rucaparib in patients with high grade ovarian carcinoma and a germline or somatic BRCA1 or BRCA2 mutation: integrated analysis of data from Study 10 and ARIEL2
Objective:
An integrated analysis was undertaken to characterize the antitumor activity and safety profile of the oral poly(ADP-ribose) polymerase inhibitor rucaparib in patients with relapsed high-grade ovarian carcinoma (HGOC).
Methods:
Eligible patients from Study 10 (NCT01482715) and ARIEL2 (NCT01891344) who received a starting dose of oral rucaparib 600 mg twice daily (BID) with or without food were included in these analyses. The integrated efficacy population included patients with HGOC and a deleterious germline or somatic BRCA1 or BRCA2 (BRCA1/2) mutation who received at least two prior chemotherapies and were sensitive, resistant, or refractory to platinum-based chemotherapy. The primary endpoint was investigator-assessed confirmed objective response rate (ORR). Secondary endpoints included duration of response (DOR) and progression-free survival (PFS). The integrated safety population included patients with HGOC who received at least one dose of rucaparib 600 mg BID, irrespective of BRCA1/2 mutation status and prior treatments.
Results:
In the efficacy population (n = 106), ORR was 53.8% (95% confidence interval [CI], 43.8–63.5); 8.5% and 45.3% of patients achieved complete and partial responses, respectively. Median DOR was 9.2 months (95% CI, 6.6–11.6). In the safety population (n = 377), the most frequent treatment-emergent adverse events (AEs) were nausea, asthenia/fatigue, vomiting, and anemia/hemoglobin decreased. The most common grade ≥ 3 treatment-emergent AE was anemia/hemoglobin decreased. Treatment-emergent AEs led to treatment interruption, dose reduction, and treatment discontinuation in 58.6%, 45.9%, and 9.8% of patients, respectively. No treatment-related deaths occurred.
Conclusions:
Rucaparib has antitumor activity in advanced BRCA1/2-mutated HGOC and a manageable safety profile
Molecular structure and biodegradation kinetics of Linear Alkylbenzene Sulphonates in sea water.
The present paper describes the results of the application of the biodegradation test proposed by the United States Environmental Protection Agency (USEPA) “Biodegradability in sea water” Office of Prevention, Pesticides, and Toxic Substances (OPPTS) 835.3160, to Linear Alkylbenzene Sulphonate (LAS), the synthetic surfactant with the highest consumption volume on a world-wide basis. High performance liquid chromatography (HPLC) has been employed for the separation and quantification of the different homologues and isomers of the surfactant. Water from the Bay of Cádiz (South–West of the Iberian peninsula) has been used as test medium. The results indicate how both lag and t50 time shows a significant linear relationship with the length of the alkyl chain of the homologue; the effect of this is that the homologues of longer chain length not only begin to degrade first but also degrade at a faster rate. Regarding the isomeric composition, it is observed that as the percentage of biodegradation increases, there is an increase in the proportion of internal isomers, in comparison with the isomeric relationships of the original test substanc
