Gait, Balance Recovery and Balance Confidence after Total Knee Replacement: Differences between Younger and Older Patients
Total knee replacement (TKR) is the gold standard treatment for improving mobility and relieving pain associated with end-stage knee osteoarthritis (OA) when other modalities have failed. Patients demonstrate significant improvements compared to pre-TKR levels, however deficiencies in function and mobility remain when compared to healthy controls. Recent national joint replacement registries have reported a substantial increase in the number of TKR procedures performed on younger patients. Over the last decade in Canada, the largest relative percent increase (â300%) in TKRs has occurred to patients who were between 45-54 years of age. Although this younger patient group (<55 years old) is rapidly growing, the vast majority of the literature investigating TKR outcomes has been based upon the âtypicalâ, older TKR patient (â„ 65 years old). This has created a therapeutic dilemma for clinicians, having little empirical data to formulate explicit statements or recommendations regarding how TKR will affect this younger population, and a concern with prosthesis failure and revision surgery, there is a hesitation to perform TKR on the younger knee OA patient.
Age-related deterioration in sensory information acquisition and musculoskeletal function has been observed between younger and older adults in the absence of knee OA and TKR, and has been linked to an increase in fall risk and falls. These age-related deficits observed in healthy adults may also distinguish younger and older TKR patients, which would have important implications to the surgical and rehabilitation practice. Currently, there are a limited number of published reports examining age-related differences in TKR patients and this gap in the literature warrants investigation. Therefore, the goal of this thesis is primarily to investigate the younger TKR patient and to compare their observations to that of the older, âtypicalâ patient and to their healthy age-matched controls
Exploring notions of genre in 'academic literacies' and 'writing across the curriculum': approaches across countries and contexts
The SIGET IV panel on genre in Writing Across the Curriculum (WAC) and âacademic literaciesâ (ACLITS) has set rolling a discussion of the similarities and differences in the two traditions, the former originating in the US in the early 1970s, the latter originating in England in the early 1990s. This paper maps out some elements of each in relation to the other and to genre, which we hope will set in motion further discussions and cross-fertilization
'It's a film' : medium specificity as textual gesture in Red road and The unloved
British cinema has long been intertwined with television. The
buzzwords of the transition to digital media, 'convergence' and
'multi-platform delivery', have particular histories in the British
context which can be grasped only through an understanding of the
cultural, historical and institutional peculiarities of the British film
and television industries. Central to this understanding must be two
comparisons: first, the relative stability of television in the duopoly
period (at its core, the licence-funded BBC) in contrast to the repeated
boom and bust of the many different financial/industrial combinations
which have comprised the film industry; and second, the cultural and
historical connotations of 'film' and 'television'. All readers of this
journal will be familiar â possibly over-familiar â with the notion that
'British cinema is alive and well and living on television'. At the end of
the first decade of the twenty-first century, when 'the end of medium
specificity' is much trumpeted, it might be useful to return to the
historical imbrication of British film and television, to explore both
the possibility that medium specificity may be more nationally specific
than much contemporary theorisation suggests, and to consider some
of the relationships between film and television manifest at a textual
level in two recent films, Red Road (2006) and The Unloved (2009)
Multi-Messenger Astronomy with Extremely Large Telescopes
The field of time-domain astrophysics has entered the era of Multi-messenger
Astronomy (MMA). One key science goal for the next decade (and beyond) will be
to characterize gravitational wave (GW) and neutrino sources using the next
generation of Extremely Large Telescopes (ELTs). These studies will have a
broad impact across astrophysics, informing our knowledge of the production and
enrichment history of the heaviest chemical elements, constrain the dense
matter equation of state, provide independent constraints on cosmology,
increase our understanding of particle acceleration in shocks and jets, and
study the lives of black holes in the universe. Future GW detectors will
greatly improve their sensitivity during the coming decade, as will
near-infrared telescopes capable of independently finding kilonovae from
neutron star mergers. However, the electromagnetic counterparts to
high-frequency (LIGO/Virgo band) GW sources will be distant and faint and thus
demand ELT capabilities for characterization. ELTs will be important and

Reading Graphic Novels in School: texts, contexts and the interpretive work of critical reading
This paper uses the example of an extra-curricular Graphic Novel Reading Group in order to explore the institutional critical reading practices that take place in English classrooms in the senior years of secondary school. Drawing on Stanley Fish's theory of interpretive communities, it questions the restrictive interpretive strategies applied to literary texts in curriculum English. By looking closely at the interpretive strategies pupils apply to a different kind of text (graphic novels) in an alternative context (an extra-curricular space) the paper suggests that there may be other ways of engaging with text that pupils find less alienating, more pleasurable and less reminiscent of 'work'
LSST: from Science Drivers to Reference Design and Anticipated Data Products
(Abridged) We describe here the most ambitious survey currently planned in
the optical, the Large Synoptic Survey Telescope (LSST). A vast array of
science will be enabled by a single wide-deep-fast sky survey, and LSST will
have unique survey capability in the faint time domain. The LSST design is
driven by four main science themes: probing dark energy and dark matter, taking
an inventory of the Solar System, exploring the transient optical sky, and
mapping the Milky Way. LSST will be a wide-field ground-based system sited at
Cerro Pach\'{o}n in northern Chile. The telescope will have an 8.4 m (6.5 m
effective) primary mirror, a 9.6 deg field of view, and a 3.2 Gigapixel
camera. The standard observing sequence will consist of pairs of 15-second
exposures in a given field, with two such visits in each pointing in a given
night. With these repeats, the LSST system is capable of imaging about 10,000
square degrees of sky in a single filter in three nights. The typical 5
point-source depth in a single visit in will be (AB). The
project is in the construction phase and will begin regular survey operations
by 2022. The survey area will be contained within 30,000 deg with
, and will be imaged multiple times in six bands, ,
covering the wavelength range 320--1050 nm. About 90\% of the observing time
will be devoted to a deep-wide-fast survey mode which will uniformly observe a
18,000 deg region about 800 times (summed over all six bands) during the
anticipated 10 years of operations, and yield a coadded map to . The
remaining 10\% of the observing time will be allocated to projects such as a
Very Deep and Fast time domain survey. The goal is to make LSST data products,
including a relational database of about 32 trillion observations of 40 billion

Health, education, and social care provision after diagnosis of childhood visual disability
Aim: To investigate the health, education, and social care provision for children newly diagnosed with visual disability.Method: This was a national prospective study, the British Childhood Visual Impairment and Blindness Study 2 (BCVIS2), ascertaining new diagnoses of visual impairment or severe visual impairment and blindness (SVIBL), or equivalent vi-sion. Data collection was performed by managing clinicians up to 1-year follow-up, and included health and developmental needs, and health, education, and social care provision.Results: BCVIS2 identified 784 children newly diagnosed with visual impairment/SVIBL (313 with visual impairment, 471 with SVIBL). Most children had associated systemic disorders (559 [71%], 167 [54%] with visual impairment, and 392 [84%] with SVIBL). Care from multidisciplinary teams was provided for 549 children (70%). Two-thirds (515) had not received an Education, Health, and Care Plan (EHCP). Fewer children with visual impairment had seen a specialist teacher (SVIBL 35%, visual impairment 28%, Ï2p < 0.001), or had an EHCP (11% vs 7%, Ï2p < 0 . 01).Interpretation: Families need additional support from managing clinicians to access recommended complex interventions such as the use of multidisciplinary teams and educational support.
