    A randomised, assessor blind, parallel group comparative efficacy trial of three products for the treatment of head lice in children - melaleuca oil and lavender oil, pyrethrins and piperonyl butoxide, and a "suffocation" product

    Background: There are many different types of pediculicides available OTC in Australia. In this study we compare the efficacy and safety of three topical pediculicides: a pediculicide containing melaleuca oil (tea tree oil) and lavender oil (TTO/LO); a head lice "suffocation" product; and a product containing pyrethrins and piperonyl butoxide (P/PB).Method: This study was a randomised, assessor-blind, comparative, parallel study of 123 subjects with live head lice. The head lice products were applied according to the manufacturer's instructions (the TTO/LO product and the "suffocation" product were applied three times at weekly intervals according to manufacturers instructions (on Day 0, Day 7 and Day 14) and the P/PB product was applied twice according to manufacturers instructions (on Day 0 and Day 7)). The presence or absence of live lice one day following the last treatment was determined.Results: The percentage of subjects who were louse-free one day after the last treatment with the product containing tea tree oil and lavender oil (41/42; 97.6%) and the head lice "suffocation" product (40/41, 97.6%) was significantly higher compared to the percentage of subjects who were louse-free one day after the last treatment with the product containing pyrethrins and piperonyl butoxide (10/40, 25.0%; adj. p < 0.0001).Conclusion: The high efficacy of the TTO/LO product and the head lice "suffocation" product offers an alternative to the pyrethrins-based product

    Anomalous crustal movements with low seismic efficiency - Campi Flegrei, Italy and some examples in Japan

    Campi Flegrei is a unique volcanic region located near Naples, Italy. Anomalous crustal movements at Pozzuoli in Campi Flegrei have been documented since the Roman period. The movements were gradual and have continued to the present, occasionally accompanying swarms of local earthquakes and volcanic eruptions. Generally the movements proceed with low seismicity. After the 1538 eruption of Monte Nuovo, Pozzuoli had subsided monotonously, but it changed to uplift abruptly in 1969. The uplift accelerated in 1983 and 1984 reaching more than 2 m, and thereafter began to subside. Many discussions of this event have been published. In Japan, we have examples of deformations similar to those at Campi Flegrei, mainly in volcanic areas, and rarely in non-volcanic areas. The former includes Iwojima, Miyakejima and Aira caldera while the latter is represented by Cape Omaezaki. Iwojima is a volcano island, and its secular uplifts since the 18th century are recognized as an unusual event. Miyakejima volcano and Aira caldera exhibited anomalous movements with low seismicity after their eruptions. Cape Omaezaki is not situated in volcanic zone but near a subduction zone, and gradually and continuously subsides as a precursor to a large earthquake. In such cases as Campi Flegrei and the Japanese localities, we would question whether the deformations are accompanied by normal seismicity or low seismicity. To examine quantitatively the relationship between seismicity and related deformation, seismic efficiency is generally useful. The crustal deformations in all the regions cited above are characterized by exceptionally low seismic efficiencies. In the present paper, the deformations at Pozzuoli and Iwojima are mainly described and a comparative discussion among these and other localities in Japan is supplemented. It is concluded that such anomalous phenomena in volcanic areas are attributable to peculiar rheological aspects of the material composing the local upper crust, and the deformation in a non-volcanic area is of tectonic origin

    Millennial scale control of European climate by the North Atlantic Oscillation from 12,500 BP: the Asiul speleothem record

    Contemporary climate in Europe is strongly influenced by the North Atlantic Oscillation (NAO), the atmospheric pressure dipole between Iceland and the Azores1. Under positive NAO conditions winter storm tracks associated with the Atlantic Westerly Jet (AWJ) migrate northwards, leading to wetter and warmer winter conditions in north-western Europe and dry conditions in southern Europe; including the Iberian Peninsula. Under the negative NAO phase, storm tracks weaken and shift southwards reversing the pattern1. Existing proxy records of the NAO suggest that this atmospheric process only began to dominate European climate at approximately 8000 years BP, related to the final breakup of the Laurentide ice shelf2. However, here we present evidence of precipitation changes from a high-resolution speleothem δ18O record from northern Iberia, which indicates NAO-like forcing extending throughout the Holocene and into the Younger Dryas (YD) at 12,500 years BP. These variations in precipitation delivery relate to an underlying millennial scale cycle in NAO dynamics. The speleothem δ18O is strongly correlated to existing records of North Atlantic Ocean ice rafted debris (IRD)3, indicating an NAO-like connection with oceanic circulation during the Holocene2. These large-scale atmospheric processes have dramatically influenced the delivery of precipitation to northern Iberia and may have played a decisive role in environmental and human development in the region, throughout the Holocene

    North Atlantic forcing of moisture delivery to Europe throughout the Holocene

    Century-to-millennial scale fluctuations in precipitation and temperature are an established feature of European Holocene climates. Changes in moisture delivery are driven by complex interactions between ocean moisture sources and atmospheric circulation modes, making it difficult to resolve the drivers behind millennial scale variability in European precipitation. Here, we present two overlapping decadal resolution speleothem oxygen isotope (δ18O) records from a cave on the Atlantic coastline of northern Iberia, covering the period 12.1–0 ka. Speleothem δ18O reveals nine quasi-cyclical events of relatively wet-to-dry climatic conditions during the Holocene. Dynamic Harmonic Regression modelling indicates that changes in precipitation occurred with a ~1500 year frequency during the late Holocene and at a shorter length during the early Holocene. The timing of these cycles coincides with changes in North Atlantic Ocean conditions, indicating a connectivity between ocean conditions and Holocene moisture delivery. Early Holocene climate is potentially dominated by freshwater outburst events, whilst ~1500 year cycles in the late Holocene are more likely driven by changes internal to the ocean system. This is the first continental record of its type that clearly demonstrates millennial scale connectivity between the pulse of the ocean and precipitation over Europe through the entirety of the Holocene

    Background: There are two components to the clinical efficacy of pediculicides: (i) efficacy against the crawling-stages (lousicidal efficacy); and (ii) efficacy against the eggs (ovicidal efficacy). Lousicidal efficacy and ovicidal efficacy are confounded in clinical trials. Here we report on a trial that was specially designed to rank the clinical ovicidal efficacy of pediculicides. Eggs were collected, pre-treatment and post-treatment, from subjects with different types of hair, different coloured hair and hair of different length.Method: Subjects with at least 20 live eggs of Pediculus capitis (head lice) were randomised to one of three treatment-groups: a melaleuca oil (commonly called tea tree oil) and lavender oil pediculicide (TTO/LO); a eucalyptus oil and lemon tea tree oil pediculicide (EO/LTTO); or a "suffocation" pediculicide. Pre-treatment: 10 to 22 live eggs were taken from the head by cutting the single hair with the live egg attached, before the treatment (total of 1,062 eggs). Treatment: The subjects then received a single treatment of one of the three pediculicides, according to the manufacturers' instructions. Post-treatment: 10 to 41 treated live eggs were taken from the head by cutting the single hair with the egg attached (total of 1,183 eggs). Eggs were incubated for 14 days. The proportion of eggs that had hatched after 14 days in the pre-treatment group was compared with the proportion of eggs that hatched in the post-treatment group. The primary outcome measure was % ovicidal efficacy for each of the three pediculicides.Results: 722 subjects were examined for the presence of eggs of head lice. 92 of these subjects were recruited and randomly assigned to: the "suffocation" pediculicide (n = 31); the melaleuca oil and lavender oil pediculicide (n = 31); and the eucalyptus oil and lemon tea tree oil pediculicide (n = 30 subjects). The group treated with eucalyptus oil and lemon tea tree oil had an ovicidal efficacy of 3.3% (SD 16%) whereas the group treated with melaleuca oil and lavender oil had an ovicidal efficacy of 44.4% (SD 23%) and the group treated with the "suffocation" pediculicide had an ovicidal efficacy of 68.3% (SD 38%).Conclusion: Ovicidal efficacy varied substantially among treatments, from 3.3% to 68.3%. The "suffocation" pediculicide and the melaleuca oil and lavender oil pediculicide (TTO/LO) were significantly more ovicidal than eucalyptus oil and lemon tea tree oil pediculicide (EO/LTTO) (P < 0.0001). Ranking: 1. "Suffocation" pediculicide (68.3% efficacy against eggs); 2. Melaleuca oil and lavender oil (44.4%) pediculicide; 3. Eucalyptus oil and lemon tea tree oil (3.3%) pediculicide. The "suffocation" pediculicide and TTO/LO are also highly efficacious against the crawling-stages. Thus, the "suffocation" pediculicide and TTO/LO should be recommended as first line treatments.Trial Registration: The study was listed at the Australian/New Zealand Clinical Trial Registry (ANZCTR): reg. no. 12609000884202

    A rapid, simple, and low-blank pumped ion-exchange column chromatography technique for boron purification from carbonate and seawater matrices

Accurate δ11B MC-ICPMS analysis requires boron purification from the sample matrix using ion-exchange column chromatography. However, the traditional gravity-drip column method is time-consuming and prone to airborne contamination due to its long duration and open resin surface. To address these issues, we designed a novel, simple, and reliable column chromatography technique called “peri-columns.” This method uses a peristaltic pump to generate vacuum on a commonly used column set up. This method uses sealed collection beakers and does not require solutions to pass through pump tubing, minimizing contamination. The duration is reduced by eight-fold, processing 12 samples in just 1.5 hr. It also yields low and consistent total procedural blanks, averaging 11 pg. The efficiency and efficacy of this method were tested by repeated boron purification from calcium carbonate and high-sodium matrices with international and in-house reference materials. The results matched those obtained using the gravity column method and fell within our laboratory long-term and international certified values. The mean δ11B and 2SD (standard deviation) of repeatedly processed NIST 8301f were 14.57 ± 0.26‰ (n = 31), NIST 8301c was 24.19 ± 0.33‰ (n = 10), STAiG-F1 was 16.20 ± 0.26‰ (n = 13), and seawater was 39.52 ± 0.32‰ (n = 10). All the components of our techniques are commercially available, and it is easily adaptable to other laboratories and isotope systems.Publisher PDFPeer reviewe

    Yield of colorectal cancer at colonoscopy according to faecal haemoglobin concentration in symptomatic patients referred from primary care

    Background: Lower gastrointestinal (GI) symptoms are poor predictors of colorectal cancer (CRC). This study examined the diagnostic yield of colonoscopy by faecal haemoglobin concentration (f‐Hb) in symptomatic patients assessed in primary care by faecal immunochemical testing (FIT). Methods: In three Scottish NHS Boards, FIT kits (HM‐JACKarc, Hitachi Chemical Diagnostics Systems Co., Ltd, Tokyo, Japan) were used by GPs to guide referrals for patients with lower GI symptoms (lab data studied for 12 months from December 2015 onward in Tayside, 18 months from June 2018 onward in Fife, and 5 months from September 2018 onward in Greater Glasgow and Clyde). CRC cases diagnosed at colonoscopy were ascertained from colonoscopy and pathology records. Results: 4841 symptomatic patients who underwent colonoscopy after FIT submission were included. Of 2166 patients (44.7%) with f‐Hb &lt;10 µg Hb/g faeces (µg/g), 14 (0.6%) were diagnosed with CRC, with a number needed to scope (NNS) of 155. Of 2675 patients (55.3%) with f‐Hb ≥10 µg/g, 252 were diagnosed with CRC (9.4%) with a NNS of 11. Of 705 patients with f‐Hb ≥400 µg/g, 158 (22.4%) were diagnosed with CRC with a NNS of 5. Over half of those diagnosed with CRC with f‐Hb &lt;10 µg/g had co‐existing anaemia. Conclusions: Symptomatic patients with f‐Hb ≥10 µg/g should undergo further investigation for CRC, while higher f‐Hb could be used to triage its urgency during the COVID‐19 recovery phase. Patients with f‐Hb &lt;10 µg/g, without anaemia, are very unlikely to be diagnosed with CRC and the majority need no further investigation

    Spectral properties of zero temperature dynamics in a model of a compacting granular column

    The compacting of a column of grains has been studied using a one-dimensional Ising model with long range directed interactions in which down and up spins represent orientations of the grain having or not having an associated void. When the column is not shaken (zero 'temperature') the motion becomes highly constrained and under most circumstances we find that the generator of the stochastic dynamics assumes an unusual form: many eigenvalues become degenerate, but the associated multi-dimensional invariant spaces have but a single eigenvector. There is no spectral expansion and a Jordan form must be used. Many properties of the dynamics are established here analytically; some are not.     The Seroepidemiology of Haemophilus influenzae Type B Prior to Introduction of an Immunization Programme in Kathmandu, Nepal.

    Haemophilus influenzae type b (Hib) is now recognized as an important pathogen in Asia. To evaluate disease susceptibility, and as a marker of Hib transmission before routine immunization was introduced in Kathmandu, 71 participants aged 7 months-77 years were recruited and 15 cord blood samples were collected for analysis of anti-polyribosylribitol phosphate antibody levels by enzyme-linked immunosorbent assay. Only 20% of children under 5 years old had levels considered protective (>0.15 µg/ml), rising to 83% of 15-54 year-olds. Prior to introduction of Hib vaccine in Kathmandu, the majority of young children were susceptible to disease
