Appropriate criteria for identification of near-miss maternal morbidity in tertiary care facilities: A cross sectional study
<p>Abstract</p> <p>Background</p> <p>The study of severe maternal morbidity survivors (near miss) may be an alternative or a complement to the study of maternal death events as a health care indicator. However, there is still controversy regarding the criteria for identification of near-miss maternal morbidity. This study aimed to characterize the near miss maternal morbidity according to different sets of criteria.</p> <p>Methods</p> <p>A descriptive study in a tertiary center including 2,929 women who delivered there between July 2003 and June 2004. Possible cases of near miss were daily screened by checking different sets of criteria proposed elsewhere. The main outcome measures were: rate of near miss and its primary determinant factors, criteria for its identification, total hospital stay, ICU stay, and number and kind of special procedures performed.</p> <p>Results</p> <p>There were two maternal deaths and 124 cases of near miss were identified, with 102 of them admitted to the ICU (80.9%). Among the 126 special procedures performed, the most frequent were central venous access, echocardiography and invasive mechanical ventilation. The mean hospital stay was 10.3 (± 13.24) days. Hospital stay and the number of special procedures performed were significantly higher when the organ dysfunction based criteria were applied.</p> <p>Conclusion</p> <p>The adoption of a two level screening strategy may lead to the development of a consistent severe maternal morbidity surveillance system but further research is needed before worldwide near miss criteria can be assumed.</p

Uncoupling human and climate drivers of late Holocene vegetation change in southern Brazil
In the highlands of southern Brazil an anthropogenitcally driven expansion of forest occurred at the
expense of grasslands between 1410 and 900cal BP, coincident with a period of demographic and
cultural change in the region. Previous studies have debated the relative contributions of increasing
wetter and warmer climate conditions and human landscape modifcations to forest expansion, but
generally lacked high resoltiuon proxies to measure these efects, or have relied on single proxies to
reconstruct both climate and vegetation. Here, we develop and test a model of natural ecosystem
distribution against vegetation histories, paleoclimate proxies, and the archaeological record to
distinguish human from temperature and precipitation impacts on the distribution and expansion of
Araucaria forests during the late Holocene. Carbon isotopes from soil profles confrm that in spite of
climatic fuctuations, vegetation was stable and forests were spatially limited to south-facing slopes
in the absence of human inputs. In contrast, forest management strategies for the past 1400 years
expanded this economically important forest beyond its natural geographic boundaries in areas of
dense pre-Columbian occupation, suggesting that landscape modifcations were linked to demographic
changes, the efects of which are still visible today
Subsequent reproductive outcome in women who have experienced a potentially life-threatening condition or a maternal near-miss during pregnancy
Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP)OBJECTIVE: To evaluate the long-term reproductive consequences that affect women who have experienced potentially life-threatening or life-threatening (near-miss) maternal complications. INTRODUCTION: Although advances have been made in reducing maternal death, few studies have investigated the long-term repercussions of significant events such as severe maternal morbidity and maternal near-misses. These repercussions may be long-lasting and negatively affect quality of life. METHODS: A total of 382 women who had experienced a potentially life-threatening pregnancy-related condition within the last five years were analyzed in this retrospective cohort study. A control group of 188 women who gave birth without complications was also included. Trained interviewers contacted the subjects by telephone and completed a pre-coded, structured questionnaire on reproductive health. Data were analyzed using odds ratios adjusted for age. The main outcome measures were occurrence and outcome of subsequent pregnancies. RESULTS: The estimated risk of becoming infertile as a result of tubal ligation or hysterectomy was 3.5 times higher in women who experienced a maternal near-miss or severe maternal morbidity during the index pregnancy as compared to controls. Likewise, the risk of complications in subsequent pregnancies was five times greater in women who had experienced severe maternal morbidity. However, no differences were found in the occurrence or number of subsequent pregnancies or perinatal outcome. Análises fitoquímicas em extrato das folhas de Anthurium affine Schott (milho de urubu)
Apesar da ampla disposição de medicamentos no mercado, muitas dificuldades ainda existem quanto ao tratamento de determinados tipos de doenças, como é o caso das dermatites nos animais domésticos, mais especificamente nos cães. Apoiando-se na hipótese de que plantas da família das Araceae podem oferecer insumos importantes no tratamento deste tipo de doença, idealizou-se este trabalho tendo-se como principal objetivo estudar mais precisamente a composição fitoquímica do Anthurium affine Schott (A. affine), conhecido popularmente como “milho de urubu”. A partir de amostras da planta colhidas na Região Metropolitana do Grande Recife, foi confeccionado o extrato hidroalcóolico (etanol a 70%), das folhas da planta, sendo colocado para maceração por sete dias. A partir desta etapa o material foi submetido a procedimentos de filtragem e caracterização do extrato final obtido. Posteriormente, alíquotas deste material foram colhidas e trabalhadas para a realização dos ensaios de identificação fitoquímica, conforme protocolo padrão disponível na literatura. Como resultados, obteve-se um extrato hidroalcóolico da planta na concentração de 85 mg/mL; as análises fitoquímicas apresentaram resposta positiva para taninos, flavonóides, alcalóides e saponinas. Verificou-se que o A. affine apresentou um potencial fitoterápico mais rico do que normalmente se apresenta, embora estudos mais precisos sejam necessários para se avaliar os reais riscos de toxicidade no uso terapêutico da planta, como também para melhor definir os mecanismos de ação por traz dos efeitos farmacológicos atribuídos aos seus extratos
Risk stratification for small for gestational age for the Brazilian population: a secondary analysis of the Birth in Brazil study.
Risk-stratification screening for SGA has been proposed in high-income countries to prevent perinatal morbidity and mortality. There is paucity of data from middle-income settings. The aim of this study is to explore risk factors for SGA in Brazil and assess potential for risk stratification. This population-based study is a secondary analysis of Birth in Brazil study, conducted in 266 maternity units between 2011 and 2012. Univariate and multivariate logistic regressions were performed, and population attributable fraction estimated for early and all pregnancy factors. We calculated absolute risk, odds ratio, and population prevalence of single or combined factors stratified by parity. Factors associated with SGA were maternal lupus (ORadj 4.36, 95% CI [2.32-8.18]), hypertensive disorders in pregnancy (ORadj 2.72, 95% CI [2.28-3.24]), weight gain < 5 kg (ORadj 2.37, 95% CI [1.99-2.83]), smoking at late pregnancy (ORadj 2.04, 95% CI [1.60-2.59]), previous low birthweight (ORadj 2.22, 95% CI [1.79-2.75]), nulliparity (ORadj 1.81, 95% CI [1.60-2.05]), underweight (ORadj 1.61, 95% CI [1.36-1.92]) and socioeconomic status (SES) < 5th centile (ORadj 1.23, 95% CI [1.05-1.45]). Having two or more risk factors (prevalence of 4.4% and 8.0%) was associated with a 2 and fourfold increase in the risk for SGA in nulliparous and multiparous, respectively. Early and all pregnancy risk factors allow development of risk-stratification for SGA. Implementation of risk stratification coupled with specific strategies for reduction of risk and increased surveillance has the potential to contribute to the reduction of stillbirth in Brazil through increased detection of SGA, appropriate management and timely delivery
Oportunidades perdidas na prevenção da sífilis congênita e da transmissão vertical do HIV
OBJECTIVE: To estimate the prevalence of missed opportunities for congenital syphilis and HIV prevention in pregnant women who had access to prenatal care and to assess factors associated to non-testing of these infections. METHODS: Cross-sectional study comprising a randomly selected sample of 2,145 puerperal women who were admitted in maternity hospitals for delivery or curettage and had attended at least one prenatal care visit, in Brazil between 1999 and 2000. No syphilis and/or anti-HIV testing during pregnancy was a marker for missed prevention opportunity. Women who were not tested for either or both were compared to those who had at least one syphilis and one anti-HIV testing performed during pregnancy (reference category). The prevalence of missed prevention opportunity was estimated for each category with 95% confidence intervals. Factors independently associated with missed prevention opportunity were assessed through multinomial logistic regression. RESULTS: The prevalence of missed prevention opportunity for syphilis or anti-HIV was 41.2% and 56.0%, respectively. The multivariate analysis showed that race/skin color (non-white), schooling (OBJETIVO: Estimar la prevalencia de oportunidad de pérdida de prevención de la sífilis y el HIV entre gestantes que tuvieron acceso al pre-natal y factores asociados con la no evaluación de estos agravios. MÉTODOS: Se realizó estudio transversal con muestra aleatoria de 2.145 puérperas de Brasil, 1999 y 2000 admitidas en maternidades para parto o curetaje y que habían realizado al menos una consulta de pre-natal. La no realización del examen de prueba para sífilis y/o anti-HIV durante el embarazo fue usada como marcador para oportunidad de pérdida de prevención. Las mujeres que realizaron sólo examen de sífilis o sólo examen de anti-HIV, o que no realizaron ninguno, fueron comparadas con las que realizaron los dos (categoría de referencia). La prevalencia de oportunidad de pérdida de prevención fue estimada para cada categoría, con intervalo de confianza de 95%. Los factores asociados con la oportunidad de pérdida de prevención fueron analizados por medio de regresión logística multinomial. RESULTADOS: La prevalencia de oportunidad de pérdida de prevención para la realización de la prueba de sífilis o anti-HIV fue de 41,2% e 56,0%, respectivamente. El análisis multivariado indicó que raza/color (no blanca), escolaridad (< 8 años de estudio), estado civil (soltera), renta < 3 salarios mínimos, relación sexual durante el embarazo, no haber tenido sífilis anterior al embarazo actual, realización de seis o mas consultas de pre-natal y la realización de la última visita antes del tercer trimestre de embarazo, estaban asociados con mayor riesgo de tener oportunidad de pérdida de prevención. Se observó una asociación negativa entre estado civil (soltera), lugar de realización de pre-natal (hospital) y la realización de la primera consulta pre-natal en el tercer trimestre con oportunidad de pérdida de prevención. CONCLUSIONES: Altos porcentajes de gestantes no evaluadas señalan fallas en la prevención y control de la infección por HIV y de la sífilis congénita en los servicios de salud. Las gestantes continúan interrumpiendo el cuidado pre-natal precozmente y no logran realizar los procedimientos de selección para HIV y sífilis.OBJETIVO: Estimar a prevalência de oportunidade perdida de prevenção a sífilis e HIV entre gestantes que tiveram acesso ao pré-natal e fatores associados a não-testagem para esses agravos. Complete experimental toolbox for alignment-free quantum communication
Quantum communication employs the counter-intuitive features of quantum
physics to perform tasks that are im- possible in the classical world. It is
crucial for testing the foundations of quantum theory and promises to rev-
olutionize our information and communication technolo- gies. However, for two
or more parties to execute even the simplest quantum transmission, they must
establish, and maintain, a shared reference frame. This introduces a
considerable overhead in communication resources, par- ticularly if the parties
are in motion or rotating relative to each other. We experimentally demonstrate
how to circumvent this problem with the efficient transmission of quantum
information encoded in rotationally invariant states of single photons. By
developing a complete toolbox for the efficient encoding and decoding of
quantum infor- mation in such photonic qubits, we demonstrate the fea- sibility
of alignment-free quantum key-distribution, and perform a proof-of-principle
alignment-free entanglement distribution and violation of a Bell inequality.
Our scheme should find applications in fundamental tests of quantum mechanics

Molecular composition of organic aerosols in central Amazonia: An ultra-high-resolution mass spectrometry study
The Amazon Basin plays key role in atmospheric chemistry, biodiversity and climate change. In this study we applied nanoelectrospray (nanoESI) ultra-high-resolution mass spectrometry (UHRMS) for the analysis of the organic fraction of PM aerosol samples collected during dry and wet seasons at a site in central Amazonia receiving background air masses, biomass burning and urban pollution. Comprehensive mass spectral data evaluation methods (e.g. Kendrick mass defect, Van Krevelen diagrams, carbon oxidation state and aromaticity equivalent) were used to identify compound classes and mass distributions of the detected species. Nitrogen-and/or sulfur-containing organic species contributed up to 60% of the total identified number of formulae. A large number of molecular formulae in organic aerosol (OA) were attributed to later-generation nitrogen-and sulfur-containing oxidation products, suggesting that OA composition is affected by biomass burning and other, potentially anthropogenic, sources. Isoprene-derived organosulfate (IEPOX-OS) was found to be the most dominant ion in most of the analysed samples and strongly followed the concentration trends of the gas-phase anthropogenic tracers confirming its mixed anthropogenic-biogenic origin. The presence of oxidised aromatic and nitro-aromatic compounds in the samples suggested a strong influence from biomass burning especially during the dry period. Aerosol samples from the dry period and under enhanced biomass burning conditions contained a large number of molecules with high carbon oxidation state and an increased number of aromatic compounds compared to that from the wet period. The results of this work demonstrate that the studied site is influenced not only by biogenic emissions from the forest but also by biomass burning and potentially other anthropogenic emissions from the neighbouring urban environments.Research at the University of Cambridge was supported by European Research Council grant no. 279405.
