    UTILIZAÇÃO TERAPÊUTICA DO CANABIDIOL NO TRATAMENTO DA ESQUIZOFRENIA

    Dentre os vários compostos identificados na planta Cannabis sativa L. (maconha), destacam-se o Δ9-tetrahidrocanabinol (Δ9-THC) e o canabidiol (CBD). O Δ9-THC é o responsável pelos efeitos psicoativos, incluindo disforia, alucinações e sonolência. Quanto ao CBD, já foi observado que é capaz de antagonizar os efeitos farmacológicos do Δ9-THC, levando à hipótese de que apresentaria uma ação antipsicótica, sugerindo sua utilização no tratamento da esquizofrenia. Deste modo, o presente trabalho teve por objetivo revisar a literatura científica acerca da potencialidade farmacológica do canabidiol no tratamento esquizofrenia através da avaliação dos estudos pré-clínicos e clínicos. Para tanto, realizou-se uma busca nas bases de dados Science Direct, PubMed e Medline, utilizando como descritores de busca: “Cannabis sativa”, “cannabidiol” e “cannabidiol and schizophrenia”. Estudos sugerem que o CBD atua principalmente no sistema endocanabinóide, por agonismo inverso nos receptores CB2, minimizando a ação dos endocanabinóides que, possivelmente, estão em altos níveis nos esquizofrênicos. A administração aguda do CBD não apresentou toxicidade significativa em animais e em humanos e sua administração crônica por um mês não demonstrou nenhuma anormalidade neurológica, psicológica ou clínica em voluntários sadios.     Interdisciplinaridade na gestão do cuidado ao idoso / Interdiscplinarity in elderly care management

    O processo do envelhecimento traz consigo grandes mudanças físicas e psicológicas que favorecem o desenvolvimento de diferentes patologias crônicas e torna-se desafiador para as áreas da saúde. Desse modo, o objetivo deste estudo foi compreender a importância da interdisciplinaridade na gestão do cuidado ao idoso e quais aspectos podem contribuir para qualidade de vida dos pacientes. Tratou-se de uma revisão bibliográfica narrativa nas bases de dados Pubmed, Scielo e Lilacs. Foram selecionados 11 artigos publicados de 2014 em diante. Verificou-se que a alta procura de indivíduos da terceira idade por assistência à saúde é consequência de suas vulnerabilidades que facilitam alterações do estado de saúde e geram restrições no seu cotidiano. Assim, a prática interdisciplinar mostra-se como uma alternativa eficaz e inovadora para suprir necessidades específicas, consiste na assistência integralizada na gestão do cuidado mediante a correlação entre os profissionais com partilhas recíprocas. Diante disso, conclui-se que existe uma fragmentação no cuidado à saúde do idoso e que é de grande relevância a elaboração de práticas articuladas em equipes que envolvam as diversas áreas que compõem a atenção à saúde, visando um atendimento integral das limitações que interferem no bem-estar do idoso, dessa forma, a interdisciplinaridade é essencial para efetivação da assistência desenvolvida

    Reducción del tiempo de tratamiento antimicrobiano en unidades de cuidados intensivos en Fortaleza, Brasil

    The authors are grateful for the contribution of the physicians, pharmacists and patients of the Walter Cantídio University Hospital who collaborated in the conduction of this study.Objective: To evaluate the application of the Antimicrobial (ATM) treatment time reduction strategy in Intensive Care Units (ICU) in an Antimicrobial Stewardship Program (ASP). Method: This is a descriptive and cross-sectional study, carried out in two ICU of a university hospital in Fortaleza, Brazil, from January/2017 to January/2019. Adult patients were included, accompanied by a pharmacist, and using ATM, in which the treatment time reduction strategy was applied. The evaluation of the strategy was made through the difference between the predicted time established at the beginning of the treatment and the effective days of use of each ATM. Results: Of the 100 patients included, 51.0 % were male and 64.0 % were elderly. The respiratory system was the most frequently affected by the infections (37.4 %) and the most prevalent classes of ATM were carbapenems (23.0 %) and glycopeptides (20.1 %). There was a decrease from 831 unnecessary days of antimicrobial therapy and from an average of 13.7 to 8.9 days of treatment. The greatest reductions in days were observed for meropenem, with 202 days reduced. The study also allowed the identification of associations between the reduction > 8 days of treatment and the variables length of stay > 22 days and patients in exclusive palliative care, and associations between hospi tal discharge and reductions of up to 7 days of therapy. Conclusions: The data obtained suggest that the presence of an ASP influences the practices of ATM use and its treatment time and emphasize the role of pharmaceutical professionals in these programs.Objetivo: Evaluar la aplicación de la estrategia reducción del tiempo de tratamiento Antimicrobiano (ATM) en Uni dades de Cuidados Intensivos (UCI) en un Programa Stewardship de Antimicrobiano (ASP).     High anti-SARS-CoV-2 antibody seroconversion rates before the second wave in Manaus, Brazil, and the protective effect of social behaviour measures: results from the prospective DETECTCoV-19 cohort

    Background: The city of Manaus, Brazil, has seen two collapses of the health system due to the COVID-19 pandemic. We report anti-SARS-CoV-2 nucleocapsid IgG antibody seroconversion rates and associated risk factors in Manaus residents before the second wave of the epidemic in Brazil. Methods: A convenience sample of adult (aged ≥18 years) residents of Manaus was recruited through online and university website advertising into the DETECTCoV-19 study cohort. The current analysis of seroconversion included a subgroup of DETECTCoV-19 participants who had at least two serum sample collections separated by at least 4 weeks between Aug 19 and Oct 2, 2020 (visit 1), and Oct 19 and Nov 27, 2020 (visit 2). Those who reported (or had no data on) having a COVID-19 diagnosis before visit 1, and who were positive for anti-SARS-CoV-2 nucleocapsid IgG antibodies at visit 1 were excluded. Using an in-house ELISA, the reactivity index (RI; calculated as the optical density ratio of the sample to the negative control) for serum anti-SARS-CoV-2 nucleocapsid IgG antibodies was measured at both visits. We calculated the incidence of seroconversion (defined as RI values ≤1·5 at visit 1 and ≥1·5 at visit 2, and a ratio >2 between the visit 2 and visit 1 RI values) during the study period, as well as incidence rate ratios (IRRs) through cluster-corrected and adjusted Poisson regression models to analyse associations between seroconversion and variables related to sociodemographic characteristics, health access, comorbidities, COVID-19 exposure, protective behaviours, and symptoms. Findings: 2496 DETECTCoV-19 cohort participants returned for a follow-up visit between Oct 19 and Nov 27, 2020, of whom 204 reported having COVID-19 before the first visit and 24 had no data regarding previous disease status. 559 participants were seropositive for anti-SARS-CoV-2 nucleocapsid IgG antibodies at baseline. Of the remaining 1709 participants who were seronegative at baseline, 71 did not meet the criteria for seroconversion and were excluded from the analyses. Among the remaining 1638 participants who were seronegative at baseline, 214 showed seroconversion at visit 2. The seroconversion incidence was 13·06% (95% CI 11·52–14·79) overall and 6·78% (5·61–8·10) for symptomatic seroconversion, over a median follow-up period of 57 days (IQR 54–61). 48·1% of seroconversion events were estimated to be asymptomatic. The sample had higher proportions of affluent and higher-educated people than those reported for the Manaus city population. In the fully adjusted and corrected model, risk factors for seroconversion before visit 2 were having a COVID-19 case in the household (IRR 1·49 [95% CI 1·21–1·83]), not wearing a mask during contact with a person with COVID-19 (1·25 [1·09–1·45]), relaxation of physical distancing (1·31 [1·05–1·64]), and having flu-like symptoms (1·79 [1·23–2·59]) or a COVID-19 diagnosis (3·57 [2·27–5·63]) between the first and second visits, whereas working remotely was associated with lower incidence (0·74 [0·56–0·97]). Interpretation: An intense infection transmission period preceded the second wave of COVID-19 in Manaus. Several modifiable behaviours increased the risk of seroconversion, including non-compliance with non-pharmaceutical interventions measures such as not wearing a mask during contact, relaxation of protective measures, and non-remote working. Increased testing in high-transmission areas is needed to provide timely information about ongoing transmission and aid appropriate implementation of transmission mitigation measures.
