    DETERMINAN KETERLAMBATAN PENCARIAN PELAYANAN KESEHATAN PADA PENDERITA KANKER PAYUDARA STADIUM LANJUT DI RUANG RB2A RSUP HAJI ADAM MALIK MEDAN TAHUN 2014

    Angka kematian dan kejadian kanker payudara masih tinggi, WHO menyebutkan, pada tahun 2004 angka kematian akibat kanker mencapai 7 juta orang, pada tahun 2020 diperkirakan bisa mencapai 10 juta orang. Angka tersebut masih tinggi karena banyak penderita kanker payudara yang datang ke pelayanan kesehatan saat kankernya sudah stadium lanjut yang menyebabkan kondisi penyakitnya semakin parah dan semakin kecil kemungkinan untuk disembuhkan. Penelitian ini bersifat analitik bertujuan untuk mengetahui determinan yang berhubungan dengan keterlambatan pencarian pelayanan kesehatan penderita kanker payudara. Terlebih dahulu dilakukan penelitian deskriptif untuk mencari data dasar. Desain penelitian yang digunakan rancangan cross sectional. Responden dalam penelitian ini ada sebanyak 30 orang. Dari hasil penelitian diketahui bahwa pasien yang menderita kanker payudara stadium lanjut yang terlambat dalam pencarian pelayanan kesehatan adalah sebanyak 24 orang (80,0%). Adapun determinan yang berhubungan dengan keterlambatan pencarian pelayanan kesehatan penderita kanker payudara adalah faktor predisposisi (pengetahuan, sikap, dan kepercayaan pengobatan), faktor pemungkin (fasilitas pengobatan, biaya, dan jarak tempat pengobatan), faktor penguat (dukungan petugas kesehatan, serta dukungan keluarga dan teman). Dari hasil penelitian didapatkan hasil bahwa determinan yang berhubungan dengan keterlambatan pencarian pelayanan kesehatan penderita kanker payudara adalah faktor pemungkin yaitu fasilitas pengobatan dengan hasil uji statistik chi square diperoleh nilai P=0,04 < 0,05, biaya yang mahal dengan nilai P=0,00<0,05, dan jarak tempat pengobatan yang jauh dari rumah responden dengan nilai p=0,00<0,05. Sedangkan faktor predisposisi dan faktor penguat dalam penelitian ini tidak berhubungan dengan keterlambatan pencarian pelayanan kesehatan penderita kanker payudara. Diharapkan kepada petugas kesehatan untuk lebih menggalakkan penyuluhan kesehatan tentang SADARI agar masyarakat dapat mengetahui cara mendeteksi dini adanya kanker payudara dan faham tentang bahaya kanker payudara, agar keterlambatan pencarian pelayanan kesehatan penderita kanker payudara dapat dihindari.Kata Kunci: Keterlambatan, Pelayanan Kesehatan, Kanker Payudar

    Pengaruh Intervensi Psikoedukasi dalam Mengatasi Depresi Postpartum di sebuah RS di Medan

    Postpartum depression is one type of depression that occur in the mother after delivering baby. This study is a quasi-experimental research, with Non-equivalent Control Group to analyze the influence of psychoeducation intervention in overcoming postpartum depression. Population in this research is amounted to 110 mothers and Research sample is 60 post-partum mothers each group as 30 people who were given treatment, and 30 persons as control group with the stated criteria of postpartum depression. The sampling is purposive sampling. Analysis of data using Pair Test and logistic regression to the extent the confidence level 95%. Results of research shows there are 48.4% mothers had postpartum depression after the psychoeducation intervention. Postpartum depression decrease 65% after the psychoeducation intervention. Pair-test results there is a difference between postpartum depression mother with psychoeducation intervention with a mother who does not get psychoeducation intervention and logistic regression test showed no relationship between age and job with post partum depression and there is a significant relationship postpartum mother education (p = 0.003), mother parity (p = 0.016) and the husband support (p = 0.000) with postpartum depression. Suggested need to improve health education on postpartum mothers in a hospital in Medan, and it is necessary to give education to the family or the husband who accompanies his wife during the labor

    Mothers describe postpartum depression as a living nightmare filled with uncontrollable anxiety, consuming guilt, andobsessive thinking. These conditions do not only harm themselves but also their infants. Mothers are enveloped in lonelinessand the quality of their lives is further compromised by a lack of all positive emotions. Up to 50% of all cases of postpartumdepression go undetected. The Edinburgh Postnatal Depression Scale (EPDS) has been the only instrument available thatwas specifically designed to screen for this mood disorder. The aim of this study is to evaluate risk of postpartum depressiondifferences of primiparous and multiparous mother by using EPDS. This study utilized survey with quantitative approach.Fourty patients were chosen with purposive sampling method. The result of the study especially the independent t-test showsthat there was significant differences of postpartum depression risk of primiparous and multiparous women (p= 0.000; α=0.05). It is recommended to apply the scale of Edinburgh as tools detection of postpartum depression in mothers primiparousand multiparous considering that postpartum depression often occurs without any known beforehand

    Penyakit Diabetes Melitus Tipe 2 (DM Tipe 2) dapat menimbulkan berbagai komplikasi seperti retinopati, nefropati, dan penyakit kardiovaskuler. Dari data yang didapatkan sebanyak 75 persen masyarakat mengalami obesitas (kegemukan) akibat tidak melakukan aktivitas olahraga sehingga presentase nilai kadar gula darah mengalami peningkatan dan beresiko mengalami Diabetes Melitus. Hal ini terjadi di wilayah Pancur Batu dan sekitarnya. Peranan kader dalam kegiatan ini merupakan perpanjangan dari tenaga Kesehatan untuk menyampaikan edukasi Kesehatan terkait skrining Diabetes Melitus dan bagaimana mengatasi serta mencegah Diabetes Melitus. Sasaran kegiatan ini adalah kader Kesehatan dan masyarakat dengan metode yang digunakan yakni ceramah dan praktek langsung dengan memperkenalkan format skrining DM kepada kader disertai dengan sesi tanya jawab antara kader dan pengabdi masyarakat. Hasil kegiatan pengabdian ini adalah respon dan tanggap kader dan masyarakat dalam melakukan pemberdayaan Diabetes Melitus tipe 2 di wilayah puskesmas Pancur Batu. Kegiatan ini mendapatkan antusias yang baik dari kader dan masyarakat ditandai dengan respon yang baik dari 6 kader dan 30 masyarakat yang menderita DM Tipe 2 dengan memotivasi masyarakat dalam mengendalikan dan mencegah DM itu sendiri

    Funktion und Design einer Ergänzungsstichprobe für das Sozio-oekonomische Panel (SOEP)

    In the year 1998 after 15 years of running the German Socio-economic Panel Study (GSOEP) well be extended by a "refreshment sample" (subsample E). The aim of this new sample is threefold: (a) stabilization of the number of cases, (b) in-depth analysis of potential "panel-effects", and (c) some extended analysis of the "representativeness" of the overall GSOEP.This paper discusses in detail the design of the refreshment sample. In addition the pros and cons of the emerging new "interview technology" by means of laptops are discussed (Computer Assisted Personal Interviewing [CAPI]). Finaly the concept of introducing CAPI into GSOEP is presented. Nach 15 Jahren Laufzeit wird das "Sozio-oekonomische Panel" (SOEP) im Jahre 1998 erstmals um eine Ergänzungsstichprobe (Teilstichprobe E) erweitert. Mit dieser Teilstichprobe werden drei Hauptziele verfolgt. Neben der Stabilisierungsfunktion für die Fallzahl soll die Ergänzungsstichprobe vor allem zur Analyse von Panel-Effekten sowie zur Verbesserung der Repräsentativitätsanalysen des gesamten SOEP genutzt werden. In diesem Papier wird das Design der Ergänzungsstichprobe ausführlich diskutiert, die dieselbe Grundgesamtheit nachbilden wird, wie die Alt-Stichproben A bis D des SOEP. Weiterhin wird der Einstieg in eine neue Befragungstechnologie (Computer Assisted Personal Interviewing [CAPI]) beim SOEP diskutiert und es wird der "Umstiegspfad" in einen komplexeren Methodenmix vorgestellt.

Although they may disavow politics as such, civic-minded young people use every means and media at their disposal to carry out the basic tasks of citizenship. Through a mix of face-to-face and digital methods, they deliberate on important issues and debate with peers and powerbrokers, redefining some key dynamics that govern civic life in the process. In Participatory Politics, Elisabeth Soep examines the specific tactics used by young people as they experiment with civic engagement. Drawing on her scholarly research and on her work as a media producer and educator, Soep identifies five tactics that are part of effective, equitable participatory politics among young people: Pivot Your Public (mobilizing civic capacity within popular culture engagements); Create Content Worlds (using inventive and interactive storytelling that sparks sharing); Forage for Information in public data archives; Code Up (using computational thinking to design tools, platforms, and spaces for public good); and Hide and Seek (protecting privacy and information sources). After describing these tactics as they manifest themselves in a range of youth-driven activities—from the runaway spread of the video Kony 2012 to community hackathons—Soep discusses concrete ideas for cultivating the new literacies that will enable young people to participate in public life. She goes on to consider some risks associated with these participatory tactics, including simplification and sensationalism, and ways to avoid them, and concludes with implications for future research and practice

    New guidelines for the administration of amikacin are presented for neonates, children and adults, with normal or impaired, stable or changing renal function. The dose is calculated according to the lean body mass, which can be estimated from weight, height and sex of the patient. The dosing interval is calculated from the predicted drug half-life, which is derived from creatinine clearance normalized to 50 kg lean body mass (LBM). A formula is presented to estimate directly the Ccr in ml/min per 50 kg LBM, based only upon serum creatinine concentrations and the age of the subject. Two electronic pocket calculators which contain the dosage guidelines of amikacin are also described.     The digital afterlife of youth-made media : implications for media literacy education

    La era digital ha reconfigurado las relaciones entre productores y usuarios. Las acciones en red son potencialmente reinterpretadas por otros usuarios. La intención de los productores mediáticos adquiere su sentido en este contexto que denominamos «el más allá digital». En este artículo analizamos cómo ha influido este fenómeno en las implicaciones de los medios y los jóvenes. El estudio se lleva a cabo a través del análisis etnográfico de las actividades llevadas a cabo por un grupo de jóvenes que trabajan creando productos mediáticos de gran impacto en una radio en California (Youth Radio). El estudio de caso que se incluye en este artículo se centra en una iniciativa de periodismo de investigación dentro de Youth Radio: una serie transmediática sobre el tráfico sexual infantil producida por un joven de 21 años con la ayuda de editores más veteranos. El análisis desvela la forma en que los medios «juveniles» dejan de serlo una vez que se convierten en un producto digital en sí, cuyo contenido se reproduce una y otra vez en manos de adultos e instituciones con su propia historia, organización y economía política. El artículo concluye identificando dimensiones clave de alfabetización que los jóvenes crean y emplean a través de su experimentación con los medios móviles y sociales, entre otros: el descubrimiento, el análisis, la movilización en red y la programación de plataformas.The digital age has fundamentally re-configured the relationship between makers and users. Every networked action by a user has the potential to be reinterpreted by other users. The original intentions of media makers emerge from this process in recontextualized form that I call the «digital afterlife». The phenomenon of digital afterlife has striking implications for youth-made media, which I explore in this article through an ethnographic analysis of behind-the-scenes activities among a group of young people working with Youth Radio, a California youth organization, where they create high-impact media. The case study examined here centers on a major investigative reporting initiative within Youth Radio – a transmedia series on child sex trafficking produced by a 21-year-old reporter in collaboration with veteran editors. The analysis reveals the ways in which youth media ceases to be «youth media», once it moves into its digital afterlife, given the extent to which the content gets re-produced, again and again, by adult institutions with their own histories, agendas, and political economies. The article concludes by identifying key dimensions of literacy that young people invent and deploy through their experiments with social and mobile media, including: discovery, analytics, network mobilization, and platform programming
