    Unwanted pregnancy and induced abortion in Rajasthan, India: A qualitative exploration

    As part of a Population Council program of research on unwanted pregnancy and induced abortion in Rajasthan, the Council and Ibtada conducted a qualitative exploration of attitudes and behaviors regarding unwanted pregnancy and induced abortion in Alwar district. The study was intended to lay the groundwork for two quantitative studies on abortion undertaken subsequently in six districts of Rajasthan. The qualitative exploration shows that women, particularly those who are poor, turn to largely untrained community-level providers for abortion services. Additionally, women use home remedies in an often unsuccessful attempt to terminate unwanted pregnancies. Women with greater financial means obtain surgical services from a private gynecologist. The remaining women are left with little choice but to avail of services from informal providers that they often recognize to be unsafe and/or to carry unwanted pregnancies to term. This report encourages innovative means to improve access to legal, safe, and effective abortion services at lower levels of the public health system, and suggests that the feasibility of training certain informal providers to offer safe abortion services, particularly at early gestations, should be explored at the policy, program, and research levels

    Field validation of a farmer supplied data approach to close soybean yield gaps in the US North Central region

    CONTEXT: Producer-reported data can be used to identify suites of management practices that lead to higher yield and profit. However, a rigorous validation of the approach in relation to its potential impact on farmer yield and profit is lacking. OBJECTIVE: This study aimed to validate a producer-data approach on its capability to guide on-farm evaluation of management practices with greatest potential for increasing producer yield and profit. We show proof of concept using soybean in the North Central US region as a case study. METHODS: We used a combination of regression tree analysis and a spatial framework to determine practices with highest influence on yield for specific climate domains across the region. These practices were used as a basis for designing an ‘improved’ management package for each domain. The impact associated with adoption of the ‘improved’ management package on producer yield, seed constituents, and profit was evaluated against a ‘reference’ treatment that follows farmer management via replicated on-farm trials across 100 sites over two crop seasons. RESULTS AND CONCLUSIONS: Average yield was 278 kg ha-1 higher in the improved versus reference management, equivalent to a closure of the current exploitable yield gap by 40%. In turn, adoption of the improved management led to an average increase of $76 ha-1 in net profit. Sensitivity analysis showed that adoption of the improved management packages should increase farmer profit across a wide range of grain price scenarios, with very small downside risk. Seed protein concentration was negatively associated with the positive yield advantage of the improved management, whereas seed oil concentration tended to increase. SIGNIFICANCE: Analysis of producer data can accelerate discovery, evaluation, and adoption of suites of management practices that consistently lead to higher farmer yield and profit, which, in turn, would help speed up current rates of yield gain

    Enhanced Neutrophil Extracellular Trap Formation in Acute Pancreatitis Contributes to Disease Severity and Is Reduced by Chloroquine

    Background: Neutrophil extracellular traps (NETs) are generated when activated neutrophils, driven by PAD4, release their DNA, histones, HMGB1, and other intracellular granule components. NETs play a role in acute pancreatitis, worsening pancreatic inflammation, and promoting pancreatic duct obstruction. The autophagy inhibitor chloroquine (CQ) inhibits NET formation; therefore, we investigated the impact of CQ mediated NET inhibition in murine models of pancreatitis and human correlative studies. Methods: L-arginine and choline deficient ethionine supplemented (CDE) diet models of acute pancreatitis were studied in wild type and PAD4−/− mice, incapable of forming NETs. Isolated neutrophils were stimulated to induce NET formation and visualized with fluorescence microscopy. CQ treatment (0.5 mg/ml PO) was initiated after induction of pancreatitis. Biomarkers of NET formation, including cell-free DNA, citrullinated histone H3 (CitH3), and MPO-DNA conjugates were measured in murine serum and correlative human patient serum samples. Results: We first confirmed the role of NETs in the pathophysiology of acute pancreatitis by demonstrating that PAD4−/− mice had decreased pancreatitis severity and improved survival compared to wild-type controls. Furthermore, patients with severe acute pancreatitis had elevated levels of cell-free DNA and MPO-DNA conjugates, consistent with NET formation. Neutrophils from mice with pancreatitis were more prone to NET formation and CQ decreased this propensity to form NETs. CQ significantly reduced serum cell-free DNA and citrullinated histone H3 in murine models of pancreatitis, increasing survival in both models. Conclusions: Inhibition of NETs with CQ decreases the severity of acute pancreatitis and improves survival. Translating these findings into clinical trials of acute pancreatitis is warranted

    High rate, fast timing Glass RPC for the high {\eta} CMS muon detectors

    The HL-LHC phase is designed to increase by an order of magnitude the amount of data to be collected by the LHC experiments. To achieve this goal in a reasonable time scale the instantaneous luminosity would also increase by an order of magnitude up to 6.1034cm2s16.10^{34} cm^{-2} s^{-1} . The region of the forward muon spectrometer (η>1.6|{\eta}| > 1.6) is not equipped with RPC stations. The increase of the expected particles rate up to 2kHz/cm22 kHz/cm^{2} (including a safety factor 3) motivates the installation of RPC chambers to guarantee redundancy with the CSC chambers already present. The actual RPC technology of CMS cannot sustain the expected background level. The new technology that will be chosen should have a high rate capability and provides a good spatial and timing resolution. A new generation of Glass-RPC (GRPC) using low-resistivity (LR) glass is proposed to equip at least the two most far away of the four high η{\eta} muon stations of CMS. First the design of small size prototypes and studies of their performance in high-rate particles flux is presented.     Unusual manifestation of Erdheim-Chester disease

    <p>Abstract</p> <p>Background</p> <p>Erdheim-Chester disease (ECD) is a rare multisystem non-Langerhans cell histiocytosis that is characterized histologically by xanthogranulomatous infiltrates and radiologically by symmetrical sclerosis of long bones. The xanthomatous process is characterized by prominent foamy histiocytes staining positive for CD68, occasionally for PS100 and negative for S100 and CD1a. Gastroenterological involvement is exceedingly rare.</p> <p>Case Presentation</p> <p>This case report describes the case of a 69-year-old man who presented otherwise well to the gastroenterology department with unspecific abdominal symptoms, nausea, vomiting and weight loss. ECD involving the gastrointestinal tract was confirmed clinically, radiologically and histologically.</p> <p>Conclusion</p> <p>Gastroenterological manifestation of ECD is rare but should be considered in the differential diagnosis in patients presenting with evidence of multi-organ disease and typical radiological features of Erdheim-Chester disease elsewhere.</p

    HPV-Associated Head and Neck Cancer: Molecular and Nano-Scale Markers for Prognosis and Therapeutic Stratification

    Over the last 10 years, it has become clear that patients with head and neck cancer can be stratified into two distinct subgroups on the basis of the etiology of their disease. Patients with human papillomavirus-related cancers have significantly better survival rates and may necessitate different therapeutic approaches than those with tobacco and/or alcohol related cancers. This review discusses the various biomarkers currently in use for identification of patients with HPV-positive cancers with a focus on the advantages and limitations of molecular and nano-scale markers

    Endoscopic Ultrasound and Related Technologies for the Diagnosis and Treatment of Pancreatic Disease - Research Gaps and Opportunities: Summary of a National Institute of Diabetes and Digestive and Kidney Diseases Workshop

    A workshop was sponsored by the National Institute of Diabetes and Digestive and Kidney Diseases to address the research gaps and opportunities in pancreatic endoscopic ultrasound (EUS). The event occurred on July 26, 2017 in 4 sessions: (1) benign pancreatic diseases, (2) high-risk pancreatic diseases, (3) diagnostic and therapeutics, and (4) new technologies. The current state of knowledge was reviewed, with identification of numerous gaps in knowledge and research needs. Common themes included the need for large multicenter consortia of various pancreatic diseases to facilitate meaningful research of these entities; to standardize EUS features of different pancreatic disorders, the technique of sampling pancreatic lesions, and the performance of various therapeutic EUS procedures; and to identify high-risk disease early at the cellular level before macroscopic disease develops. The need for specialized tools and accessories to enable the safe and effective performance of therapeutic EUS procedures also was discussed
