    Lo‐tech tools as episteme: rethinking student engagement in the writing process and beyond

    In this paper, five teacher‐scholars describe pedagogical inquiry into the use of ‘lo‐tech’ tools and what we discovered about the affordances of these tools. These include but are not limited to technologies like sticky notes that help students to organize written thoughts and physically move them around, crayons that allow students to highlight, trace, and categorize different types of thoughts on their paper, and index cards that they can use in a variety of interactive ways for their own writing and to write collaboratively. We found that the use of lo‐tech tools complemented our work with digital technology, engaging the kinesthetic learners in our classrooms and encouraging a spirit of play in students and teachers alike. We also discuss how teachers can encourage the use of lo‐tech tools epistemologically to help students process information, create knowledge, and to come to their own understandings or demonstrate understandings of course content ‐ with no product in mind other than knowledge‐making

    High-Stakes Accountability Systems: Creating Cultures of Fear

    A phenomenological case study by Counsell (2007) explored and investigated the phenomenon of what happened as Florida’s A+ Plan intersected with the life histories of one beginning African American female third grade teacher and one veteran White female third grade teacher at demographically different school communities in one Florida school district. Habermas’ theory of communicative action served as an instructive framework used to examine and contemplate two key emerging trends based on lived experiences of the various social (communicative) actors. In addition to the beginning and veteran third grade teachers, other participating social actors included third graders, their parents, other third grade teachers and school principals at the beginning and veteran teachers’ schools as well as urban school teachers and personnel at urban schools in the same school district. Across the different social actors at each respective middle-income, low-income, and high-poverty school community, the emerging trends and patterns revealed: (a) a continuum of moral and ethical dilemmas specifically and (b) an overall continuum of fear in general. Social actors’ varied lived experiences with fear in relation to high-stakes testing illuminated the following: (a) a fear of speaking out; (b) a fear regarding children’s emotional welfare; (c) fear as it intersects with race; and (d) a fear of taking the test. From these testimonials, the continuum of fear toward the FCAT (according to third grade student reflections), proved to be the most serious (if not detrimental) consequence of Florida’s high-stakes accountability system. These insights can help guide and inform future accountability decisions under the new Every Student Succeeds Act

    Evaluation of the Safety of MRI Scanning of Patients with MR Conditional Pacemakers

    Magnetic resonance imaging (MRI) of patients with cardiac implantable electronic devices (CIED) has been associated with risks such as device/lead movement, device dysfunction, and lead heating. New technological advancements have made it possible for MRI to be safely performed when adhering to an evidence-based protocol; however, this practice has not yet been widely adopted. The purpose of this practice-focused question project was to examine the safety of MRI as a diagnostic modality for the aggregate population of adult patients with MR conditional pacemakers when a nurse-practitioner-led, evidence-based protocol was used. The Iowa model served as the guide for implementation of the program, and the Donabedian framework was used to evaluate the program through process, structure, and outcomes. Evidence was obtained through a documentation template that served as the procedural record in the electronic health record. Demographic information, program fidelity, and manufacturer adherence were analyzed through descriptive statistics. Clinical outcomes related to device function were measured pre- and post- MRI and analyzed with chi square and paired t-test inferential statistics to determine if statistically significant change occurs in the setting of MRI scanning. According to data analysis of 34 studies, there were no statistically significant changes in lead impedance, pacing thresholds, or patient reported symptoms pre- and post- MRI. The pilot program has been recommended for organizational adoption and will increase the scope of advanced practice nurses within the organization and provide the CIED aggregate population with access to an important diagnostic modality

    A cross-sectional feasibility study of nutrient intake patterns in people with Parkinson’s compared to government nutrition guidelines

    Background: Diet could have implications for disease progression and management in people with Parkinson’s disease (PwP). However, the knowledge of diet intake patterns in PwP is limited. Objectives: We set out to assess the feasibility of collecting diet data in PwP to determine food and nutrient intake, in order to compare to national nutrition guidelines and thus understand the habits in this population. Methods: In this cross-sectional feasibility study, PwP were approached through local support groups throughout the Thames Valley and were asked to complete a Food Frequency Questionnaire. Eligibility criteria included a self-reported neurologist confirmed diagnosis of Parkinson’s disease. Completeness of questionnaires was reported and 80% was considered appropriate for each measure including demographic information. Results: Response rate was 61% and missing data rate for the 121 returned questionnaires was 74%; however, of the 90 used for analysis there was 100% completion of the questionnaires. Compared to the UK government guidelines, protein was significantly higher for both males and females and fluid intake was lower for both genders (p < 0.001). There were several other differences in nutrient intake compared to guidelines. Conclusion: We observed high levels of engagement from PwP and found that assessing food and nutrient patterns in PwP was feasible. Importantly, the diet was generally healthy overall, yet there were specific nutrients that may affect medication metabolism in PwP that were found to be high. Therefore further research into this emerging and important area is warranted

    Successful Surgical Treatment of a Patient with a Solitary Asymptomatic Cardiac Metastasis from Breast Cancer, Identified by Elevated Tumor Markers and Circulating Tumor Cells

    Indroduction: Cardiac metastases are a not infrequent autopsy finding in patients dying of metastatic cancer, but are less commonly diagnosed during life (1). Although the autopsy incidence of cardiac metastases ranges may be as high as 25%, solitary cardiac metastases in the absence of metastatic involvement of other organs are rare (2).Case Presentation: We report here the case of a 66-year old woman with a history of bilateral breast cancer, where a solitary metastasis in the right atrium was successfully resected.Conclusion: In the absence of any symptoms or clinical findings on physical examination, the presence of metastatic disease was first suggested by the detection of elevated tumor markers and circulating tumor cells during routine follow up after treatment for early stage breast cancer.

    Measurements of Submicron Particle Adsorption and Particle Film Elasticity at Oil-Water Interfaces

    The influence of particle adsorption on liquid/liquid interfacial tension is not well understood, and much previous research has suggested conflicting behaviors. In this paper we investigate the surface activity and adsorption kinetics of charge stabilized and pH-responsive polymer stabilized colloids at oil/water interfaces using two tensiometry techniques: (i) pendant drop and (ii) microtensiometer. We found, using both techniques, that charge stabilized particles had little or no influence on the (dynamic) interfacial tension, although dense silica particles affected the "apparent" measured tension in the pendent drop, due to gravity driven elongation of the droplet profile. Nevertheless, this apparent change additionally allowed the study of adsorption kinetics, which was related qualitatively between particle systems by estimated diffusion coefficients. Significant and real interfacial tension responses were measured using ∼53 nm core-shell latex particles with a pH-responsive polymer stabilizer of poly(methyl methacrylate)-b-poly(2-(dimethylamino)ethyl methacrylate) (pMMA-b-pDMAEMA) diblock copolymer. At pH 2, where the polymer is strongly charged, behavior was similar to that of the bare charge-stabilized particles, showing little change in the interfacial tension. At pH 10, where the polymer is discharged and poorly soluble in water, a significant decrease in the measured interfacial tension commensurate with strong adsorption at the oil-water interface was seen, which was similar in magnitude to the surface activity of the free polymer. These results were both confirmed through droplet profile and microtensiometry experiments. Dilational elasticity measurements were also performed by oscillation of the droplet; again, changes in interfacial tension with droplet oscillation were only seen with the responsive particles at pH 10. Frequency sweeps were performed to ascertain the dilational elasticity modulus, with measured values being significantly higher than previously reported for nanoparticle and surfactant systems, and similar in magnitude to protein stabilized droplets.</p

    Standardizing Terminology and Assessment for Orofacial Conditions in Juvenile Idiopathic Arthritis : International, Multidisciplinary Consensus-based Recommendations

    Objective. To propose multidisciplinary, consensus-based, standardization of operational terminology and method of assessment for temporomandibular joint (TMJ) involvement in juvenile idiopathic arthritis (JIA). Methods. Using a sequential expert group–defined terminology and methods-of-assessment approach by (1) establishment of task force, (2) item generation, (3) working group consensus, (4) external expert content validity testing, and (5) multidisciplinary group of experts final Delphi survey consensus. Results. Seven standardized operational terms were defined: TMJ arthritis, TMJ involvement, TMJ arthritis management, dentofacial deformity, TMJ deformity, TMJ symptoms, and TMJ dysfunction. Conclusion. Definition of 7 operational standardized terms provides an optimal platform for communication across healthcare providers involved in JIA-TMJ arthritis management.publishersversionPeer reviewe
