    EPIdemiology of Surgery-Associated Acute Kidney Injury (EPIS-AKI) : Study protocol for a multicentre, observational trial

    More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University MĂŒnster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials. Trial registration number NCT04165369

    Morphological, physiological and behavioural evaluation of a ‘Mice in Space’ housing system

    Environmental conditions likely affect physiology and behaviour of mice used for life sciences research on Earth or in Space. Here, we analysed the effects of cage confinement on the weightbearing musculoskeletal system, behaviour and stress of wild-type mice (C57BL/6JRj, 30 g b.wt., total n = 24) housed for 25 days in a prototypical ground-based and fully automated life support habitat device called “Mice in Space” (MIS). Compared with control housing (individually ventilated cages) the MIS mice revealed no significant changes in soleus muscle size and myofiber distribution (type I vs. II) and quality of bone (3-D microarchitecture and mineralisation of calvaria, spine and femur) determined by confocal and micro-computed tomography. Corticosterone metabolism measured non-invasively (faeces) monitored elevated adrenocortical activity at only start of the MIS cage confinement (day 1). Behavioural tests (i.e., grip strength, rotarod, L/D box, elevated plus-maze, open field, aggressiveness) performed subsequently revealed only minor changes in motor performance (MIS vs. controls). The MIS habitat will not, on its own, produce major effects that could confound interpretation of data induced by microgravity exposure during spaceflight. Our results may be even more helpful in developing multidisciplinary protocols with adequate scenarios addressing molecular to systems levels using mice of various genetic phenotypes in many laboratories

    Xeroderma Pigmentosum Groups C and A in Algerian Patients with Deregulation of both Transcription and DNA Repair

    International audienceXeroderma Pigmentosum (XP) is a rare autosomal recessive disorder characterized by an extreme sensitivity to UV rays from sunlight, a high incidence of skin cancer and occasional neurological symptoms. XP, primarily defined as a DNA repair syndrome, has been found associated with defects in the Nucleotide Excision Repair (NER) pathway, and more recently by transcriptional deregulation. XP results from mutations in eight genes (XPA to XPG and XPV) coding for proteins involved in NER. Abstract We report here two cases of XP patients from Algeria, describe their clinical features, identify the causative mutations, and molecularly define their etiology. We determined that each XP individual bears XPC and XPA mutations respectively. Both mutations disrupt expression of their corresponding genes: while the XPC p.Val548Alafs*25 variant was not expressed, the truncated XPA p.Arg228* variant was detected in the patient's cells. Unscheduled DNA synthesis (UDS) and Recovery of RNA synthesis after DNA damage (RRS) assays, as well as immunofluorescence on Ultraviolet-irradiated patient cells showed deficiency in the NER pathway. Moreover, we also found that the patients' cells were defective in transcription, especially certain Retinoic-acid receptor (RAR)-responsive genes. Altogether our data revealed both DNA repair and transcriptional defaults that defined the molecular etiology for these two XP individuals, and may help to understand some of the patients' clinical features

    Thermal-infrared remote sensing of surface water-groundwater exchanges in a restored anastomosing channel (Upper Rhine River, France)

    Ecohydrological processes are a key element to consider in functional river restorations. In the framework of a LIFE+ European restoration program, we have investigated the potential for airborne thermal‐infrared remote sensing to map surface water–groundwater exchanges and to identify their driving factors. We focused our attention on anastomosing channels on an artificial island of the Upper Rhine River (Rohrschollen), where a new channel was excavated from the floodplain to reconnect an older channel in its upstream part. These hydraulic engineering works led to an increased inflow from the Rhine Canal. Here, we propose an original data treatment chain to (a) georeference the thermal‐infrared images in geographic information system based on visible images, (b) detect and correct data errors, and (c) identify and locate thermal anomalies attributed to groundwater inputs and hyporheic upwellings. Our results, which have been compared to morpho‐sedimentary data, show that groundwater upwelling in the new channel is controlled by riffle–pool sequences and bars. This channel is characterized by large bedload transport and morphodynamic activity, forming riffles and bars. In the old channel, where riffle–pool sequences no longer exist, due to impacts of engineering works and insufficient morphodynamic effects of the restoration, thermal anomalies appeared to be less pronounced. Groundwater inputs seem to be controlled by former gravel bars outcropping on the banks, as well as by local thinning of the low‐permeability clogging layer on the channel bed
