Com a utilização de lentes intraoculares (LIOs) asféricas na cirurgia de catarata, o oftalmologista é capaz de oferecer, além da melhora da acuidade visual pela remoção do cristalino opacificado, benefícios adicionais como, por exemplo, a redução de aberrações ópticas oculares, que diminuem a sensibilidade ao contraste.Foram revisadas publicações recentes sobre LIOs asféricas e adicionadas conclusões de nossos estudos na área, a fim de expor neste artigo nossa percepção a respeito da influência da utilização de LIOs que diminuem a aberração esférica do olho, na qualidade visual após a cirurgia da catarata.With aspherical intraocular lens (IOL), phakectomy could be more than removal of cataract, it also could means an improvement in optical quality of the eye. Role of Optical Coherence Tomography on Corneal Surface Laser Ablation
This paper focuses on reviewing the roles of optical coherence tomography (OCT) on corneal surface laser ablation procedures. OCT is an optical imaging modality that uses low-coherence interferometry to provide noninvasive cross-sectional imaging of tissue microstructure in vivo. There are two types of OCTs, each with transverse and axial spatial resolutions of a few micrometers: the time-domain and the fourier-domain OCTs. Both have been increasingly used by refractive surgeons and have specific advantages. Which of the current imaging instruments is a better choice depends on the specific application. in laser in situ keratomileusis (LASIK) and in excimer laser phototherapeutic keratectomy (PTK), OCT can be used to assess corneal characteristics and guide treatment decisions. OCT accurately measures central corneal thickness, evaluates the regularity of LASIK flaps, and quantifies flap and residual stromal bed thickness. Changes on the Amsler grid test in patients with senile cataract
OBJETIVO: Verificar a influência de alguns tipos de opacidade lenticular no teste da tela de Amsler entre portadores de catarata senil. MÉTODOS:Foram avaliados 200 pacientes portadores de catarata senil através da aplicação da tela de Amsler, antes e após a cirurgia. O estudo foi feito em um único centro, simples cego, randomizado e prospectivo. Os pacientes foram submetidos a exame de acuidade visual (AV) corrigida, para longe e perto, biomicroscopia do segmento anterior e da região macular. RESULTADOS:A acuidade visual média pré-operatória para longe dos 200 pacientes foi de 0,5 logMAR (20/60 Tabela de Snellen). Após a cirurgia, dos 11 pacientes que haviam apresentado alteração na tela de Amsler pré-operatória, dez não tiveram alteração no exame de reavaliação da tela. Os 10 indivíduos apresentaram acuidade visual pós-operatória, com a melhor correção óptica, para distância de 20/20 e para perto de J1. No presente estudo foi constatado 5,5% com alteração na tela de Amsler antes da cirurgia, em 5% dos pacientes estas alterações estavam diretamente relacionados a opacidade do cristalino e 0,5% relacionados a opacidade vítrea. CONCLUSÃO: Conclui-se que a alteração no exame da tela de Amsler pode ser diretamente relacionada à catarata, na ausência de alterações maculares clínicas.PURPOSE: To assess the influence of some forms of lenticular opacification in the Amsler grid test among patients suffering from senile cataract. METHODS: Randomized, prospective interventional trial. Two hundred patients with senile cataract were evaluated with Amsler grid, measurement of best corrected visual acuity for near distance and far distance, biomicroscopy of the anterior segment and macular region. RESULTS: With a two months minimum of follow-up, the average visual acuity after surgery for far distance among the 200 patients was 0.48 logMAR, 11 (5.5%) had changes in the Amsler grid test before surgery. After surgery, from the 11 subjects with changes in the preoperative Amsler grid test, 10 had no more changes in the Amsler grid. These 10 subjects had postoperative best corrected visual acuity of 20/20, and Jaeger 1, no abnormalities of the fundus having been detected. Under this study conditions, the prevalence of changes in the Amsler grid test was 5.5%, and in 5% of the patients these changes were likely related to cataract whereas in 0.5% they were related to vitreous opacification. CONCLUSION: Some patient with cataract showed changes in the Amsler grid test that can be directly related to cataract, in the absence of clinically detected macular disorders
Mini-rhexis for white intumescent cataracts
PURPOSE: To compare the intraoperative safety of two techniques of capsulorhexis for intumescent white cataracts: traditional one-stage continuous curvilinear capsulorhexis and two-stage continuous curvilinear capsulorhexis. METHODS: This prospective comparative randomized study included two groups: the 1-CCC group (11 patients) received traditional one-stage continuous curvilinear capsulorhexis with 5-6 mm diameter, and the 2-CCC (13 patients) group received a deliberately small continuous curvilinear capsulorhexis that was secondarily enlarged, or a two-stage continuous curvilinear capsulorhexis. Patients were stratified according to cataract subset, which was characterized echographically. Six patients were considered as type 1, fifteen as type 2 and three as type 3. Type 1 included intumescent white cataracts with cortex liquefaction and extensive internal acoustic reflections, type 2 included white cataracts with voluminous nuclei, a small amount of whitish solid cortex, and minimal internal acoustic reflections, and type 3 included white cataracts with fibrous anterior capsules and few internal echo spikes. RESULTS: With the one-stage technique, 46.15% of patients had leakage of the liquefied cortex; in addition, the surgeon perceived high intracapsular pressure in 61.53% of cases. Anterior capsule tears occurred in 23.07% of cases, discontinuity of capsulorhexis in 30.79% of cases and no posterior capsular rupture occurred. With the two-stage technique, leakage of the liquefied cortex occurred in 45.45% of cases; additionally, the surgeon perceived high intracapsular pressure in 36.36% of cases. No anterior capsule tears, discontinuity of capsulorhexis or posterior capsular rupture occurred. Considering each cataract subset, there was a higher incidence of leakage for type 2 as compared to types 1 and 3. CONCLUSIONS: Two-stage continuous curvilinear capsulorhexis helps prevent unexpected radial tears of the initial capsulotomy from high intracapsular pressure, sudden radialization of the CCC and other intraoperative complications due to high intracapsular pressure, thus providing a safe cataract surgery in cases of white cataracts. Determination of Force Corresponding to Maximal Lactate Steady State in Tethered Swimming
The main aim of the present investigation was to verify if the aerobic capacity (AC) measured in tethered swimming corresponds to the maximal lactate steady state (MLSS) and its correlation with 30 min and 400m free style swimming. Twenty-five swimmers were submitted to an incremental tethered swimming test (ITS) with an initial load of 20N and increments of 10N each 3min. After each stage of 3min, the athletes had 30s of interval to blood sample collections that were used to measure blood lactate concentrations ([La-]). The ACBI was determined by the abrupt increase in [La-] versus force (F). The points obtained between [La-] versus force (N) were adjusted by an exponential curve model to determine AC corresponding to 3.5mmol.l-1 (AC3.5) and 4.0mmol.l-1 (AC4.0). After these procedures, the swimmers performed maximal efforts of 30min and 400m in free style swimming. We used the distance performed in 30min and the time performed in 400m to calculate the median velocities (i.e. V30 and V400) of these protocols. After one week, in order to measure the MLSS, nine athletes performed three 30-min tethered swimming efforts with intensities of 90, 100, and 110% of ACBI. The ANOVA one-way was used to compare the ACBI, AC3.5 and AC4.0. Correlations between ACs, and between ACs and V30 and V400 (p\u3c0.05) were determined using the Pearson’s correlation coefficient. The intensity corresponding to 100% of ACBI was similar to the MLSS. It was observed significant correlations of the aerobic capacities (i.e. ACBI, AC3.5 and AC4.0) with V30 (r\u3e0.91) and V400 (r\u3e0.63). According to our results, it is possible to conclude that the ACBI corresponds to the MLSS, and both the AC - individually determined - and the AC - determined using fixed blood lactate concentrations of 3.5 and 4.0mmol.l-1 - can be used to predict the mean velocity of 30min and 400m in free style swimming. In addition to that, the tethered swimming system can be used for aerobic development in places where official sized swimming pools are not available, such as rehabilitation clinics and health clubs
Profile of patient with aphakic/pseudopfakic bullous keratopaty attended at public hospital
Objetivo: Identificar e analisar criticamente o perfil dos pacientes com ceratopatia bolhosa atendidos em um hospital municipal. Métodos: Foi realizada análise retrospectiva de 35 olhos de 35 pacientes em acompanhamento no setor de córnea e doenças externas do Hospital da Piedade, que apresentavam ceratopatia bolhosa pós-facectomia. Foram excluídos aqueles onde não se conseguiu determinar características pré e per-operatórias fundamentais em nossa pesquisa. Esta seleção de pacientes foi realizada durante o período de janeiro de 2007 a junho de 2007. Resultados: Como principais resultadosobservou-se que a ceratopatia bolhosa ocorreu em 22.86 % (8 olhos) dos casos pós-facectomia extracapsular (FEC) e 74.28 % (26 olhos) pós- facoemulsificação. Em 25.71 % (9 olhos) foi documentado rotura de cápsula posterior (RCP) com perda vítrea. Em 94.29 % (33 olhos) a lente intraocular se localizava na câmara posterior, e dois pacientes estavam afácicos. A acuidade visual de todos os pacientes era pior que 20/200. O tempo médio de evolução entre a facectomia e o diagnostico de ceratopatia bolhosa foi 11,93 meses (1 - 44 meses). Conclusão: A análise adequada do endotélio corneano é condição essencial para o sucesso da cirurgia de catarata com transparência corneal, e que a ceratopatia bolhosa parece estar mais associada à facoemulsificação. Esta informação deve ser interpretada com cautela, já que esta relação pode vir do maior numero de facoemulsificações realizadas no Hospital onde foi realizado o estudo e muitas destas por cirurgiões em treinamento.Objective: Analyze and identify critically the profile of patients with aphakic/pseudophakic bullous keratopathy attended at a public hospital. Methods: Retrospective analysis of 35 eyes of 35 patients with diagnosis of bullous keratopathy, which has been followed in the department of cornea and external diseases of Hospital da Piedade. The one whose pre and per operative data we did not find were excluded. This selection of patients was done from January to June of 2007. Results: As main results we found that bullous keratopathy occurred in 22.86 % (8 eyes) of cases after extracapsular phakectomy and 74.28 % (26 eyes) after phacoemulsification. Rupture of posterior capsule and vitreous loss was documented in 25.71% (9 eyes). In 94.29% (33eyes) the intraocular lenses was in posterior chamber, and two patients were aphakic. The visual acuity of all patients was worse than 20/200. The mean time from phakectomy to diagnosis of bullous keratopathy was 11.93 months (1-44 months). Conclusion: Adequate analysis of corneal endothelium is an essential condition to obtain success of cataract surgery with corneal transparency, and bullous keratopathy seems to be more related to phacoemulsification. That should be read with caution since this relation may occur because of the higher number of phacoemulsifications that are practiced in that Hospital where the study was conducted and many of them was performed by training surgeons

She graduated in Music and is recognized as the first woman to play the French horn in Brazil. Astrid worked in illustration and landscape representation and now calls herself a painter. In the conversation, she recounts, among other points, her encounter with sertanist Orlando Villas-Bôas, which was crucial for her to dedicate her visual art production to reinterpreting indigenous graphic patterns
Phakectomy in childhood: who are our patients?
OBJETIVO: Analisar o perfil dos pacientes até 10 anos de idade, submetidos à facectomia no setor de catarata do Hospital da Piedade, determinando as principais indicações, perfil sócioeconômico e características pré-natais nos casos relevantes. MÉTODOS: Foram analisados retrospectivamente, os dados de 60 pacientes com até 10 anos de idade, submetidos à lensectomia, dentro do período de janeiro de 2006 a dezembro de 2007. Todas as cirurgias foram realizadas no setor de catarata do Hospital da Piedade, pelo mesmo cirurgião. Os responsáveis pelas crianças foram submetidos a questionário. RESULTADOS: Dos 60 pacientes, 25 eram do sexo feminino (42 %) e 35 do sexo masculino (58 %). 42 pacientes (70%) apresentavam o diagnóstico de catarata congênita, 8 (13%) tinham subluxação de cristalino e 10 (17%) foram submetidos à lensectomia devido à baixa da acuidade visual associada à catarata traumática. Entre as mães dos pacientes com catarata congênita, 40 (95%) realizaram pré natal com mais de 5 consultas, destas 9 mães (22,5%) foram infectadas pelo vírus da rubéola durante a gestação. Nos casos referentes à subluxação todos os 8 apresentavam síndrome de Marfan. Os 10 casos de trauma foram em meninos. CONCLUSÕES: Foi possível verificar que a grande maioria de nossos pacientes submetidos à facectomia apresentava catarata congênita e um número relevante dos casos encontrava-se ainda relacionado à rubéola. É fundamental diminuir o tempo entre a suspeita diagnóstica e o atendimento especializado, favorecendo o prognóstico visual dessas crianças.OBJECTIVE: To analyze the profile of the patients who were submitted to phakectomy at the cataract service of the Hospital da Piedade, under the age of 10 years old, evaluating the main indications, the socioeconomic and gestational profiles in the relevant cases. METHODS: Data of 60 patients under the age of 10 years old submitted to lensectomy, were retrospectively analyzed, during the period between January of 2006 and December of 2007. All the surgeries were performed at the cataract service of the Hospital da Piedade, by the same surgeon. RESULTS: Of the 60 patients, 25 were female (42%) and 35 were male (58 %). 42 patients (70%) had the diagnostic of congenital cataract, 8 (13%) had subluxated lens and 10 (17%) had lensectomy for traumatic cataract. Considering congenital cataract patient's mothers, 40 (95%) underwent prenatal exams with more than 5 appointments, 9 (22.5%) of them were infected by rubella virus during the pregnancy. In subluxation cases, 8 had Marfan's syndrome. 10 cases of trauma occurred in boys. CONCLUSION: It was possible to verify that the majority of our patients submitted to phakectomy had congenital cataract and there is an important number of cases that still associated to rubella. Decreasing time since the suspect until the specialized consultation, will improve the visual prognosis of the children

This article proposes a reflection on the methodological challenges presented and faced by oral history research from the outbreak of the Covid-19 pandemic and the physical isolation it entailed. It conceptualizes and focuses on actions for the present’s “urgent documentation,” arguing that these are neither a discontinuation in relation to oral history research traditions, nor a return to the documentary ambition that marks the very emergence of the oral history practice – but a practice that merges plural methodological principles and theoretical propositions developed from the use of the method. Finally, the article assumes the construction of narrative communities and the creation of visibility of experiences of subjects and groups in situations of vulnerability as public commitments that are imperatives for researchers in and of the present, for whom technology presents itself as a challenging ally.O presente artigo propõe uma reflexão acerca dos desafios metodológicos apresentados e enfrentados pela pesquisa em história oral a partir da eclosão da pandemia de Covid-19 e da situação de isolamento físico por ela ensejada. Conceitua e focaliza as ações de “documentação urgente” do presente, argumentando que elas constituem não um gesto de descontinuidade em relação às tradições de pesquisa em história oral, nem um retorno à ambição documentarista que marca o próprio surgimento da prática de história oral – mas uma prática que se baseia na combinação de propostas metodológicas e proposições teóricas plurais desenvolvidas a partir do uso do método. O artigo assume, por fim, a construção de comunidades narrativas e a visibilização de experiências de sujeitos e grupos em situações de vulnerabilidade como compromissos públicos que se apresentam como imperativos para pesquisadores no e do presente, para os quais a tecnologia apresenta-se como desafiadora aliada
