    Frequency, Etiology and Leading Causes of Pre-Senile Cataract: A Descriptive Study

    Background: Cataract is generally caused due to physical aging, but in some cases, there are chances that it may develop in early ages (Pre-senile cataract). Pre-senile cataract is believed not to be primary in nature and some underlying ocular/systemic/environmental factor is causing the lens opacification at an early age. This study aim was to determine frequency and risk factors of pre-senile cataract in our populationMethods: this observational study at Outdoor Patient - Eye in Benazir Bhutto Hospital, Rawalpindi (BBH), for the period from December 2017 to June 2018. Cataract was diagnosed on slit lamp examination. Detail history, Ocular and systemic examination was done for all pre-senile cataract cases. Laboratory/radiological investigations were performed when and where required. Using the Excel spreadsheet, Statistical analysis of descriptive data was carried out.Results: A total of 11448 patients were examined, whereby cataract was found in 849 (7.41%) patients. Out of these 849, 165 (19.43%) cases were pre-senile cataract. Pre-senile cataract was most seen in age group 30-49 86 (52.13%) and among women 93 (56.36%). The identified causes were idiopathic 55 (33.3%), diabetes 45(27.3%) and Steroids and other drugs related 33 (20.0%). posterior sub-capsular cataract is 60(36.36%) was most identified type in the study among pre-senile cataract

    Abnormalities in motor-control behavior, which have been with concussion and head acceleration events (HAE), can be quantified using virtual reality (VR) technologies. Motor-control behavior has been consistently mapped to the brain's somatomotor network (SM) using both structural (sMRI) and functional MRI (fMRI). However, no studies habe integrated HAE, motor-control behavior, sMRI and fMRI measures. Here, brain networks important for motor-control were hypothesized to show changes in tractography-based diffusion weighted imaging [difference in fractional anisotropy (dFA)] and resting-state fMRI (rs-fMRI) measures in collegiate American football players across the season, and that these measures would relate to VR-based motor-control. We firther tested if nine inflammation-related miRNAs were associated with behavior-structure-function variables. Using permutation-based mediation and moderation methods, we found that across-season dFA from the SM structural connectome (SM-dFA) mediated the relationship between across-season VR-based Sensory-motor Reactivity (dSR) and rs-fMRI SM fingerprint similarity (p = 0.007 and Teff = 47%). The interaction between dSR and SM-dFA also predicted (pF = 0.036, pbeta3 = 0.058) across-season levels of dmiRNA-30d through permutation-based moderation analysis. These results suggest (1) that motor-control is in a feedback relationship with brain structure and function, (2) behavior-structure-function can be connected to HAE, and (3) behavior-structure might predict molecular biology measures.Comment: 62 pages, 4 figures, 2 table

    Polarization Insensitive Compact Chipless RFID Tag

    This research article proposes a highly dense, inexpensive, flexible and compact 29 x 29 mm(2) chipless radio frequency identification (RFID) tag. The tag has a 38-bit data capacity, which indicates that it has the ability to label 238 number of different objects. The proposed RFID tag has a bar-shape slot/resonator based structure, which is energized by dual-polarized electromagnetic (EM) waves. Thus, portraying polarization insensitive nature of the tag. The radar cross-section (RCS) response of the proposed tag design is analyzed using different substrates, i.e., Rogers RT/duroid (R)/5880, Taconic (TLX-0), and Kapton (R) HN (DuPont (TM)). A comparative analysis is done, which reveal the changes observed in the RCS curve, as a result of using different substrates and radiators. Moreover, the effect on the RCS response of the tag is also examined, by bending the tag at different bent radii. The compactness and flexible nature of the tag makes it the best choice for Internet of things (IoT) based smart monitoring applications

    Corticotrophin releasing factor increases ascending colon volume after a fructose test meal in healthy humans: a randomised control trial

    Background: Poorly absorbed, fermentable carbohydrates can provoke irritable bowel syndrome (IBS) symptoms by escaping absorption in the small bowel and being rapidly fermented in the colon in some susceptible subjects. IBS patients are often anxious and stressed and stress accelerates small bowel transit which may exacerbate malabsorption. Objective: In this study we investigated the effect of intravenous injection of corticotrophin releasing factor (CRF) on fructose malabsorption and the resulting volume of water in the small bowel. Design: We performed a randomised, placebo controlled, cross-over study of CRF versus saline injection in 11 male and 10 female healthy subjects, examining the effect on the malabsorption of a 40 g fructose test meal and its transit through the gut which was assessed by serial Magnetic Resonance imaging (MRI) and breath hydrogen measurement. Orocaecal transit was assessed using the lactose-ureide C13 breath test and the adrenal response to CRF assessed by serial salivary cortisol measurements. Results: (Mean ± SD) CRF injection caused a significant rise in salivary cortisol which lasted 135 minutes. Small bowel water content (SBWC) rose from baseline, peaking at 45 minutes after fructose ingestion while breath hydrogen peaked later at 75 minutes. The area under the curve (AUC) for SBWC from -15 - 135 minutes was significantly lower after CRF versus saline (mean difference [95% CI] 7433 [275, 14591] mL.min, P = 0.04). Ascending colon volume rose after CRF, significantly more for male volunteers than female (P = 0.025). Conclusions: CRF constricts the small bowel and increases fructose malabsorption as shown by increased ascending colon volumes. This mechanism may help to explain the increased sensitivity of some stressed individuals to fructose malabsorption. This trial was registered at ClinicalTrials.gov as NCT0176328

    Effectiveness of unconditional cash transfers combined with lipid-based nutrient supplement and/or behavior change communication to prevent stunting among children in Pakistan: A cluster randomized controlled trial

    Background: In Pakistan, the prevalence of stunting among children under-five years has remained above WHO critical thresholds (≥30%) over the last two decades.Objective: We hypothesized that an unconditional cash transfer (UCT) combined with lipid-based nutrient supplement (LNS) and/or social and behavior change communication (SBCC) will prevent stunting among children 6-23 months of age.Design: This was a four-arm, community-based cluster randomized controlled trial conducted in the district of Rahim Yar Khan, Pakistan. A total of 1729 children (UCT n = 434); (UCT+SBCC n = 433); (UCT+LNS n = 430) and (UCT+LNS+SBCC n = 432) were enrolled at 6 months of age and measured monthly for 18 months until the age of 24 months.Results: At 24 months of age, children who received UCT+LNS (rate ratio [RR], 0.85 [95% CI 0.74, 0.97]; P = 0.015); and UCT+LNS+SBCC (RR, 0.86 [95% CI 0.77, 0.96]; P = 0.007) had significantly lower risk of being stunted as compared to the UCT arm. No significant difference was noted among children who received UCT+SBCC (RR, 1.03 [95% CI 0.91, 1.16]; P = 0.675) in the risk of being stunted as compared to the UCT arm. The pooled prevalence of stunting among children 6-23 months was 41.7%, 44.8%, 38.5% and 39.3% in UCT, UCT+SBCC, UCT+LNS and UCT+LNS+SBCC, respectively. In pairwise comparisons, a significant impact on stunting among children in UCT+LNS (P = 0.029) and UCT+LNS+SBCC (P = \u3c0.001) was noted as compared to UCT arm.Conclusions: UCT combined with LNS and UCT+LNS+SBCC were effective in reducing the prevalence of stunting among children aged 6-23 months in marginalized populations. UCT+SBCC was not effective in reducing the child stunting prevalence.Clinical trial registration number: ClinicalTrials.gov NCT03299218

    Measuring routine childhood vaccination coverage in 204 countries and territories, 1980-2019 : a systematic analysis for the Global Burden of Disease Study 2020, Release 1

    Background Measuring routine childhood vaccination is crucial to inform global vaccine policies and programme implementation, and to track progress towards targets set by the Global Vaccine Action Plan (GVAP) and Immunization Agenda 2030. Robust estimates of routine vaccine coverage are needed to identify past successes and persistent vulnerabilities. Drawing from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2020, Release 1, we did a systematic analysis of global, regional, and national vaccine coverage trends using a statistical framework, by vaccine and over time. Methods For this analysis we collated 55 326 country-specific, cohort-specific, year-specific, vaccine-specific, and dosespecific observations of routine childhood vaccination coverage between 1980 and 2019. Using spatiotemporal Gaussian process regression, we produced location-specific and year-specific estimates of 11 routine childhood vaccine coverage indicators for 204 countries and territories from 1980 to 2019, adjusting for biases in countryreported data and reflecting reported stockouts and supply disruptions. We analysed global and regional trends in coverage and numbers of zero-dose children (defined as those who never received a diphtheria-tetanus-pertussis [DTP] vaccine dose), progress towards GVAP targets, and the relationship between vaccine coverage and sociodemographic development. Findings By 2019, global coverage of third-dose DTP (DTP3; 81.6% [95% uncertainty interval 80.4-82 .7]) more than doubled from levels estimated in 1980 (39.9% [37.5-42.1]), as did global coverage of the first-dose measles-containing vaccine (MCV1; from 38.5% [35.4-41.3] in 1980 to 83.6% [82.3-84.8] in 2019). Third- dose polio vaccine (Pol3) coverage also increased, from 42.6% (41.4-44.1) in 1980 to 79.8% (78.4-81.1) in 2019, and global coverage of newer vaccines increased rapidly between 2000 and 2019. The global number of zero-dose children fell by nearly 75% between 1980 and 2019, from 56.8 million (52.6-60. 9) to 14.5 million (13.4-15.9). However, over the past decade, global vaccine coverage broadly plateaued; 94 countries and territories recorded decreasing DTP3 coverage since 2010. Only 11 countries and territories were estimated to have reached the national GVAP target of at least 90% coverage for all assessed vaccines in 2019. Interpretation After achieving large gains in childhood vaccine coverage worldwide, in much of the world this progress was stalled or reversed from 2010 to 2019. These findings underscore the importance of revisiting routine immunisation strategies and programmatic approaches, recentring service delivery around equity and underserved populations. Strengthening vaccine data and monitoring systems is crucial to these pursuits, now and through to 2030, to ensure that all children have access to, and can benefit from, lifesaving vaccines.     Stress and the Multiple-Role Woman: Taking a Closer Look at the “Superwoman”

    <div><p>In the academic literature there is debate as to whether women who engage in multiple social roles experience more or less stress than women in fewer roles. For the present research we examined the relationship between levels of engagement in seven distinct roles and perceived stress and life satisfaction in a small non-random sample of women in North America (N = 308). We did not find a significant correlation between role engagement and perceived stress, though we did find a small but significant positive correlation between role engagement and life satisfaction. Similarly, in a subset of the participants (N = 31), there was not a significant relationship between the level of role engagement and physiological stress as measured by hair or urinary cortisol levels. We found a significant negative correlation between perceived stress and life satisfaction, and role satisfaction. The results from multiple regression models did not identify the level of role engagement as a significant predictor of either perceived stress or life satisfaction. Role satisfaction in addition to several life style variables such as the frequency of sex and exercise were identified as significant predictors of both outcome variables. We also examined the popularized notion of the “superwoman”, which we defined as women who fell within the 4<sup>th</sup> quartile of role engagement, or those engaged in the wife/mother/worker/homemaker role combination. Based on popular discourses surrounding the superwoman we expected that superwomen would exhibit higher levels of perceived stress. Our results revealed that superwomen do not experience a significantly higher level of perceived stress than non-superwomen. The results of our study therefore suggest that multiple role engagement in women, even at a relatively high level as experienced by “superwomen”, is not associated with significantly higher stress, or reduced life satisfaction.</p></div
