    Investigation of in-vehicle speech intelligibility metrics for normal hearing and hearing impaired listeners

    The effectiveness of in-vehicle speech communication can be a good indicator of the perception of the overall vehicle quality and customer satisfaction. Currently available speech intelligibility metrics do not account in their procedures for essential parameters needed for a complete and accurate evaluation of in-vehicle speech intelligibility. These include the directivity and the distance of the talker with respect to the listener, binaural listening, hearing profile of the listener, vocal effort, and multisensory hearing. In the first part of this research the effectiveness of in-vehicle application of these metrics is investigated in a series of studies to reveal their shortcomings, including a wide range of scores resulting from each of the metrics for a given measurement configuration and vehicle operating condition. In addition, the nature of a possible correlation between the scores obtained from each metric is unknown. The metrics and the subjective perception of speech intelligibility using, for example, the same speech material have not been compared in literature. As a result, in the second part of this research, an alternative method for speech intelligibility evaluation is proposed for use in the automotive industry by utilizing a virtual reality driving environment for ultimately setting targets, including the associated statistical variability, for future in-vehicle speech intelligibility evaluation. The Speech Intelligibility Index (SII) was evaluated at the sentence Speech Receptions Threshold (sSRT) for various listening situations and hearing profiles using acoustic perception jury testing and a variety of talker and listener configurations and background noise. In addition, the effect of individual sources and transfer paths of sound in an operating vehicle to the vehicle interior sound, specifically their effect on speech intelligibility was quantified, in the framework of the newly developed speech intelligibility evaluation method. Lastly, as an example of the significance of speech intelligibility evaluation in the context of an applicable listening environment, as indicated in this research, it was found that the jury test participants required on average an approximate 3 dB increase in sound pressure level of speech material while driving and listening compared to when just listening, for an equivalent speech intelligibility performance and the same listening task

    The paper analyzed linear type traits and influence of summer and winter nutrition on body development of Simmental cattle. Test cows (paternal a half-sisters) were located in identical breedingconditions on the eastern slopes of Mount Kopaonik.Considering that expression of the genetic potential depends on the environment in which the organisms exist, the differences between summer and winter period feeding of Simmental cattle were not statistically significant (Tukey's test, p> 0.05) for the investigated properties of linear type traits and body development.U radu su analizirane linearne ocene tipa i uticaj letnjeg i zimskog perioda ishrane na telesnu razvijenost krava simentalske rase.     Hypoglycemia in Non-Diabetic In-Patients: Clinical or Criminal?

    BACKGROUND AND AIM: We wished to establish the frequency of unexpected hypoglycemia observed in non diabetic patients outside the intensive care unit and to determine if they have a plausible clinical explanation. METHODS: We analysed data for 2010 from three distinct sources to identify non diabetic hypoglycaemic patients: bedside and laboratory blood glucose measurements; medication records for those treatments (high-strength glucose solution and glucagon) commonly given to reverse hypoglycemia; and diagnostic codes for hypoglycemia. We excluded from the denominator admissions of patients with a diagnosis of diabetes or prescribed diabetic medication. Case notes of patients identified were reviewed. We used capture-recapture methods to establish the likely frequency of hypoglycemia in non-diabetic in-patients outside intensive care unit at different cut-off points for hypoglycemia. We also recorded co-morbidities that might have given rise to hypoglycemia. RESULTS: Among the 37,898 admissions, the triggers identified 71 hypoglycaemic episodes at a cut-off of 3.3 mmol/l. Estimated frequency at 3.3 mmol/l was 50(CI 33-93), at 3.0 mmol/l, 36(CI 24-64), at 2.7 mmol/l, 13(CI 11-19), at 2.5 mmol/l, 11(CI 9-15) and at 2.2 mmol/l, 8(CI 7-11) per 10,000 admissions. Admissions of patients aged above 65 years were approximately 50% more likely to have an episode of hypoglycemia. Most were associated with important co-morbidities. CONCLUSION: Significant non-diabetic hypoglycemia in hospital in-patients (at or below 2.7 mmol/l) outside critical care is rare. It is sufficiently rare for occurrences to merit case-note review and diagnostic blood tests, unless an obvious explanation is found

    Enforced Expression of the Transcriptional Coactivator OBF1 Impairs B Cell Differentiation at the Earliest Stage of Development

    OBF1, also known as Bob.1 or OCA-B, is a B lymphocyte-specific transcription factor which coactivates Oct1 and Oct2 on B cell specific promoters. So far, the function of OBF1 has been mainly identified in late stage B cell populations. The central defect of OBF1 deficient mice is a severely reduced immune response to T cell-dependent antigens and a lack of germinal center formation in the spleen. Relatively little is known about a potential function of OBF1 in developing B cells. Here we have generated transgenic mice overexpressing OBF1 in B cells under the control of the immunoglobulin heavy chain promoter and enhancer. Surprisingly, these mice have greatly reduced numbers of follicular B cells in the periphery and have a compromised immune response. Furthermore, B cell differentiation is impaired at an early stage in the bone marrow: a first block is observed during B cell commitment and a second differentiation block is seen at the large preB2 cell stage. The cells that succeed to escape the block and to differentiate into mature B cells have post-translationally downregulated the expression of transgene, indicating that expression of OBF1 beyond the normal level early in B cell development is deleterious. Transcriptome analysis identified genes deregulated in these mice and Id2 and Id3, two known negative regulators of B cell differentiation, were found to be upregulated in the EPLM and preB cells of the transgenic mice. Furthermore, the Id2 and Id3 promoters contain octamer-like sites, to which OBF1 can bind. These results provide evidence that tight regulation of OBF1 expression in early B cells is essential to allow efficient B lymphocyte differentiation

    Effects of hospital facilities on patient outcomes after cancer surgery: an international, prospective, observational study

    Background Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide.Methods A multimethods analysis was performed as part of the GlobalSurg 3 study-a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital.Findings Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. After adjustment for case-mix and country income group, hospitals with three or fewer of these facilities (62 hospitals, 1294 patients) had higher mortality compared with those with four or five (adjusted odds ratio [OR] 3.85 [95% CI 2.58-5.75]; p<0.0001), with excess mortality predominantly explained by a limited capacity to rescue following the development of major complications (63.0% vs 82.7%; OR 0.35 [0.23-0.53]; p<0.0001). Across LMICs, improvements in hospital facilities would prevent one to three deaths for every 100 patients undergoing surgery for cancer.Interpretation Hospitals with higher levels of infrastructure and resources have better outcomes after cancer surgery, independent of country income. Without urgent strengthening of hospital infrastructure and resources, the reductions in cancer-associated mortality associated with improved access will not be realised

    Improved radiation dose efficiency in solution SAXS using a sheath flow sample environment.

    Radiation damage is a major limitation to synchrotron small-angle X-ray scattering analysis of biomacromolecules. Flowing the sample during exposure helps to reduce the problem, but its effectiveness in the laminar-flow regime is limited by slow flow velocity at the walls of sample cells. To overcome this limitation, the coflow method was developed, where the sample flows through the centre of its cell surrounded by a flow of matched buffer. The method permits an order-of-magnitude increase of X-ray incident flux before sample damage, improves measurement statistics and maintains low sample concentration limits. The method also efficiently handles sample volumes of a few microlitres, can increase sample throughput, is intrinsically resistant to capillary fouling by sample and is suited to static samples and size-exclusion chromatography applications. The method unlocks further potential of third-generation synchrotron beamlines to facilitate new and challenging applications in solution scattering

    Effect of SARS-CoV-2 (COVID-19) Pandemic and Lockdown on Body Weight, Maladaptive Eating Habits, Anxiety, and Depression in a Bariatric Surgery Waiting List Cohort

    Purpose: On January 30, 2020, the World Health Organization declared COVID-19 as a \u201cpublic health emergency of international concern.\u201d The primary aim of the study was to evaluate weight and food habit changes during COVID-19 outbreak. The secondary endpoint was to explore the psychological factors, arising during the pandemic, influencing weight and dietary variations. Materials and Methods: A survey composed of four different items was conducted by telephone interview: (1) anthropometric data and type of procedure, (2) Hospital Anxiety and Depression Scale (HADS), (3) maladaptive eating behaviors, and (4) personal feelings moved by the COVID-19 spread and lockdown. Results: Fifty-six patients were enrolled. No significant changes in weight, BMI, and maladaptive eating habits were observed. A significant reduction in the anxiety index score was observed. In 17.8% of cases, a change in obesity class was reported, and among these patients, a substantial modification in bariatric procedures was planned (60%). Conclusion: This study showed no effect on weight and BMI nor on rates of maladaptive eating habits associated with quarantine/social isolation among severely obese individuals waiting for the bariatric surgery. At the end of lockdown, a considerable proportion of patients modified their initial obesity class, and in selected cases, it could represent a criteria for rearrangement of the planned bariatric procedure. In obese patients, the lockdown and social distancing generated a reduction of fear of confronting and being negatively judged by others. This psychological aspect was assessed with the reduction of the HADS score

Vertical distribution of weed seed banks in extensive and intensive methods of grapevine cultivation. Bulg. J. Agric. Sci., This study deals with the influence of herbicide application (glyphosate and glufosinate-ammonium), soil cultivation methods on the size and quantity of weed seed banks. The application of herbicides in a row (intra-row spacing) of the intensive vineyards studied as well as the cultivation between rows (inter-row spacing) affect the soil weed seed bank in the vineyard. The research was carried out in 2011 and 2012 in three different vineyard sites (Erdut, Subotica-Horgos and Fruska Gora vineyards) in intensive vineyards grown for commercial purposes, as well as in extensive vineyards. Samples were taken in ten replications from the inter-row and intra-row zones to the depth of 0-30cm of intensive and extensive vineyards. The samples were rinsed through copper sieves of specific diameters and dried at room temperature, after which the weed seeds were extracted and identified. The comparison of the two weed seed banks in the two methods of grapevine cultivation studied revealed the range of weed seed number in extensive and intensive vine growing within a one year period from 37.006 seeds/m 2 in the inter-row spacing of an intensive vineyard to 69.069 seeds/m 2 in a row of extensive vineyards at various localities. Despite the great diversity of the weeds, totaling 38 species, whose seeds were identified in the samples studied, only several species were predominant in number in comparison to other species: Amaranthus retroflexus L., Portulaca oleracea L., Chenopodium album L., Stellaria media (L.) Vill. Lamium purpureum L. The germination of the seeds identified showed a significantly higher potential for the predominant weed species in comparison to others
