The Juvenile Arthritis Multidimensional Assessment Report (JAMAR) is a new parent/patient reported outcome measure that enables a thorough assessment of the disease status in children with juvenile idiopathic arthritis (JIA). We report the results of the cross-cultural adaptation and validation of the parent and patient versions of the JAMAR in the Paraguayan Spanish language. The reading comprehension of the questionnaire was tested in 10 JIA parents and patients. Each participating centre was asked to collect demographic, clinical data and the JAMAR in 100 consecutive JIA patients or all consecutive patients seen in a 6-month period and to administer the JAMAR to 100 healthy children and their parents. The statistical validation phase explored descriptive statistics and the psychometric issues of the JAMAR: the 3 Likert assumptions, floor/ceiling effects, internal consistency, Cronbach\u2019s alpha, interscale correlations, and construct validity (convergent and discriminant validity). A total of 51 JIA patients (2% systemic, 27.4% oligoarticular, 37.2% RF negative polyarthritis, 33.4% other categories) and 100 healthy children, were enrolled. The JAMAR components discriminated well healthy subjects from JIA patients. Notably, there was no significant difference between healthy subjects and their affected peers in the school-related problem variable. All JAMAR components revealed good psychometric performances. In conclusion, the Paraguayan Spanish version of the JAMAR is a valid tool for the assessment of children with JIA and is suitable for use both in routine clinical practice and clinical research

Omic Technologies in HIV: Searching Transcriptional Signatures Involved in Long-Term Non-Progressor and HIV Controller Phenotypes

RevisiónThis article reviews the main discoveries achieved by transcriptomic approaches on HIV controller (HIC) and long-term non-progressor (LTNP) individuals, who are able to suppress HIV replication and maintain high CD4+ T cell levels, respectively, in the absence of antiretroviral therapy. Different studies using high throughput techniques have elucidated multifactorial causes implied in natural control of HIV infection. Genes related to IFN response, calcium metabolism, ribosome biogenesis, among others, are commonly differentially expressed in LTNP/HIC individuals. Additionally, pathways related with activation, survival, proliferation, apoptosis and inflammation, can be deregulated in these individuals. Likewise, recent transcriptomic studies include high-throughput sequencing in specific immune cell subpopulations, finding additional gene expression patterns associated to viral control and/or non-progression in immune cell subsets. Herein, we provide an overview of the main differentially expressed genes and biological routes commonly observed on immune cells involved in HIV infection from HIC and LTNP individuals, analyzing also different technical aspects that could affect the data analysis and the future perspectives and gaps to be addressed in this field. This study was funded by a fellowship from GILEAD Sciences (GLD18/00090), Instituto de Salud Carlos III (PI19CIII/00004), and has been conducted within the Spanish AIDS Research Network (RIS), funded by Instituto de Salud Carlos III (Plan Estatal de I+D+I 2013-2016) and co-funded by European Regional Development Fund (ERDF) "A way to build Europe" (RD16CIII/0002/0001). The purpose of this investigative work is to identify the different risk factors that are present in the different activities that workers in the registration area of the Cotopaxi Commonwealth Public Mobility Company must comply with during their workday. Based on a prior quantitative identification of risks, a prior analysis will be carried out focused on ergonomic risks and in this way carry out an evaluation of the risk factors that present a high level of risk, which are the adoption of forced postures and the use of of data display screens (PVD). To carry out these evaluations, specific methodologies were used; For what is the adoption of forced postures, it will be developed based on the RULA method, where the exposure of workers to risk factors that can cause musculoskeletal disorders in both the upper and lower limbs of the body will be evaluated. The methodology used to evaluate the use of data visualization screens is the JUNTA ANDALUCIA method, developed by the National Institute of Occupational Safety and Hygiene of Spain. This will help us to quantify the magnitude of the existing risks in each job.

Elite controllers long-term non progressors present improved survival and slower disease progression

Different phenotypes exhibiting no evidences of disease progression have been described in ART-naïve HIV-1 positive individuals. Long-term non progressors (LTNP) and elite controllers (EC) are low frequent examples of immunological and virological control in HIV-1 positive subjects, respectively. The combination of both phenotypes is even less frequent and studied despite being considered as models of HIV-1 functional cure. A multicenter, prospective study in retrospect including clinical and epidemiological data collected from 313 LTNP of 21 Spanish hospitals was carried out. LTNPs maintaining CD4+ T cell counts over 500 cells/µl and viral loads (VL) under 10,000 copies/mL for at least 10 years in the absence of antiretroviral therapy were followed for a median of 20.8 years (IQR = 15.6-25.5). A 52.1% were considered EC (undetectable VL) and LTNP (EC-LTNP) and a total of 171 (54.8%) and 42 (13.5%) out of the 313 participants maintained LTNP status for at least 20 and 30 years, respectively. EC-LTNP showed lower CD4+ T cell count loss (9.9 vs 24.2 cells/µl/year), higher CD4/CD8 ratio (0.01 vs - 0.09 in ratio), and lesser VL increase (no increase vs 197.2 copies/mL/year) compared with LTNPs with detectable VL (vLTNP). Survival probabilities for all-cause mortality at 30 years from HIV + diagnosis were 0.90 for EC-LTNP and 0.70 for vLTNP (p = 2.0 × 10-3), and EC-LTNP phenotype was the only factor associated with better survival in multivariate analyses (HR = 0.28; 95% CI 0.10-0.79). The probability to preserve LTNP status at 30 years was 0.51 for EC-LTNP and 0.18 for vLTNP (p < 2.2 × 10-16). Risk factors associated to the loss of LTNP status was: higher age at diagnosis and the increase of VL, whereas the increase of CD4+ T cell counts and CD4/CD8 ratio, the initial EC-LTNP phenotype and HCV coinfection were protective factors. EC-LTNP phenotype was associated with improved survival and slower disease progression compared with other phenotypes of LTNP. EC-LTNP individuals represent one of the most favorable phenotypes of immune activation against HIV-1 found in nature and, therefore, are strong candidates to be considered a model of functional cure of HIV-1 infection. This study has been conducted within the Infectious Diseases Networking Biomedical Research Centre (CIBER INFEC) and the Spanish AIDS Research Network (RIS), the latter being funded by the Instituto de Salud Carlos III (Project RD16CIII/0002/0001, Plan Estatal de I+D+I 2013–2016) and co-fnanced by the European Regional Development Fund (ERDF) “A way to build Europe”.

Intersectionalities of the Left and Right in Latin America and Europe. An exploration of contemporary political processes

    Get PDF
The current rise of new right-wing groups with a particular combination of intransigent economic liberalism and conservative nationalist fascism, a kind of liberfascism, advocates for both a strict apology for the capitalist market and the exclusion of anything that challenges its model in the political and cultural spheres. Based on the populist discourse that exploits the discontent caused by neoliberal democracy, they gain followers for nationalist, cultural, and racist reasons that in the end are nothing more than right-wing deception to protect the rule of capital. Faced with the restoration of an already exhausted neoliberal model, an intersectionality appears between the Left and the Right which, far from confirming the diagnosis of the end of ideologies and the deactivation of the class struggle, rekindles conflicts over capital. This article deals with such a situation. It argues how intersectionality is not a privilege of the Left, but is also used by the extreme populist Right to channel discontent and propagate its particular brand of oppression. It is concluded, based on examples of struggles in Latin America and Europe, that an intersectional and transversal politics must be developed in the face of the hegemony of the new populist and nationalist Right; the political tactic must not be local, but must pursue a new international strategy

Redefining the Common Causes of Social Struggles: An Examination of the Antinomies of Value, Labor and Subsumption

In the global contemporary political context, diverse social struggles are being alienated from each other to the point that the illusion of capitalism as the only possible socio-economic system is blurring all horizons of social change. In this article, we will aim to redefine the common causes of social struggles, by demonstrating their intersectionality and interdependence. In order to do so, we will engage with a number of concepts from Marx’s philosophy. In the introduction, we will examine the notion of value, claiming that Marx’s value theory is not simply a labor theory of value, but that it rather reflects the parallax structure of production and circulation, crystallized in the ultimate value-form of money. Having obtained these preliminary insights, we will go back to the phenomenon of labor in capitalism, to reinterpret, in the first section of the article, the Marxian distinction between productive and unproductive labor. From this will follow the first concrete examples of the intersectionality of social struggles against the abstraction of capital: namely, showing that gender and racial struggles have certain common causes, rooted in the Gramscian hegemony. In the second section, we will examine the distinction that Marx establishes between the formal and the real subsumption. The latter, we will claim, is decisive for understanding how capital structures the quasi-totality of our social relations. Following an interpretation of Maren Ade’s film Toni Erdmann, we will propose some possible means of intersectional struggle against real subsumption, that will find their theoretical backing in the concept of subversive universals. The concluding remarks will address the nuclear logic of the distribution and accumulation of capital, a symptom that survives throughout history preserved and driven by the omnipresence of ideology, and stress again implicitly the importance of common struggle in the unfreezing of emergence of a new revolutionary subject
