    Dynamical and hydrological changes in climate simulations of the last millennium

    Simulations of climate of the last millennium (LM) show that external forcing had a major contribution to the evolution of temperatures; warmer and colder periods like the Medieval Climate Anomaly (MCA; ca. 950–1250 CE) and the Little Ice Age (LIA; ca. 1450–1850 CE) were critically influenced by changes in solar and volcanic activity. Even if this influence is mainly observed in terms of temperatures, evidence from simulations and reconstructions shows that other variables related to atmospheric dynamics and hydroclimate were also influenced by external forcing over some regions. In this work, simulations from the Coupled Model Intercomparison Project Phase 5 and Paleoclimate Modelling Intercomparison Project Phase 3 (CMIP5/PMIP3) are analyzed to explore the influence of external forcings on the dynamical and hydrological changes during the LM at different spatial and temporal scales. Principal component (PC) analysis is used to obtain the modes of variability governing the global evolution of climate and to assess their correlation with the total external forcing at multidecadal to multicentennial timescales. For shorter timescales, a composite analysis is used to address the response to specific events of external forcing like volcanic eruptions. The results show coordinated long-term changes in global circulation patterns, which suggest expansions and contractions of the Hadley cells and latitudinal displacements of westerlies in response to external forcing. For hydroclimate, spatial patterns of drier and wetter conditions in areas influenced by the North Atlantic Oscillation (NAO), Northern Annular Mode (NAM), and Southern Annular Mode (SAM) and alterations in the intensity and distribution of monsoons and convergence zones are consistently found. Similarly, a clear short-term response is found in the years following volcanic eruptions. Although external forcing has a greater influence on temperatures, the results suggest that dynamical and hydrological variations over the LM exhibit a direct response to external forcing at both long and short timescales that is highly dependent on the particular simulation and model

    Temporal response to drought stress in several Prunus rootstocks and wild species

    Prunus species are important crops in temperate regions. In these regions, drought periods are predicted to occur more frequently due to climate change. In this sense, to reduce the impact of climate warming, obtaining new tolerant/resistant cultivars and rootstocks is a mandatory goal in Prunus breeding. Therefore, the current study assembled three Prunus species including almond, (P. dulcis Mill D.A. Webb), apricot (P. armeniaca L.) and peach (P. persica L.) to model the temporal effects of drought. A hybrid peach × almond and a wild almond-relative species Prunus webbii were also included in the study. Physiological traits associated with photosynthetic activity, leaf water status, and chlorophyll content were assessed under three watering treatments. Results showed that effects of time, genotype, and treatment interact significantly in all traits. In addition, results confirmed that P. webbii have a greater tolerance to drought than commercial rootstocks. However, “Real Fino” apricot showed the fastest recovery after re-irrigation while being one of the most affected cultivars. In addition, from the better response to these watering treatments by the almond genotypes, two different trends were observed after re-irrigation treatment that clearly differentiate the response of the almond cultivar “Garrigue” from the rest of Prunus genotypes. A better characterization of the short-term drought response in Prunus, an accurate and more efficient evaluation of the genotype effect was obtained from the use of mixed models considering appropriate variance–covariance structures.     Driven progressive evolution of genome sequence complexity in Cyanobacteria

    Progressive evolution, or the tendency towards increasing complexity, is a controversial issue in biology, which resolution entails a proper measurement of complexity. Genomes are the best entities to address this challenge, as they encode the historical information of a species’ biotic and environmental interactions. As a case study, we have measured genome sequence complexity in the ancient phylum Cyanobacteria. To arrive at an appropriate measure of genome sequence complexity, we have chosen metrics that do not decipher biological functionality but that show strong phylogenetic signal. Using a ridge regression of those metrics against root-to-tip distance, we detected positive trends towards higher complexity in three of them. Lastly, we applied three standard tests to detect if progressive evolution is passive or driven—the minimum, ancestor– descendant, and sub-clade tests.     [Resumen] El informe sobre criterios de calidad específicos de recursos materiales y profesorado para el título de grado de Fisioterapia ha sido elaborado por la Asociación Española de Fisioterapeutas que convocó, con tal finalidad, a una comisión de expertos en Educación en Fisioterapia. El documento, realizado a propuesta de la Dirección General de Universidades en diciembre de 2004, tiene por objetivo el establecer los criterios mínimos de recursos materiales y humanos derivados de la especificidad de la titulación de Fisioterapia, que permita cubrir las nuevas demandas de la sociedad. En relación a los recursos materiales, se ha procurado darle un suficiente grado de especificación cuantitativa y cualitativa a los instrumentos que constituyen los recursos materiales específicos. La intención es, así mismo, facilitar su valoración en el momento de acreditar la titulación. Se abordan, por una parte, los recursos específicos de la titulación para la docencia teórico-práctica en el centro, dentro de los laboratorios destinados a tal efecto. Por otra parte, se definen las características o perfil de los centros sociosanitarios donde debe impartirse la formación clínica del alumno. En relación a los recursos humanos, se hace especial hincapié en el perfil y la experiencia del profesorado para la docencia, así como en el ratio profesor-alumno en la formación práctica y en la formación clínica.[Abstract] The report about specific quality criteria of resources material (items) and teaching profession to obtain the Physiotherapy Grade, has been elaborated by the Spanish Association of Physiotherapists, which aimed to convoke a Physiotherapy Education experts commission. The document was carried out on Universities General Chairmene’s proposal, aiming to establish the minimum criteria of human and Material (Items) resources, derived from the specific Physical Therapy Grade (Diploma), Which may cover the latest Society requirements. According to the Material resources, there has been a serious attempt to give the instruments, which constitute the specific material resources, an appropriate degree, quantitative and qualitative specification. The main intention by itself, consists of providing its assessment when the time to accredit the title comes. On the one hand the specific resources of the Physiotherapy Grade for the Theoric-practical Teaching in the Centre are taken up even in the laboratories aimed to get such goal. On the other hand we define the characteristics or the profile of the social. Health centres, where Pupils’ Clinical Education, must be carried out. Relating to the Human Resources, we do underline the Teaching Profession experience and profile, as well as the rate Teacher-Pupil, both in the practical Training and the Clinical Education, too

    The combined use of PSInSAR and UAV photogrammetry techniques for the analysis of the kinematics of a coastal landslide affecting an urban area (SE Spain)

    In the present work, the case of the Cármenes del Mar resort (Granada, Spain) is shown. It can be considered one of the most extreme examples on the Mediterranean coast of severe pathologies associated with urban development on coastal landslides. The resort, with 416 dwellings, was partially built on a deep-seated landslide which affects a soft formation composed of dark graphite schists. In November 2015, the City Council officially declared a state of emergency in the resort and 24 dwellings have already been evacuated. We have used two remote sensing techniques to monitor the landslide with the aim of identifying and measuring a wide range of displacements rates (from mm/year to m/year): (1) PSInSAR, exploiting 25 ENVISAT SAR images acquired from May 2003 to December 2009, and (2) photogrammetry, considering the output from two Unmanned Aerial Vehicle (UAV) flights made in June 2015 and January 2016 and the outdated photos from a conventional flight in 2008. The relationship between the geology of the site, data from PS deformation measurements, building displacements, rainfall and damage observed and their temporal occurrence allows a better understanding of the landslide kinematics and both the spatial and temporal evolution of the instability. Results indicate building displacements of up to 1.92 m in 8 years, a clear lithological control in the spatial distribution of damage and a close relationship between the most damaging events and water recharge episodes (rainy events and leaks from swimming pools and the water supply network).     Results of the cooperative study of Spanish peritoneal dialysis registries: analysis of 12 years of follow-up

    [Resumen] Introducción y objetivos: Actualmente no existe un registro que muestre en su conjunto y globalidad la realidad de la diálisis peritoneal (DP) en España. Sin embargo, para distintos congresos y reuniones se ha elaborado durante varios años un informe sobre la DP en España a partir de datos comunicados por cada uno de los registros de las comunidades autónomas y regiones. El objetivo fundamental del presente trabajo es analizar todos estos datos en forma agrupada y comparativa, con objeto de conseguir una muestra representativa de la población española en DP en los últimos años, para su análisis y resultados en cuanto a datos demográficos, penetración de la técnica, diferencias geográficas, incidencia y prevalencia, aspectos técnicos, indicadores intermedios, comorbilidad y resultados finales como supervivencia del paciente y de la técnica puedan ser extrapolables a todo el territorio nacional. Diseño, material y métodos: Estudio observacional de cohortes de registros autonómicos de DP, abarcando el mayor porcentaje posible de la población española adulta (mayores de 14 años) en DP, al menos en la última década (1999-2010), y en la mayor área geográfica que nos ha sido posible reclutar. Se ha seguido una estrategia precisa de recogida de información de cada registro autonómico. Una vez recibida la información y depurada, se integran como datos agregados, para su estudio estadístico. Resultados: Los registros autonómicos que han participado representan un área geográfica total que engloba a 32 853 251 habitantes mayores de 14 años, el 84 % de la población española total a partir de esa edad. La tasa anual media de incidentes por millón de habitantes (ppm) es variable (entre los 17,81 ppm de Andalucía y los 29,90 ppm del País Vasco), observándose en los últimos años un discreto y permanente aumento de la incidencia global en la DP en España. La prevalencia media anual por millón de población (ppm) es muy heterogénea (desde 42 a 99 ppm). Se observa un aumento progresivo medio en el uso de la diálisis peritoneal automática (DPA). La tasa de peritonitis es de aproximadamente un episodio cada 25-30 meses/paciente, observándose una ligera disminución en los años más recientes. Las causas de salida del programa de DP se distribuyen, de forma bastante homogénea entre las distintas comunidades, prácticamente en un tercio por muerte del paciente (media 28 %), un tercio por trasplante renal (media 39 %) y un tercio pasan a hemodiálisis (fracaso de la técnica: media 32 %). Las principales comorbilidades fueron la enfermedad cardiovascular (30,2 %) y la diabetes mellitus (24,2 %). La supervivencia global media acumulada ha sido del 92,2 %, 82,8 %, 74,2 %, 64,8 % y 57 %, al año, dos, tres, cuatro y cinco años, respectivamente. Proporcionaron de forma significativa e independiente una peor supervivencia para el paciente una mayor edad, la enfermedad cardiovascular, la diabetes mellitus, la técnica de diálisis peritoneal continua ambulatoria (frente a DPA), el inicio de la DP antes de 2004 (analizado en Andalucía y Cataluña) y la menor función renal residual al inicio de la DP (analizado en el registro de Levante). De igual forma, actualmente ha mejorado la supervivencia de la técnica, presentando unas cifras promedio superiores al 50 % a los 5 años Conclusiones: La incidencia y la prevalencia de la DP en España están creciendo moderadamente de forma generalizada, si bien siguen manteniendo una distribución por comunidades autónomas irregular. Tanto la supervivencia del paciente como de la técnica es superior al 50 % a los 5 años, habiendo mejorado en los últimos años, y siendo comparable a los países de mejores resultados en este tratamiento.[Abstract] Introduction and objectives: There is currently no registry that gives a complete and overall view of the peritoneal dialysis (PD) situation in Spain. However, a report on PD in Spain was developed for various conferences and meetings over several years from data provided by each registry in the autonomous communities and regions. The main objective of this study is to analyse this data in aggregate and comparatively to obtain a representative sample of the Spanish population on PD in recent years, in order that analysis and results in terms of demographic data, penetration of the technique, geographical differences, incidence and prevalence, technical aspects, intermediate indicators, comorbidity, and outcomes such as patient and technique survival may be extrapolated to the whole country Design, material and method: Observational cohort study of autonomous PD registries, covering the largest possible percentage of the adult Spanish population (over 14 years of age) on PD, at least in the last decade (1999-2010), and in the largest possible geographical area in which we were able to recruit. A precise data collection strategy was followed for each regional registry. Once the information was received and clarified, they were added as aggregate data for statistical study Results: The regional registries that participated represent a total geographical area that encompasses 32,853,251 inhabitants over 14 years of age, 84% of the total Spanish population older than that age. The mean annual rate of incidents per million inhabitants (ppm) was variable (between 17.81ppm in Andalusia and 29.90ppm in the Basque Country), with a discrete and permanent increase in the overall PD incidence in Spain being observed in recent years. The mean annual prevalence per million population (ppm) was very heterogeneous (from 42 to 99ppm). A mean progressive increase in the use of automated peritoneal dialysis (APD) was observed. The peritonitis rate was approximately one episode every 25-30 months/patient, with a slight decrease being observed in recent years. The causes of discontinuing PD were distributed fairly evenly between communities; almost a third was due to patient death (mean 28%), a third was due to renal transplantation (mean 39%) and a third was due to transfer to haemodialysis (technique failure: mean 32%). The main comorbidities were cardiovascular disease (30.2%) and diabetes mellitus (24.2%). The overall accumulated mean survival was 92.2%, 82.8%, 74.2%, 64.8% and 57% after one, two, three, four and five years respectively. There was significantly and independently worse survival for older patients and those with cardiovascular disease, patients with diabetes mellitus, those on continuous ambulatory peritoneal dialysis (vs. APD), those who started PD before 2004 (analysed in Andalusia and Catalonia), and patients with lower residual renal function at the start of PD (analysed in the Levante registry). Similarly, the technique survival has improved, showing a mean figure above 50% after 5 years. Conclusions: The incidence and prevalence of PD in Spain are growing moderately and in a generalised manner and continue to maintain an irregular distribution by autonomous community. Both patient and technique survival were greater than 50% after 5 years, with an improvement being observed in recent years, and are comparable to countries with better results in this treatment

    Chronological and biological aging of the human left ventricular myocardium: Analysis of microRNAs contribution

    Aging is the main risk factor for cardiovascular diseases. In humans, cardiac aging remains poorly characterized. Most studies are based on chronological age (CA) and disregard biological age (BA), the actual physiological age (result of the aging rate on the organ structure and function), thus yielding potentially imperfect outcomes. Deciphering the molecular basis of ventricular aging, especially by BA, could lead to major progresses in cardiac research. We aim to describe the transcriptome dynamics of the aging left ventricle (LV) in humans according to both CA and BA and characterize the contribution of microRNAs, key transcriptional regulators. BA is measured using two CA-associated transcriptional markers: CDKN2A expression, a cell senescence marker, and apparent age (AppAge), a highly complex transcriptional index. Bioinformatics analysis of 132 LV samples shows that CDKN2A expression and AppAge represent transcriptomic changes better than CA. Both BA markers are biologically validated in relation to an aging phenotype associated with heart dysfunction, the amount of cardiac fibrosis. BA-based analyses uncover depleted cardiac-specific processes, among other relevant functions, that are undetected by CA. Twenty BA-related microRNAs are identified, and two of them highly heart-enriched that are present in plasma. We describe a microRNA-gene regulatory network related to cardiac processes that are partially validated in vitro and in LV samples from living donors. We prove the higher sensitivity of BA over CA to explain transcriptomic changes in the aging myocardium and report novel molecular insights into human LV biological aging. Our results can find application in future therapeutic and biomarker research

    Tocilizumab in refractory Caucasian Takayasu's arteritis: a multicenter study of 54 patients and literature review

    Objective: To assess the efficacy and safety of tocilizumab (TCZ) in Caucasian patients with refractory Takayasu's arteritis (TAK) in clinical practice. Methods: A multicenter study of Caucasian patients with refractory TAK who received TCZ. The outcome variables were remission, glucocorticoid-sparing effect, improvement in imaging techniques, and adverse events. A comparative study between patients who received TCZ as monotherapy (TCZMONO) and combined with conventional disease modifying anti-rheumatic drugs (cDMARDs) (TCZCOMBO) was performed. Results: The study comprised 54 patients (46 women/8 men) with a median [interquartile range (IQR)] age of 42.0 (32.5-50.5) years. TCZ was started after a median (IQR) of 12.0 (3.0-31.5) months since TAK diagnosis. Remission was achieved in 12/54 (22.2%), 19/49 (38.8%), 23/44 (52.3%), and 27/36 (75%) patients at 1, 3, 6, and 12 months, respectively. The prednisone dose was reduced from 30.0 mg/day (12.5-50.0) to 5.0 (0.0-5.6) mg/day at 12 months. An improvement in imaging findings was reported in 28 (73.7%) patients after a median (IQR) of 9.0 (6.0-14.0) months. Twenty-three (42.6%) patients were on TCZMONO and 31 (57.4%) on TCZCOMBO: MTX (n = 28), cyclosporine A (n = 2), azathioprine (n = 1). Patients on TCZCOMBO were younger [38.0 (27.0-46.0) versus 45.0 (38.0-57.0)] years; difference (diff) [95% confidence interval (CI) = -7.0 (-17.9, -0.56] with a trend to longer TAK duration [21.0 (6.0-38.0) versus 6.0 (1.0-23.0)] months; diff 95% CI = 15 (-8.9, 35.5), and higher c-reactive protein [2.4 (0.7-5.6) versus 1.3 (0.3-3.3)] mg/dl; diff 95% CI = 1.1 (-0.26, 2.99). Despite these differences, similar outcomes were observed in both groups (log rank p = 0.862). Relevant adverse events were reported in six (11.1%) patients, but only three developed severe events that required TCZ withdrawal. Conclusion: TCZ in monotherapy, or combined with cDMARDs, is effective and safe in patients with refractory TAK of Caucasian origin.Funding: This work was partially supported by RETICS Programs, RD08/0075 (RIER), RD12/0009/0013 and RD16/0012 from “Instituto de Salud Carlos III” (ISCIII) (Spain)
