Critical Illness Myopathy: Diagnostic Approach and Resulting Therapeutic Implications.

    Get PDF
Purpose of review Critical illness myopathy (CIM) is a common neuro-muscular complication of intensive care treatment associated with increased morbidity and mortality. The current guidelines for diagnosis include clinical and electrophysiological criteria as well as a muscle biopsy, and allow diagnosis only at an advanced stage of the disease. To date, there is no treatment for CIM available, apart from symptomatic and rehabilitative interventions. In this review, we discuss different diagnostic approaches and describe new treatment possibilities for CIM. Recent findings Of the diagnostic approaches evaluated, a new electrophysiological technique for measuring muscle excitability has the greatest potential to allow earlier diagnosis of CIM than the current guidelines do and thereby may facilitate the conduction of future pathophysiological and therapeutic studies. Although clinical trials are still lacking, in animal models, BGP-15, vamorolone, and ruxolitinib have been shown to have anti-inflammatory effects, to reduce muscle wasting and to improve muscle function and survival. Summary In recent years, promising methods for early and confirmatory diagnosis of CIM have been developed, but still need validation. Experimental studies on novel pharmacological interventions show promising results in terms of preventive CIM treatments, but future clinical studies will be needed to study the effectiveness and safety of these drugs

Introduction: The rapid worldwide spread of carbapenem-resistant Enterobacteriaceae (CRE) constitutes a major challenge. The aim of the EUropean prospective cohort study on Enterobacteriaceae showing REsistance to CArbapenems (EURECA), which is part of the Innovative Medicines Initiative Joint Undertaking (IMI JU) funded COMBACTE-CARE project, is to investigate risk factors for and outcome determinants of CRE infections to inform randomised clinical trial designs and to provide a historical cohort that could eventually be used for future comparisons with new drugs targeting CRE. Methods: A multicentre (50 sites), multinational (11 European countries), analytical observational project was designed, comprising 3 studies. The aims of study 1 (a prospective cohort study) include characterising the features, clinical management and outcomes of hospitalised patients with intra-abdominal infection, pneumonia, complicated urinary tract infections and bloodstream infections caused by CRE (202 patients in each group). The main outcomes will be 30-day all-cause mortality and clinical response. Study 2 (a nested case–control study) will identify the risk factors for target infections caused by CRE; 248 selected patients from study 1 will be matched with patients with carbapenem-susceptible Enterobacteriaceae (1:1) and with hospitalised patients (1:3) and will provide a historical cohort of patients with CRE infections. Study 3 (a matched cohort study) will follow patients in study 2 in order to assess mortality, length of stay and hospital costs associated with CRE. All patients will be followed for 30 days. Different, up-to-date statistical methods will be applied to come to unbiased estimates for all 3 studies. Ethics and dissemination: Before-study sites will be initiated, approval will be sought from appropriate regulatory agencies and local Ethics Committees of Research or Institutional Review Boards (IRBs) to conduct the study in accordance with regulatory requirements. This is an observational study and therefore no intervention in the diagnosis, management or treatment of the patients will be required on behalf of the investigation. Any formal presentation or publication of data collected from this study will be considered as a joint publication by the participating physician(s) and will follow the recommendations of the International Committee of Medical Journal Editors (ICMJE) for authorship.Innovative Medicine Initiative (IMI)European Union's Seventh Framework Programme (FP7)Spanish Network for Research in Infectious Diseases [REIPI RD12/0015, RD16/2016

Treballs Finals del Màster en Ciència Cognitiva i Llenguatge, Facultat de Filosofia, Universitat de Barcelona, Curs: 2015-2016, Tutora: Pilar Ferré RomeuThe aim of the present study was to investigate whether wordtype influenced the first stages of the acquisition of novel words in a foreign language. Specifically, if emotional content produced any facilitatory effect when learning new words and if this effect was modulated by concreteness. We also aimed to test whether the newly acquired words had the same affective properties and behaved as the emotional words in the first language or the words in proficient bilinguals’ second language. We also investigated if these newly acquired words had become integrated in the bilingual semantic system. Finally, we also studied whether individual differences in terms of anxiety levels could have affected the acquisition of emotional words. Using a paired-associate learning paradigm, participants had to learn 48 pseudowords by associating them to their Spanish translations. We used a Translation Production task to assess whether emotional content and concreteness affected the acquisition of the novel words. Furthermore, Pleasantness Rating tasks were administered before and after the acquisition to study if the pseudowords had acquired affective properties. In addition, Free Recall tasks were used to investigate whether the newly acquired pseudowords behaved as words in the first language. Finally, to assess the semantic integration of these novel words, a Translation Recognition task was conducted. All these tasks were done immediately after the acquisition and one week later. Lastly, the effect of individual differences was explored by analyzing the relationship between the results of a questionnaire of anxiety and the performance in the Translation Production and the Free Recall tasks. Overall, the results showed a better performance for concrete words over abstract words in the different tasks. Concerning emotional content, no facilitatory effect of this variable was found in the acquisition of the novel words. However, the affective properties of emotional words seemed to have been acquired and to remain over time. The Free Recall tasks showed that although there were no effects of emotional content, there was a tendency for emotional pseudowords to behave as words in the native language. Furthermore, the overall results of the Translation Recognition task showed that the novel words had been successfully integrated in the semantic system of the bilinguals. The analysis of the errors in this task also showed that the interference produced by associates was bigger for emotional words what it would indicate a better integration of those emotional pseudowords that had been successfully learnt. Finally, the analysis of individual differences showed no correlation at all

Diversity of the cell-wall associated genomic island of the archaeon Haloquadratum walsbyi

Background: Haloquadratum walsbyi represents up to 80 % of cells in NaCl-saturated brines worldwide, but is notoriously difficult to maintain under laboratory conditions. In order to establish the extent of genetic diversity in a natural population of this microbe, we screened a H. walsbyi enriched metagenomic fosmid library and recovered seven novel version of its cell-wall associated genomic island. The fosmid inserts were sequenced and analysed. Results: The novel cell-wall associated islands delineated two major clades within H. walsbyi. The islands predominantly contained genes putatively involved in biosynthesis of surface layer, genes encoding cell surface glycoproteins and genes involved in envelope formation. We further found that these genes are maintained in the population and that the diversity of this region arises through homologous recombination but also through the action of mobile genetic elements, including viruses. Conclusions: The population of H. walsbyi in the studied saltern brine is composed of numerous clonal lineages that differ in surface structures including the cell wall. This type of variation probably reflects a number of mechanisms that minimize the infection rate of predating virusesAll authors were supported by project MICROGEN (Programa CONSOLIDER-INGENIO 2010 CSD2009-00006) from the Spanish Ministerio de Ciencia e Innovación.FR-V and AMBC received support from MEDIMAX BFPU2013- 48007-P from the Spanish Ministerio de Economía y CompetitividadMaCuMBA Project 311975 of the European Commission FP7ACOMP/2014/024, AORG 2014/032 and PROMETEO II/2014/012. Analytical Characterization and Sensory Analysis of Distillates of Di erent Varieties of Grapes Aged by an Accelerated Method

    Get PDF
The wine spirits used for the elaboration of Brandy de Jerez are mainly obtained from wines produced from the Airen type of grape, which comes from the vineyards located in the region of La Mancha (Central Spain). This entails a limitation when achieving a product classified as "protected geographic designation". For that purpose, it is necessary that the grape used for the wine spirit comes from the area and not from Castile la Mancha, as has happened until now. Due to this fact, it is necessary to search for a possible alternative grape variety which allows the produced brandy to be eligible for a "protected geographic designation". For that purpose, an accelerated ageing process has been implemented with a method previously optimized to distillates obtained from wines from different varieties of grapes (Airen, Colombard, Corredera, Doradilla, Garrido Fino, Jaen blanco, Moscatel de Alejandria, Palomino Fino, Ugni Blanc, and Zalema) grown in the Jerez Area. They were evaluated, both from the analytical and sensory points of view. The distillates made from Jaen Blanco and Zalema have properties that make them interesting for future development and incorporation into oenological practice

Achievement Goal Task Framing and Fit With Personal Goals Modulate the Neurocognitive Response to Corrective Feedback

Past studies have demonstrated the educational impact of achievement goals, but have not yet captured their effects at a critical learning moment—students’ response to negative feedback and their subsequent engagement with error remediation opportunities. We used event-related potentials to investigate how neural substrates of feedback processing were influenced by a within subjects manipulation of mastery and performance goals. Task goal framing did not affect event-related potentials to performance feedback, but did modulate neural activity predicting successful learning. Under a mastery frame, successful learning modulated fronto-temporal activity linked with semantic processing; under a performance frame, it modulated parieto-occipital activity linked with perceptual processing. A match (“fit”) between task and personal goals intensified these neural differences under both goal frames, but mastery goals were additionally sensitive to goal presentation order. Mastery goals may motivate better learning strategies, but are more vulnerable to modulation by students’ own goal dispositions and prior experiences

    Get PDF
Palabras Clave: Cereales, cereal para el desayuno, diabetes mellitus, extrusor, edulcorantesSince ancient times, humans have cultivated cereals to produce different foods such as bread, arepas, pasta, cookies and sweets. The lack of food in humans, not only affects their nutritional status, but this directly affects their health, being responsible for an increase in diseases, such as Diabetes Mellitus (DM). People affected with this pathology present a pathology defined as chronic disease, which occurs when the pancreas cannot produce insulin, or when the body cannot make good use of the insulin it produces. Because of this, it is necessary for diabetics to be able to control and develop a nutritional diet that allows them to eat a balanced and nutritious diet low in sugar. For this, it is recommended the intake of cereals such as rice, wheat, corn, oats, rye and barley, which represent one of the main nutrients of humanity, due to the high content of carbohydrates and therefore the present amount of fiber, lipids, proteins, vitamins and minerals, which provide the main source of high quality energy. Under these premises the importance of this monograph is established, which deals with the factors related to the elaboration of a sucrose-free breakfast cereal as an alternative for diabetics in the city of Barquisimeto, Venezuela during the period 2019, for this The historical evolution of cereals and their transformation into food products was investigated. After this, we proceeded to analyze the importance of the consumption of cereals in the diet of people suffering from DM and finally determine the raw materials, equipment and type of sweetener to be used in the preparation of a cereal for breakfast suitable for diabetic people. The results showed that corn, brown rice and oats represent cereals with ideal nutritional and technological properties composed of starches, slow-digesting carbohydrates that produce flattened and sustained glycemic curves over time, maintaining blood sugar levels, avoiding sudden increases in glucose levels. Finally, the research carried out allowed to expose the advantages of the composition of cereals for the elaboration of expanded foods, through extrusion and the use of stevia, a sweetener that does not affect glycemia in the coating stage. Based on the study carried out, it is concluded that the preparation of a nutritious food from cereals makes it sensory acceptable and does not put the health of people with DM at risk. Key Words: Cereals, breakfast cereal, diabetes mellitus, extruder, sweetener
