    Analysis and proposal of intervention in a case of pregnancy delusion

    Introducción: Se presenta el caso de una mujer con diagnóstico de esquizofrenia paranoide. Mantiene la creencia delirante de estar embarazada a pesar de la certeza y evidencia de lo contrario. Ha rechazado el tratamiento farmacológico así como realizarse determinadas pruebas médicas necesarias para la mejora de su salud por miedo a dañar al feto. Por motivos de accesibilidad para intervenir con la paciente, se presenta una evaluación exhaustiva junto al planteamiento de los objetivos de tratamiento, siendo este último diseñado de forma hipotética sin que haya sido posible su aplicación. Método: Se han utilizado diferentes fuentes de información, principalmente, la entrevista clínica y varios instrumentos sobre sintomatología positiva y ajuste psicosocial. Se sugiere como hipótesis explicativa de esta convicción delirante de embarazo, la interpretación errónea de experiencias perceptivas anómalas (amenorrea, náuseas, mareos, distensión abdominal…). Resultados: La focalización de la atención únicamente a las experiencias sobre el estado físico, debido al deseo de ser madre, junto a este sesgo en la interpretación estaría reforzando la propia ideación delirante de embarazo al convertirse en la única alternativa para explicar tal complejidad. No se aplicó la intervención propuesta debido al abandono del tratamiento. Discusión: Para una mejor comprensión de este tipo de problemática, en cuanto a su naturaleza y factores implicados así como para el diseño de intervenciones psicológicas eficaces, se insiste en la importancia de realizar un mayor número de investigacionesIntroduction: The case of a woman with paranoid schizophrenia diagnosis is presented. She showed the delusional belief of being pregnant, conception that faithfully remains despite of the certainty and evidence of the opposite. She has denied drug treatment and medical tests needed to improve their health made for fear of damaging the fetus. Due to receptiveness reasons to interact with the patient, a thorough examination is made along with the treatment objectives, being this last one designed hypothetically because its application has never been possible. Method: Many different information sources has been used, mainly the clinical interview and few instruments related to positive symptomatology and psychosocial adjustment. As an explanatory hypothesis suggested for the delusional conviction of being pregnant, the wrong interpretation of abnormal perceptive experiences (amenorrhea, nauseas, dizziness, abdominal distension…). Results: To focus the attention uniquely on the experiences of the physical condition, due to the desire of being a mother, along with this bias on the interpretation would be reinforcing its own delusional conception of pregnancy becoming the only alternative to explain such a complexity. Did not apply the proposed intervention due to the abandonment of treatment. Discussion: For a better comprehension of this problematic type, regarding to its nature and implied factors as well as for the design of effective psychological interventions, it insists on the importance of carry out a greater number of researches

    Histopathological image analysis for breast cancer diagnosis by ensembles of convolutional neural networks and genetic algorithms

    One of the most invasive cancer types which affect women is breast cancer. Unfortunately, it exhibits a high mortality rate. Automated histopathological image analysis can help to diagnose the disease. Therefore, computer aided diagnosis by intelligent image analysis can help in the diagnosis tasks associated with this disease. Here we propose an automated system for histopathological image analysis that is based on deep learning neural networks with convolutional layers. Rather than a single network, an ensemble of them is built so as to attain higher recognition rates, which are obtained by computing a consensus decision from the individual networks of the ensemble. A final step involves the optimization of the set of networks that are included in the ensemble by a genetic algorithm.     Determinants of the Likelihood of Tourist Spending in Cultural Micro-Destinations: Type, Timing, and Distance of the Activity as Predictors

    Although tourism expenditure has long been a pertinent topic in studies dealing with cultural tourism, its importance in recent years has become even more marked due to the consequences of low-cost tourism that many destinations are suffering. This need has been further aggravated by the impact of the ongoing Coronavirus pandemic. Now, the only sustainable means of increasing destination revenue is to generate greater financial profitability of tourism-related activities, as opposed to increasing tourist volumes. The present study analyzes the determinants of the likelihood of tourist spending in an urbancultural destination whose economic sustainability is being threatened by low-cost tourism. To this end, all the tourism activities of the sample (672 in total) were recorded in real time during the stay via a purpose-designed mobile application. Given the nested structure of the sample, multilevel modeling was used: the characteristics of the different tourism activities were used as predictor variables, while characteristics of the tourist were used as control variables. Regarding the former, the results suggest that spending is more likely at the beginning of the stay and in relation to non-cultural tourism activities (restaurants, shopping, transport, etc.). An interaction effect between activity location and timing (beginning vs. end of stay) was also demonstrated: at the beginning of the stay, the greater likelihood of spending was related to services or attractions outside the city center; and, toward the end, spending patterns become more static, based close to the city center. The aim is to explain the probability of tourist expenditure at each spending opportunity, thus contributing to the current knowledge of total tourist spending.     The neutron time-of-flight facility n-TOF at CERN: Phase II

    Neutron-induced reactions are studied at the neutron time-of-flight facility n-TOF at CERN. The facility uses 6∼ns wide pulses of 20 GeV/c protons impinging on a lead spallation target. The large neutron energy range and the high instantaneous neutron flux combined with high resolution are among the key characteristics of the facility. After a first phase of data taking during the period 2001-2004, the facility has been refurbished with an upgraded spallation target and cooling system for a second phase of data taking which started in 2009. Since 2010, the experimental area at 185 m where the neutron beam arrives, has been modified into a worksector of type A, allowing the extension of the physics program to include neutron-induced reactions on radioactive isotopes

El objetivo principal era definir los perfiles de paciente susceptibles prioritariamente de la vacunación frente al neumococo considerando los grupos de riesgo, las patologías concomitantes, las posibles consecuencias de una infección neumocócica y la elaboración, a su vez, de un algoritmo de vacunación en el adulto. Se han definido igualmente las indicaciones de la vacuna así como la propuesta de entrevista por parte de la farmacia comunitaria.El documento, avalado por las principales sociedades científicas médico-farmacéuticas, será de gran utilidad para el farmacéutico comunitario de cara a abordar a pacientes con mayor factor de riesgo de contraer enfermedad neumocócica. Será también una manera muy gráfica para conseguir detectar, asesorar y, llegado el caso, derivar al facultativo médico a todo este tipo de paciente

    Variety in local development strategies and employment: LEADER programme in Andalusia

    For the period 2007–2013 LEADER became the fourth axis of rural development policy. One of the main characteristics of LEADER is that it adopts a bottom-up approach. Local Action Groups (LAGs) have to define and implement area-based local development strategies (LDSs). In this paper, we examine the relationship between variety in the LDSs implemented by LAGs and employment safeguarding over the programming period 2007–2013 in Andalusia, the most populated region of Spain. Firstly, we construct several indicators to capture differences in the number of projects carried out, the grants awarded, the investments made and the safeguarded employment. Secondly, we carry out an exploratory factor analysis. We use cluster analysis to classify LAGs applying similar LDSs. The results obtained show that there is no ideal strategy for employment safeguarding and that spending high amounts of money in a few numbers of projects does not guarantee success. Thus, most LAGs do not show any clear specialisation pattern but obtain moderate results in terms of employment safeguarding. This supports the idea that LAGs need to have sufficient flexibility to find a balance among the different objectives of the rural development policy and to translate this balance into the funding of projects.Supported by the Ministry of Economics and Competitiveness of Spain (Project “Development Programmes and Rural Change in the European Union: governance, results and lessons to share 2007–2013”; Grant No. CSO2014-56223-P)
