    Cytokine Production but Lack of Proliferation in Peripheral Blood Mononuclear Cells from Chronic Chagas' Disease Cardiomyopathy Patients in Response to T. cruzi Ribosomal P Proteins

    Background:Trypanosoma cruzi ribosomal P proteins, P2β and P0, induce high levels of antibodies in patients with chronic Chagas' disease Cardiomyopathy (CCC). It is well known that these antibodies alter the beating rate of cardiomyocytes and provoke apoptosis by their interaction with β1-adrenergic and M2-muscarinic cardiac receptors. Based on these findings, we decided to study the cellular immune response to these proteins in CCC patients compared to non-infected individuals.Methodology/Principal findings:We evaluated proliferation, presence of surface activation markers and cytokine production in peripheral blood mononuclear cells (PBMC) stimulated with P2β, the C-terminal portion of P0 (CP0) proteins and T. cruzi lysate from CCC patients predominantly infected with TcVI lineage. PBMC from CCC patients cultured with P2β or CP0 proteins, failed to proliferate and express CD25 and HLA-DR on T cell populations. However, multiplex cytokine assays showed that these antigens triggered higher secretion of IL-10, TNF-α and GM-CSF by PBMC as well as both CD4+ and CD8+ T cells subsets of CCC subjects. Upon T. cruzi lysate stimulation, PBMC from CCC patients not only proliferated but also became activated within the context of Th1 response. Interestingly, T. cruzi lysate was also able to induce the secretion of GM-CSF by CD4+ or CD8+ T cells.Conclusions/Significance:Our results showed that although the lack of PBMC proliferation in CCC patients in response to ribosomal P proteins, the detection of IL-10, TNF-α and GM-CSF suggests that specific T cells could have both immunoregulatory and pro-inflammatory potential, which might modulate the immune response in Chagas' disease. Furthermore, it was possible to demonstrate for the first time that GM-CSF was produced by PBMC of CCC patients in response not only to recombinant ribosomal P proteins but also to parasite lysate, suggesting the value of this cytokine to evaluate T cells responses in T. cruzi infection.Fil: Longhi, Silvia Andrea.     Depressão pós-parto: uma revisão que aborda seus fatores

    A depressão pós-parto é um transtorno caracterizado por tristeza profunda que afeta diversas mulheres no período próximo ao parto ou no período puerperal e é desencadeada por diversos fatores, sejam sociais, físicos e psicológicos. Esta mini revisão de literatura objetiva analisar a relação entre os fatores que desencadeiam a depressão pós-parto e a saúde da mulher, e, para isso, baseou-se em cinco artigos para responder à pergunta: “Quais os fatores que levam à depressão pós-parto?”. Dessa forma, ao serem analisados os artigos encontrou-se 18 principais fatores associados aos sintomas da depressão pós-parto, havendo discordância entre alguns resultados de pesquisas distintas, como a influência do acompanhamento paterno, mas muitos outros resultados se corresponderam, entre eles pode-se concluir que a baixa renda familiar, falta de escolaridade, falta de um acompanhamento na hora do parto e o uso de drogas são os principais fatores que contribuem para desencadear a depressão pós-parto na mulher. Portanto, a fim de ajudar diversas mulheres, o conhecimento sobre o assunto é de extrema importância

    Revisão integrativa das principais formas de tentativa de suicídio no Brasil

    A temática do suicídio ao longo dos anos foi sempre tratada como um tabu social. Uma sociedade que não pesquisa, não discute abertamente e não estuda o suicídio, ignora um problema que pode ser combatido e evitado. Assim, o objetivo dessa revisão se pauta em analisar quais são os principais tipos de tentativa de suicídio no Brasil, adentrando as principais causas de tentativa de suicídio, aos principais fatores agravantes dos pacientes suicidas, a possível existência de fatores predominantes para a escolha do tipo de tentativa de suicídio, além de analisar se a idade e o gênero interferem nessa escolha e identificar a existência de medidas preventivas. Como metodologia, foram realizadas pesquisas em duas bases de dados: SciELO (Scientific Eletronic Library Online) e Portal de Periódicos CAPES. No tocante aos descritores, utilizou-se "Tentativa de Suicídio," Suicídio" e "Fatores Epidemiológicos", ligados pelo boleando AND. Os critérios para escolha dos artigos foram: artigos originais, com publicação realizadas até 5 anos atrás e com o idioma de língua portuguesa, que respondessem à pergunta norteadora “Quais os principais tipos de suicídio ocorridos no Brasil?”. Em suma, foram encontrados 243 artigos; 45 foram selecionados para leitura de resumo, 29 para leitura íntegra e 20 para serem utilizados na revisão, totalizando 223 artigos excluídos, pois não se adequavam aos objetivos e parâmetros dessa revisão. Os 20 artigos selecionados mostraram que as principais causas de tentativa de suicídio se associam a depressão; o gênero masculino tem maior índice de suicídio; a faixa etária predominante de casos se concentra na jovem adulta; os métodos mais utilizados foram enforcamento e autointoxicação; como fatores agravantes de tentativa de suicídio relacionou-se a presença de auto violência, deficiência e transtornos; nas medidas preventivas foi observado que na maioria dos casos de suicídio houveram tentativas anteriores e a necessidade de mais campanhas sociais

    Protective Human Leucocyte Antigen Haplotype, HLA-DRB1*01-B*14, against Chronic Chagas Disease in Bolivia

    Chronic Chagas disease consists of four different forms categorized on the basis of their clinical manifestations, namely; cardiac, digestive, cardiodigestive and indeterminate. In Latin America, there are 8–10 million seropositive persons who are at risk of, or have already developed serious clinical complications and who have limited access to effective treatment. The cardiac and digestive forms are characterized by tissue damage caused by persistent infection of Trypanosoma cruzi and are thought to be modulated by host immunity. In our large scale screening for chronic Chagas disease in Santa Cruz, Bolivia, hearts and colons of 229 seropositive patients were examined. We found 31.4% of patients had abnormal electrocardiograms (ECGs), 15.7% presented with megacolon, 5.2% had a combination of abnormal ECG and megacolon, and 58.1% were of indeterminate status. Previously, we attempted to ascertain whether parasite genetic polymorphism might account for the differences in clinical manefestations, by analyzing parasite DNA taken from the same study group (with the addition of a further 62 megacolon post-operational patients).     Short-term follow-up of chagasic patients after benznidazole treatment using multiple serological markers

    <p>Abstract</p> <p>Background</p> <p>Conventional serological tests, using total soluble proteins or a cocktail of recombinant proteins from <it>T. cruzi </it>as antigens, are highly sensitive for Chagas disease diagnosis. This type of tests, however, does not seem to be reliable tools for short- and medium-term monitoring of the evolution of patients after antiparasitic treatment. The aim of the present study was to search for immunological markers that could be altered in the sera from Chagas disease patients after benznidazole treatment, and therefore have a potential predictive diagnostic value.</p> <p>Methods</p> <p>We analyzed the reactivity of sera from chagasic patients during different clinical phases of the disease against a series of immunodominant antigens, known as KMP11, PFR2, HSP70 and Tgp63. The reactivity of the sera from 46 adult Chronic Chagas disease patients living in a non-endemic country without vector transmission of <it>T. cruzi </it>(15 patients in the indeterminate stage, 16 in the cardiomiopathy stage and 16 in the digestive stage) and 22 control sera from non-infected subjects was analyzed. We also analyzed the response dynamics of sera from those patients who had been treated with benznidazole.</p> <p>Results</p> <p>Regardless of the stage of the sickness, the sera from chagasic patients reacted against KMP11, HSP70, PFR2 and Tgp63 recombinant proteins with statistical significance relative to the reactivity against the same antigens by the sera from healthy donors, patients with autoimmune diseases or patients suffering from tuberculosis, leprosy or malaria. Shortly after benznidazole treatment, a statistically significant decrease in reactivity against KMP11, HSP70 and PFR2 was observed (six or nine month). It was also observed that, following benznidazole treatment, the differential reactivity against these antigens co-relates with the clinical status of the patients.</p> <p>Conclusions</p> <p>The recombinant antigens KMP11, PFR2, Tgp63 and HSP70 are recognized by Chagas disease patients' sera at any clinical stage of the disease. Shortly after benznidazole treatment, a drop in reactivity against three of these antigens is produced in an antigen-specific manner. Most likely, analysis of the reactivity against these recombinant antigens may be useful for monitoring the effectiveness of benznidazole treatment.</p

    Usefulness of PCR-based assays to assess drug efficacy in Chagas disease chemotherapy: value and limitations

    One major goal of research on Chagas disease is the development of effective chemotherapy to eliminate the infection from individuals who have not yet developed cardiac and/or digestive disease manifestations. Cure evaluation is the more complex aspect of its treatment, often leading to diverse and controversial results. The absence of reliable methods or a diagnostic gold standard to assess etiologic treatment efficacy still constitutes a major challenge. In an effort to develop more sensitive tools, polymerase chain reaction (PCR)-based assays were introduced to detect low amounts of Trypanosoma cruzi DNA in blood samples from chagasic patients, thus improving the diagnosis and follow-up evaluation after chemotherapy. In this article, I review the main problems concerning drug efficacy and criteria used for cure estimation in treated chagasic patients, and the work conducted by different groups on developing PCR methodologies to monitor treatment outcome of congenital infections as well as recent and late chronic T. cruzi infections

    The Potential Economic Value of a Trypanosoma cruzi (Chagas Disease) Vaccine in Latin America

    The substantial burden of Chagas disease, especially in Latin America, and the limitations of currently available treatment and control strategies have motivated the development of a Trypanosoma cruzi (T. cruzi) vaccine. Evaluating a vaccine's potential economic value early in its development can answer important questions while the vaccine's key characteristics (e.g., vaccine efficacy targets, price points, and target population) can still be altered. This can assist vaccine scientists, manufacturers, policy makers, and other decision makers in the development and implementation of the vaccine. We developed a computational economic model to determine the cost-effectiveness of introducing a T. cruzi vaccine in Latin America. Our results showed vaccination to be very cost-effective, in many cases providing both cost savings and health benefits, even at low infection risk and vaccine efficacy. Moreover, our study suggests that a vaccine may actually “pay for itself”, as even a relatively higher priced vaccine will generate net cost savings for a purchaser (e.g., a country's ministry of health). These findings support continued investments in and efforts toward the development of a human T. cruzi vaccine

    Abstract Chagas disease is a neglected chronic condition with a high burden of morbidity and mortality. It has considerable psychological, social, and economic impacts. The disease represents a significant public health issue in Brazil, with different regional patterns. This document presents the evidence that resulted in the Brazilian Consensus on Chagas Disease. The objective was to review and standardize strategies for diagnosis, treatment, prevention, and control of Chagas disease in the country, based on the available scientific evidence. The consensus is based on the articulation and strategic contribution of renowned Brazilian experts with knowledge and experience on various aspects of the disease. It is the result of a close collaboration between the Brazilian Society of Tropical Medicine and the Ministry of Health. It is hoped that this document will strengthen the development of integrated actions against Chagas disease in the country, focusing on epidemiology, management, comprehensive care (including families and communities), communication, information, education, and research
