National trends in the use of oral chemotherapy over 13 years

If we consider the actual pipeline in oncology, oral chemotherapy will be the main component of cancer treatment in the next few years. This shift in the administration route requires different care models in order to guarantee treatment efficacy and safety. Objective: To analyze time trends in oral chemotherapy consumption in Portugal. Method: Oral chemotherapy consumption over a 13-year period (2008–2020) was analyzed, considering dispensed units by the administration route with respective costs, resorting to the drug regulatory agency (INFARMED I.P.) database. Oral consumption patterns were further explored using common daily doses (CDD) for three conditions, including chronic myeloid leukemia (CML), non-small-cell lung cancer (NSCLC), and breast cancer (BC), to adjust for the effect of varying doses. Data were analyzed descriptively resorting to Microsoft Office Excel 2010. Results: Overall chemotherapy consumption increased +Δ54.7%, with the highest contribution in units observed in oral forms (+Δ58.8%). The total expenditure increased +Δ96.5%, and despite the increase in oral forms (+Δ221.6%), intravenous forms continued to be the major cost driver, with an important contribution from immunotherapy. Much of the increase was led by the approval of 40 new IV and 48 new oral cancer medications with higher costs introduced in the market. Using CDD as an alternative metric to units had varying impacts by indication. The observed increases seemed to focus on specific cancer sites with varying effect; in CML, there was a 2.39-fold increase, compared to 4.41 for NSCLC and 1.86 for BC. However, for BC, two distinct sub-patterns were observed for hormone therapy (increasing 1.83) and for the novel tyrosine kinase inhibitors (increasing 40.8). Conclusion: The growing use of oral chemotherapy is obvious and calls for investments in supporting patients in managing medication adherence and adverse events. The shifts in the healthcare system are complex and need to be prioritized. Electrical behaviour of the pump working as turbine in off grid operation
[EN] The use of pumps working as turbines (PATs) connected to the electric system, in the replacement of pressure reduction valves to reduce the excessive pressure in water distribution networks, have been studied for the last years. The introduction of PATs is very important in the water-energy nexus to promote the increase of the energy savings. As consequence, the majority of the water systems does not have access to the electrical grid and, therefore, the need to study the PATS operation off-grid is necessary. In this line, the novelty of this research is the application and optimization of a PAT in water systems when the recovery solution is off-grid type. To operate correctly, the induction machine requires an external source of reactive power, which is typically provided by the electrical grid. To supply the required reactive power, a bank of capacitors is installed at the machine terminals, so-called self-excited induction generator (SEIG). The analytical model, simulation and experimental works were performed, to analyse the SEIG behaviour. The results were applied in a SEIG-PAT system obtaining the global efficiency of the system for different speeds and loads. The global efficiency decreases 47% when off-grid operation, showing the need to optimize the electrical parameters of the generator to operate as off- grid with acceptable efficiency levels. The Faecal Microbiome of the Wild European Badger Meles meles:A Comparison Against Other Wild Omnivorous Mammals from Across the Globe
Here we investigate the faecal microbiome of wild European badgers Meles meles using samples collected at post-mortem as part of the All Wales Badger Found Dead study. This is the first published characterisation of the badger microbiome. We initially undertook a sex-matched age comparison between the adult and cub microbiomes, based on sequencing the V3–V4 region of the 16S rRNA gene. Analysis used the QIIME 2 pipeline utilising DADA2 and the Silva database for taxonomy assignment. Fusobacteria appeared to be more abundant in the microbiomes of the cubs than the adults although no significant difference was seen in alpha or beta diversity between the adult and cub badger microbiomes. Comparisons were also made against other wild, omnivorous, mammals’ faecal microbiomes using publicly available data. Significant differences were seen in both alpha and beta diversity between the microbiomes from different species. As a wildlife species of interest to the disease bovine tuberculosis, knowledge of the faecal microbiome could assist in identification of infected badgers. Our work here suggests that, if comparisons were made between the faeces of bTB infected and non-infected badgers, age may not have a significant impact on the microbiome. Deficits in psychosocial functioning are present in the early stages of psychosis. Several factors, such as premorbid adjustment, neurocognitive performance, and cognitive reserve (CR), potentially influence functionality. Sex differences are observed in individuals with psychosis in multiple domains. Nonetheless, few studies have explored the predictive factors of poor functioning according to sex in first-episode psychosis (FEP). This study aimed to explore sex differences, examine changes, and identify predictors of functioning according to sex after onset. The initial sample comprised 588 individuals. However, only adults with non-affective FEP (n = 247, 161 males and 86 females) and healthy controls (n = 224, 142 males and 82 females) were included. A comprehensive assessment including functional, neuropsychological, and clinical scales was performed at baseline and at 2-year follow-up. A linear regression model was used to determine the predictors of functioning at 2-year follow-up. FEP improved their functionality at follow-up (67.4% of both males and females). In males, longer duration of untreated psychosis (β = 0.328, p = 0.003) and worse premorbid adjustment (β = 0.256, p = 0.023) were associated with impaired functioning at 2-year follow-up, while in females processing speed (β = 0.403, p = 0.003), executive function (β = 0.299, p = 0.020) and CR (β = −0.307, p = 0.012) were significantly associated with functioning. Our data indicate that predictors of functioning at 2-year follow-up in the FEP group differ according to sex. Therefore, treatment and preventative efforts may be adjusted taking sex into account. Males may benefit from functional remediation at early stages. Conversely, in females, early interventions centered on CR enhancement and cognitive rehabilitation may be recommended

Assistimos, atualmente a inúmeras parcerias e projetos de âmbito nacional e internacional na
área da formação ativa, com recurso à utilização de ferramentas digitais e assentes em processos
de cocriação entre diferentes parceiros, com o objetivo de se obterem processos/percursos mais
criativos e adequados, que respondam de forma eficaz aos desafios científicos que vivemos e se
avizinham. Neste artigo procede-se à apresentação, de forma sumária, da participação de um grupo
de docentes do Instituto Politécnico de Setúbal no Projeto DEMOLA®. Apresentamos os objetivos,
a constituição das equipas e a metodologia de trabalho estabelecida. Descrevemos as principais
fases que constituem o desenvolvimento do projeto no âmbito do IPS e os resultados que obtivemos
da análise de todas componentes (formação estudantes, docentes, desafios com as empresas/
organizações e perceções de toda a equipa). Salientam-se o desenvolvimento de competências
instrumentais (Canva, Miro, PESTLE, Portefólio); competências científicas/pedagógicas (Trabalho
em Equipa, Abordagem Multiprofissional, Processos de Cocriação, Design Thinking, Pesquisa,
Facilitação, Pensamento Disruptivo e Coconstrução, entre outros); e situações de “win-win”, ou
melhor “learn-learn” entre facilitadores e estudantes, inovação, intergerações, sustentabilidade e
