    The Bacteriology of the Biliary Tract: A preliminary report.

    The bacterial flora of acute biliary tract disease has not been completely elucidated. The organisms identified most often were coliforms. Anaerobic culture technics were not precise enough to provide proper identification. In a 12-month study of 250 patients operated upon for inflammatory gallbladder disease, culture specimens were collected in a most strict and rigorous fashion. It was hoped that a more exact understanding of the relationship between bacteria and inflammatory biliary tract disease might be appreciated. Important technics were developed for gathering specimens, delivery to the laboratory and culturing. More meaningful results are to be expected In the future

    This study analyses the different methods of preparation and the nutritional value of three Cameroonian dishes. These are: "Corn chaff", "Nnam Owondo/Ebobolo" and "Nnam Ngon/Ebobolo". "Corn chaff" is a mixture of corn (Zea mays L.), beans (Phaseolus vulgaris) and palm oil (Elaeis guinensis). "Nnam Owondo/Ebobolo" is made up of groundnut paste (Arachis  hypogaea) and cassava paste (Manihot  utilisima). "Nnam  Ngon/Ebobolo" is made up of egusi paste (Cucumeropsis mannii)  and cassava paste (Manihot utilisima). The moisture, ash, protein, lipid, fibre and carbohydrate contents were determined using standard methods. The mineral contents (magnesium, copper, iron and zinc) were determined by Atomic Absorption Spectrophotometry. A survey of these dishes in the market gave the quantity and the cost of each dish sold: A serving of "Corn chaff" which costs 200F CFA, weighs 641g; that of "Nnam Owondo/Ebobolo" which costs 150F CFA weighs 566g and that of "Nnam Ngon/Ebobolo" which costs 150F CFA weighs 507g. The proximate analysis gave the following nutritive  content: moisture (67.9; 49.5 and 53.4 g/100g F.W), ash (4.30; 1.77 and 1.54 g/100g D.W), protein (11.7; 8.96 and 7.36 g/100g D.W), lipid (17.6; 13.5 and 7.79 g/100g D.W), fibre (3.18; 1.86 and 0.84 g/100g D.W), carbohydrate (63.3; 73.8 and 82.5 g/100g D.W), magnesium (95; 108.9 and 97.9 mg/100g D.W), copper (0.57; 0.27 and 0.23 mg/100g D.W), iron (6.08; 2.97 and 3,87 mg/100g D.W) and zinc (3.74; 3,29 and 3,14 mg/100g D.W) for "Corn chaff", "Nnam Owondo/Ebobolo" and "Nnam Ngon/Ebobolo" respectively.  The content in nutrients shows that ″Corn chaff″ is balanced with respect to the  proportions of protein, lipid and carbohydrate energy. On the other hand, "Nnam  Owondo/Ebobolo" and "Nnam Ngon/Ebobolo" have low proportions of protein and lipid energy. The contents in minerals (Mg, Cu, Fe, and Zn) are low. The quantities actually ingested enable the coverage of 64; 69 and 45 % of the protein allowances and 31; 42 and 33% of the energy allowances for the tree dishes respectively, for an averagely active adult. For a good nutritional balance, it is recommended to consume a greater quantity of "Nnam Owondo " and "Nnam Ngon with smaller quantities of "Ebobolo".Key words: Cameroonian dishes, preparation, nutritional value. Dans le cadre général de la valorisation des aliments camerounais prêts à la consommation, trois plats alimentaires sont étudiés du point de vue de leurs protocoles de cuisson, de leurs valeurs nutritionnelles et des quantités de protéine, d'énergie et de minéraux qu'ils apportent aux consommateurs. Le "Corn chaff" est un mélange de maïs (Zea mays L.), de haricot  (Phaseolus vulgaris) et d’huile de palme (Elaeis guinensis). Le "Nnam Owondo/Ebobolo" est un plat composé à base de pâte  d'arachide (Arachis hypogaea) et de pâte de tubercule de manioc (Manihot utilisima). Le "Nnam Ngon/Ebobolo" est un plat composé à base de pâte de graines de courge (Cucumeropsis mannii) et de pâte de tubercule de manioc (Manihot  utilissima). La teneur en eau, cendre, protéine, lipide, fibre et glucide est déterminée par les méthodes standards. Les minéraux  (magnésium, cuivre, fer et zinc) par spectrophotométrie d’absorption atomique. L’enquête auprès des vendeuses a   donné les quantités d’aliments vendues actuellement sur le marché pour chacun des plats : 641,7g (200F CFA); 566,7 g (150F CFA) et 507,8 g (150F CFA) respectivement pour le "Corn chaff", le "Nnam Owondo /Ebobolo" et le "Nnam Ngon/Ebobolo". Les analyses ont donné les teneurs en principes nutritifs suivantes: eau (67,9; 49,5 et 53,4 g/100g M.F), cendre (4,30; 1,77 et 1,54 g/100g M.S), protide (11,7; 8,96 et 7,36 g/100g M.S), lipide (17,6; 13,5 et 7,79 g/100g M.S), fibre brute (3,18; 1,86 et 0,84 g/100g M.S), glucide (63,3; 73,9 et 82,5 g/100g M.S), magnésium (95,0; 108,1; 97,9 mg/100g M.S), cuivre (0,58; 0,27; 0,24 mg/100g M.S), fer (6,08; 2,99; 3,87 mg/100g M.S), zinc (3,74; 3,29; 3,14  mg/100g M.S) respectivement pour le "Corn chaff", le "Nnam Owondo/Ebobolo" et "Nnam Ngon/Ebobolo". Les taux de couverture des apports journaliers pour les adultes modérément actifs sont les suivants: protides (64,8; 69,3 et 45,7 %), énergie (31,4; 42,9 et 33,8%), magnésium (16,3; 25,9; 19,3%), cuivre (11,8; 7,70; 5,50), fer (6,9; 4,7; 5,1), zinc (12,8; 15,7; 12,4) respectivement pour le "Corn chaff", le "Nnam Owondo/Ebobolo" et le "Nnam Ngon/Ebobolo".Mots clés : Plats camerounais, préparation, apports nutritionnels

    Short term effects of ambient sulphur dioxide and particulate matter on mortality in 12 European cities : results from time series data from the APHEA project

    Objectives: To carry out a prospective combined quantitative analysis of the associations between all cause mortality and ambient particulate matter and sulphur dioxide. Design: Analysis of time series data on daily number of deaths from all causes and concentrations of sulphur dioxide and particulate matter (measured as black smoke or particles smaller than 10 ìm in diameter (PM10)) and potential confounders. Setting: 12 European cities in the APHEA project (Air Pollution and Health: a European Approach). Main outcome measure: Relative risk of death. Results:In western European cities it was found that an increase of 50 ìg/m3 in sulphur dioxide or black smoke was associated with a 3% (95% confidence interval 2% to 4%) increase in daily mortality and the corresponding figure for PM10 was 2% (1% to 3%). In central eastern European cities the increase in mortality associated with a 50 ìg/m3 change in sulphur dioxide was 0.8% (−0.1% to 2.4%) and in black smoke 0.6% (0.1% to 1.1%). Cumulative effects of prolonged (two to four days) exposure to air pollutants resulted in estimates comparable with the one day effects. The effects of both pollutants were stronger during the summer and were mutually independent. Conclusions:The internal consistency of the results in western European cities with wide differences in climate and environmental conditions suggest that these associations may be causal. The long term health impact of these effects is uncertain, but today's relatively low levels of sulphur dioxide and particles still have detectable short term effects on health and further reductions in air pollution are advisable

    Hydrazone chelators for the treatment of iron overload disorders: iron coordination chemistry and biological activity

    The potentially tridentate ligand 2-pyridinecarbaldehyde isonicotinoyl hydrazone (HPCIH) and its analogues are an emerging class of orally effective Fe chelators that show great promise for the treatment of Fe overload diseases. Herein, we present an extensive study of the Fe coordination chemistry of the HPCIH analogues including the first crystallographically characterised Fe-II complex of these chelators. Unlike most other clinically effective Fe chelators, the HPCIH analogues bind Fe-II and not F-III. In fact, these chelators form low-spin bis-ligand F-II complexes, although NMR suggests that the complexes are close to the high-spin/low-spin crossover. All the Fe complexes show a high potential Fe-III/(II) redox couple (> 500 mV vs. NHE) and cyclic voltammetry in aqueous or mixed aqueous/organic solvents is irreversible as a consequence of a rapid hydration reaction that occurs upon oxidation. A number of the HPCIH analogues show high activity at inducing Fe efflux from cells and also at preventing Fe uptake by cells from the serum Fe transport protein transferrin. As a class of ligands, these chelators are more effective at reducing Fe uptake from transferrin than inducing Fe mobilisation from cells. This may be related to their ability to intercept Fe-II after its release from transferrin within the cell. Our studies indicate that their Fe chelation efficacy is due, at least in part, to the fact that these ligands and their Fe-II complexes are neutral at physiological pH (7.4) and sufficiently lipophilic to permeate cell membranes

    Heme-Oxygenases during Erythropoiesis in K562 and Human Bone Marrow Cells

    In mammalian cells, heme can be degraded by heme-oxygenases (HO). Heme-oxygenase 1 (HO-1) is known to be the heme inducible isoform, whereas heme-oxygenase 2 (HO-2) is the constitutive enzyme. Here we investigated the presence of HO during erythroid differentiation in human bone marrow erythroid precursors and K562 cells. HO-1 mRNA and protein expression levels were below limits of detection in K562 cells. Moreover, heme was unable to induce HO-1, at the protein and mRNA profiles. Surprisingly, HO-2 expression was inhibited upon incubation with heme. To evaluate the physiological relevance of these findings, we analyzed HO expression during normal erythropoiesis in human bone marrow. Erythroid precursors were characterized by lack of significant expression of HO-1 and by progressive reduction of HO-2 during differentiation. FLVCR expression, a recently described heme exporter found in erythroid precursors, was also analyzed. Interestingly, the disruption in the HO detoxification system was accompanied by a transient induction of FLVCR. It will be interesting to verify if the inhibition of HO expression, that we found, is preventing a futile cycle of concomitant heme synthesis and catabolism. We believe that a significant feature of erythropoiesis could be the replacement of heme breakdown by heme exportation, as a mechanism to prevent heme toxicity

    Early neoplastic and metastatic mammary tumours of transgenic mice detected by 5-aminolevulinic acid-stimulated protoporphyrin IX accumulation

    A photodynamic technique for human breast cancer detection founded upon the ability of tumour cells to rapidly accumulate the fluorescent product protoporphyrin IX (PpIX) has been applied to transgenic mouse models of mammary tumorigenesis. A major goal of this investigation was to determine whether mouse mammary tumours are reliable models of human disease in terms of PpIX accumulation, for future mechanistic and therapeutic studies. The haeme substrate 5-aminolevulinic acid (5-ALA) (200 mg kg−1) was administered to mouse strains that develop mammary tumours of various histological subtypes upon expression of the transgenic oncogenes HRAS, Polyoma Virus middle T antigen, or Simian Virus 40 large T antigen in the mammary gland. Early neoplastic lesions, primary tumours and metastases showed consistent and rapid PpIX accumulation compared to the normal surrounding tissues, as evidenced by red fluorescence (635 nm) when the tumours were directly illuminated with blue light (380–440 nm). Detection of mouse mammary tumours at the stage of ductal carcinoma in situ by red fluorescence emissions suggests that enhanced PpIX synthesis is a good marker for early tumorigenic processes in the mammary gland. We propose the mouse models provide an ideal experimental system for further investigation of the early diagnostic and therapeutic potential of 5-ALA-stimulated PpIX accumulation in human breast cancer patients

    The importance of iron in long-term survival of maintenance hemodialysis patients treated with epoetin-alfa and intravenous iron: analysis of 9.5 years of prospectively collected data

    <p>Abstract</p> <p>Background</p> <p>In patients treated by maintenance hemodialysis the relationship to survival of hemoglobin level and administered epoetin-alfa and intravenous iron is controversial. The study aim was to determine effects on patient survival of administered epoetin-alfa and intravenous iron, and of hemoglobin and variables related to iron status.</p> <p>Methods</p> <p>The patients were 1774 treated by maintenance hemodialysis in 3 dialysis units in New York, NY from January 1998 to June, 2007. A patient-centered, coded, electronic patient record used in patient care enabled retrospective analysis of data collected prospectively. For survival analysis, patients were censored when transplanted, transferred to hemodialysis at home or elsewhere, peritoneal dialysis. Univariate Kaplan-Meier analysis was followed by multivariate analysis with Cox's regression, using as variables age, race, gender, major co-morbid conditions, epoetin-alfa and intravenous iron administered, and 15 laboratory tests.</p> <p>Results</p> <p>Median age was 59 years, epoetin-alfa (interquartile range) 18,162 (12,099, 27,741) units/week, intravenous iron 301 (202, 455) mg/month, survival 789 (354, 1489) days. Median hemoglobin was 116 (110, 120)g/L, transferrin saturation 29.7 (24.9, 35.1)%, serum ferritin 526 (247, 833) μg/L, serum albumin 39.0 (36.3, 41.5) g/L. Survival was better the higher the hemoglobin, best with > 120 g/L. Epoetin-alfa effect on survival was weak but had statistically significant interaction with intravenous iron. For intravenous iron, survival was best with 1–202 mg/month, slightly worse with 202–455 mg/month; it was worst with no intravenous iron, only slightly better with > 455 mg/month. Survival was worst with transferrin saturation ≤ 16%, serum ferritin ≤ 100 μg/L, best with transferrin saturation > 25%, serum ferritin > 600 μg/L The effects of each of hemoglobin, intravenous iron, transferrin saturation, and serum ferritin on survival were independently significant and not mediated by other predictors in the model.</p> <p>Conclusion</p> <p>Long term survival of maintenance hemodialysis patients was favorably affected by a relatively high hemoglobin level, by moderate intravenous iron administration, and by indicators of iron sufficiency. It was unfavorably influenced by a low hemoglobin level, and by indicators of iron deficiency.</p
