    Consensus statement of the Spanish Society of Internal Medicine and the Spanish Society of Medical Oncology on secondary thromboprophylaxis in patients with cancer

    Up to 20% of cancer patients will develop some manifestation of venous thromboembolic disease (VTD) during their clinical course. VTD greatly impacts morbidity, mortality, quality of life and pharmaceutical expenditure. In addition, both thrombotic relapse and major haemorrhages derived from VTD treatment are more likely in oncological patients. To make the decision to establish secondary thromboprophylaxis as an indefinite treatment in these patients, it is important to review all the risk factors involved, whether related to the disease, the patient or the prior thrombotic event. The objectives of this consensus of the Spanish Society of Internal Medicine (Sociedad Española de Medicina Interna -SEMI) and the Spanish Society of Medical Oncology (Sociedad Española de Oncología Médica -SEOM) are to establish recommendations that help assess the risk of recurrence of VTD and haemorrhagic risk in patients with cancer, as well as to analyse the evidence that exists on the currently available drugs, which will allow the establishment of a protocol for shared decision-making with the informed patient

    Trachelomonas (Euglenophyta) from a eutrophic reservoir In Central Mexico

    This study provides valuable information on the ultrastructure and environmental conditions of the Trachelomonas Ehr. (Euglenophyceae) genus in the Guadalupe Dam, a eutrophic reservoir located in the suburbs of Mexico City, which receives a considerable volume of wastewaters. Specimens were collected at surface level between November 2005 and May 2006. Using LM and SEM twelve taxa from phytoplankton were identified of which, 9 are new records for Mexico. The reservoir is warm monomictic, with basic pH values (7.4-10.1), a high concentration of chlorophyll a(18-101 microg l(-1), a permanent anoxic bottom, specific conductivity (K25) of 205 to 290 microS cm(-1), N-NO3, 0.19-1.2 mg l(-1) and P-PO4 0.22-1.6 mg l(-1). Water temperature was 15.6-23.0 degrees C. Most of the Trachelomonas species were found during the dry season, when concentrations of organic matter, nitrogen and phosphorus as well as the temperature were the highest. Higher species richness was also associated with the warmer months. This research contributes to increase our knowledge on Trachelomonas in Mexico and constitutes the first detailed description of lorica ultrastructure of 12 taxa that grow in a body of water with high concentration of nutrients and a moderate amount of mineral contents.Fil: Garduño Solorzano, Gloria.     Se describen las características anatómicas y morfológicas de la hojas compuestas deAlvaradoaamorphoidesLiebm.procedentede una selva baja caducifolia ubicada en el municipio deTlaquiltenango, Morelos, México. Las hojas se fijaron y se prepararon para obtener secciones transversales, longitudinales yparadermalesen un criostatoLeicaa -20°C. Se tiñeron con safranina-azulalcianoy azul de toluidina; se deshidrataron y montaron con resina. Se siguieron los protocolos dediafanizaciónpara describir la arquitectura foliar. Para las descripciones anatómicas y morfológicas se midieron: los pecíolos ypeciólulos, la lámina, los pelos, las células epidérmicas y los parénquimas en empalizada y esponjoso. A los caracteres mensurables se les aplicaron los siguientes análisis: de varianzas de una vía o en su caso pruebas no paramétricas. Se registraron datos deΨxy valores de conductancia estomática. Los resultados muestran: hojas compuestas y pubescentes imparipinnadas confilotaxishelicoidal, con venaciónbroquidódromafestonada. Hojasbifaciadashipostomáticas, con estomasanomocíticosy hundidos en cámaras pre-estomáticas con células epidérmicas modificadas en papilas. Elpecióluloes móvil, presenta parénquima cortical con drusas y depósitos de origenpolifenólico. Los valores deΨhfueron del orden de -3.29 y -2.01MParegistrados en junio y septiembre respectivamente. Se observó que aunque la morfología de la epidermis y el movimiento foliar reducen el área de exposición de la hoja a la radiación solar directa, los espacios intercelulares delmesófilopermiten el desarrollo de unainterfaselíquido-vapor que logra valores hasta de 313mmol/m2sdegsen promedio, lo que sugiere el continuo abastecimientode una capa límite que evita un posible sobrecalentamiento de las hojas

    Lack of cytomegalovirus detection in human glioma

    Gliomas are the most common brain tumors and include a variety of histologic types and grades of malignancy. They arise from glial cells and represent approximately 70% of the primary brain tumors. According to the criteria of the World Health Organization (WHO), the majority of gliomas can be classified into four grades of malignancy (I-IV). Virus infection, especially by DNA viruses and retroviruses, which may cause insertion of viral DNA sequences into the host genome, often triggers the host defense mechanisms. Particularly, the DNA methylation machinery can be activated to cause the methylation of foreign movable viral sequences and, therefore, silence viral gene expression. Several studies have shown the presence of Human Cytomegalovirus (HCMV) in glioblastoma, suggesting that the virus may participate in tumor pathogenesis. But this relationship is controversial because many other studies did not detect HCMV in these tumors. This study aims to detect the presence of HCMV in several samples of human glioma (94 formalin-fixed, paraffin-embedded samples and 28 snap-frozen samples) by different sensitive techniques. We have been unable to detect HCMV DNA and proteins in glioma samples.     Current professional standing of young medical oncologists in Spain : a nationwide survey by the Spanish Society of Medical Oncology + MIR section

    There is a lack of knowledge about the career paths and employment situation of young medical oncologists. The aim of our study was to evaluate the current professional standing of these professionals in Spain. The Spanish Society of Medical Oncology + MIR section conducted a national online survey in May 2021 of young medical oncology consultants (< 6 years of expertise) and final year medical oncology residents. A total of 162 responses were eligible for analysis and included participants from 16 autonomous communities; 64% were women, 80% were consultants, and 20% were residents. More than half of the participants performed routine healthcare activity and only 7% research activity. Almost three quarters (73%) were subspecialized in a main area of interest and almost half of these chose this area because it was the only option available after residency. Half of the respondents (51%) considered working abroad and 81% believed the professional standing in Spain was worse than in other countries. After finishing their residency, only 22 were offered a job at their training hospital. Just 16% of participants had a permanent employment contract and 87% were concerned (score of ≥ 5 on a scale of 1-10) about their job stability. In addition, one quarter of the participants in our study showed an interest in increasing their research activity. The choice of subspecialty in medical oncology may depend on job opportunities after residency rather than personal interest. The abundance of temporary contracts may have influenced the job stability concerns observed. Future mentoring strategies should engage in building a long-term career path for young medical oncologists.     Lead, Cadmium and Cobalt (Pb, Cd, and Co) Leaching of Glass-Clay Containers by pH Effect of Food

    Recent studies have shown that handcrafted glass-clay containers are a health risk because they can be contaminated by heavy metals, which can be transferred to food, thus reaching the human body to potentially cause illness. Therefore, in the present work, we evaluate the leaching of lead, cadmium, and cobalt from glass-clay containers into two types of food: tomato sauce (salsa), and chickpea puree. The containers were obtained from four regions in the Mexican state of Hidalgo. Repetitive extractions from the containers were carried out to quantify the leaching of the heavy metals into the salsa, the chickpea puree, and acetic acid using the technique proposed by the USFDA. The results show that greater use of the containers leads to more leaching of heavy metals into both types of food and into the acetic acid, with the greatest metal extraction recorded for the Ixmiquilpan vessels. These results indicate that the metals present in the glass-clay containers leach into the food and that increased reuse increases the risk to the people who use them in food preparation

    Good practice recommendations to better coordinate the management of oncological pain: a Delphi survey

    Abstract Treatment of oncological pain is complex and requires a multidisciplinary management approach between oncology services and pain units. Although significant improvements have been achieved in the treatment and overall survival of cancer patients, the management of oncological pain has not followed the same directions. Many patients are not referred to pain units even though they could benefit from it. The purpose of this Delphi survey was to map the current situation in the management of cancer pain, identify barriers and propose recommendations to improve its management by emphasizing the importance of collaboration and coordination between oncology services and pain units. A survey among members with recognized experience in the management of oncology patients and oncological pain was held based on the Delphi method principles. The experts were asked to vote preselected statements on cancer pain management in two rounds and conclusions and recommendations were formulated based on the consensus reached for each statement. Barriers and areas for improvement were identified: need of multidisciplinary management approach, effective communication between oncology services and pain units, timely referral of cancer patients to pain units, training of health care professionals dealing with cancer aspects and identification of those patients that could benefit from a multidisciplinary management of their oncological disease. The experts issued recommendations targeting the identified barriers and areas for improvement by defining the service requirements of hospital and units treating cancer pain patients, establishing referral pathways necessities and adopted measures to improve the care of cancer patients

    A Delphi Study on the Management of Neuropathic Cancer Pain in Spain: The DOLNEO Study

    Purpose: The objectives of this project were to assess the current situation and management of cancer-related neuropathic pain (CRNP) in Spain and to provide specific recommendations for the assessment, diagnosis and treatment of CRNP using a Delphi methodology.Methods: This was a qualitative study that followed a Delphi methodology using a questionnaire with 56 statements that were grouped into 5 areas related to CRNP: prevalence and impact, pathophysiology, assessment and diagnosis, specific syndromes, treatment, and multidisciplinary approach. Based on the responses, the scientific committee prepared an algorithm and a recommended pathway for the management of CRNP.Results: Seventy-nine physicians attended the meeting and completed the questionnaire. Consensus was reached for all statements relating to the prevalence and impact of CRNP. However, the perceptions of specialists from palliative care of the frequency and impact of CRNP differed from those of other specialists. A high degree of consensus was reached for all statements concerning the assessment and diagnosis of CRNP. Regarding specific syndromes, the only statement with a lack of consensus was that on the frequency of NP in patients undergoing radiotherapy. There were some disagreements regarding the multidisciplinary approach and referral criteria for the management of NP.Conclusion: Our results show a large degree of agreement on the assessment, diagnosis and treatment of cancer-related neuropathic pain among the specialists involved in its management.     La madera deTrema micrantha (L.) Blume de Veracruz, MĂ©xico

    The microscopic anatomy, hydraulic and mechanical indices of the wood of 28 trees of the species Trema micrantha of four localities in the municipalities of Xico and Coatepec in Veracruz State, Mexico were studied. In addition, macroscopic characteristics such as relative density and moisture content were determined. Results showed that T. micrantha has a mesomorphic xylem characterized by low resistance to cavitation, low values of relative density, low indices for vessel and fiber collapse and a high index of vulnerability. An analysis of variance shows that of all the characteristics studied, only the rate of collapse of the vessels is similar between individuals and populations. Multiple comparison tests show that wall thickness and the number of rays separate populations Bola de Oro and Barrales from the other sites of study.Carpentry tests reveal that wood from T. micrantha is resistant to splitting when nailed, is well suited to assemblage, and thus, it is recommended for use in carpentry in rural areas.Se estudió la anatomía microscópica, los índices hidráulicos y mecánicos de la madera de 28 árboles de Trema micrantha de cuatro localidades ubicadas en los municipios de Xico y Coatepec en el estado de Veracruz. Adicionalmente, se determinaron las características macroscópicas, la densidad relativa, el contenido de humedad y algunas pruebas de carpintería en probetas de diferentes tamaños de la madera de tres árboles, con el fin de proponer usos alternativos en zonas rurales. Los resultados mostraron que T. micrantha presenta un xilema mesomórfico, poco resistente a la cavitación, de baja densidad relativa, con valores bajos de índices de colapso de vaso y fibras y altos para el índice de vulnerabilidad. Los análisis de varianza confirman que de todas las características estudiadas solamente el índice de colapso de los vasos es similar entre indivíduos y poblaciones. Las comparaciones múltiples arrojan que los caracteres que separan a las localidades Bola de Oro y Los Barrales de las otras son el grosor de la pared y el número de radios. Las pruebas tecnológicas describen una madera que no se raja al clavado, responde bien al ensamblado y al torneado por lo que se recomienda para su uso en la carpintería rural
