Vitamin D deficiency: awareness and practice
Within the UK rickets is emerging as a national public health issue, particularly among at-risk groups. There is concern that health professionals are overlooking recommendations for vitamin supplementation and opportunities for prevention. This paper reports on a study that aimed to identify current knowledge and practice regarding vitamin D deficiency and supplementation
among community midwifery and health visiting teams employed in one NHS trust. A questionnaire was distributed to all health visiting and community midwifery team members (n=96), with a 76% response rate (n=73). Results suggest varying levels of awareness about vitamin
D deficiency and groups at increased risk. Only 52% reported that they were aware of health department recommendations for vitamin supplementation. Health visiting teams recommended supplements more frequently than community midwives, but overall the recommendations were implemented inconsistently. Participants identified families eligible for the government Healthy Start scheme, but fewer were recommending Healthy Start vitamins. A deeper understanding of vitamin D deficiency and health department recommendations for
vitamin supplementation is required by health professionals to ensure families are advised about appropriate prevention messages and to implement recommendations effectively.

A thematic analysis of smokersâ and non-smokersâ accounts of E-cigarettes
This study explored smokersâ and non-smokersâ accounts of E-cigarettes. A total of 51 UK-based participants, 20 men and 31 women, responded to open-ended questions online. Inductive thematic analysis identified that the factors that influence E-cigarette behaviour and opinion in adult smokers and non-smokers are related to social context, informative sources, practical aspects and health implications. Participants presented varying accounts of E-cigarettes, suggesting that individual narratives regarding E-cigarettes are multi-faceted. This is important information for health professionals and policy makers tasked with advising on E-cigarette use
Detection of Leishmania spp. in Chronic Dermatitis: Retrospective Study in Exposed Horse Populations
Leishmania infantum is a protozoan causing human zoonotic visceral leishmaniasis (ZVL) and visceralâcutaneous canine leishmaniosis (CanL) in the Mediterranean Basin. L. infantum is able to infect a large number of wild and domestic species, including cats, dogs, and horses. Since the 1990s, clinical cases of equine leishmaniasis (EL), typically characterized by cutaneous forms, have been increasingly diagnosed worldwide. The aim of the present study was to evaluate the presence of clinical forms of EL in CanL-endemic areas in Italy, where exposure of equine populations was ascertained from recent serological surveys. For this purpose, formalin-fixed and paraffin-embedded skin biopsies of 47 horses presenting chronic dermatitis compatible with EL were retrospectively selected for the study and subjected to conventional and q-PCR. A singular positivity for L. infantum was found; BLAST analysis of sequence amplicons revealed a 99â100% homology with L. infantum sequences. The histological examination revealed a nodular lymphoplasmacytic and histiocytic infil-trate; immunohistochemistry showed rare macrophages containing numerous positive amastigotes. The present retrospective study reports, for the first time, a case of a cutaneous lesion by L. infantum occurring in an Italian horse. Pathological and healthy skin samples should be investigated on a larger scale to provide information on the potential clinical impact of EL in the practice, and to define the role of horses in epidemiological ZVL and CanL scenarios
Welsh Primary Schoolchildren's Perceptions of Electronic Cigarettes: A Mixed Methods Study.
There are concerns that the growing popularity of e-cigarettes promotes experimentation among children. Given the influence of the early years on attitude and habit formation, better understanding of how younger children perceive vaping before experimentation begins is needed, to prevent uptake and inform tobacco control strategies. We explored Welsh primary schoolchildren's (aged 7-11) awareness of e-cigarettes relative to tobacco smoking, their understanding of the perceived risks and benefits and their intentions and beliefs about vaping. Data was collected using a mix of methods in June and July 2017 from 8 purposively selected primary schools across Wales. Four hundred and ninety-five children (52% female) aged 7 years (n = 165), 9 years (n = 185) and 11 years (n = 145) completed a class-administered booklet encompassing a draw and write exercise and survey. Ninety-six children participated in 24 peer discussion groups comprised of 2 boys and 2 girls from each year group. Data were analysed independently and findings triangulated. Survey analyses used frequencies, descriptive statistics and chi-squared tests. Content analysis was undertaken on the draw and write data and peer discussion groups were analysed thematically. Study findings highlight that primary schoolchildren have general awareness of e-cigarettes. Vaping was perceived to be healthier than smoking and there was some recognition that e-cigarettes were used for smoking cessation. Understanding of any health harms was limited. Few children intended to smoke or vape in the future but almost half thought it was okay for grownups. Children's perceptions were influenced by exposure through family and friends. Findings suggest a need for e-cigarette education in primary schools, to highlight the associated risks of e-cigarette experimentation including the potential for tobacco initiation
Using discursive approaches to examine the utility and functions of language in public health and health promotion: highlighting social constructions of e-cigarettes
This article uses discursive approaches to examine the utility and functions of language in public health, focusing on social constructions of e-cigarettes. Due to the ambiguity surrounding the use of e-cigarettes, understanding may be negotiated collaboratively through co-construction in talk. Ten participants, three men and seven women aged 26â47 years, took part in two focus groups in Manchester, UK, where they discussed e-cigarettes. Data were analysed using blended discourse analysis, with a view to identifying dominant repertoires used by speakers. Participants drew from two discursive frameworks to communicate perceptions of e-cigarettes: (1) uncertainty and risk and (2) the social acceptability and stigma spectrum. The ambiguity surrounding e-cigarettes was reflected in the linguistic devices used in talk. This article demonstrates the value of drawing on discourse analysis to better understand the impact of health-related communication by providing insight into how existing messages are interpreted, co-constructed, and assigned meaning through shared interactions
Influence of family and friend smoking on intentions to smoke and smoking-related attitudes and refusal self-efficacy among 9-10 year old children from deprived neighbourhoods: a cross-sectional study.
BACKGROUND: Smoking often starts in early adolescence and addiction can occur rapidly. For effective smoking prevention there is a need to identify at risk groups of preadolescent children and whether gender-specific intervention components are necessary. This study aimed to examine associations between mother, father, sibling and friend smoking and cognitive vulnerability to smoking among preadolescent children living in deprived neighbourhoods. METHODS: Cross-sectional data was collected from 9-10 year old children (n =1143; 50.7% girls; 85.6% White British) from 43 primary schools in Merseyside, England. Children completed a questionnaire that assessed their smoking-related behaviour, intentions, attitudes, and refusal self-efficacy, as well as parent, sibling and friend smoking. Data for boys and girls were analysed separately using multilevel linear and logistic regression models, adjusting for individual cognitions and school and deprivation level. RESULTS: Compared to girls, boys had lower non-smoking intentions (Pâ=â0.02), refusal self-efficacy (Pâ=â0.04) and were less likely to agree that smoking is 'definitely' bad for health (Pâ<â0.01). Friend smoking was negatively associated with non-smoking intentions in girls (Pâ<â0.01) and boys (Pâ<â0.01), and with refusal self-efficacy in girls (Pâ<â0.01). Sibling smoking was negatively associated with non-smoking intentions in girls (Pâ<â0.01) but a positive association was found in boys (Pâ=â0.02). Boys who had a smoking friend were less likely to 'definitely' believe that the smoke from other people's cigarettes is harmful (OR 0.57, 95% CI: 0.35 to 0.91, Pâ=â0.02). Further, boys with a smoking friend (OR 0.38, 95% CI: 0.21 to 0.69, Pâ<â0.01) or a smoking sibling (OR 0.45, 95% CI: 0.21 to 0.98) were less likely to 'definitely' believe that smoking is bad for health. CONCLUSION: This study indicates that sibling and friend smoking may represent important influences on 9-10 year old children's cognitive vulnerability toward smoking. Whilst some differential findings by gender were observed, these may not be sufficient to warrant separate prevention interventions. However, further research is needed
Using formative research to develop the healthy eating component of the CHANGE! school-based curriculum intervention
Background: Childhood obesity is a significant public health concern. Many intervention studies have attempted
to combat childhood obesity, often in the absence of formative or preparatory work. This study describes the
healthy eating component of the formative phase of the Childrenâs Health Activity and Nutrition: Get Educated!
(CHANGE!) project. The aim of the present study was to gather qualitative focus group and interview data
regarding healthy eating particularly in relation to enabling and influencing factors, barriers and knowledge in
children and adults (parents and teachers) from schools within the CHANGE! programme to provide populationspecific
evidence to inform the subsequent intervention design.
Methods: Semi-structured focus group interviews were conducted with children, parents and teachers across 11
primary schools in the Wigan borough of North West England. Sixty children (N = 24 boys), 33 parents (N = 4 male)
and 10 teachers (N = 4 male) participated in the study. Interview questions were structured around the PRECEDE
phases of the PRECEDE-PROCEED model. Interviews were transcribed verbatim and analysed using the pen-profiling
technique.
Results: The pen-profiles revealed that childrenâs knowledge of healthy eating was generally good, specifically
many children were aware that fruit and vegetable consumption was âhealthyâ (N = 46). Adultsâ knowledge was also
good, including restricting fatty foods, promoting fruit and vegetable intake, and maintaining a balanced diet. The
important role parents play in childrenâs eating behaviours and food intake was evident. The emerging themes
relating to barriers to healthy eating showed that external drivers such as advertising, the preferred sensory
experience of âunhealthyâ foods, and food being used as a reward may play a role in preventing healthy eating.
Conclusions: Data suggest that; knowledge related to diet composition was not a barrier per se to healthy eating,
and education showing how to translate knowledge into behavior or action is required. The key themes that
emerged through the focus groups and pen-profiling data analysis technique will be used to inform and tailor the
healthy eating component of the CHANGE! intervention study.

Write, draw, show, and tell: a child-centred dual methodology to explore perceptions of out-of-school physical activity
Background
Research to increase childrenâs physical activity and inform intervention design has, to date, largely underrepresented childrenâs voices. Further, research has been limited to singular qualitative methods that overlook childrenâs varied linguistic ability and interaction preference. The aim of this study was to use a novel combination of qualitative techniques to explore childrenâs current views, experiences and perceptions of out-of-school physical activity as well as offering formative opinion about future intervention design.
Methods
Write, draw, show and tell (WDST) groups were conducted with 35 children aged 10â11 years from 7 primary schools. Data were analysed through a deductive and inductive process, firstly using the Youth Physical Activity Promotion Model as a thematic framework, and then inductively to enable emergent themes to be further explored. Pen profiles were constructed representing key emergent themes.
Results
The WDST combination of qualitative techniques generated complimentary interconnected data which both confirmed and uncovered new insights into factors relevant to childrenâs out-of-school physical activity. Physical activity was most frequently associated with organised sports. Fun, enjoyment, competence, and physical activity provision were all important predictors of childrenâs out-of-school physical activity. Paradoxically, parents served as both significant enablers (i.e. encouragement) and barriers (i.e. restricting participation) to physical activity participation. Some of these key findings would have otherwise remained hidden when compared to more traditional singular methods based approaches.
Conclusions
Parents are in a unique position to promote health promoting behaviours serving as role models, physical activity gatekeepers and choice architects. Given the strong socialising effect parents have on childrenâs physical activity, family-based physical activity intervention may offer a promising alternative compared to traditional school-based approaches. Parents' qualitative input is important to supplement childrenâs voices and inform future family-based intervention design. The WDST method developed here is an inclusive, interactive and child-centred methodology which facilitates the exploration of a wide range of topics and enhances data credibility
Barriers and facilitators to extended working lives in Europe : a gender focus
Background: There is a global imperative to respond to the challenge of a growing âold-age dependency ratioâ by ensuring the workforce is healthy enough to remain in work for longer. Currently more than half of older workers leave before the default retirement age, and in some countries (e.g. the United Kingdom) the time spent in retirement is increasing. At the same time across Europe there is a gender employment gap, with 14.5% fewer female workers between 55-64 years old, and a large variation in the participation of older women in the workforce (ranging from 30%-75%). As older women are under-represented in the workforce, increasing employment in this group has the propensity to go some way towards reducing the old-age dependency ratio to ensure continued economic growth.
Objectives: This review explores the barriers and facilitators to extended working lives in Europe, particularly those than impact on women.
Methods: A systematic mapping review process was undertaken using four electronic databases, Medline, PsychoInfo, Psych Extra via Ovid and Age Line via EBSCO, using the terms, âworkâ, âageingâ, âretirementâ, âpensionâ, âoldâ, âbarrierâ, âextended working lifeâ, âgenderâ, âhealth and wellbeingâ. Hand searching was also carried out in the Journal of Aging and Human Development and the Journal of Ageing and Society.
Results: The search resulted in 15 English language studies published from 1st January 2005 to the current date that met the inclusion criteria.
Key findings: The key factors that influenced decisions to retire or extend working lives in Europe, were health; social factors; workplace factors; and financial security and pension arrangements.
Conclusions and implications of key findings: Health was found to be the most commonly cited barrier to extended working lives in Europe, and a number of social inequalities to work exist by gender. Structural factors exist, such as the gender pay gap, which disadvantage women, while the nature of work itself differs by gender and can have a negative impact on health. Currently women tend to exit the labour market earlier than men, however, changes in the state pension age are resulting in women being required to work for as long as men, in most countries. For women to remain healthy at work, workplaces need to consider a range of interventions, including flexible arrangements to both work and retirement to enable women to balance the demands of work with domestic and caring responsibilities that particularly impact on them
