Shared decision-making performance of general practice residents: an observational study combining observer, resident, and patient perspectives
Background: Shared decision making (SDM) is considered fundamental to person-centred care. However, applying SDM may be a challenge for residents in general practice, since it is a complex competence that requires the integration of knowledge and skills from several competency domains. Objective: To support learning of SDM during medical residency, we aimed to gain insight in Dutch residents’ observed and perceived SDM performance in general practice. Methods: We evaluated residents’ SDM performance from an observer, resident, and patient perspective. Consultations of first- and third-year residents were recorded. Trained observers used the validated Observing Patient Involvement (OPTION5) scale to assess observed SDM performance of residents in 98 actual recorded consultations. Perceived SDM performance was evaluated by residents and patients completing validated SDM questionnaires, supplemented with questions about (the context of) the consultation and perceived relevance of SDM immediately after the consultation. The data were analysed using descriptive statistics (mean, SD, minimums, and maximums) and explorative bivariate analyses. Results: The residents’ observed mean SDM performance was 19.1 (range, 0–100, SD = 10.9), mean resident self-reported SDM performance was 56.9 (range, 0–100, SD = 18.5), and mean patient-reported SDM performance was 73.3 (range, 0–100, SD = 26.8). We found a significant and positive correlation between observed SDM performance and residents’ perceived relevance of SDM for the consultation (t = 4.571, P ≤ 0.001) and the duration of the consultation (r = 0.390, P ≤ 0.001). Conclusions: This study showed that there is room for increasing awareness of the potential incongruence between observed and perceived SDM performance during medical residency, in order to facilitate the implementation of SDM in clinical practice

To study whether glucocorticoid (GC) resistance delineates disease-relevant T helper (Th)
subsets that home to the CNS of patients with early MS.

ABCB1) and glucocorticoid receptor (GR/NR3C1), was investigated in proinflammatory Th
subsets and compared between natalizumab-treated patients with MS and healthy individuals.

Search for composite and exotic fermions at LEP 2
A search for unstable heavy fermions with the DELPHI detector at LEP is
reported. Sequential and non-canonical leptons, as well as excited leptons and
quarks, are considered. The data analysed correspond to an integrated
luminosity of about 48 pb^{-1} at an e^+e^- centre-of-mass energy of 183 GeV
and about 20 pb^{-1} equally shared between the centre-of-mass energies of 172
GeV and 161 GeV. The search for pair-produced new leptons establishes 95%
confidence level mass limits in the region between 70 GeV/c^2 and 90 GeV/c^2,
depending on the channel. The search for singly produced excited leptons and
quarks establishes upper limits on the ratio of the coupling of the excited
fermio
Search for lightest neutralino and stau pair production in light gravitino scenarios with stau NLSP
Promptly decaying lightest neutralinos and long-lived staus are searched for
in the context of light gravitino scenarios. It is assumed that the stau is the
next to lightest supersymmetric particle (NLSP) and that the lightest
neutralino is the next to NLSP (NNLSP). Data collected with the Delphi detector
at centre-of-mass energies from 161 to 183 \GeV are analysed. No evidence of
the production of these particles is found. Hence, lower mass limits for both
kinds of particles are set at 95% C.L.. The mass of gaugino-like neutralinos is
found to be greater than 71.5 GeV/c^2. In the search for long-lived stau,
masses less than 70.0 to 77.5 \GeVcc are excluded for gravitino masses from 10
to 150 \eVcc . Combining this search with the searches for stable heavy leptons
and Minimal Supersymmetric Standard Model staus a lower limit of 68.5 \GeVcc
may be set for the stau mas
Measurement of spin correlation in ttbar production using dilepton final states
We measure the correlation between the spin of the top quark and the spin of
the anti-top quark in (ttbar -> W+ W- b bbar -> l+ nu b l- nubar bbar) final
states produced in ppbar collisions at a center of mass energy sqrt(s)=1.96
TeV, where l is an electron or muon. The data correspond to an integrated
luminosity of 5.4 fb-1 and were collected with the D0 detector at the Fermilab
Tevatron collider. The correlation is extracted from the angles of the two
leptons in the t and tbar rest frames, yielding a correlation strength C=
0.10^{+0.45}_{-0.45}, in agreement with the NLO QCD prediction within two
standard deviations, but also in agreement with the no correlation hypothesis.Comment: 10 pages, 3 figures, submitted to PL
Search for single top quarks in the tau+jets channel using 4.8 fb of collision data
We present the first direct search for single top quark production using tau
leptons. The search is based on 4.8 fb of integrated luminosity
collected in collisions at =1.96 TeV with the D0 detector
at the Fermilab Tevatron Collider. We select events with a final state
including an isolated tau lepton, missing transverse energy, two or three jets,
one or two of them tagged. We use a multivariate technique to discriminate
signal from background. The number of events observed in data in this final
state is consistent with the signal plus background expectation. We set in the
tau+jets channel an upper limit on the single top quark cross section of
\TauLimObs pb at the 95% C.L. This measurement allows a gain of 4% in expected
sensitivity for the observation of single top production when combining it with
electron+jets and muon+jets channels already published by the D0 collaboration
with 2.3 fb of data. We measure a combined cross section of
\SuperCombineXSall pb, which is the most precise measurement to date.Comment: 12 pages, 5 figure
Measurement of the ttbar Production Cross Section in ppbar Collisions at sqrt{s} = 1.96 TeV using Kinematic Characteristics of Lepton + Jets Events
We present a measurement of the top quark pair ttbar production cross section
in ppbar collisions at a center-of-mass energy of 1.96 TeV using 230 pb**{-1}
of data collected by the DO detector at the Fermilab Tevatron Collider. We
select events with one charged lepton (electron or muon), large missing
transverse energy, and at least four jets, and extract the ttbar content of the
sample based on the kinematic characteristics of the events. For a top quark
mass of 175 GeV, we measure sigma(ttbar) = 6.7 {+1.4-1.3} (stat) {+1.6- 1.1}
(syst) +/-0.4 (lumi) pb, in good agreement with the standard model prediction.Comment: submitted to Phys.Rev.Let
Measurement of Z/gamma*+jet+X angular distributions in ppbar collisions at sqrt{s}=1.96 TeV
We present the first measurements at a hadron collider of differential cross
sections for Z+jet+X production in delta phi(Z, jet), |delta y(Z, jet)| and
|y_boost(Z, jet)|. Vector boson production in association with jets is an
excellent probe of QCD and constitutes the main background to many small cross
section processes, such as associated Higgs production. These measurements are
crucial tests of the predictions of perturbative QCD and current event
generators, which have varied success in describing the data. Using these
measurements as inputs in tuning event generators will increase the

We present a measurement of the top quark pair () production cross
section () in collisions at TeV
using 230 pb of data collected by the D0 experiment at the Fermilab

lifetime-based b-jet identification techniques to further enhance the

b-Jet Identification in the D0 Experiment
Algorithms distinguishing jets originating from b quarks from other jet
flavors are important tools in the physics program of the D0 experiment at the
Fermilab Tevatron p-pbar collider. This article describes the methods that have
been used to identify b-quark jets, exploiting in particular the long lifetimes
of b-flavored hadrons, and the calibration of the performance of these
