The 4.0 revolution is driving a digital transformation, in which emerging technologies are changing
business models. Blockchain technology thanks to its decentralisation, security and traceability
promises to revolutionise the tourism industry. But, how and which areas of tourism are most likely
to be transformed? How will it affect the intermediaries? What acceptance will it have among
tourists?
To address these questions, a study about the functioning and applications of this technology in
tourism has been carried out among the population of Tenerife. Analysing the results, a great
acceptance of the applications of this technology is observed, especially among millennials. A
series of actions and recommendations are proposed to adapt the industry to the new tourism

Efficacy of clozapine versus standard treatment in adult individuals with intellectual disability and treatment-resistant psychosis (CLOZAID): study protocol of a multicenter randomized clinical trial
BackgroundIntellectual disability (ID) affects approximately 1% of the worldwide population and individuals with ID have a higher comorbidity with mental illness, and specifically psychotic disorders. Unfortunately, among individuals with ID, limited research has been conducted since ID individuals are usually excluded from mental illness epidemiological studies and clinical trials. Here we perform a clinical trial to investigate the effectiveness of clozapine in the treatment of resistant psychosis in individuals with ID. The article highlights the complexity of diagnosing and treating psychopathological alterations associated with ID and advocates for more rigorous research in this field.MethodsA Phase IIB, open-label, randomized, multicenter clinical trial (NCT04529226) is currently ongoing to assess the efficacy of oral clozapine in individuals diagnosed with ID and suffering from treatment-resistant psychosis. We aim to recruit one-hundred and fourteen individuals (N=114) with ID and resistant psychosis, who will be randomized to TAU (treatment as usual) and treatment-with-clozapine conditions. As secondary outcomes, changes in other clinical scales (PANSS and SANS) and the improvement in functionality, assessed through changes in the Euro-QoL-5D-5L were assessed. The main outcome variables will be analyzed using generalized linear mixed models (GLMM), assessing the effects of status variable (TAU vs. Clozapine), time, and the interaction between them.DiscussionThe treatment of resistant psychosis among ID individuals must be directed by empirically supported research. CLOZAID clinical trial may provide relevant information about clinical guidelines to optimally treat adults with ID and treatment-resistant psychosis and the benefits and risks of an early use of clozapine in this underrepresented population in clinical trials.Trial registrationClinicaltrials.gov: NCT04529226. Evaluation of appendicitis risk prediction models in adults with suspected appendicitis
Background
Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis.
Methods
A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis).
Results
Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent).
Conclusion
Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy. Risk prediction models to support shared decision‐making by identifying adults in the UK at low risk of appendicitis were identified
Modelling and Analysis of Vibrations in a UAV Helicopter with a Vision System
The analysis of the nature and damping of unwanted vibrations on Unmanned Aerial Vehicle (UAV) helicopters are important tasks when images from on-board vision systems are to be obtained. In this article, the authors model a UAV system, generate a range of vibrations originating in the main rotor and design a control methodology in order to damp these vibrations. The UAV is modelled using VehicleSim, the vibrations that appear on the fuselage are analysed to study their effects on the on-board vision system by using Simmechanics software. Following this, the authors present a control method based on an Adaptive Neuro-Fuzzy Inference System (ANFIS) to achieve satisfactory damping results over the vision system on board
Optimal Arrangements of Renewable Energy Systems for Promoting the Decarbonization of Desalination Plants
In this research, a renewable energy hybrid system (PV-Wind) is modeled to compare different design options based on their economic and technical features. The energy requirements of a Reversible Osmosis desalination plant located on the island of Tenerife with a water production capacity of up to 20,000 m3/day was considered. The system is connected to the electricity grid. The HOMER software, version 2.75 was used to produce optimum strategies for renewable energy. The assumptions input into the model were: the technical specifications of the devices, electricity demand of the desalination plant, as well as the solar radiation and the wind speed potentials. Numerous arrangements were considered by the software, version 2.75. The optimal results were obtained based on the use of renewable energy. The data used in the study were recorded in Tenerife in the Canary Islands. The experience of this research could be transferred to other Atlantic islands with similar renewable energy sources (specifically the wind) and water scarce conditions
