    Stroke prevalence among the Spanish elderly: an analysis based on screening surveys

    BACKGROUND: This study sought to describe stroke prevalence in Spanish elderly populations and compare it against that of other European countries. METHODS: We identified screening surveys -both published and unpublished- in Spanish populations, which fulfilled specific quality requirements and targeted prevalence of stroke in populations aged 70 years and over. Surveys covering seven geographically different populations with prevalence years in the period 1991–2002 were selected, and the respective authors were then asked to provide descriptions of the methodology and raw age-specific data by completing a questionnaire. In addition, five reported screening surveys in European populations furnished useful data for comparison purposes. Prevalence data were combined, using direct adjustment and logistic regression. RESULTS: The overall study population, resident in central and north-eastern Spain, totalled 10,647 persons and yielded 715 cases. Age-adjusted prevalences, using the European standard population, were 7.3% for men, 5.6% for women, and 6.4% for both sexes. Prevalence was significantly lower in women, OR 0.79 95% CI 0.68–0.93, increased with age, particularly among women, and displayed a threefold spatial variation with statistically significant differences. Prevalences were highest, 8.7%, in suburban, and lowest, 3.8%, in rural populations. Compared to pooled Spanish populations, statistically significant differences were seen in eight Italian populations, OR 1.39 95%CI (1.18–1.64), and in Kungsholmen, Sweden, OR 0.40 95%CI (0.27–0.58). CONCLUSION: Prevalence in central and north-eastern Spain is higher in males and in suburban areas, and displays a threefold geographic variation, with women constituting the majority of elderly stroke sufferers. Compared to reported European data, stroke prevalence in Spain can be said to be medium and presents similar age- and sex-specific traits

    Plan de AcciĂłn Tutorial Integrado, en EducaciĂłn

    En el proyecto que se presenta se parte no solamente de una amplia trayectoria de los Grados de la Facultad de Ciencias de la EducaciĂłn de la UGR en orientaciĂłn universitaria, sino, ademĂĄs, de una necesidad patente plasmada en los Objetivos EstratĂ©gicos que se incluyen dentro del Sistema para la GarantĂ­a de la Calidad de la Facultad; siendo el segundo objetivo “OrientaciĂłn y tutorĂ­a acadĂ©mica”, y una de sus acciones (Plan Director 19-21): Crear curriculum complementario para los estudiantes (desarrollo de competencias transversales a travĂ©s de un PAT). Otras acciones relacionadas y en las que continuamos trabajando en el Plan Director 22 24 son Establecer acciones de tutorizaciĂłn coordinadas y complementarias al PAT, formar a los tutores de grupo-curso, establecer un plan general de orientaciĂłn profesional y empleabilidad, potenciar la orientaciĂłn acadĂ©mica y profesional de los estudiantes NEAE, entre otras. Con el diseño del Plan de AcciĂłn Tutorial Integrado, en EducaciĂłn, se pretende desarrollar en el estudiantado de los Grados en PedagogĂ­a, Ed. Social, Ed. Infantil y Ed. Primaria, competencias transversales que complementen su formaciĂłn acadĂ©mica y le ayuden en su desarrollo acadĂ©mico, personal y profesional. En Ă©l participarĂĄ profesorado, alumnado y PAS de la Facultad, no solamente para fomentar su sentimiento de pertenencia y favorecer un clima de comunidad universitaria, sino ademĂĄs, para enriquecerse unos a otros, aprendiendo juntos, solventando dudas, guiando, orientando y haciendo que el paso por la Universidad de todo el alumnado que participe del mismo, sea lo mĂĄs enriquecedor y productivo posible, logrando un elevado rendimiento acadĂ©mico, un adecuado desarrollo personal y un posterior futuro profesional colmado de Ă©xitos. Para ello se ha llevado a cabo un anĂĄlisis de necesidades mediante el Cuestionario para detecciĂłn de necesidades formativas transversales del alumnado en la Facultad de Ciencias de la EducaciĂłn (Escala de competencias transversales del cuestionario CECTGRA de MartĂ­nez y GonzĂĄlez, 2018), a partir del cual se diseñan diferentes talleres orientados a cada curso segĂșn sus intereses, motivaciones y necesidades, haciendo una posterior evaluaciĂłn de los resultados y un anĂĄlisis de los mismos. Se ha logrado ademĂĄs del diseño del PAT, dotar de unidad a todas las actividades que desde la Facultad se realizaban en torno a la orientaciĂłn del estudiantado. Se ha realizado un esfuerzo porque los resultados obtenidos se vean reflejados en las diferentes materias, no solamente a nivel transversal sino de forma directa. Por ejemplo, al trabajarse la normativa APA se ha introducido este punto en las rĂșbricas de evaluaciĂłn de diferentes asignaturas para, de este modo, relacionar su aprendizaje con la evaluaciĂłn y rendimiento acadĂ©mico. Al centrarse el primer curso fundamentalmente en el anĂĄlisis de necesidades, la asistencia ha sido mucho mayor en el segundo curso (21-22), haciendo especial hincapiĂ© en la difusiĂłn de las actividades, siguiendo el Plan de ComunicaciĂłn del Centro, a travĂ©s de la web de la Facultad y de la figura de los tutores de grupo curso. Un objetivo a largo plazo, es incluir las competencias aprendidas en los talleres dirigidos al estudiantado de 4Âș en la calificaciĂłn del Trabajo Fin de Grado. Este punto no ha dado tiempo a consolidarlo en dos cursos acadĂ©micos, pero seguiremos trabajando en ello ya que consideramos que es fundamental ademĂĄs de poder dar de esta forma respuesta a una de las alegaciones realizadas por la DEVA en el informe de la renovaciĂłn de la acreditaciĂłn de los Grados.This project is based not only on a wide trajectory of the Degrees of the Faculty of Education Sciences of the UGR in university orientation, but, in addition, from a patent need embodied in the Strategic Objectives that are included within the System for the Guarantee of the Quality of the Faculty; the second objective being "Academic guidance and tutoring", and one of its actions (Master Plan 19-21): Create complementary curriculum for students (development of transversal competences through a PAT). Other related actions and in which we continue to work in the Master Plan 22 24 are To establish coordinated and complementary tutoring actions to the PAT, to train the group-course tutors, to establish a general plan of professional orientation and employability, to enhance the academic and professional orientation of the NEAE students, among others. With the design of the Integrated Tutorial Action Plan, in Education, it is intended to develop in the students of the Degrees in Pedagogy, Social Ed., Infant Ed. and Primary Ed., transversal competences that complement their academic training and help them in their academic, personal and professional development. It will involve teachers, students and PAS of the Faculty, not only to promote the feeling of belonging of the same and favor a climate of university community, but, in addition, to enrich each other, learning together, solving doubts, guiding, guiding and making the passage through the University of all the students who participate in it, as enriching and productive as possible, achieving a high academic performance, a competent personal development and a subsequent professional future full of successes. To this end, a needs analysis has been carried out (through the Questionnaire for the detection of transversal training needs of students in the Faculty of Education Sciences (Scale of transversal competences of the CECTGRA questionnaire of MartĂ­nez y GonzĂĄlez, 2018), reflected in different workshops oriented to each course according to their interests, motivations and needs, making a subsequent evaluation of the results and analysis of them. It has been achieved in addition to the design of the PAT, to provide unity to all the activities that from the Faculty were carried out around the orientation of the students. We have worked so that the results obtained are reflected in the different subjects, not only at a transversal level but directly. For example, when working on the APA regulations, it has been introduced in the evaluation rubrics of different subjects in order to relate their learning with evaluation and academic performance. By focusing the first course mainly on the analysis of needs, the attendance has been much higher in the second year (21 22), with special emphasis on the dissemination of the activities, following the Communication Plan of the Center, through the website of the Faculty and the figure of the tutors of the course group. A long-term objective is to include the skills learned in the workshops of the 4th grade students in the TFG qualification. This point has not given time to consolidate it in two academic years but we will continue working on it since we consider that it is fundamental in addition to being able to respond in this way to one of the allegations made by the DEVA in the report of the renewal of the accreditation of the Degrees.Unidad de Calidad, InnovaciĂłn Docente y Prospectiva.     Evolving trends in the management of acute appendicitis during COVID-19 waves. The ACIE appy II study

    Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide

    Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≄20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≄30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference) and obesity (BMI >2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents, the increases in double burden were driven by increases in obesity, and decreases in double burden by declining underweight or thinness. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesit

    Omecamtiv mecarbil in chronic heart failure with reduced ejection fraction, GALACTIC‐HF: baseline characteristics and comparison with contemporary clinical trials

    Aims: The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials. Methods and Results: Adults with established HFrEF, New York Heart Association functional class (NYHA) ≄ II, EF ≀35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure < 100 mmHg (n = 1127), estimated glomerular filtration rate < 30 mL/min/1.73 m2 (n = 528), and treated with sacubitril‐valsartan at baseline (n = 1594). Conclusions: GALACTIC‐HF enrolled a well‐treated, high‐risk population from both inpatient and outpatient settings, which will provide a definitive evaluation of the efficacy and safety of this novel therapy, as well as informing its potential future implementation

    Neste artigo, respondemos algumas interrogantes que formulamos a respeito da emergĂȘncia histĂłrica das funçÔes elĂ­pticas na primeira metade do sĂ©culo XIX. Em primeiro lugar, queremos determinar as forças mais relevantes que produziam tensĂ”es na disciplina durante a Ă©poca considerada. Para isso, vamos propor uma hipĂłtese explicativa sobre o giro na esfera do pensamento matemĂĄtico, o qual sucedeu quando foram preferidas as funçÔes em lugar das integrais elĂ­pticas. Essa hipĂłtese se centra no estado da "AnĂĄlise" e da "Álgebra" durante a Ă©poca da emergĂȘncia. A seguir mostraremos que as construçÔes de Abel e Jacobi sobre as funçÔes elĂ­pticas suportam a nossa hipĂłtese de trabalho. Por Ășltimo, delinearemos algumas conclusĂ”es referentes Ă s nossas reflexĂ”es.Dans ce mĂ©moire nous rĂ©pondons Ă  certaines questions que nous avons posĂ©es sur et autour de l'Ă©mergence historique des fonctions elliptiques dans la premiĂšre moitiĂ© du XIXe siĂšcle. Avec cela, nous proposons une hypothĂšse expliquant le changement dans le domaine de la pensĂ©e mathĂ©matique qui a abouti Ă  des fonctions de prĂ©fĂ©rence au lieu d'intĂ©grales elliptiques. Cette hypothĂšse est enracinĂ©e dans l'Ă©tat de l'analyse et l'algĂšbre Ă  l'Ă©poque de l'Ă©mergence. Tout de suite nous montrerons que les constructions d'Abel et de Jacobi supportent notre hypothĂšse de travail. First, we want to determine the most relevant forces that produced tensions in the discipline during the period considered. To do this, we propose an explanatory hypothesis on the overturning in the field of mathematical thinking that resulted when the functions were preferred instead of the elliptic integrals. This hypothesis is rooted in the state of "Analysis" and "Algebra" during the emergence time. Then we show that the constructions of Abel and Jacobi for the elliptic functions support our working hypothesis. Finally, we outline some conclusions concerning our reflection

    Durante varios años Gabriel Pareja estuvo buscando la llave que abre el bruñido cofre de las funciones elípticas. Como dedicado profesor de Cålculo, Gabriel buscaba sobre todo una manera sencilla de explicarlas a sus alumnos. Y un día del año 2011, alentado por un conocido maestro ruso, le fue revelada la respuesta evidente que no se dejaba ver: las funciones elípticas han de entenderse como una generalización de las funciones circulares. Desde aquel día, sus dos compañeros de trabajo y él mismo se dieron a la tarea de escribir un libro que explicara cabalmente los fundamentos de las funciones elípticas y que, a la vez, recreara su circunstancia histórica desde Euler hasta Jacobi. Dicho libro es el que el lector tiene ahora en sus manos. Con esta publicación pulimos los resultados de nuestro libro anterior titulado Integrales elípticas con notas históricas, en el que nos dedicamos mås a las integrales y al estado del arte de la disciplina en el siglo XVIII. Este nuevo libro contiene, ademås, algunos resultados del proyecto de investigación La emergencia de las funciones elípticas en la primera mitad del siglo XIX, que ha sido cofinanciado por el Comité Central de Investigaciones de la Universidad del Tolima y la Vicerrectoría de Investigaciones de la Universidad de Medellín. Esta vez tenemos también mayor dominio de la técnica de escribir a tres manos. El libro se ha marinado y cocinado muy lentamente
