    Barreiras de acesso a tratamento para mães com depressão pós-parto em centros de atenção primária: um modelo preditivo

    Objective to develop a predictive model to evaluate the factors that modify the access to treatment for Postpartum Depression (PPD). Methods prospective study with mothers who participated in the monitoring of child health in primary care centers. For the initial assessment and during 3 months, it was considered: sociodemographic data, gyneco-obstetric data, data on the services provided, depressive symptoms according to the Edinburgh Postpartum Depression Scale (EPDS) and quality of life according to the Short Form-36 Health Status Questionnaire (SF-36). The diagnosis of depression was made based on MINI. Mothers diagnosed with PPD in the initial evaluation, were followed-up. Results a statistical model was constructed to determine the factors that prevented access to treatment, which consisted of: item 2 of EPDS (OR 0.43, 95%CI: 0.20-0.93) and item 5 (OR 0.48, 95%CI: 0.21-1.09), and previous history of depression treatment (OR 0.26, 95%CI: 0.61-1.06). Area under the ROC curve for the model=0.79; p-value for the Hosmer-Lemershow=0.73. Conclusion it was elaborated a simple, well standardized and accurate profile, which advises that nurses should pay attention to those mothers diagnosed with PPD, presenting low/no anhedonia (item 2 of EPDS), scarce/no panic/fear (item 5 of EPDS), and no history of depression, as it is likely that these women do not initiate treatment.Objetivo desarrollar un modelo predictivo para evaluar los factores que modifiquen el acceso a tratamiento para la DPP. Métodos estudio prospectivo de madres que asistieron a control de niño sano en centros de atención primaria. En evaluación basal y 3 meses, se registraron: datos sociodemográficos, gineco-obstétricos, variables de uso de servicios, síntomas depresivos con la Escala de Depresión Posparto de Edimburgo (EPDS) y calidad de vida con el Cuestionario de Salud SF-36. El diagnóstico de depresión se hizo con el MINI. Se siguió a madres que en evaluación basal tenían DPP. Resultados se construyó un modelo estadístico para determinar los factores que impidieron el acceso a tratamiento, compuesto por: ítems del EPDS 2 (OR 0,43, IC95%: 0,20-0,93) y 5 (OR 0,48, IC95%: 0,21-1,09), e historia previa de tratamiento de depresión (OR 0,26, IC95%: 0,61-1,06). Área bajo la curva ROC para el modelo=0,79; valor de p para la prueba de Hosmer-Lemershow=0,73. Conclusión se elaboró un perfil simple, bien calibrado y discriminante, que sugiere que los/las enfermeros/as estén atentos/as a aquellas madres con DPP que presenten baja/nula anhedonia (ítem 2 EPDS), escaso/nulo pánico/miedo (ítem 5 EPDS), y sin antecedentes de depresión, ya que es probable que estas mujeres no ingresen a tratamiento.Objetivo desenvolver um modelo preditivo para avaliar os fatores que modificam o acesso a tratamento para a DPP. Métodos estudo prospectivo com mães que participaram do acompanhamento da saúde da criança em centros de atenção primária. Na avaliação inicial e durante 3 meses, foram registrados: dados sociodemográficos, gineco-obstétricos, dados sobre o uso dos serviços, sintomas depressivos de acordo com a Escala de Depressão Pós-parto de Edimburgo (EPDS) e qualidade de vida de acordo com o Questionário de Saúde SF-36. O diagnóstico de depressão foi feito com o MINI. Foram acompanhadas as mães que tinham DPP na avaliação inicial. Resultados foi construído um modelo estatístico para determinar os fatores que impediram o acesso a tratamento, constituído por: item 2 da EPDS (OR 0,43, IC95%: 0,20-0,93) e 5 (OR 0,48, IC95%: 0,21-1,09), e história prévia de tratamento para depressão (OR 0,26, IC95%: 0,61-1,06). Área sob a curva ROC para o modelo=0,79; valor de p para o teste de Hosmer-Lemershow=0,73. Conclusão foi elaborado um perfil simples, bem padronizado e preciso, que recomenda que os/as enfermeiros/as estejam atentos/as àquelas mães com DPP que apresentem anedonia baixa/nula (item 2 da EPDS), pânico/medo escasso/nulo (item 5 da EPDS) e sem antecedentes de depressão, já que é provável que estas mulheres não entrem em tratamento

    Análisis, diseño y desarrollo de un prototipo de sistema SaaS para las plantas de packing de manzanas

    148 p.Chile es el quinto país exportador de manzanas del mundo, y a nivel nacional las regiones del Maule y O’Higgins concentran el 87% de la superficie plantada con manzanos. Las manzanas exportadas necesariamente pasan por las plantas de packing, en las cuales los frutos son seleccionados y embalados, para posteriormente ser trasladados a los puertos de embarque. Más detalladamente, el proceso ejecutado dentro de las plantas de packing se divide en vaciado, selección y embalado. Es a lo largo de este proceso que la empresa de servicios agroindustriales Just in Time a identificado diversos cuellos de botella. Dicha empresa se dedica a prestar servicios a distintas plantas de packing de la región del Maule, ejecutando procesos de selección y embalajes completos. Como empresa Just in Time se ha propuesto crear un sistema informático que permita mejorar la toma de decisiones de las plantas de packing de manzanas, para de esta forma mejorar la planificación y por ende la producción de las mismas. Por otra parte, es sabido que las empresas del rubro en general son reacias a adquirir sistemas tecnológicos para mejorar sus procesos productivos, dado a que gran parte de los empresarios consideran esta inversión en función de los costos actuales en lugar de las ganancias futuras. Tomando en cuenta el nuevo modelo de distribución de software SaaS, el cual disminuye sustancialmente los costos de adquisición de sistemas por parte de las empresas, es que Just in Time a tomado la decisión de desarrollar un prototipo de sistema en base a este modelo de distribución. En esencia, este documento contiene todo el proceso de construcción del prototipo del sistema mencionado anteriormente, incluyendo la revisión de las arquitecturas de sistemas SaaS, la arquitectura Rest, patrones y principios de diseño, junto con la metodología utilizada y el paso a paso de las funcionalidades implementadas

    Preyssler Heteropolyacids in the Self-Etherification of 5-Hydroxymethylfurfural to 5,5′-[Oxybis(methylene)]bis-2-furfural Under Mild Reaction Conditions

    The synthesis of 5,5′-[oxybis(methylene)]bis-2-furfural (OBMF) from 5-hydroxymethylfurfural (5-HMF) was studied using bulk and alumina-supported Preyssler heteropolyacids (HPAs). The formation of OBMF was related to the amount of Brønsted acid sites, and the lowest yield of OBMF was obtained with supported HPAs. However, the Lewis acidity of the HPA supported on Al2O3 favored the formation of 2,5-dimethylfurane. The effects of solvent, catalyst loading, temperature, and reaction time on the selectivity to OBMF from 5-HMF were studied to optimize OBMF production using bulk Preyssler HPAs; a yield of 84 % to OBMF was obtained at 5 h and 343 K. These results demonstrate that bulk Preyssler HPA is a good candidate for OBMF synthesis under mild reaction conditions.Fil: Páez, Alexander.     Synthesis of 1,4-dihydropyrimidines with immobilized urease: Effect of method immobilization on magnetic supports

    The effect of the urease immobilization method was studied on magnetic supports for the Biginelli/Hantzsch reaction. For this purpose, Fe3O4/SiO2 was modified with 3-Aminopropyl-Triethoxysilane and then activated with glutaraldehyde. A ratio of 500 mg of enzyme per gram of support at 4°C and 18 h were sufficient for the physical adsorption, while 24 h were required for covalent bonding. The Biginelli and Hantzsch reactions were used to evaluate urease application in multicomponent reactions (MCRs). The synthesis of 1,4-dihydropyrimidines was successfully performed using immobilized urease favoring the Hantzsch product. The magnetic properties of the supports allow easy separation, and the urease immobilized by both methods improved the enzymatic activity compared to that of free urease.Centro de Investigación y Desarrollo en Ciencias Aplicada

    Oral diseases : a 14-year experience of a Chilean institution with a systematic review from eight countries

    Retrospective studies to assess the distribution of oral diseases (ODs) are helpful in estimating the prevalence of oral diagnoses in the population, and thus help in preventive and curative services. Prevalence and frequency data for ODs are available from many countries, but information from Chile is scarce. This study investigated the frequency of ODs in a Chilean population. For this, we included all patients treated at the University of Talca (UTALCA, Chile) between 2001 and 2014. Patient characteristics were retrieved from medical files. To contextualize our results, we conducted a systematic review (SystRev) using Publish or Perish software (PoP), Google Scholar and MEDLINE/PubMed. One hundred sixty-six ODs were diagnosed, and the most prevalent groups were soft tissue tumours, epithelial pathology and salivary gland pathology. Individually, irritation fibroma, oral lichen planus (OLP) and mucocele were the most common diagnoses. ODs frequently affected unspecified parts of the mouth (including cheek, vestibule and retromolar area), gum, lips, tongue and palate. In the SystRev, the more studied diagnoses were leukoplakia, OLP and recurrent aphthous stomatitis; prevalent lesions included Fordyce?s spots, recurrent aphthous stomatitis and fissured tongue. Chilean patients and SistRev shared almost all ODs. The results reflect ODs diagnosed in a specialized service of oral pathology and medicine in Chile and will allow the establishment of preventive/curative policies, adequate health services and dentistry curriculum

    Synthesis of mesoporous Ca-MCM catalysts and their use in suitable multicomponent synthesis of polyfunctionalized pyrans

    Mesoporous Ca-MCM catalysts were prepared using a mixture of CaCO3, TEOS, cetyl trimethyl ammonium bromide (CTAB), triethanolamine (TEA), ethanol and H₂O. Three catalysts with different CaCO3 contents, M-Ca 1 %, M-Ca 5 % and M-Ca 10 %, respectively, were prepared and characterized by N₂ physisorption, XRD, FT-IR, XPS, TPD-CO₂ and Pyridine-FTIR. A green and simple protocol was developed for the synthesis of 6-amino-4H-pyran scaffolds through a multicomponent coupling reaction of an aldehyde, malononitrile, and 1,3-dicarbonyl compound in different reaction solvents. This procedure was performed at room temperature (20 °C), obtaining good and excellent yields of 4H-pyran derivatives. The mesoporous materials are insoluble in polar media, which allows easy removal of the reaction products without affecting their catalytic activity.     Ternary hydrotalcites in the multicomponent synthesis of 4H-pyrans

    Lamellar double hydroxides (LDH) with double divalent cations were synthesized by the co-precipitation method and studied in the multicomponent synthesis of 4H-pyrans. The solids obtained were characterized by X-ray diffraction (XRD), infrared spectroscopy (FTIR), N2 adsorption isotherms, and thermogravimetric analysis (TGA). The XRD patterns confirmed the formation of LDHs in which the incorporation of Ni2+ or Co2+ improves their crystalline and textural properties. The results of catalytic activity showed that the synthesis of 4H-pyrans is favored in solvent-free conditions with the LDH–Ni catalyst, avoiding calcination processes. In addition, it was found that hydrotalcite with double divalent cations can conduct this reaction through multicomponent synthesis or by the Michael addition reaction, which can be performed by different types of basicity that depend on the composition of another divalent cation in the brucite layer or a calcination process.Fil: Nope Vargas, Eliana Rocio.     Fungal colonization with Pneumocystis correlates to increasing chloride channel accessory 1 (hCLCA1) suggesting a pathway for up-regulation of airway mucus responses, in infant lungs

    AbstractFungal colonization with Pneumocystis is associated with increased airway mucus in infants during their primary Pneumocystis infection, and to severity of COPD in adults. The pathogenic mechanisms are under investigation. Interestingly, increased levels of hCLCA1 – a member of the calcium-sensitive chloride conductance family of proteins that drives mucus hypersecretion – have been associated with increased mucus production in patients diagnosed with COPD and in immunocompetent rodents with Pneumocystis infection. Pneumocystis is highly prevalent in infants; therefore, the contribution of Pneumocystis to hCLCA1 expression was examined in autopsied infant lungs. Respiratory viruses that may potentially increase mucus, were also examined. hCLCA1 expression was measured using actin-normalized Western-blot, and the burden of Pneumocystis organisms was quantified by qPCR in 55 autopsied lungs from apparently healthy infants who died in the community. Respiratory viruses were diagnosed using RT-PCR for RSV, metapneumovirus, influenza, and parainfluenza viruses; and by PCR for adenovirus. hCLCA1 levels in virus positive samples were comparable to those in virus-negative samples. An association between Pneumocystis and increased hCLCA1 expression was documented (P=0.028). Additionally, increasing Pneumocystis burden correlated with increasing hCLCA1 protein expression levels (P=0.017). Results strengthen the evidence of Pneumocystis-associated up-regulation of mucus-related airway responses in infant lungs. Further characterization of this immunocompetent host-Pneumocystis-interaction, including assessment of potential clinical significance, is warranted

    Examination of the Feynman-Hibbs Approach in the Study of NeN_N-Coronene Clusters at Low Temperatures

    Feynman-Hibbs (FH) effective potentials constitute an appealing approach for investigations of many-body systems at thermal equilibrium since they allow us to easily include quantum corrections within standard classical simulations. In this work we apply the FH formulation to the study of NeN_N-coronene clusters (N=N= 1-4, 14) in the 2-14 K temperature range. Quadratic (FH2) and quartic (FH4) contributions to the effective potentials are built upon Ne-Ne and Ne-coronene analytical potentials. In particular, a new corrected expression for the FH4 effective potential is reported. FH2 and FH4 cluster energies and structures -obtained from energy optimization through a basin-hoping algorithm as well as classical Monte Carlo simulations- are reported and compared with reference path integral Monte Carlo calculations. For temperatures T>4T> 4 K, both FH2 and FH4 potentials are able to correct the purely classical calculations in a consistent way. However, the FH approach fails at lower temperatures, especially the quartic correction. It is thus crucial to assess the range of applicability of this formulation and, in particular, to apply the FH4 potentials with great caution. A simple model of NN isotropic harmonic oscillators allows us to propose a means of estimating the cut-off temperature for the validity of the method, which is found to increase with the number of atoms adsorbed on the coronene molecule
