    Assessment of the Right-to-Try Law

    On May 30, 2018, the Right-To-Try bill, allowing the use of experimental, non-Food and Drug Administration (FDA)-approved drugs as a last resort for those unable to participate in clinical testing who have also exhausted all other treatment options, was signed into law (1). At the time this bill was signed, 38 states had passed similar Right-To-Try laws; this law is nationwide (1). The requirements for granting approval of experimental drug treatment under Right-To-Try laws are: (1) a terminally ill patient has exhausted all other treatment options and is ineligible to participate in clinical trials, (2) the experimental drug passes FDA phase 1 clinical testing, (3) the patients’ health-care provider or treating physician must recommend and approve the experimental treatment, (4) the pharmaceutical manufacturer must approve the drug for use as an experimental treatment, and (5) patients’ written informed consent is required (2,3).     Dynamic Surface Tension of Surfactants Through Separation of Diffusion and Dilation Effects

    Surfactants are biphilic monomers that adsorb to interfaces and have the ability to reduce interfacial tension. From this reduction in interfacial tension, surfactants are able to function as emulsifiers, detergents, and wetting agents among a variety of other things. Despite their ubiquity surfactants and surfactant behaviors, in particular those in highly dynamic environments, are not yet well characterized. In these environments, surfactants undergo significant distortions to droplet, bubble, and foam geometries which induce changes to the surface tension. This establishes a need to understand how surfactants respond to distortions of geometry through diffusion and dilational effects. To address this need, we characterize the dilational interfacial rheology of a pulsating pendant bubble in water-surfactant mixtures using the nonionic surfactants TWEEN 20 and TWEEN 40. This study considers two separate but concurrent processes: dilation of the bubble interface and diffusion of surfactants to the bubble interface. Through a timescale comparison of both of these separate processes, a new way to contextualize surfactant adsorption behavior and bubble mechanics was developed based on a single dimensionless Peclet number. This dimensionless parameter accurately describes changes to the surface tension for different surfactants and concentrations through a combination of both diffusion and dilation effects. We hope that a detailed analysis consisting of a single dimensionless number capable of capturing the effects caused by the combination of both diffusion and dilation will better inform a selection of surfactants for specific industries and applications

    Surfactant Use in Boiling Heat Transfer Alludes to a Universal Concentration?

    Surfactants—molecules that change the surface properties of liquids—can enhance the rate of heat transfer in boiling, which could improve power generation, heating/cooling/refrigeration, and other applications. Boiling involves cycles of bubble nucleation, growth, and departure. These behaviors are modified as surfactants adsorb to the bubble surface, lowering surface tension and changing the bubble contact angle. Unique to each type of surfactant, there is a critical micelle concentration (CMC) above which surfactant molecules aggregate into larger structures called micelles. From conventional wisdom, adding surfactants beyond the CMC increases liquid viscosity, resulting in an overall lowering of heat transfer. Thus, to optimize heat transfer, the optimal concentration should be near the CMC. This is because the Reynolds and Rayleigh numbers of the liquid decrease—reducing the ability of the liquid to convect heat. However, our testing has found that different surfactants experience a marked “bump” in heat transfer performance occurring at the 6–7 mM range. Our results suggest the existence of a universal optimized concentration for surfactants. We hypothesize that this concentration is due to the rate surfactants enter the bubbly region via liquid motion (advection) matching the rate at which surfactants exit the region via bubble surfaces. We are continuing experiments with nonionic (TWEEN families) and ionic (sodium sulfate families) surfactants with vastly different CMCs to see if they all experience a similar “bump” around 6–7 mM.     Illness trajectory from prodromal symptoms to incident bipolar disorder and schizophrenia

    Background: The concept of prodrome is highly variable but, when applied retrospectively in cohorts who progress to meet diagnostic criteria for bipolar disorder (BD) and schizophrenia (SZ), it can contribute to clarify early origins and trajectory of the illness. In the context of race and ethnicity, studies with population-based and registry-based cohorts, such as the Rochester Epidemiology Project (REP), can potentially identify, through retrospective assessment of the prodromes of BD and SZ, differences while addressing the heterogeneous nature of the illness. Objective: We aim to describe and compare the illness trajectories of individuals with BD or SZ prior to the incident case. Additionally, we assessed for differences in access to healthcare in racially diverse patients and within those with schizophrenia, we sought to analyze differences in prodrome duration based on place of birth (U.S. born vs. foreign born). Methods: Using a records-linkage system from Minnesota, we searched for subjects born after 1985 that had been diagnosed with BD or SZ. Cases were ascertained for diagnosis and identification of the first episode of mania or psychosis. Using the medical records we extracted data from their medical history prior to their first episode. Results: We identified 205 cases with the first episode of psychosis or mania (SZ = 131; BD = 74). The mean age of onset for BD was 21.34 years and 20.45 years for SZ. We did not find a difference between the mean duration of the trajectory of mental health problems to a first episode. Both disorders were preceded by high healthcare utilization and had similar rates of psychiatric diagnoses, substance use, and prescriptions of psychiatric medications. SZ was more common in Non-white patients and in immigrants and BD had a higher rate of depressive disorder and adjustment disorders. Conclusions: Results from this study provide critical information on social and clinical features that precede a first manic or psychotic episode that may help early illness detection, identification of individuals at high risk of BD and SZ and address health disparities

    How Dynamic Adsorption Controls Surfactant‑enhanced Boiling

    Improving boiling is challenging due to the unpredictable nature of bubbles. One way to enhance boiling is with surfactants, which alter the solid–liquid and liquid–vapor interfaces. The conventional wisdom established by previous studies suggests that heat transfer enhancement is optimized near the critical micelle concentration (CMC), which is an equilibrium property that depends on surfactant type. However, these studies only tested a limited number of surfactants over small concentration ranges. Here, we test a larger variety of nonionic and anionic surfactants over the widest concentration range and find that a universal, optimal concentration range exists, irrespective of CMC. To explain this, we show that surfactant-enhanced boiling is controlled by two competing phenomena: (1) the dynamic adsorption of surfactants to the interfaces and (2) the increase in liquid dynamic viscosity at very high surfactant concentrations. This dynamic adsorption is time-limited by the millisecond-lifetime of bubbles on the boiling surface—much shorter than the timescales required to see equilibrium behaviors such as CMC. At very high concentrations, increased viscosity inhibits rapid bubble growth, reducing heat transfer. We combine the effects of adsorption and viscosity through a simple proportionality, providing a succinct and useful understanding of this enhancement behavior for boiling applications

    La presente investigación Evaluación de riesgos laborales en la empresa manufacturera Scandinavian Tobacco GROUP S.A, que se encuentra ubicada en el kilómetro 145 carretera panamericana Estelí, Nicaragua. El diagnóstico fue elaborado en base a la Ley 618 ley de Higiene y Seguridad en el trabajo, la cual establece criterios que deben ser cumplidos con el fin de crear un ambiente seguro e higiénico para disminuir riesgos laborales que puedan provocar enfermedades profesionales y accidentes. Se realizó inspecciones en cada una de las áreas, mediante una guía de observación basada en la ley, mediciones y encuestas que fueron aplicadas para recopilación de información para mostrar las condiciones en que se encuentra la empresa. Posteriormente se procesaron las encuestas para el análisis de los riesgos existente en cada puesto de trabajo a los cuales los colaboradores se encontraban expuestos con mayor frecuencia. De acuerdo al resultado del diagnóstico se procedió a realizar la matriz de riesgo para identificar el nivel de riesgo que están expuestos los colaboradores. Se procedió a realizar la propuesta de un plan de acción para planificar medidas preventivas que se deben implementar en las áreas requeridas de acuerdo a los resultados del diagnóstico. Por lo tanto, se determinó la factibilidad económica de la propuesta del plan de acción mediante el costo-beneficio dando como resultado 1.52 la relación, esto indicando que se recuperara la inversión

    Theseus: A Library for Differentiable Nonlinear Optimization

    We present Theseus, an efficient application-agnostic open source library for differentiable nonlinear least squares (DNLS) optimization built on PyTorch, providing a common framework for end-to-end structured learning in robotics and vision. Existing DNLS implementations are application specific and do not always incorporate many ingredients important for efficiency. Theseus is application-agnostic, as we illustrate with several example applications that are built using the same underlying differentiable components, such as second-order optimizers, standard costs functions, and Lie groups. For efficiency, Theseus incorporates support for sparse solvers, automatic vectorization, batching, GPU acceleration, and gradient computation with implicit differentiation and direct loss minimization. We do extensive performance evaluation in a set of applications, demonstrating significant efficiency gains and better scalability when these features are incorporated.
