    Metafluid dynamics as a gauge field theory

    In this paper, the analog of Maxwell electromagnetism for hydrodynamic turbulence, the metafluid dynamics, is extended in order to reformulate the metafluid dynamics as a gauge field theory. That analogy opens up the possibility to investigate this theory as a constrained system. Having this possibility in mind, we propose a Lagrangian to describe this new theory of turbulence and, subsequently, analyze it from the symplectic point of view. From this analysis, a hidden gauge symmetry is revealed, providing a clear interpretation and meaning of the physics behind the metafluid theory. Also, the geometrical interpretation to the gauge symmetries is discussed.

    A feasibility study with survival in swine model

    Transrectal access still has some unsolved issues such as spatial orientation, infection, access and site closure. This study presents a simple technique to perform transcolonic access with survival in a swine model series. A new technique for NOTES perirectal access to perform retroperitoneoscopy, peritoneoscopy, liver and lymphnode biopsies was performed in 6 pigs, using Totally NOTES technique. The specimens were extracted transanally. The flexible endoscope was inserted through a posterior transmural incision and the retrorectal space. Cultures of bacteria were documented for the retroperitoneal space and intra abdominal cavity after 14 days. Rectal site was closed using non-absorbable sutures. There was no bowel cleansing, nor preoperative fasting. The procedures were performed in 6 pigs through transcolonic natural orifice access using available endoscopic flexible instruments. All animals survived 14 days without complications, and cultures were negative. Histopathologic examination of the rectal closure site showed adequate healing of suture line and no micro abscesses. The results of feasibility and safety of experimental Transcolonic NOTES potentially brings new frontiers and future wider applications for minimally invasive surgery.     Perception of sleep duration in adult patients with suspected obstructive sleep apnea

    PURPOSE: Discrepancies between subjective and objective measures of total sleep time (TST) are frequent among insomnia patients, but this issue remains scarcely investigated in obstructive sleep apnea (OSA). We aimed to evaluate if sleep perception is affected by the severity of OSA. METHODS: We performed a 3-month cross-sectional study of Brazilian adults undergoing overnight polysomnography (PSG). TST was objectively assessed from PSG and by a self-reported questionnaire (subjective measurement). Sleep perception index (SPI) was defined by the ratio of subjective and objective values. Diagnosis of OSA was based on an apnea/hypopnea index (AHI) ≥ 5.0/h, being its severity classified according to AHI thresholds: 5.0-14.9/h (mild OSA), 15.0-29.9/h (moderate OSA), and ≥ 30.0/h (severe OSA). RESULTS: Overall, 727 patients were included (58.0% males). A significant difference was found in SPI between non-OSA and OSA groups (p = 0.014). Mean SPI values significantly decreased as the OSA severity increased: without OSA (100.1 ± 40.9%), mild OSA (95.1 ± 24.6%), moderate OSA (93.5 ± 25.2%), and severe OSA (90.6 ± 28.2%), p = 0.036. Using logistic regression, increasing SPI was associated with a reduction in the likelihood of presenting any OSA (p = 0.018), moderate/severe OSA (p = 0.019), and severe OSA (p = 0.028). However, insomnia was not considered as an independent variable for the presence of any OSA, moderate/severe OSA, and severe OSA (all p-values > 0.05). CONCLUSION: In a clinical referral cohort, SPI significantly decreases with increasing OSA severity, but is not modified by the presence of insomnia symptoms.publishersversionpublishe

    Estimation of Genetic Parameters of Heat Tolerance for Production Traits in Canadian Holsteins Cattle.

    Understanding how cows respond to heat stress has helped to provide effective herd management practices to tackle this environmental challenge. The possibility of selecting animals that are genetically more heat tolerant may provide additional means to maintain or even improve the productivity of the Canadian dairy industry, which is facing a shifting environment due to climate changes. The objective of this study was to estimate the genetic parameters for heat tolerance of milk, fat, and protein yields in Canadian Holstein cows. A total of 1.3 million test-day records from 195,448 first-parity cows were available. A repeatability test-day model fitting a reaction norm on the temperature-humidity index (THI) was used to estimate the genetic parameters. The estimated genetic correlations between additive genetic effect for production and for heat tolerance ranged from -0.13 to -0.21, indicating an antagonistic relationship between the level of production and heat tolerance. Heritability increased marginally as THI increased above its threshold for milk yield (0.20 to 0.23) and protein yield (0.14 to 0.16) and remained constant for fat yield (0.17). A Spearman rank correlation between the estimated breeding values under thermal comfort and under heat stress showed a potential genotype by environmental interaction.     Anuloplastia mitral com tira de pericárdio bovino resultados de 18 anos

    PURPOSE: Prosthetic annuloplasty rings are currently used in mitral reconstruction. Posterior annuloplasty with a bovine pericardial strip is a technique largely used in the Heart Institute of University of São Paulo Medical School. The purpose of the study was to analyze the late results of mitral valve repair with posterior annuloplasty using a bovine pericardial strip. METHODS: Between January 1984 and December 2002, 273 patients underwent mitral valve repair with posterior pericardial annuloplasty in the Heart Institute of University of São Paulo Medical School. One hundred and forty four (52.7%) were women and ages ranged between 1 and 76 years (38.3 ± 21.1). Rheumatic fever was present in 52.0% of the patients. Associated techniques were employed in 26.0% of the patients, and the most frequent was chordal shortening (9.2%). RESULTS: Hospital mortality was 3.3% (9 patients), with the major cause being low cardiac output (6 patients). Actuarial survival was 55.1% ± 16.8% in 18 years. During the 18-year follow-up: patients were free from the following: reoperation (59.1% ±± 13.9%, (percent +/- Standard Error), thromboembolism (97.4% ± 2.3%), hemolysis (99.2% ± 0.2%), and endocarditis (99.6% ± 1.0%). In the late follow-up period, 83.9% were classified as New York Heart Association functional class I. CONCLUSIONS: Late results with mitral valve repair with posterior annuloplasty using a bovine pericardial strip were satisfactory. The technique is feasible, reproducible, and cost effective.OBJETIVO: Anéis protéticos para anuloplastia são usados nas plásticas da valva mitral de forma rotineira. A anuloplastia posterior com tira de pericárdio bovino é uma técnica largamente utilizada no Instituto do Coração - HC - FMUSP. O objetivo deste estudo foi analisar os resultados tardios da plástica da valva mitral com a utilização desta técnica na nossa instituição. MÉTODOS: Entre janeiro de 1984 e dezembro de 2002, 273 pacientes foram submetidos a plástica da valva mitral com anuloplastia posterior no Instituto do Coração. Foram excluídos os pacientes submetidos a outros procedimentos no anel posterior. Cento e quarenta e quatro (52,7%) eram do sexo feminino e a idade variou entre 1 e 76 anos (média: 38,3 ± 21,1 anos). Em 52% dos pacientes, havia história de febre reumática. Técnicas associadas para reconstrução valvar foram empregadas em 26% dos casos, sendo a mais utilizada, o encurtamento de cordas (9,2%). RESULTADOS: A mortalidade hospitalar foi de 3,3% (9 casos). A principal causa de óbito foi baixo débito cardíaco (6 pacientes). A sobrevida actuarial foi de 55,1 ± 16,8% (percent +/- Standard Error) em 18 anos, sobrevida livre de reoperação, tromboembolismo, hemólise e endocardite foram respectivamente de 59,1 ± 13,9%, 97,4 ± 2,3%, 99,2 ± 0,2%, 99,6 ± 1,0% em 18 anos. No seguimento tardio, 83,9% dos pacientes se encontravam em classe funcional I (NYHA). CONCLUSÕES: Concluímos que os resultados tardios foram satisfatórios. A técnica é fácil, reprodutível e de baixo custo

    Development, validation and comparative study with no-apnea, STOP-bang, and NoSAS

    Background: Obstructive sleep apnea (OSA) is a very prevalent disorder. Here, we aimed to develop and validate a practical questionnaire with yes-or-no answers, and to compare its performance with other well-validated instruments: No-Apnea, STOP-Bang, and NoSAS. Methods: A cross-sectional study containing consecutively selected sleep-lab subjects underwent full polysomnography. A 4-item model, named GOAL questionnaire (gender, obesity, age, and loud snoring), was developed and subsequently validated, with item-scoring of 0–4 points (≥2 points indicating high risk for OSA). Discrimination was assessed by area under the curve (AUC), while predictive parameters were calculated using contingency tables. OSA severity was classified based on conventionally accepted apnea/hypopnea index thresholds: ≥5.0/h (OSA≥5), ≥15.0/h (OSA≥15), and ≥30.0/h (OSA≥30). Results: Overall, 7377 adults were grouped into two large and independent cohorts: derivation (n = 3771) and validation (n = 3606). In the derivation cohort, screening of OSA≥5, OSA≥15, and OSA≥30 revealed that GOAL questionnaire achieved sensitivity ranging from 83.3% to 94.0% and specificity ranging from 62.4% to 38.5%. In the validation cohort, screening of OSA≥5, OSA≥15, and OSA≥30, corroborated validation steps with sensitivity ranging from 83.7% to 94.2% and specificity from 63.4% to 37.7%. In both cohorts, discriminatory ability of GOAL questionnaire for screening of OSA≥5, OSA≥15, and OSA≥30 was similar to No-Apnea, STOP-Bang or NoSAS. Conclusion: All four instruments had similar performance, leading to a possible greater practical implementation of the GOAL questionnaire, a simple instrument with only four parameters easily obtained during clinical evaluation.publishersversionpublishe

    Reproductive tract size and position score: Estimation of genetic parameters for a novel fertility trait in dairy cows.

    The dairy industry is moving toward selecting animals with better fertility to decrease the economic losses linked to reproductive issues. The reproductive tract size and position score (SPS) was recently developed in physiological studies as an indicator of pregnancy rate and the number of services to conception. Cows are scored as SPS 1, 2, or 3 based on the size of their reproductive tract and its position in the pelvis, as determined by transrectal palpation. The objective of this study was to estimate genetic parameters for SPS to assess its potential as a novel fertility trait. Phenotypes were collected at the University of British Columbia's research herd from 2017 to 2020, consisting of 3,247 within- and across-lactation SPS records from 490 Holstein cows. A univariate animal model was used to estimate the variance components for SPS. Both threshold and linear models were fit under a Bayesian approach and the results were compared using the Spearman rank correlation (r) between the estimated breeding values. The 2 models ranked the animals very similarly (r = 0.99), and the linear model was selected for further analysis. Genetic correlations with other currently evaluated traits were estimated using a bivariate animal model. The posterior means (± posterior standard deviation) for heritability and repeatability within- and across-lactation were 0.113 (± 0.013), 0.242 (± 0.012), and 0.134 (± 0.014), respectively. The SPS showed null correlations with production traits and favorable correlations with traditional fertility traits, varying from -0.730 (nonreturn rate) to 0.931 (number of services). Although preliminary, these results are encouraging because SPS seems to be more heritable than and strongly genetically correlated with number of services, nonreturn rate, and first service to conception, indicating potential for effective indirect selection response on these traits from SPS genetic selection. Therefore, further studies with larger data sets to validate these findings are warranted

    Role of PheE15 gate in ligand entry and nitric oxide detoxification function of Mycobacterium tuberculosis truncated hemoglobin N

    The truncated hemoglobin N, HbN, of Mycobacterium tuberculosis is endowed with a potent nitric oxide dioxygenase (NOD) activity that allows it to relieve nitrosative stress and enhance in vivo survival of its host. Despite its small size, the protein matrix of HbN hosts a two-branched tunnel, consisting of orthogonal short and long channels, that connects the heme active site to the protein surface. A novel dual-path mechanism has been suggested to drive migration of O(2) and NO to the distal heme cavity. While oxygen migrates mainly by the short path, a ligand-induced conformational change regulates opening of the long tunnel branch for NO, via a phenylalanine (PheE15) residue that acts as a gate. Site-directed mutagenesis and molecular simulations have been used to examine the gating role played by PheE15 in modulating the NOD function of HbN. Mutants carrying replacement of PheE15 with alanine, isoleucine, tyrosine and tryptophan have similar O(2)/CO association kinetics, but display significant reduction in their NOD function. Molecular simulations substantiated that mutation at the PheE15 gate confers significant changes in the long tunnel, and therefore may affect the migration of ligands. These results support the pivotal role of PheE15 gate in modulating the diffusion of NO via the long tunnel branch in the oxygenated protein, and hence the NOD function of HbN
