Numerical modelling of massif zonal structuring around underground working
Purpose. To identify indicators of massif zonal structuring around underground working using numerical modeling techniques.
Methods. Research into massif zonal structuring was performed using finite element method and thermodynamic method by which the size and number of zones formed around development workings and stopes have been simulated.
Findings. The ratio of zonesβ vertical and horizontal semiaxes in the massif has been established and reliability of the obtained results was determined. The prospects of new modeling techniques for the study of massif zonal structuring parameters around underground workings have been identified.
Originality. The opportunities for wide application of numerical simulation methods to study the phenomenon of zonal encapsulation by the massif of underground workings have been revealed.
Practical implications. The sizes and shapes of zones in the massif around workings were determined and requirements were formulated stating that synergetic research methods should allow to more accurately determine the number, size and shape of zones, as well as fading sinusoidal stress and massif strain domains.Π¦Π΅Π»Ρ. Aim. To study the mechanisms of the reparative action of a new combined cream on the model of thermal burn injury in rats.Materials and methods. On the model of thermal burn in rats the reparative properties of the new combined cream under the conditional name βDermalipoinβ containing Ξ±-lipoic acid, urea, olive oil, tea tree oil, PEG-400 were studied. To determine the level of the protein synthetic activity of cells (epitheliocytes of the stratified squamous epithelium in the epidermis and cells of the fibroblast cell in the edges of healing of thermal damage) the content of ribonucleoproteins (RNP) in the cytoplasm and deoxyribonucleoproteins (DNP) in the nuclei of the cells was assessed by the cytometric method.Results and discussion. On the model of thermal burn in rats βDermalipoinβ cream showed a more active effect on reparative processes compared to the reference drugs β methyluracil ointment and βTitriolβ gel. It indicates a significant increase in the protein synthetic function of epitheliocytes of the stratified squamous epithelium in the epidermis and cells of the fibroblastic series at the healing edges. The reparative activity of the cream was, first of all, provided by the presence of thiol (sulfhydryl) groups in the molecule of lipoic acid, giving it the properties of an antioxidant. The antioxidant effect of lipoic acid promotes more efficient DNA molecule reparation after damage as a result of the oxidative stress.Conclusions. βDermalipoinβ cream shows a marked reparative effect due to the increase of the protein synthetic activity and acceleration of reparation of DNA molecules of epithelial cells after damage; as a result, the process of healing burns accelerates.Π¦Π΅Π»Ρ β ΠΈΡΡΠ»Π΅Π΄ΠΎΠ²Π°Π½ΠΈΠ΅ ΠΌΠ΅Ρ

The first coronavirus NΠ‘oV-B814 was isolated from humans in 1965 and did not survive to the present time. For a long time, it was believed that coronaviruses were not pathogenic to humans. They were not included in the list of particularly dangerous infections and represented a serious problem exclusively in veterinary medicine. But in 2002, after the SARS outbreak, scientistsβ opinions changed. A new subtype of the coronavirus called SARS-CoV penetrated the human population. In 2012, it was possible to discover natural foci of Middle East Respiratory Syndrome. The epidemic of a new coronavirus infection that emerged in late 2019 and early 2020 attracted the attention of scientists around the world. Research of topography of influence and classification of the requirements for uniform of passenger car attendants
The article investigates the theoretical foundations of the study of reliability indicators in order to identify priorities in development of efficient uniform for the passenger car attendants with predictable properties. Impact of neoadjuvant targeted therapy on perioperative complications in patients with localized reno-cellular cancer
Objective. To study the efficacy of neoadjuvant targeted therapy in treatment of localized reno-cellular cancer and to determine its impact on the perioperative complications rate.
Materials and methods. There are presented the results of randomized investigation, conducted in 152 patients with localized reno-cellular cancer. Into the main group 75 (49.3%) patients were included, to whom neoadjuvant targeted therapy and operative treatment were conducted, and into the control group - 77 (50.7%) patients, to whom operative treatment was performed only. The groups were randomized in accordance to the main clinical parameters. Efficacy of treatment was estimated in accordance to RECIST 1.1 scale, complications β in accordance to Clavien-Dindo classification.
Results. Neoadjuvant targeted therapy did not influence the operation duration: (95 Β± 19) min β while conduction of targeted therapy and (108 Β± 53) min β without conduction of targeted therapy, and duration of postoperative treatment: (5.6 Β± 1.9) and (5.2 Β± 2.3) bed-days, accordingly. The intraoperative blood loss volume in renal resection of the patientsβ main group in comparison with patients of a control group have constituted (407 Β± 224) and (317 Β± 210) ml accordingly (p=0.013). Neoadjuvant targeted therapy did not influence the perioperative complications rate, which have occurred in 7 (9.3%) patients of the main and in 8 (10.7%) patients of a control group, and their severity did not exceed 3 degrees in accordance to Clavien-Dindo classification.
Conclusion. Neoadjuvant targeted therapy do not increase the operation duration, the perioperative complications rate and postoperative treatment time, but increases the blood loss volume while performance of renal resection (p=0.013)
Quality of life in patients, suffering localized reno-cellular cancer after operative treatment
Objective. To rise the efficacy of treatment in patients, suffering localized reno-cellular cancer (LRCC), studying quality of life in patients and determination of optimal procedure of operative treatment.
Materials and methods. Of 511 patients, suffering LRCC (T1-T2 N0M0), in whom quality of life was estimated in accordance to the SF-36 questionnaire, renal resectionΒ was performed in 422 (82.6%), nephrectomy - 89 (17.4%). The observation duration median have constituted 28.9 mo (from 3 to 131 mo).
Results. A patientβs cognition of the oncological disease missing have raised the quality of life self-estimation, predominantly in accordance to indices of psychological component of health after nephrectomy and after renal resection. Comparing the quality of life between the patientsβ groups, there was obtained a statistically significant advantage of renal resection over nephrectomy in accordance to five indices of quality of life, including: physical functioning - 75 [65; 85] points after nephrectomy and 80 [70; 90] points after renal resection (p < 0.005); general state of health - 65 [57; 72] and 70 [65; 80] (p < 0.001); vital activity - 65 [55; 70] and 70 [60; 75] (p < 0.005); psychological health - 76 [60; 80] and 80 [68; 88] (p < 0.005); the emotionally-role functioning - 66.7 [66.7; 100] and 100 [66,7; 100] (p < 0.05) points, accordingly.
Conclusion. Renal resection has a statistically proven advantage over nephrectomy in accordance to patientsβ quality of life, determined, basing on data from the SF-36 questionnaire, predominantly due to better psychological self estimation of the patientβs state of health (p < 0.01). Difference between the investigated groups in accordance to indices of pain intensity, social and physical-role functioning was not established

The most common localization of GIST is the gastrointestinal tract, however, extragastrointestinal forms are sometimes found. Most GISTs are small in size, which most often does not exceed 5 - 8 cm in maximum dimension. This article describes the case of a patient with a non-metastatic giant tumor of the stomach without any clinical manifestations. The tumor was localized in the abdominal cavity, occupying almost all of it, squeezing and displacing the surrounding organs. Due to the cystic-solid structure, its gigantic size and intimate adherence to the surrounding organs, it was not possible to carry out an accurate differential diagnosis before surgery. Intraoperative imaging of the tumor, further histological and immunohistochemical (CD117, CD34, S100, CD45, PanCK; Ki-67) studies made possible to establish the diagnosis of gastric GIST T4N0M0 stage II. Based on the data obtained, despite the gigantic size, the prognosis of the disease was considered favorable (low mitotic index, no metastasis). During the period of clinical observation, within 1.5 years after surgery, no relapse or disease progression was recorded, which indicates a favorable prognosis for the patient
