    Egypt is already the most populous Arab country in the world with 93 million citizens in 2016 which may grow to about 120 million by 2030 if the same level of fertility continues. This paper aims to offer an overview of the evolution over time of the ideal number of children in Egypt, assessing previous researches and giving a particular emphasis on most recent data on such topic. In a context of raising fertility, whose causes are still unknown, we test the persistence of a high ideal number of children among younger cohorts

    Involvement of Autophagic Pathway in the Progression of Retinal Degeneration in a Mouse Model of Diabetes

    The notion that diabetic retinopathy (DR) is essentially a micro-vascular disease has been recently challenged by studies reporting that vascular changes are preceded by signs of damage and loss of retinal neurons. As to the mode by which neuronal death occurs, the evidence that apoptosis is the main cause of neuronal loss is far from compelling. The objective of this study was to investigate these controversies in a mouse model of streptozotocin (STZ) induced diabetes. Starting from 8 weeks after diabetes induction there was loss of rod but not of cone photoreceptors, together with reduced thickness of the outer and inner synaptic layers. Correspondingly, rhodopsin expression was downregulated and the scotopic electroretinogram (ERG) is suppressed. In contrast, cone opsin expression and photopic ERG response were not affected. Suppression of the scotopic ERG preceded morphological changes as well as any detectable sign of vascular alteration. Only sparse apoptotic figures were detected by terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay and glia was not activated. The physiological autophagy flow was altered instead, as seen by increased LC3 immunostaining at the level of outer plexiform layer (OPL) and upregulation of the autophagic proteins Beclin-1 and Atg5. Collectively, our results show that the streptozotocin induced DR in mouse initiates with a functional loss of the rod visual pathway. The pathogenic pathways leading to cell death develop with the initial dysregulation of autophagy well before the appearance of signs of vascular damage and without strong involvement of apoptosis

    Benzimidazole-based derivatives as privileged scaffold developed for the treatment of the RSV infection: a computational study exploring the potency and cytotoxicity profiles

    Respiratory syncytial virus (RSV) has been identified as a main cause of hospitalisation in infants and children. To date, the current therapeutic arsenal is limited to ribavirin and palivizumab with variable efficacy. In this work, starting from a number of in-house series of previously described anti-RSV agents based on the benzimidazole scaffold, with the aim at gaining a better understanding of the related chemical features involved in potency and safety profiles, we applied a computational study including two focussed comparative molecular fields analysis (CoMFA) and comparative molecular similarity indices analysis (CoMSIA). The results allowed us to derive useful suggestions for the design of derivatives and also to set up statistical models predicting the potency and selectivity index (SI1/4CC50/EC50) of any new analogue prior to synthesis. Accordingly, here, we discuss preliminary results obtained through the applied exhaustive QSAR analyses, leading to design and synthesise more effective anti-RSV agents

    Vitamin D and tuberculosis: A multicenter study in children

    BACKGROUND: The aim of this study is to evaluate vitamin D levels in children with latent and active TB compared to healthy controls of the same age and ethnical background. METHODS: A multicenter observational study has been conducted in three tertiary care paediatric centres: Anna Meyer Children's University Hospital, Florence, Italy; Evelina London Children's Hospital, London, United Kingdom and Great Ormond Street Hospital, London, United Kingdom. Vitamin D was considered deficient if the serum level was <25 nmol/L, insufficient between 25 and 50 nmol/L and sufficient for a level >50 nmol/L. RESULTS: The study population included 996 children screened for TB, which have been tested for vitamin D. Forty-four children (4.4%) had active TB, 138 (13.9%) latent TB and 814 (81.7%) were controls. Our study confirmed a high prevalence of hypovitaminosis D in the study population. A multivariate analysis confirmed an increased risk of hypovitaminosis D in children with latent and active TB compared to controls [(P = 0.018; RR = 1.61; 95% CI: 1.086-2.388), (P < 0.0001; RR = 4.587; 95% CI:1.190-9.608)]. CONCLUSIONS: Hypovitaminosis D was significantly associated with TB infection in our study. Further studies are needed to evaluate a possible role of vitamin D in the treatment and prevention of tuberculosis in children.     Expression analysis of miRNA hsa-let7b-5p in naso-oropharyngeal swabs of COVID-19 patients supports its role in regulating ACE2 and DPP4 receptors

    Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) is the novel coronavirus responsible for worldwide coronavirus disease (COVID-19). We previously observed that Angiotensin-converting enzyme 2 (ACE2) and Dipeptidyl peptidase-4 (DPP4) are significantly overexpressed in naso-oropharyngeal swabs (NPS) of COVID-19 patients, suggesting their putative functional role in the disease progression. ACE2 and DPP4 overexpression in COVID-19 patients may be associated to epigenetic mechanism, such as miRNA differential expression. We investigated if hsa-let7b-5p, reported to target both ACE2 and DPP4 transcripts, could be involved in the regulation of these genes. We verified that the inhibition and overexpression of hsa-let7b-5p matched to a modulation of both ACE2 and DPP4 levels. Then, we observed a statistically significant downregulation (FC = -1.5; p &lt; 0.05) of hsa-let7b-5p in the same COVID-19 and control samples of our previous study. This is the first study that shows hsa-let7b-5p low expression in naso-oropharyngeal swabs of COVID-19 patients and demonstrates a functional role of this miR in regulating ACE2 and DPP4 levels. These data suggest the involvement of hsa-let7b-5p in the regulation of genes necessary for SARS-CoV-2 infections and its putative role as a therapeutic target for COVID-19

    Seasonal, synoptic, and diurnal-scale variability of biogeochemical trace gases and O2 from a 300-m tall tower in central Siberia

    We present first results from 19 months of semicontinuous concentration measurements of biogeochemical trace gases (CO2, CO, and CH4) and O2, measured at the Zotino Tall Tower Observatory (ZOTTO) in the boreal forest of central Siberia. We estimated CO2 and O2 seasonal cycle amplitudes of 26.6 ppm and 134 per meg, respectively. An observed west-east gradient of about -7 ppm (in July 2006) between Shetland Islands, Scotland, and ZOTTO reflects summertime continental uptake of CO2 and is consistent with regional modeling studies. We found the oceanic component of the O2 seasonal amplitude (Atmospheric Potential Oxygen, or APO) to be 51 per meg, significantly smaller than the 95 per meg observed at Shetlands, illustrating a strong attenuation of the oceanic O2 signal in the continental interior. Comparison with the Tracer Model 3 (TM3) atmospheric transport model showed good agreement with the observed phasing and seasonal amplitude in CO2; however, the model exhibited greater O2 (43 per meg, 32%) and smaller APO (9 per meg, 18%) amplitudes. This seeming inconsistency in model comparisons between O2 and APO appears to be the result of phasing differences in land and ocean signals observed at ZOTTO, where ocean signals have a significant lag. In the first 2 months of measurements on the fully constructed tower (November and December 2006), we observed several events with clear vertical concentration gradients in all measured species except CO. During “cold events” (below -30°C) in November 2006, we observed large vertical gradients in CO2 (up to 22 ppm), suggesting a strong local source. The same pattern was observed in CH4 concentrations for the same events. Diurnal vertical CO2 gradients in April to May 2007 gave estimates for average nighttime respiration fluxes of 0.04 ± 0.02 mol C m-2 d-1, consistent with earlier eddy covariance measurements in 1999–2000 in the vicinity of the tower
