    Kepulauan Mentawai di Provinsi Sumatera Barat menyimpan banyak keindahan dan keunikan budaya yang masih terjaga. Pesona pantai dengan ombak yang langsung menghadap Samudera Hindia menjadikan Kepulauan Mentawai sebagai surga bagi para peselancar dunia. Menurut Peraturan Gubernur Sumatera Barat Nomor 32 Tahun 2016, Kepulauan Mentawai ditetapkan sebagai Kawasan Strategis Pariwisata Provinsi (KSPP). Dengan adanya potensi wisata di kawasan tersebut, pada Tugas Akhir ini dilakukan desain catamaran yacht dengan sistem kelistrikan hibrida yang ramah lingkungan untuk menambah opsi daya tarik bagi wisatawan domestik maupun mancanegara sehingga dapat meningkatkan perekonomian daerah setempat. Dari kuesioner yang telah disebar kepada masyarakat, maka ditentukan fasilitas kapal dan didapatkan payload luasan pada catamaran yacht seluas 735,16 m2. Dengan mengambil rata-rata jumlah penumpang pada kapal dengan jenis serupa, maka ditentukan penumpang catamaran yacht yaitu sebanyak 12 orang dengan 10 orang kru. Ukuran utama yang memenuhi kriteria untuk catamaran yacht pada Tugas Akhir ini adalah LoA = 37 m; Lpp = 34,2 m; B = 15 m; H = 4 m, T = 2,2 m. Analisis teknis yang dilakukan berupa perhitungan berat, perhitungan trim, perhitungan stabilitas, perhitungan freeboard, dan Motion Sickness Insidence (MSI). Kemudian, dilakukan desain Rencana Garis, Rencana Umum, Perencanaan Keselamatan, dan Model 3D. Pada Tugas Akhir ini juga dilakukan analisis ekonomis dengan total biaya pembangunan senilai Rp14.860.960.503, dengan biaya operasional sebesar Rp6.620.488.654/tahun. Dilakukan perhitungan dan didapatkan nilai Net Present Value (NPV) sebesar Rp8.580.816.994, Internal Rate of Return (IRR) senilai 16,74%, dan Payback Period selama 8 tahun 6 bulan 28 hari

    Associations of water quality and bacteria presence in cage culturedred hybrid tilapia, Oreochromis niloticus × O. mossambicus

    This study was conducted to understand the associations between water quality and the presence of bacteria in cage cultured red hybrid tilapia, Oreochromis niloticus × O. mossambicus. Tilapia from commercial floating net cage culture systems in lakes and river in Peninsular Malaysia were randomly sampled over a 24-month period. The eye, brain and kidney were sampled for bacterial isolation, following identification using biochemical test and commercial identification kits. The water clarity, velocity, depth, temperature, pH, iron, sulfide, ammonia, nitrite, phosphate, conductivity and dissolved oxygen at each sampling site were also measured. A total of 44 bacterial species were isolated, which are comprised of 28 Gram-positive and 16 Gram-negative bacteria. Terengganu River (506 isolates, 41 species) recorded the highest number of bacterial isolation and species, compared to Pedu Lake (286 isolates, 25 species) and Kenyir Lake (179 isolates, 25 species). The highest number of isolates was Streptococcus agalactiae (28.3%), followed by Lactococcus lactis (8.4%) and Micrococcus spp. (7.3%). Terengganu River had significantly lower (P < 0.05) water dissolved oxygen, temperature and pH compared to Pedu and Kenyir lakes. On the other hand, water iron, nitrite, sulfide, ammonia and velocity were significantly higher (P < 0.05) in Terengganu River compared to Pedu and Kenyir lakes. Multivariate analyses showed that each sampling site has different water quality parameters that were associated with the presence of bacteria. However, water temperature and ammonia have been identified as the most significant parameters, as they were observed to have strong associations with the bacteria presence in all of the study sites

    We confirmed a bacteremic typhoid fever incidence of 3.9 episodes/1,000 person-years during fever surveillance in a Dhaka urban slum. The relative risk for preschool children compared with older persons was 8.9. Our regression model showed that these children were clinically ill, which suggests a role for preschool immunization

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P &lt; 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Prevalence of non-suicidal self-injury in medical students of Rawalpindi; its socio-demographics, methods, and functions

    Objective: To investigate the prevalence of non-suicidal self-injury among Pakistani medical students, its association with demographic variables, and the distribution of its methods and functions. Method: The cross-sectional study was conducted in from April 2021 to March 2022 at Rawalpindi Medical University, Rawalpindi, Pakistan, and comprised medical students regardless of gender and year of study. Data was collected using the inventory of statements about self-harm scale to assess their socio-demographics, non-suicidal self-injury behaviors and functions. Data was analyzed using SPSS 28. Results: Of the 411 subjects approached, 386(94%) correctly filled the forms; 170(44%) males and 216(55.9%) females. The overall mean age was 19.7 ±1.5 (range: 17-27 years). There were 132(34.1%) students from first year, 146(37.8%) second year, 44(11.3%) third year, 54(13.9%) fourth year and 10(2.5%) from the final year. There were 110(28.4%) with non-suicidal self-injury; 60(54.5%) males and 50(45.4%) females (p=0.008). There were 18(4.7%) subjects aged 18 years, and, of them, 14(77.7%) exhibited self-harm behavior. Among specific non-suicidal self-injury behaviors, ‘interfering with scabs and wound healing’ was the most common method 80(72.7%). As for the functions of non-suicidal self-injury behavior, there was a significant difference between intrapersonal and interpersonal functions (p<0.05). Conclusion: Non-suicidal self-injury was highly prevalent among the young subjects, and it needs proper intervention for timely management.
