    Measuring success: Program fidelity of Queensland\u27s child health home visiting services. a document analysis

    Background: The Family CA.R.E. (Community-based Assistance Resourcing and Education) program was introduced in Queensland two decades ago. It aimed to redress health inequalities for infants from families experiencing specific social stressors. The program has been locally adapted over time and has not been evaluated against the original program. This study assessed the extent to which selected hospital and health services in Queensland, Australia have modified the original Family C.A.R.E. program. Methods: Altheide\u27s model was used to facilitate a critical document analysis of policies and guidelines for adapted Family C.A.R.E. home visiting programs in use by hospital and health services (target n = 7). Results: Five of seven eligible services provided service model documentation. There was low alignment with the original Family C.A.R.E. program across four of the five participating services. While the program delivered within Service 4 was highly aligned to the structure and intent of the original model, variation to the program was still evident. Importantly, four of the five participating programs were not collecting evaluation measures. Conclusions: Health services have adapted the original Family C.A.R.E program format to \u27fit\u27 the local service environment but have largely failed to collect data to facilitate evaluation. Inability to evaluate the program leads to uncertainty about program success and benefits as well as any unintended consequences for families engaging in unevaluated home visiting programs. This study highlights the importance of monitoring program fidelity and evaluating success given the potential ramifications for this vulnerable cohort and for health service delivery

    Critical Reflections on Experiential Learning for Food Justice

    This essay will reflect on Santa Clara University\u27s (SCU) forays into experiential learning around food justice through the Bronco Urban Gardens (BUG) program. BUG works with urban schools and a community center in San José, California, using a garden-based education approach. This program emerged out of our student garden, The Forge. University student farms and gardens provide opportunities for students to learn how to grow, manage, and market food. At Santa Clara University, our half-acre (0.2 hectare) garden plays that role. However, because of our institution\u27s commitment to social justice and a strong network of community partners, our campus garden has blossomed into a larger food justice outreach program. We will first discuss the motivation behind experiential learning for social justice and reflect on its connection to food justice. We then focus on several observations, challenges, and questions that have emerged out of our BUG experiences. Some of those observations involve the challenge of working with students and community partners where the interests of both groups must be served. We also explore what food justice means in this context, and what it means when a program expands beyond the committed few to an entire student body. By engaging in food justice with low-income communities of color through innovative campus programs such as BUG, our students are likely to see the food system from a very different vantage point than if they stayed on campus, resulting in deep learning experiences and also benefits for communities

    A 2 Earth Radius Planet Orbiting the Bright Nearby K-Dwarf Wolf 503

    Since its launch in 2009, the Kepler telescope has found thousands of planets with radii between that of Earth and Neptune. Recent studies of the distribution of these planets have revealed a rift in the population near 1.5-2.0RR_{\bigoplus}, informally dividing these planets into "super-Earths" and "sub-Neptunes". The origin of this division is not well understood, largely because the majority of planets found by Kepler orbit distant, dim stars and are not amenable to radial velocity follow-up or transit spectroscopy, making bulk density and atmospheric measurements difficult. Here, we present the discovery and validation of a newly found 2.030.07+0.08 R2.03^{+0.08}_{-0.07}~R_{\bigoplus} planet in direct proximity to the radius gap, orbiting the bright (J=8.32J=8.32~mag), nearby (D=44.5D=44.5~pc) high proper motion star Wolf 503 (EPIC 212779563). We classify Wolf 503 as a K3.5V star and member of the thick disc population. We determine the possibility of a companion star and false positive detection to be extremely low using both archival images and high-contrast adaptive optics images from the Palomar observatory. The brightness of the host star makes Wolf 503b a prime target for prompt radial velocity follow-up, HST transit spectroscopy, as well as detailed atmospheric characterization with JWST. With its measured radius near the gap in the planet radius and occurrence rate distribution, Wolf 503b offers a key opportunity to better understand the origin of this radius gap as well as the nature of the intriguing populations of "super-Earths" and "sub-Neptunes" as a whole

    How to promote healthy behaviours in patients? An overview of evidence for behaviour change techniques

    To identify the evidence for the effectiveness of behaviour change techniques, when used by health-care professionals, in accomplishing health-promoting behaviours in patients. Reviews were used to extract data at a study level. A taxonomy was used to classify behaviour change techniques. We included 23 systematic reviews: 14 on smoking cessation, 6 on physical exercise, and 2 on healthy diets and 1 on both exercise and diets. None of the behaviour change techniques demonstrated clear effects in a convincing majority of the studies in which they were evaluated. Techniques targeting knowledge (n = 210 studies) and facilitation of behaviour (n = 172) were evaluated most frequently. However, self-monitoring of behaviour (positive effects in 56% of the studies), risk communication (52%) and use of social support (50%) were most often identified as effective. Insufficient insight into appropriateness of technique choice and quality of technique delivery hinder precise conclusions. Relatively, however, self-monitoring of behaviour, risk communication and use of social support are most effective. Health professionals should avoid thinking that providing knowledge, materials and professional support will be sufficient for patients to accomplish change and consider alternative strategies which may be more effective

    Results of a participatory needs assessment demonstrate an opportunity to involve people who use alcohol in drug user activism and harm reduction

    BACKGROUND: Drug users’ organizations have made progress in recent years in advocating for the health and human rights of people who use illicit drugs but have historically not emphasized the needs of people who drink alcohol. METHODS: This paper reports on a qualitative participatory needs assessment with people who use illicit substances in British Columbia, Canada. We held workshops in 17 communities; these were facilitated by people who use illicit drugs, recorded with ethnographic fieldnotes, and analyzed using critical theory. RESULTS: Although the workshops were targeted to people who use illicit drugs, people who primarily consume alcohol also attended. An unexpected finding was the potential for drug users’ organizations and other harm reduction programs to involve “illicit drinkers”: people who drink non-beverage alcohol (e.g. mouthwash, rubbing alcohol) and those who drink beverage alcohol in criminalized ways (e.g., homeless drinkers). Potential points of alliance between these groups are common priorities (specifically, improving treatment by health professionals and the police, expanding housing options, and implementing harm reduction services), common values (reducing surveillance and improving accountability of services), and polysubstance use. CONCLUSIONS: Despite these potential points of alliance, there has historically been limited involvement of illicit drinkers in drug users’ activism. Possible barriers to involvement of illicit drinkers in drug users’ organizations include racism (as discourses around alcohol use are highly racialized), horizontal violence, the extreme marginalization of illicit drinkers, and knowledge gaps around harm reduction for alcohol. Understanding the commonalities between people who use drugs and people who use alcohol, as well as the potential barriers to alliance between them, may facilitate the greater involvement of illicit drinkers in drug users’ organizations and harm reduction services

    Identification of the top TESS objects of interest for atmospheric characterization of transiting exoplanets with JWST

While there are over 5000 confirmed planets, more than 4000 Transiting Exoplanet Survey Satellite (TESS) planet candidates are still unconfirmed and many of the best planets for atmospheric characterization may remain to be identified. We present a sample of TESS planets and planet candidates that we identify as “best-in-class” for transmission and emission spectroscopy with JWST. These targets are sorted into bins across equilibrium temperature Teq and planetary radius Rp and are ranked by a transmission and an emission spectroscopy metric (TSM and ESM, respectively) within each bin. We perform cuts for expected signal size and stellar brightness to remove suboptimal targets for JWST. Of the 194 targets in the resulting sample, 103 are unconfirmed TESS planet candidates, also known as TESS Objects of Interest (TOIs). We perform vetting and statistical validation analyses on these 103 targets to determine which are likely planets and which are likely false positives, incorporating ground-based follow-up from the TESS Follow-up Observation Program to aid the vetting and validation process. We statistically validate 18 TOIs, marginally validate 31 TOIs to varying levels of confidence, deem 29 TOIs likely false positives, and leave the dispositions for four TOIs as inconclusive. Twenty-one of the 103 TOIs were confirmed independently over the course of our analysis. We intend for this work to serve as a community resource and motivate formal confirmation and mass measurements of each validated planet.
