    Before COVID-19, most therapists had concerns about telepsychology, and only treated patients in person. During the COVID-19 lockdown, patients still needed therapy, but in-person therapy sessions became unsafe. The current study measured how many therapists are using online therapy before vs. during COVID-19, how much training they have received, and their knowledge about legal restrictions on using telepsychology. A sample of 768 U.S.A. mental health professionals completed a 29-item online survey. Results show that before COVID-19, most therapists only saw their patients in person (e.g., at the therapists office), but during the COVID-19 pandemic, nearly all therapists used a wide range of telecommunication technologies to communicate with their quarantined patients, including texting, telephones, video conferences, and even virtual reality. According to within-subject related samples comparisons, 39% of survey respondents used telepsychology before COVID-19, vs. 98% during COVID-19 (χ2 = 450.02, p < 0.001). Therapists reported high treatment effectiveness using telepsychology (7.45 on 0–10 scale). However, overall, on a 0–10 scale, therapists reported a significant increase in feeling burned out during the COVID-19 pandemic, Mean = 3.93 (SD = 1.93) before vs. 6.22 (SD = 2.27) during the pandemic (Z = −18.57, p < 0.001). Although the APA ethics guidelines encourage therapists to use telepsychology with their patients during the crisis, gaps in respondents' knowledge identify a need for increased specialized training and education. Although the current study showed that virtual reality is rarely used by the therapists surveyed, virtual reality is a promising new telepsychology technology. Billions of dollars are currently being invested in mass producing immersive virtual reality systems. In the future, as networked immersive Virtual Reality becomes more widely available, therapists and patients in physically different locations will be able to “meet” in a shared computer-generated world designed for therapy sessions, potentially including group sessions. Telepsychology and virtual reality have the potential to be increasingly valuable tools to help therapists mitigate the consequences of COVID-19.     Climate change and impact on renewable energies in the Azores strategic visions for sustainability

    The energy sector is the largest contributor to global greenhouse gas emissions, but could also be seriously affected by climate change, calling into question society’s current consumption patterns. In this communication, climate projections based on a set of numerical models of global circulation are used to simulate the climate until the end of the century and keep in mind the alternative scenarios of pollutant emissions. Apart from solar energy, the results for the Azores region show a negative impact on the production and consumption of renewable energies. In the regional context, this issue assumes special relevance, given the geographical constraints, such as territorial discontinuity and insularity. Based on these assumptions, measures and recommendations are pointed out for the sectors that most penalize greenhouse gas emissions, considering the energy sustainability in the Azores and the commitments and goals assumed under international agreements.info:eu-repo/semantics/publishedVersio

    Know Your Value: Negotiation Skill Development for Junior Investigators in the Academic Environment—A Report from the American Society of Preventive Oncology\u27s Junior Members Interest Group

    The American Society of Preventive Oncology (ASPO) is a professional society for multidisciplinary investigators in cancer prevention and control. One of the aims of ASPO is to enable investigators at all levels to create new opportunities and maximize their success. One strategy adopted by ASPO was to develop the Junior Members Interest Group in 1999. The Interest Group membership includes predoctoral fellows, postdoctoral fellows, and junior faculty members who are provided career development and training opportunities (1). Responsibilities of the members of the Junior Members Interest Group include serving on the ASPO Executive Committee and the Program Planning Committee and organizing professional development sessions at ASPO\u27s annual meeting. As part of the 2014 ASPO annual meeting, the Junior Members Interest Group organized a session entitled “Negotiation Skill Development for Junior Investigators in the Academic Environment.” This interactive session was designed to provide early-career investigators an opportunity to practice their negotiation skills and to receive expert advice and strategies to effectively negotiate new faculty positions in an academic environment. The session focused primarily on negotiating an initial academic appointment from a graduate student or postdoctoral fellow to an assistant professor–level position. In addition to the main focus, the session also covered renegotiation for assistant and associate-level investigators as they navigate through their careers. The session began with an interactive exercise led by Dr. Stephanie A.N. Silvera (Associate Professor of Public Health, Montclair State University, Montclair, NJ) where participants engaged in a mock salary negotiation session with another member of the audience (Table 1). Following the negotiation exercise, Dr. Silvera led a debriefing session. Next, four panelists at different levels in their academic careers were invited to provide their personal perspectives on the topic of effective negotiation: Dr. Faith Fletcher (Assistant Professor of Community Health Sciences, the University of Illinois at Chicago, Chicago, IL) to provide the perspective of a first-year faculty member; Dr. Stephanie A.N. Silvera (Associate Professor of Public Health, Montclair State University, Montclair, NJ) to provide the perspective of a recently tenured faculty member; Dr. Karen Basen-Engquist (Professor of Behavioral Science and Director of the Center for Energy Balance, University of Texas MD Anderson Cancer Center, Houston, TX) to provide the perspective of a senior faculty member; and Dr. Peter G. Shields (Professor and Deputy Director of the Ohio State University Comprehensive Cancer Center, Columbus, OH) to provide the perspective of a senior faculty member with extensive experience on the employer side of an academic appointment negotiation. This report summarizes the main themes that emerged from the negotiation exercise debriefing, the speakers\u27 advice and recommendations, and responses to audience questions during the session

    Energy Efficiency Optimization in Battery-Based Photovoltaic Pumping Schemes

    [EN] This article deals with the analysis of energy efficiency optimization in battery-based photovoltaic pumping schemes. The study builds on previous findings derived from the comparison between a direct photovoltaic water pumping system (DPVWPS) and the equivalent system including a lithium-ion battery (LIB). The initial experimental results of the battery-based photovoltaic water pumping system (PVWPS+LIB) were obtained with the motor-pump group operating at its rated condition of 50 Hz. In the present work, an analysis of the efficiency and the performance ratio of the variable speed drive (VSD) and the motor-pump group, showed that both parameters improve when operating in the low frequency range of the VSD and far from the rated frequency where the flow rate is maximized. Several fitting models were performed with the data obtained with the monitoring system and it was concluded that an overall energy efficiency optimization can be achieved with a VSD frequency equal to 37 Hz. A comparison of the experimental results obtained with the VSD working in the direct mode and with the battery-based solution (setting different VSD frequencies - 50 Hz, 37 Hz, and a combination of the two frequencies designed as 37/50/37 Hz) was made to determine the efficiency and performance ratios in each case.     Recapitulating the Bayesian framework for neurosurgical outpatient care and a cost-benefit analysis of telemedicine for socioeconomically disadvantaged patients in the Philippines during the pandemic

    Objective: In the Philippines during recent months, a neurosurgical center that caters primarily to socioeconomically disadvantaged patients has encountered unprecedented changes in practice patterns brought about by the COVID-19 pandemic. In particular, the usual task of outpatient care has shifted to the telemedicine format, bringing along all of its attendant advantages and gargantuan challenges. The authors sought to determine the responsiveness of this telemedicine setup to the needs of their disadvantaged patients and explored the application of Bayesian inference to enhance the use of teleconsultation in daily clinical decision-making. Methods: The authors used the following methods to assess the telemedicine setup used in a low-resource setting during the pandemic: 1) a cross-sectional survey of patients who participated in a medical consultation via telemedicine during the 16-week period from March 16, 2020, to July 15, 2020; 2) a cost-benefit analysis of the use of telemedicine by patients; and 3) a case illustration of a Bayesian approach application unique to the teleconsultation scenario. Results: Of the 272 patient beneficiaries of telemedicine in a 16-week period, 57 responded to the survey. The survey responses regarding neurosurgical outpatient care through telemedicine yielded high ratings of utility for the patients and their caregivers. According to 64% of respondents, the affordability of the telemedicine setup also prevented them from borrowing money from others, among other adverse life events prevented. There were realized financial gains on the part of the patients in terms of cost savings and protection from further impoverishment. The benefit-cost ratio was 3.51 for the patients, signifying that the benefits outweighed the costs. An actual teleconsultation case vignette was reported that is meant to be instructive and contributory to the preparedness of the neurosurgeon on the provider end of the service delivery. Conclusions: Telemedicine holds promise as a viable and safe method for health service delivery during the pandemic. In the setting of a health system that is continually challenged by shortages of resources, this study shows that an effective telemedicine setup can come with high benefit-cost ratios and quality of care, along with the assurance of patient satisfaction. The potential for high-quality care can be enhanced by the inclusion of the Bayesian framework to the basic toolkit of remote clinical assessment.     Combined sub-optimal doses of Rosuvastatin and Bexarotene impairs angiotensin II-induced arterial mononuclear cell adhesion through inhibition of Nox5 signaling pathways and increased RXR/PPARα and RXR/PPARγ interactions

    Aim: Mononuclear cell (MC) infiltration into the arterial subendothelium is a key event in atherogenesis. Rosuvastatin (Rosu) and bexarotene (Bex) exert anti-inflammatory activity, but serious dose-related adverse effects have emerged. The need for safer and effective strategies to prevent and treat atherosclerosis led us to test the effect of combined use of both drugs on angiotensin II (Ang-II)-induced arterial MC recruitment. Results: Vehicle, Rosu (10-30 nM), Bex (0.3-1 μM), or a combination of both were administered to human umbilical arterial endothelial cells (HUAECs) 20 h before stimulation with 1 μM Ang-II (4 h). Surprisingly, a combination of Rosu (10 nM)+Bex (0.3 μM), which did not influence Ang-II-induced MC recruitment when either stimulus was studied alone, significantly reduced this response. This effect was accompanied by diminished Ang-II-induced ICAM-1, VCAM-1, and CX3CL1 endothelial expression and CXCL1, CXCL8, CCL2, and CCL5 production. Preincubation of HUAECs with Rosu+Bex inhibited Nox5 expression and Nox5-induced RhoA activation stimulated by Ang-II through increased RXRα, PPARα, and PPARγ expression in addition to RXRα/PPARα and RXRα/PPARγ interactions. In vivo, combined but not single administration of Rosu (1.25 mg/kg/day) and Bex (10 mg/kg/day) significantly diminished Ang-II-induced arteriolar leukocyte adhesion in the cremasteric microcirculation of C57BL/6 mice and atherosclerotic lesion formation in apoE(-/-) mice subjected to an atherogenic diet. Innovation and conclusion: Combined administration of Bex+Rosu at suboptimal doses may constitute a new alternative and effective therapy in the control of the vascular inflammation associated to cardiometabolic disorders, since they synergize in their anti-inflammatory actions and may counteract their associated adverse effects

    Using immersive virtual reality distraction to reduce fear and anxiety before surgery

    Presurgical anxiety is very common and is often treated with sedatives. Minimizing or avoiding sedation reduces the risk of sedation-related adverse events. Reducing sedation can increase early cognitive recovery and reduce time to discharge after surgery. The current case study is the first to explore the use of interactive eye-tracked VR as a nonpharmacologic anxiolytic customized for physically immobilized presurgery patients. Method: A 44-year-old female patient presenting for gallbladder surgery participated. Using a within-subject repeated measures design (treatment order randomized), the participant received no VR during one portion of her preoperative wait and interactive eye-tracked virtual reality during an equivalent portion of time in the presurgery room. After each condition (no VR vs. VR), the participant provided subjective 0–10 ratings and state–trait short form Y anxiety measures of the amount of anxiety and fear she experienced during that condition. Results: As predicted, compared to treatment as usual (no VR), the patient reported having 67% lower presurgical anxiety during VR. She also experienced “strong fear” (8 out of 10) during no VR vs. “no fear” (0 out of 10) during VR. She reported a strong sense of presence during VR and zero nausea. She liked VR, she had fun during VR, and she recommended VR to future patients during pre-op. Interactive VR distraction with eye tracking was an effective nonpharmacologic technique for reducing anticipatory fear and anxiety prior to surgery. The results add to existing evidence that supports the use of VR in perioperative settings. VR technology has recently become affordable and more user friendly, increasing the potential for widespread dissemination into medical practice. Although case studies are scientifically inconclusive by nature, they help identify new directions for future larger, carefully controlled studies.
