Perspectives on domestic violence: case study from Karachi, Pakistan.
There is no adequate profile of domestic violence in Pakistan although this issue is frequently highlighted by the media. This case study used qualitative and quantitative methods to explore the nature and forms of domestic violence, circumstances, impact and coping mechanisms amongst selected women victims in Karachi. Violence was a continuum: all the women reported verbal abuse, often escalating into physical, emotional, sexual and economic abuse. The husband was the most common perpetrator. Women suffered in silence due to sociocultural norms, misinterpretation of religious beliefs, subordinate status, economic dependence and lack of legal redress. Besides short-term local measures, public policy informed by correct interpretation of religion can bring about a change in prevailing societal norms
Interference of five problematic weed species with rice growth and yield
Five weed species namely, Cyperus rotundus L., Cyperus difformis L., Echinochloa colonum (L.) Link., Paspalum paspaloides (Mich.) Scribner, and Marsilea minuta L. were selected for the assessment of their level of competition with two commonly grown rice varieties viz. Basmati-385 and Super Basmati. Root and shoot growth as well as grain yield, in both rice cultivars, were adversely affected due to the weed competition in the field experiments. In general, Super Basmati was found to be comparatively more tolerant to weed infestation than Basmati-385. There were 6 to 40% and 21 to 56% reduction in grain yield of Super Basmati and Basmati-385, respectively, due to different weed species. E. colonum was found to be the most damaging weed which resulted in the highest grain yield losses of 56 and 42% in Basmati-385 and Super Basmati, respectively. P. paspaloides was found to be the second most damaging weed species which caused 47% yield losses in Basmati-385. It was concluded from the study that E. colonum was the most competitive weed. It resulted in the highest yield losses in rice especially in var. Basmati-385.Keywords: Rice, weeds, yield losses
Using SERVQUAL for assessing and improving patient satisfaction at a rural health facility in Pakistan.
A cross-sectional study of Patient satisfaction with care was conducted over a period of 1 year from March 2004 to March 2005 in a secondary-level hospital in a peri-urban area of Karachi, Pakistan. Using the SERVQUAL tool and exit interviews, data were collected quarterly from a total of 1533 Patients. Results sharing and capacity-building workshops were arranged during the 4 phases of the survey to sensitize the staff of the hospital to work towards improving Patient satisfaction. The level of satisfaction of the Patients with the outPatient health services provided showed a gradual increase from 34.4% to 82.0% over the 1-year period
Mutation Analysis of the CYP21A2 Gene in the Iranian Population
Background: Defects in the CYP21A2 gene cause steroid 21-hydroxylase deficiency, which is the most frequent cause of congenital adrenal hyperplasia. Forty four affected families were investigated to identify the mutation spectrum of the CYP21A2 gene. Methods: Families were subjected to clinical, biochemical, and molecular analyses. Allele-specific polymerase chain reaction amplification was used for eight common mutations followed by dosage analysis to exclude CYP21A2 deletions. Results: The most frequent mutations detected were gene deletions and chimera (31.8). Other mutation frequencies were as follows: Q318X, 15.9; I2G, 14.8; I172N, 5.8; gene duplication, 5.7; R356W, 8; and E6 cluster mutations, 2.3. Direct sequencing of the CYP21A2 gene revealed R316X, P453S, c.484insT, and a change at the start codon. Different modules carried by patients were classified into five different haplotypes. The genotype phenotype correlation (positive predictive value) for group null, A, B, and C were 92.3, 85.7, 100, and 0, respectively. Conclusions: Methods used will be helpful for carrier detection and antenatal diagnosis, especially with inclusion of the multiplex ligation probe dependent amplification technique, which is easier for routine tests in comparison with other methods. Mutation frequencies indicate that Iranians are possible descendants of Asians and Europeans
Glucose-induced down regulation of thiamine transporters in the kidney proximal tubular epithelium produces thiamine insufficiency in diabetes
Increased renal clearance of thiamine (vitamin B1) occurs in experimental and clinical diabetes producing thiamine insufficiency mediated by impaired tubular re-uptake and linked to the development of diabetic nephropathy. We studied the mechanism of impaired renal re-uptake of thiamine in diabetes. Expression of thiamine transporter proteins THTR-1 and THTR-2 in normal human kidney sections examined by immunohistochemistry showed intense polarised staining of the apical, luminal membranes in proximal tubules for THTR-1 and THTR-2 of the cortex and uniform, diffuse staining throughout cells of the collecting duct for THTR-1 and THTR-2 of the medulla. Human primary proximal tubule epithelial cells were incubated with low and high glucose concentration, 5 and 26 mmol/l, respectively. In high glucose concentration there was decreased expression of THTR-1 and THTR-2 (transporter mRNA: −76% and −53% respectively, p<0.001; transporter protein −77% and −83% respectively, p<0.05), concomitant with decreased expression of transcription factor specificity protein-1. High glucose concentration also produced a 37% decrease in apical to basolateral transport of thiamine transport across cell monolayers. Intensification of glycemic control corrected increased fractional excretion of thiamine in experimental diabetes. We conclude that glucose-induced decreased expression of thiamine transporters in the tubular epithelium may mediate renal mishandling of thiamine in diabetes. This is a novel mechanism of thiamine insufficiency linked to diabetic nephropathy
A novel PKP2 mutation and intrafamilial phenotypic variability in ARVC/D
Background: Arrhythmogenic ventricular cardiomyopathy (AVC) is an inherited cardiac disorder affecting 1 in 1000 individuals worldwide. The mean diagnosed age of disease is 31 years. In this article, an Iranian family reported that they were affected by ARVC due to a novel PKP2 mutation. Methods: Clinical evaluations, 12-lead ECG, CMR, and signal-averaged ECG were performed. After DNA extraction, genetic testing was done, and PCR-sequencing was applied to find causal mutations. Segregation analysis was also performed for the family. Results: ARVC criteria were documented in the patients. Genetic testing revealed a novel chain termination mutation (p.Tyr168Ter) in PKP2 gene; this mutation was transmitted from the mother to her 23-year-old son, but only the son was affected with ARVC. Conclusion: Modifier genes were indicated using interactome analysis of Plakophilin 2 protein (PKP2); they might have led to phenotypic variability through cellular mechanisms, such as nonsense-mediated mRNA decay. At least, 9 proteins were identified that might have affected Plakophilin 2 protein function, and consequently, rationalizing this intrafamilial phenotypic variability. This study highlighted the role of modifier genes involved in ARVC as well as the major role of PKP2 mutation in developing the disease in our population. © Iran University of Medical Sciences
Feedback Revisited: Definitional, Structural, and Functional Issues
This article provides a more fundamental overview that addresses the following provisions associated with feedback in an integrative way. First, several linguistic and definitional issues relating to the term ‘feedback’ are discussed. Second, the article draws attention to the challenges and implications associated with online feedback being increasingly used primarily during the contemporary shift toward online learning. Then, the article spots light on the importance of feedback at multiple levels, followed by a discussion of feedback purposes and potential benefits on student learning and progress, both in the short and long term. This article seeks to contribute to the literature on formative feedback by providing a more profound understanding of the conceptualizations of feedback by incorporating definitional, structural, and functional issues about feedback both as a construct and a practice
Gain properties of dye-doped polymer thin films
Hybrid pumping appears as a promising compromise in order to reach the much
coveted goal of an electrically pumped organic laser. In such configuration the
organic material is optically pumped by an electrically pumped inorganic device
on chip. This engineering solution requires therefore an optimization of the
organic gain medium under optical pumping. Here, we report a detailed study of
the gain features of dye-doped polymer thin films. In particular we introduce
the gain efficiency , in order to facilitate comparison between different
materials and experimental conditions. The gain efficiency was measured with
various setups (pump-probe amplification, variable stripe length method, laser
thresholds) in order to study several factors which modify the actual gain of a
layer, namely the confinement factor, the pump polarization, the molecular
anisotropy, and the re-absorption. For instance, for a 600 nm thick 5 wt\% DCM
doped PMMA layer, the different experimental approaches give a consistent value
80 cm.MW. On the contrary, the usual model predicting the gain
from the characteristics of the material leads to an overestimation by two
orders of magnitude, which raises a serious problem in the design of actual
devices. In this context, we demonstrate the feasibility to infer the gain
efficiency from the laser threshold of well-calibrated devices. Besides,
temporal measurements at the picosecond scale were carried out to support the

Increased glycation and oxidative damage to apolipoprotein B100 of LDL cholesterol in patients with type 2 diabetes and effect of metformin
OBJECTIVE The aim of this study was to investigate whether apolipoprotein B100 of LDL suffers increased damage by glycation, oxidation, and nitration in patients with type 2 diabetes, including patients receiving metformin therapy.
RESEARCH DESIGN AND METHODS For this study, 32 type 2 diabetic patients and 21 healthy control subjects were recruited; 13 diabetic patients were receiving metformin therapy (median dose: 1.50 g/day). LDL was isolated from venous plasma by ultracentrifugation, delipidated, digested, and analyzed for protein glycation, oxidation, and nitration adducts by stable isotopic dilution analysis tandem mass spectrometry.
RESULTS Advanced glycation end product (AGE) content of apolipoprotein B100 of LDL from type 2 diabetic patients was higher than from healthy subjects: arginine-derived AGE, 15.8 vs. 5.3 mol% (P < 0.001); and lysine-derived AGE, 2.5 vs. 1.5 mol% (P < 0.05). Oxidative damage, mainly methionine sulfoxide residues, was also increased: 2.5 vs. 1.1 molar equivalents (P < 0.001). 3-Nitrotyrosine content was decreased: 0.04 vs. 0.12 mol% (P < 0.05). In diabetic patients receiving metformin therapy, arginine-derived AGE and methionine sulfoxide were lower than in patients not receiving metformin: 19.3 vs. 8.9 mol% (P < 0.01) and 2.9 vs. 1.9 mol% (P < 0.05), respectively; 3-nitrotyrosine content was higher: 0.10 vs. 0.03 mol% (P < 0.05). Fructosyl-lysine residue content correlated positively with fasting plasma glucose. Arginine-derived AGE residue contents were intercorrelated and also correlated positively with methionine sulfoxide.
CONCLUSIONS Patients with type 2 diabetes had increased arginine-derived AGEs and oxidative damage in apolipoprotein B100 of LDL. This was lower in patients receiving metformin therapy, which may contribute to decreased oxidative damage, atherogenicity, and cardiovascular disease
Studi Eksperimental Pengaruh Inklusi Baja Terhadap Perilaku Mortar-inklusi
Beton merupakan material komposit yang tersusun dari mortar dan agregat kasar, serta disatukan oleh semen melalui proses hidrasi. Kuat tekan material beton sangat tergantung dari karakteristik material penyusunnya, dan kekuatan lekatan daerah peralihan antara mortar dengan permukaan agregat kasar yang disebut Interfacial Transition Zone (ITZ). Penelitian mengenai mortar inklusi sudah banyak dilakukan, namun masih sedikit penelitian yang memodelkan inklusi dengan baja. Untuk mengetahui lebih lanjut mengenai pengaruh ITZ baja terhadap mortar inklusi, maka dalam penelitian ini menggunakan beberapa macam mutu mortar yaitu dengan kuat tekan 20, 40, dan 60 MPa. Benda uji menggunakan ukuran 150 x 150 x 50 mm dengan inklusi silinder baja tipe BJTP-40 berdiameter 25 mm dan 32 mm. Silinder baja diletakkan tepat di pusat titik berat benda uji. Hasil penelitian menunjukkan bahwa mutu mortar dan diameter inklusi sangat mempengaruhi perilaku dari benda uji. Kuat tekan mengalami penurunan diakibatkan oleh semakin luasnya diameter inklusi, sedangkan angka deformasi menurun dengan kenaikan pada nilai mutu mortar. Pengamatan yang dilakukan saat pengujian menunjukkan bahwa pola retak yang terjadi adalah columnar yang terjadi di daerah inklusi yang merupakan daerah terlemah dikarenakan oleh ITZ
