A systematic review of frameworks for the interrelationships of mental health evidence and policy in low- and middle-income countries
Background: The interrelationships between research evidence and policy-making are complex. Different theoretical frameworks exist to explain general evidenceâpolicy interactions. One largely unexplored element of these interrelationships is how evidence interrelates with, and influences, policy/political agenda-setting. This review aims to identify the elements and processes of theories, frameworks and models on interrelationships of research evidence and health policy-making, with a focus on actionability and agenda-setting in the context of mental health in low- and middle-income countries (LMICs).
Methods: A systematic review of theories was conducted based on the BeHeMOTh search method, using a tested and refined search strategy. Nine electronic databases and other relevant sources were searched for peer-reviewed and grey literature. Two reviewers screened the abstracts, reviewed full-text articles, extracted data and performed quality assessments. Analysis was based on a thematic analysis. The included papers had to present an actionable theoretical framework/model on evidence and policy interrelationships, such as knowledge translation or evidence-based policy, specifically target the agenda-setting process, focus on mental health, be from LMICs and published in English.
Results: From 236 publications included in the full text analysis, no studies fully complied with our inclusion criteria. Widening the focus by leaving out âagenda-settingâ, we included ten studies, four of which had unique conceptual frameworks focusing on mental health and LMICs but not agenda-setting. The four analysed frameworks confirmed research gaps from LMICs and mental health, and a lack of focus on agenda-setting. Frameworks and models from other health and policy areas provide interesting conceptual approaches and lessons with regards to agenda-setting.
Conclusion: Our systematic review identified frameworks on evidence and policy interrelations that differ in their elements and processes. No framework fulfilled all inclusion criteria. Four actionable frameworks are applicable to mental health and LMICs, but none specifically target agenda-setting. We have identified agenda-setting as a research theory gap in the context of mental health knowledge translation in LMICs. Frameworks from other health/policy areas could offer lessons on agenda-setting and new approaches for creating policy impact for mental health and to tackle the translational gap in LMICs
Honeybee linguisticsĂąâŹâa comparative analysis of the waggle dance among species of Apis
All honeybees use the waggle dance to recruit nestmates. Studies on the dance precision of Apis mellifera have shown that the dance is often imprecise. Two hypotheses have been put forward aimed at explaining this imprecision. The first argues that imprecision in the context of foraging is adaptive as it ensures that the dance advertises the same patch size irrespective of distance. The second argues that the bees are constrained in their ability to be more precise, especially when the source is nearby. Recent studies have found support for the latter hypothesis but not for the âtuned-errorâ hypothesis, as the adaptive hypothesis became known. Here we investigate intra-dance variation among Apis species. We analyse the dance precision of A. florea, A. dorsata, and A. mellifera in the context of foraging and swarming. A. mellifera performs forage dances in the dark, using gravity as point of reference, and in the light when dancing for nest sites, using the sun as point of reference. Both A. dorsata and A. florea are open-nesting species; they do not use a different point of reference depending on context. A. florea differs from both A. mellifera and A. dorsata in that it dances on a horizontal surface and does not use gravity but instead âpointsâ directly toward the goal when indicating direction. Previous work on A. mellifera has suggested that differences in dance orientation and point of reference can affect dance precision. We find that all three species improve dance precision with increasing waggle phase duration, irrespective of differences in dance orientation, and point of reference. When dancing for sources nearby, dances are highly variable. When the distance increases, dance precision converges. The exception is dances performed by A. mellifera on swarms. Here, dance precision decreases as the distance increases. We also show that the size of the patch advertised increases with increasing distance, contrary to what is predicted under the tuned-error hypothesis
Childhood febrile illness and the risk of myopia in UK Biobank participants
Purpose Historical reports suggest febrile illness during childhood is a risk factor for myopia. The establishment of the UK Biobank provided a unique opportunity to investigate this relationship.
Patients and methods We studied a sample of UK Biobank participants of White ethnicity aged 40â69 years old who underwent autorefraction (N=91â592) and were classified as myopic (â€â0.75 Dioptres (D)), highly myopic (â€â6.00âD), or non-myopic (>â0.75âD). Self-reported age at diagnosis of past medical conditions was ascertained during an interview with a nurse at a Biobank assessment centre. Logistic regression analysis was used to calculate the odds ratio (OR) for myopia or high myopia associated with a diagnosis before age 17 years of each of nine febrile illnesses, after adjusting for potential confounders (age, sex, highest educational qualification, and birth order).
Results Rubella, mumps, and pertussis were associated with myopia: rubella, OR=1.38, 95% CI: 1.03â1.85, P=0.030; mumps, OR=1.32, 95% CI: 1.07â1.64, P=0.010; and pertussis, OR=1.39, 95% CI 1.03â1.87, P=0.029. Measles, rubella, and pertussis were associated with high myopia: measles, OR=1.48, 95% CI: 1.07â2.07, P=0.019; rubella, OR=1.94, 95% CI: 1.12â3.35, P=0.017; and pertussis, OR=2.15, 95% CI: 1.24â3.71, P=0.006. The evidence did not support an interaction between education and febrile illness in explaining the above risks.
Conclusion A history of childhood measles, rubella, or pertussis was associated with high myopia, whereas a history of childhood rubella, mumps, or pertussis was associated with any myopia. The reasons for these associations are unclear
Update and guidance on management of myopia. European Society of Ophthalmology in cooperation with International Myopia Institute
The prevalence of myopia is increasing extensively worldwide. The number of people with myopia in 2020 is predicted to be 2.6 billion globally, which is expected to rise up to 4.9 billion by 2050, unless preventive actions and interventions are taken. The number of individuals with high myopia is also increasing substantially and pathological myopia is predicted to become the most common cause of irreversible vision impairment and blindness worldwide and also in Europe. These prevalence estimates indicate the importance of reducing the burden of myopia by means of myopia control interventions to prevent myopia onset and to slow down myopia progression. Due to the urgency of the situation, the European Society of Ophthalmology decided to publish this update of the current information and guidance on management of myopia. The pathogenesis and genetics of myopia are also summarized and epidemiology, risk factors, preventive and treatment options are discussed in details
The genetics of myopia
Myopia is the most common eye condition worldwide and its prevalence is increasing. While changes in environment, such as time spent outdoors, have driven myopia rates, within populations myopia is highly heritable. Genes are estimated to explain up to 80% of the variance in refractive error. Initial attempts to identify myopia genes relied on family studies using linkage analysis or candidate gene approaches with limited progress. More genome-wide association study (GWAS) approaches have taken over, ultimately resulting in the identification of hundreds of genes for refractive error and myopia, providing new insights into its molecular machinery. These studies showed myopia is a complex trait, with many genetic variants of small effect influencing retinal signaling, eye growth and the normal process of emmetropization. The genetic architecture and its molecular mechanisms are still to be clarified and while genetic risk score prediction models are improving, this knowledge must be expanded to have impact on clinical practice
Ocular significance of intraventricular haemorrhage in premature infants
AIMâTo document ocular outcome in premature infants with intraventricular haemorrhages (IVH).âšMETHODSâ68 preterm infants with IVH were examined.âšRESULTSâMean gestational age was 28.1Â weeks (range 24-35). Mean birth weight was 1045.9Â g (630-2240). Mean follow up was 54.6Â months (6-150). IVH is graded from 1Â to 4Â based on the severity of haemorrhages. The incidence of ocular abnormalities was compared between low grade IVH (grade 1Â and 2) and high grade IVH (grade 3Â and 4). Of the 68Â infants with IVH, ROP occurred in 33Â infants (48.5%); 13Â (43.3%) had low grade IVH; 20Â (52.6%) had high grade IVH. Strabismus developed in 30Â infants (44.1%); 14Â (46.6%) had low grade IVH; 16Â (42.1%) had high grade IVH. Infants with high grade IVH were at significant greater risk than infants with low grade IVH for the development of optic atrophy (31.5% v 16.6%), hydrocephalus (57.8% v 10%).âšCONCLUSIONâThis study highlights the serious significance of all grades of IVH with the higher incidence of optic atrophy and hydrocephalus with high grade IVH.âš
