    Comportamiento mecánico en función de la temperatura de aleaciones wolfranio-vanadio

    Libro de proceedings de: XI Congreso Nacional de Materiales, Zaragoza, 23 junio 2010En esta investigación se evalúa el comportamiento mecánico del wolframio puro y de dos de sus aleaciones con contenidos de 2% y 4% de vanadio, fabricadas mediante prensado isostático en caliente (HIP). La caracterización mecánica se ha realizado mediante ensayos de flexión en tres puntos en atmósfera oxidante a distintas temperaturas comprendidas entre temperatura ambiente y 1000 ºC. Adicionalmente, se han realizado ensayos en inmersión en nitrógeno líquido para los de -197 ºC. Estos últimos valores pueden servir para una determinación más precisa de la temperatura la transición dúctil-frágil en éstos materiales, y de su posible comportamiento plástico o frágil a temperatura ambiente. Mediante estos ensayos ha sido posible obtener la tenacidad de fractura, la resistencia mecánica, el límite elástico y el módulo de elasticidad en función de la temperatura. Así mismo, se ha determinado la densidad y la dureza Vickers para dos cargas distintas, lo que permitido determinar la influencia de este parámetro en la medida realizada. Adicionalmente, se han examinado mediante microscopia electrónica de barrido las superficies de fractura.     3D printed scaled setup for smoke transport analysis in a subterranean passenger platform

    In this work, the study of smoke fire transportation inside of a subway passenger platform is presented. The study includes a set of numerical simulations to observe the behavior of the smoke inside the platform. Two smoke transport simulations using the FDS program are also included. Subsequently, the development of a 3D - 1:100 scale model is described and it was used to perform an experimental observation of the phenomenon. The model was built by using a 3D printer which allowed to include more architectural details of the real scenario. The inclusion of these details allowed to observe qualitative similarity between the results of the simulation and the experimental work. Although there are clear differences between what could happen in a real scenario and what was observed in the scale model, it was identified that the model is an important complement to the simulations. In addition to the simulations, the use of this type of 3D models allows the observation of the phenomenon by different specialists such as firefighters, policeman, medical personnel, etc., in the same place and its intention is to provide a more interactive tool to the observation group, increasing the time devoted to the development of contingency actions and reducing the costs associated with the logistics of a real simulacrum. The model allows to better identify the strengths, opportunities, weaknesses and threats of the contingency procedures developed by the safety and hygiene groups and to make their corresponding adjustments if necessary.Peer Reviewe

    Spanish Collaboration on Endocarditis—Grupo de Apoyo al Manejo de la Endocarditis infecciosa en España (GAMES).[Introduction] Mural infective endocarditis (MIE) is a rare type of endovascular infection. We present a comprehensive series of patients with mural endocarditis.[Methods] Patients with infectious endocarditis (IE) from 35 Spanish hospitals were prospectively included in the GAMES registry between 2008 and 2017. MIEs were compared to non-MIEs. We also performed a literature search for cases of MIE published between 1979 and 2019 and compared them to the GAMEs series.[Results] Twenty-seven MIEs out of 3676 IEs were included. When compared to valvular IE (VIE) or device-associated IE (DIE), patients with MIE were younger (median age 59 years, p < 0.01). Transplantation (18.5% versus 1.6% VIE and 2% DIE, p < 0.01), hemodialysis (18.5% versus 4.3% VIE and 4.4% DIE, p = 0.006), catheter source (59.3% versus 9.7% VIE and 8.8% DIE, p < 0.01) and Candida etiology (22.2% versus 2% DIE and 1.2% VIE, p < 0.01) were more common in MIE, whereas the Charlson Index was lower (4 versus 5 in non-MIE, p = 0.006). Mortality was similar. MIE from the literature shared many characteristics with MIE from GAMES, although patients were younger (45 years vs. 56 years, p < 0.001), the Charlson Index was lower (1.3 vs. 4.3, p = 0.0001), catheter source was less common (13.9% vs. 59.3%) and there were more IVDUs (25% vs. 3.7%). S. aureus was the most frequent microorganism (50%, p = 0.035). Systemic complications were more common but mortality was similar.[Conclusion] MIE is a rare entity. It is often a complication of catheter use, particularly in immunocompromised and hemodialysis patients. Fungal etiology is common. Mortality is similar to other IEs.Peer reviewe

    A retrospective, multicenter study of the efficacy of lapatinib plus trastuzumab in HER2-positive metastatic breast cancer patients previously treated with trastuzumab, lapatinib, or both: the Trastyvere study

    [Purpose]: To evaluate the efficacy and safety of lapatinib (L) and trastuzumab (T) combination in HER2-positive metastatic breast cancer (MBC) patients previously treated with T and/or L.[Materials and methods]: We conducted a retrospective, post-authorized, multicenter study including patients with HER2-positive MBC or locally advanced breast cancer (ABC) treated with the combination of L–T. Concomitant endocrine therapy, as well as brain metastasis and/or prior exposure to L, were allowed.[Results]: One hundred and fifteen patients from 14 institutions were included. The median age was 59.8 years. The median number of prior T regimens in the advanced setting was 3 and 73 patients had received a prior L regimen. The clinical benefit rate (CBR) was 34.8% (95% CI 26.1–43.5). Among other efficacy endpoints, the overall response rate was 21.7%, and median progression-free survival (PFS) and overall survival were 3.9 and 21.6 months, respectively. Heavily pretreated and ≥ 3 metastatic organ patients showed lower CBR and PFS than patients with a low number of previous regimens and < 3 metastatic organs. Moreover, CBR did not significantly change in L-pretreated compared with L-naïve patients (31.5% versus 40.5% for L-pretreated versus L-naïve). Grade 3/4 adverse events were reported in 19 patients (16.5%).[Conclusion]: The combination of L–T is an effective and well-tolerated regimen in heavily pretreated patients and remains active among patients progressing on prior L-based therapy.     Professional quality of life and organizational changes: a five-year observational study in Primary Care

    <p>Abstract</p> <p>Background</p> <p>The satisfaction and the quality of life perceived by professionals have implications for the performance of health organizations. We have assessed the variations in professional quality of life (PQL) and their explanatory factors during a services management decentralization process.</p> <p>Methods</p> <p>It was designed as a longitudinal analytical observational study in a Health Area in Madrid, Spain. Three surveys were sent out during an ongoing management decentralization process between 2001 and 2005. The professionals surveyed were divided into three groups: Group I (97.3% physicians), group II (92.5% nurses) and group III (auxiliary personnel). Analysis of the tendency and elaboration of an explanatory multivariate model was made. The PQL -35 questionnaire, based on Karasek's demand-control theory, was used to measure PQL. This questionnaire recognizes three PQL dimensions: management support (MS), workload (WL) and intrinsic motivation (IM).</p> <p>Results</p> <p>1444 responses were analyzed. PQL increased 0.16 (CI 95% 0.04 – 0.28) points in each survey. Group II presents over time a higher PQL score than group I of 0.38 (IC 95% 0.18 – 0.59) points. There is no difference between groups I and III.</p> <p>For each point that MS increases, PQL increases between 0.44 and 0.59 points. PQL decreases an average of between 0.35 and 0.49 point, for each point that WL increases.</p> <p>Age appears to have a marginal association with PQL (CI 95% 0.00 – 0.02), as it occurs with being single or not having a stable relationship (CI 95% 0.01 – 0.41). Performing management tasks currently or in the past is related to poorer PQL perception (CI 95% -0.45 – -0.06), and the same occurs with working other than morning shifts (CI 95% -0.03 – -0.40 points).</p> <p>PQL is not related to sex, location of the centre (rural/urban), time spent working in the organization or contractual situation.</p> <p>Conclusion</p> <p>With the improvement in work control and avoiding increases in workloads, PQL perception can be maintained despite deep organizational changes at the macro-management level. Different professional groups experience different perceptions depending on how the changes impact their position in the organization.</p

    Morphosedimentary infilling and human settlement of the Tinto-Odiel Estuary (Huelva) during the Mid-Late Holocene.

    The first human settlements (6000−4000 BP) in the early estuary of the Tinto and Odiel rivers were located in the ancient coastal banks or in the nearby hills. From 4000 years BP onwards, the estuarine sediments started to emerge as sand barriers and chenier plains, prograding towards the mouth. As the littoral strands stabilized morphologically, they are colonized by human settlements in successive periods, the oldest inland (Almendral) and more recent outward (Cascajera). These emerged morphologies were surrounded by muddy tidal flats in the estuary and only the high-energy marine events such as a tsunami or extraordinary storms were able to modify both the landscape and human settlements installed on them.
