Do Diabetic Patients with Acute Coronary Syndromes Have a Higher Threshold for Ischemic Pain?
Background: Data from over 4 decades have reported a higher incidence of silent infarction among patients with diabetes mellitus (DM), but recent publications have shown conflicting results regarding the correlation between DM and presence of pain in patients with acute coronary syndromes (ACS). Objective: Our primary objective was to analyze the association between DM and precordial pain at hospital arrival. Secondary analyses evaluated the association between hyperglycemia and precordial pain at presentation, and the subgroup of patients presenting within 6 hours of symptom onset. Methods: We analyzed a prospectively designed registry of 3,544 patients with ACS admitted to a Coronary Care Unit of a tertiary hospital. We developed multivariable models to adjust for potential confounders. Results: Patients with precordial pain were less likely to have DM (30.3%) than those without pain (34.0%; unadjusted p = 0.029), but this difference was not significant after multivariable adjustment, for the global population (p = 0.84), and for subset of patients that presented within 6 hours from symptom onset (p = 0.51). In contrast, precordial pain was more likely among patients with hyperglycemia (41.2% vs 37.0% without hyperglycemia, p = 0.035) in the overall population and also among those who presented within 6 hours (41.6% vs. 32.3%, p = 0.001). Adjusted models showed an independent association between hyperglycemia and pain at presentation, especially among patients who presented within 6 hours (OR = 1.41, p = 0.008). Conclusion: In this non-selected ACS population, there was no correlation between DM and hospital presentation without precordial pain. Moreover, hyperglycemia correlated significantly with pain at presentation, especially in the population that arrived within 6 hours from symptom onset
Real-time torque estimation using human and sensor data fusion for exoskeleton assistance
Robotic assistive devices have emerged as a potential complement for repeti-tive and user-centered gait rehabilitation. In this field, the development of electromyography (EMG)-based torque controls has played a crucial role in improving the user experience with robotic assistive devices. However, most existing approaches for EMG-based joint torque estimation (i) are designed for upper limbs; (ii) often do not consider the complexity of the walking mo-tion, focusing only on the stance phase; and (iii) rely on complex mathemat-ical models that result in time-consuming estimations. This study aims to address these shortcomings by evaluating the generalization ability of a Deep Learning regressor (Convolutional Neural Network (CNN)) for estimating ankle torque trajectories, in real-time. Several inputs were incorporated, namely, EMG signals from Tibialis Anterior and Gastrocnemius Lateralis, hip kinematic data in the sagittal plane (angle, angular velocity, angular acceler-ation), walking speed (from 1.5 to 2.0 km/h), user's demographic (gender and age) and anthropometric information (height and mass, ranging from 1.50 to 1.90 m and 50.0 to 90.0 kg, respectively, and shank and foot lengths). Re-sults showed that a CNN model with two convolutional layers showed the highest generalization ability (Root Mean Square Error: 23.4±8.36, Normal-ized Mean Square Error: 0.494±0.299, and Spearman Correlation 0.754±0.105). CNN model’s time-effectiveness was tested in an active ankle orthosis, being able to estimate ankle joint torques in less than 2 millisec-onds. Platelet function, coagulation and fibrinolysis in patients with previous coronary and cerebrovascular ischemic events
OBJECTIVES: Ischemic stroke (IS) or transient ischemic attack (TIA) history is present in 4-17% of patients with coronary artery disease (CAD). This subgroup of patients is at high risk for both ischemic and bleeding events. The aim of this study was to determine the role of platelet aggregability, coagulation and endogenous fibrinolysis in patients with CAD and previous IS or TIA. METHODS: A prospective case-control study that included 140 stable CAD patients divided into two groups: the CASE group (those with a previous IS/TIA, n=70) and the CONTROL group (those without a previous IS/TIA, n=70). Platelet aggregability (VerifyNow Aspirins and VerifyNow P2Y12s), coagulation (fibrinogen and thromboelastography by Reoroxs) and endogenous fibrinolysis (D dimer and plasminogen activator inhibitor-1) were evaluated. RESULTS: Patients in the CASE group presented significantly higher systolic blood pressure levels (135.84±16.09 vs 123.68±16.11, po0.01), significantly more previous CABG (25.71% vs 10%, p=0.015) and significantly higher calcium channel blocker usage (42.86% vs 24.29%, p=0.02) than those in the control group. In the adjusted models, low triglyceride values, low hemoglobin values and higher systolic blood pressure were significantly associated with previous IS/TIA (CASE group). Most importantly, platelet aggregability, coagulation and fibrinolysis tests were not independently associated with previous cerebrovascular ischemic events (CASE group). CONCLUSION: Platelet aggregability, coagulation and endogenous fibrinolysis showed similar results among CAD patients with and without previous IS/TIA. Therefore, it remains necessary to identify other targets to explain the higher bleeding risk presented by these patients
Adult culicidae captured in a rural area in southern Brazil
During the period from September 1988 to April 1990, mosquitoes were captured using Shannon light trap and Falcão light traps, in Terra Boa county, in the north of Paraná State, Brazil. The Shannon trap was installed in the outlying modified forest and Falcão traps were installed in the modified forest and domiciliary areas. The prevalence of species was verified and the methods of capture compared.São apresentados os resultados obtidos de coletas de mosquitos, com armadilha luminosa de Shannon, às margens de uma mata modificada, e armadilhas de Falcão na mesma mata e em ecótopos extra-florestais, no Município de Terra Boa, Estado do Paraná, Brasil, de setembro de 1988 a abril de 1990. Verificou-se a prevalência das espécies de culicídeos e compararam-se os dois métodos de coletas
Flebotomíneos em área de transmissão de leishmaniose tegumentar na região norte do Estado do Paraná - Brasil: Variação Sazonal e Atividade Noturna
In two years of phlebotomine captures in a modified forest an appreciable decrease in the size of this insect population has been observed when compared to previous data from the same locality. The captures with the Shannon light trap were made in the outlying forest of Palmital farm in Terra Boa county, Parana State, Brazil. The predominant species were Lutzomyia withmani, Lutzomyia migonei, Lutzomyia intermedius e Lutzomyia fischeri. These insects were active from 6 p.m. to 6 a.m. and the period of greatest activity varied according to the species. These species' densities were higher in the summer and autum months. Environmental modifications may have influenced the observed changes in phlebotomine behavior.No período de dois anos de capturas de flebotomíneos com armadilha luminosa de Shannon, às margens de uma mata residual alterada, na fazenda Palmital, Município de Terra Boa, Estado do Paraná, Brasil, observou-se que a densidade populacional desses insetos diminuiu sensivelmente, quando comparada com resultados anteriores, no mesmo local. As espécies predominantes, Lutzomyia whitmani, Lutzomyia migonei, Lutzomyia intermedius e Lutzomyia fischeri tiveram atividade das 18 às 6h e o pico horário de freqüência variou de acordo com cada espécie. As espécies citadas mostraram maior densidade nos meses de verão e outono. Concluem-se que as alterações introduzidas no ambiente podem ter resultado em mudanças no comportamento dos flebotomíneos
UTILIZANDO A PLATAFORMA KAHOOT COMO FERRAMENTA DE APRENDIZAGEM PARA 9º ANO DO ENSINO FUNDAMENTAL II NO CENTRO EDUCACIONAL JOÃO LOBO NO MUNICÍPIO DE CAREIRO – AM
This article explores the supervised internship experience as an essential part of academic preparation, highlighting its application in the context of the 9th grade of elementary school. Performed as a component of the Supervised Internship in Computing I discipline, integrated into the curriculum of the Licentiate in Computing at the Nucleus of Higher Education in Careiro Castanho (NESCAC), linked to the University of the State of Amazonas (UEA), this practical activity is supported by the mandatory Law of Guidelines and Bases of National Education (LDB). Teachers' concern with students' lack of interest and motivation in transmitting knowledge in the traditional way. The introduction of technology, especially the use of technological resources such as digital images and gamified platforms, has been adopted as a strategy to enrich the pedagogical experience and promote interaction among students. Gamification, based on game controls, is seen as an approach that can engage and motivate students, making the teaching-learning process more attractive and effective. The study intends to explore the application of this innovative approach on the Kahoot platform, sharing experiences lived in the classroom at the João Lobo Educational Center, analyzing the impact and potential that it can offer for Elementary School II students.Este artículo explora la experiencia de la pasantía supervisada como parte esencial de la preparación académica, destacando su aplicación en el contexto del 9º grado de la escuela primaria. Realizada como un componente de la disciplina de Pasantía Supervisada en Computación I, integrada en el plan de estudios de la Licenciatura en Computación en el Centro de Educación Superior de Careiro Castanho (NESCAC), vinculada a la Universidad del Estado de Amazonas (UEA), esta actividad práctica está respaldada por la obligación de la Ley de Directrices y Bases de la Educación Nacional (LDB). La preocupación de los profesores por la falta de interés y motivación de los alumnos a la hora de transmitir conocimientos de forma tradicional. La introducción de la tecnología, especialmente el uso de recursos tecnológicos como imágenes digitales y plataformas gamificadas, se ha adoptado como una estrategia para enriquecer la experiencia pedagógica y promover la interacción entre los estudiantes. La gamificación, basada en el control del juego, se ve como un enfoque que puede involucrar y motivar a los estudiantes, haciendo que el proceso de enseñanza-aprendizaje sea más atractivo y efectivo. El estudio tiene como objetivo explorar la aplicación de este enfoque innovador en la plataforma Kahoot, compartiendo experiencias vividas en el aula del Centro Educativo João Lobo, analizando el impacto y el potencial que puede ofrecer a los estudiantes de la Escuela Primaria II.Este artigo explora a experiência do estágio supervisionado como parte essencial da preparação acadêmica, com destaque para sua aplicação no contexto do 9º ano do ensino fundamental.
